    Análisis de interconsultas derivadas desde atención primaria y rechazadas por salud mental en el Departamento de Salud Arnau de Vilanova-Llíria (Valencia)

    In recent years, there has been an increase in the demand for care in Mental Health services and, therefore, an increase in interconsultations received from Primary Care, making it necessary to filter said referrals in order to carry out adequate care. The main objective of this study is to evaluate the referral model of the Mental Health Service of the Department of Arnau de Vilanova-Llíria through the descriptive analysis and follow-up over 6 months of interconsultations derived from Primary Care and rejected by Mental Health in 2019. During those months, 2029 interconsultations were received, of which 623 were rejected, corresponding to 504 patients. Of these patients, 164 were finally attended to in Mental Health after carrying out a new interconsultation. This analysis shows the need to implement a collaborative model between Primary Care and Mental Health that turns into greater cases detection, better referral sand, ultimately, a better patient care.En los últimos años se ha constatado un aumento de la demanda asistencial en los servicios de salud mental y, por tanto, un aumento de interconsultas que se reciben desde atención primaria, siendo necesario filtrar dichas derivaciones para poder llevar a cabo una atención adecuada. El principal objetivo de este estudio es la evaluación del modelo de derivación del Servicio de Salud Mental del Departamento del Arnau de Vilanova-Llíria mediante un análisis descriptivo y el seguimiento a lo largo de 6 meses de las interconsultas derivadas desde atención primaria y rechazadas por salud mental en el año 2019. Durante esos meses se recibieron 2029 interconsultas, de las cuales 623 fueron rechazadas, correspondiéndose estas a 504 pacientes. De estas personas, 164 fueron finalmente citadas en salud mental tras realizar una nueva interconsulta. Este análisis pone de manifiesto la necesidad de implementar un modelo colaborativo entre atención primaria y salud mental que se traduzca en una mayor detección de casos, una mejor derivación y, en definitiva, una mejor atención a los y las pacientes

    The article discusses the importance of Carlos Castilla del Pino's teaching in Spanish culture, especially as a mentor for the whole generation that promoted the change. This is done by following the works of his intellectual period in the sixties, books and articles that had an enormous acceptance among the youth that later played a leading role in that change. Carlos Castilla was, in Spain and also in Latin America, the teacher of a whole generation that read him avidly and relied on his critical analysis and terminology. He became the super-ego of an entire intellectual elite through his writings of those years, particularly during the 1968-1975 period.El artículo glosa la importancia del magisterio de Carlos Castilla del Pino en la cultura española, especialmente como mentor que fue de toda la generación que impulsó el cambio. Lo hace siguiendo las obras de su período intelectual sesentaiochista, libros y artículos que tuvieron una enorme acogida entre la juventud que luego protagonizó ese cambio. Carlos Castilla fue, en España y también en Hispanoamérica, el maestro de toda una generación que lo leyó con avidez y contó con sus análisis críticos y su terminología. Se convirtió en el superego de toda una élite intelectual mediante sus escritos de esos años década en particular, 1968-1975

    Anti-inflammatory and antioxidative effects of dexpanthenol on nicotine-induced lung injury in rats

    Aim Lung inflammation is a consequence of smoking, tobacco use, nicotine addiction, and the accumulation of toxicants in the body. This study aimed to investigate the association between nicotine-induced lung injury and NF-kappa B activation, as well as changes in redox balance. Furthermore, the protective role of Dexpanthenol against this damage was examined. Method A total of 32 male rats were divided into four groups: Control, DEX, Nicotine, and Nicotine + DEX. Nicotine (0.5 mg/kg/day) and Dexpanthenol (500 mg/kg/day) were administered intraperitoneally. Subsequently, the levels of nuclear and cytoplasmic NF-kappa B in lung tissue were analyzed. Inflammation and oxidative stress markers and histopathological evaluations of the lung tissue were conducted. Results Nicotine administration resulted in increased levels of tissue MDA and TOS, as well as decreased levels of GSH-Px, GSH, GST, SOD, and TAS. Additionally, nicotine administration led to elevated nuclear expression of NF-kappa B protein, IL-1 beta, IL-6 proinflammatory cytokine levels, and Galectin-3 levels, which modulate cytokine release. Moreover, histopathological examinations revealed a higher population of diffuse lymphocytes and macrophages, indicating increased lung inflammation. Dexpanthenol administration ameliorated these adverse effects of nicotine and reduced them to levels comparable to the control group. Conclusion Nicotine-induced lung injury promoted oxidative stress and inflammation through modulation of NF-kappa B's nuclear translocation.     The effect of thrombocytapheresis on hemogram and biochemistry parameters in patients with essential thrombocytemia

    Background: Essential thrombocythemia is one of the chronic myeloproliferative neoplasms characterized by clonal proliferation of myeloid cells with variable morphological maturation and hematopoietic activity. It is characterized by excessive clonal platelet production with a tendency to thrombosis and bleeding. Thrombocytapherests is the removal of platelets by apheresis techniques. Thrombocytapherests is generally recommended in patients with essential thrombocythemia with acute, severe thrombotic or hemorrhagic events.Methods: The study included 39 patients who were diagnosed with essential thrombocythemia, started cytoreductive and aspirin therapy, and underwent thrombocytapherests due to the development of acute severe thrombotic or hemorrhagic events, diagnosed in the adult hematology clinic of I_nonu University Turgut Ozal Medical Center. Hemogram and biochemistry values of the patients were scanned retrospectively.Results: After thrombocytapherests, a statistically significant difference was found between the first and last measurements of hemoglobin, mean platelet volume, White blood cell, neutrophil, platelet, platelet distribution width, creatine, lactate dehydrogenase, fibronogen and calcium levels of the patients. Conclusion: The use of thrombocytapherests in patients with essential thrombocytosis causes a rapid decrease in platelet values as well as an effect on hemogram and biochemistry parameters.     Detection of hateful twitter users with graph convolutional network model

    Today, hate speech is widespread and persistent in various forms on social networking platforms, targeting different minority groups. These attacks can be carried out using various factors such as racial, religious, gender, and physical disability, etc. Considering the number of people and their interactions, social networks are the most important channels through which these discourses spread. The social network structure is considered a set of nodes and edges and is very suitable for the graph structure. The multidimensional structure of social networks carries social network data from Euclidean space to non-Euclidean space. In non-Euclidean space, the graph structure is used to represent data effectively. In this respect, solving the hate speech problem with graph-based methods in a complex dimensional space can produce more impressive results. In this study, a powerful method based on the Graph Convolutional Network (GCN) model, which is rarely used in this field, was proposed for the detection of hateful Twitter users in social networks. Well-known machine learning methods were used to measure the performance of this method. According to the results obtained, the proposed GCN model gave the most successful result

    Is acute appendicitis complicated or uncomplicated? Approaching the question via computed tomography

    Background The distinction between complicated and uncomplicated appendicitis is very important for the selection of the treatment method. Purpose To investigate the sensitivity and specificity of computed tomography (CT) in differentiating between complicated and uncomplicated appendicitis to demonstrate that false negativity in differentiating these cases can be reduced when CT findings are incorporated into the clinical evaluation of patients. Material and Methods All patients aged >= 18 years who underwent appendectomy at Malatya Training and Research Hospital in 2020 and 2021 were retrospectively screened. Of them, 283 patients were included in the study who had undergone CT before the operation. Patients with appendicitis were divided into two groups: complicated and uncomplicated, according to the results of their pathology tests. Demographic data, laboratory results, and CT images of the patients were evaluated. Results The patients with complicated appendicitis had a significantly higher mean age (P<0.001). The most common CT findings in patients with complicated appendicitis were moderate or severe peri-appendiceal fat stranding (PFS) and appendix wall enhancement defect (AWD). The findings with the highest sensitivity were PFS (77.9%) and AWD (69.4%). Although abscess, phlegmon, and peri-appendiceal air had the highest specificity (100%), these findings were the ones with the lowest sensitivity. According to the scoring system was developed for the differential diagnosis, CT had a sensitivity of 83.3% and a specificity of 79.2%. Conclusion Based on the sensitivity and specificity values measured for CT according to the findings of our study, the scoring system may be useful for the differential diagnosis of complicated appendicitis

    Acute interstitial pneumonia associated with vinorelbine usage

    Vinorelbine, an analogue of vinca alkaloids, has an antineoplastic effect. Adverse effects related to vinca alkaloids are rare. A 57-year-old patient who presented to the emergency room with dyspnea and back pain was hospitalized with the diagnosis of overload related to cardiac decompensation and complicated urinary tract infection due to its acute clinical and radiological findings. While under treatment, his respiratory failure progressed. Considering his history of vinorelbine usage 2 weeks prior, it was thought to be interstitial pneumonia. High-flow oxygen treatment and 250 mg/day pulse methylprednisolone for 3 days was started; after the fourth day, methylprednisolone was reduced and continued at 1 mg/kg/day. His oxygen need decreased on the seventh day of the treatment, and the findings on the chest X-ray regressed almost completely. However, the patient died due to acute gastrointestinal bleeding

    Encapsulation of vitamin D with different encapsulation techniques

    Bu çalışmada, D vitamini enkapsülasyonu ultrason yöntemi ile geleneksel emülsiyon ve yüksek basınçlı homojenizatör kullanılarak pickering emülsiyon üretimi yapılmıştır. Emülsiyon üretiminde; farklı zincir uzunluğu ve doymamışlık oranına sahip üç farklı yağ (Orta zincirli yağ asidi içeren yağ (MCT), Ayçiçek ve keten tohumu yağı; %5 a/a) ve % 95 su fazı (bezelye ve mercimek proteini içeren: %0,5 a/a) kullanılmıştır. Pickering emülsiyon üretimi için bitkisel proteinlerden jel üretilerek nanoagregatlar elde edilmiş ve emülgatör olarak kullanılmışlardır. Her iki yöntemle elde edilen D vitamini (%0,01) içeren emülsiyonların fiziksel, oksidatif stabiliteleri ve D vitamini biyogeçişliliği ile serbest yağ asidi salımları karşılaştırılmıştır. Geleneksel emülsiyonlarda partikül büyüklüğü 0,205±0,00 ile 0,98±0,07 μm, partikül yükleri ise -31,2±1,27 ile -49,2±1,90 mV arasında; pickering emülsiyonlarda partikül boyutu 0,15±0,00 ile 12,6±0,26 μm, partikül yükleri ise -25,2±0,21 ile -41,2±2,40 mV arasında belirlenmiş ve fiziksel olarak stabil emülsiyonlar elde edilmiştir. Oksidatif stabilite testlerinde geleneksel ve pickering emülsiyonda; mercimek proteini ile stabilize edilen ayçicek ve keten tohumu emülsiyonlarının lipit hidroperoksit ve TBARS değerlerindeki artış aynı yağların bezelye proteini ile stabilize edilen emülsiyonlarına göre daha fazladır. Tüm pickering emülsiyonlarının PV ve TBARS değerlerindeki oluşumunun daha yavaş olduğu görülmüştür. In vitro sindirim testlerinde; pickering ve geleneksel emülsiyonlarda % SYA salınımları %53,9±1,04 ile %115,9±5,21 arasında ölçülmüş ve aralarında anlamlı bir farklılık görülmezken, pickering emülsiyonlarda D vitamini biyogeçişliliğinin yaklaşık 2 kat daha fazla olduğu gözlemlenmiştir. Sonuç olarak, iki yöntem karşılaştırıldığında geleneksel emülsiyonlara göre pickering emülsiyonlarin daha iyi oksidatif stabiliteye, kontrollü SYA salınımına ve yüksek D vitamini biyogeçişliliğine sahip olduğu belirlenmiştir.In this study, vitamin D encapsulation was produced in conventional emulsion by ultrasound method and pickering emulsion using high pressure homogenizer. In emulsion production; three different oils with different chain lengths and unsaturation ratios (Medium-chain fatty acid containing oil (MCT), Sunflower and linseed oil; 5% w/w) and 95% water phase (containing pea and lentil protein: 0,5% w /w) was used. For the production of Pickering emulsion, gels were produced from vegetable proteins and nanoaggregates were obtained and used as emulsifiers. The physical and oxidative stability of emulsions containing vitamin D (0.01%) obtained by both methods, bioavailability of vitamin D and free fatty acid releases were compared. In conventional emulsions, particle sizes were determined between 0,205±0,00 and 0,98±0,07 μm, and particle charges were determined between -31,2±1,27 and -49,2±1,90 mV; In pickering emulsions, particle sizes were 0,15±0,00 to 12,6±0,26 μm, particle charges were -25,2±0,21 and -41,2±2,40 mV. All emulsions were physically stable. The oxidative stability in conventional and pickering emulsions; the increase in hydroperoxide and TBARS values of sunflower seed and flaxseed emulsions stabilized with lentil protein is higher than that of the same oil emulsions stabilized with pea protein. The formation of all pickering emulsions in PV and TBARS values were observed to be slower. In in vitro digestion test; while %FFA emulsions in pickering and conventional emulsions were measured between %53,9±1,04 and %115,9±5,21 and no significant difference was found between them, in pickering emulsions, bioavailability of vitamin D was observed to be approximately 2 times higher. As a result, when compared, it was determined that pickering emulsions had better oxidative stability, controlled FFA release and higher vitamin D bioavailability than conventional emulsions
