    Roma imparatoru Diocletianus'un 284-305 yılları arasındaki yirmi bir yıllık egemenliği Roma tarihindeki önemli dönüm noktalarından biridir. 235 yılında başlayan ve imparatorluğun bir zayıflama dönemine girmesine neden olan elli yıllık kriz dönemi 284 yılında iktidara gelen Diocletianus'un icraatlarıyla sona ermiştir. Diocletianus'un iktidara geldiği dönemde Roma askerleri devlet yönetimi üzerinde benzersiz bir etkiye sahipti ve Diocletianus'un kendisi de Roma ordusunda yetişen bir askerdi. Diocletianus asker kökenli bir imparator olsa da iktidara geldiğinde geniş sınırları olan imparatorluğu tek başına yönetmenin zorluğunu görmüş olacak ki yönetim modelinde değişikliğe gitmiştir. 286-293 tarihleri arasında eş augustusu ile ikili yönetim modelinde devleti idare etmişken 293 tarihinden 305 tarihine kadar Roma İmparatorluğu'nda daha önce görülmeyen bir sistem olan ve Tetrarchia olarak bilinen dörtlü yönetim modelini uygulamıştır. Bu yönetim modelinin başında Diocletianus'un kendisi yer almaktaydı, diğer augustus ile caesarlar bu modelin uygulayıcılarıydı. Uyumlu bir dörtlü yönetim biçiminin oluşturulmasıyla, yarım yüzyıl yaşanan siyasi ve askeri bir kaostan sonra imparatorluğa yeniden istikrar kazandırdılar. Diocletianus ve caesarı Galerius imparatorluğun Doğu bölümünü idare ederken, eş Augustus Maximianus ve caesarı Constantius Chlorus imparatorluğun Batı bölümünü kontrol etmekteydi. Yönetimin dört yönetici arasında paylaşılmasıyla Roma İmparatorluğu'nun resmi olarak 395 yılında bölünmesine kadar imparatorluk ilk kez bu dönemde coğrafi açıdan ikiye bölünmüştür. Diocletianus, meslektaşlarının sorumluluk bölgelerinden kaynaklı olarak ikiye bölünen imparatorluğu geçmişteki krizin etkilerinden kurtarmak ve yeniden istikrar kazandırmak amacıyla pek çok reform girişiminde bulunmuştur. Yaşanan siyasi olaylarda üstünlük elde etmesiyle, imparatorluğun sınırlarını güvenceye almasıyla ve pek çok alanda gerçekleştirdiği düzenlemelerle esasen Constantinus'un güçlü yönetimine temel hazırlamış görünmektedir. Bu tez çalışmasında Diocletianus'un Roma İmparatorluğu'na siyasi, idari, askeri, iktisadi, dini ve hukuki alanda ne derece de katkı yaptığı ve ne kadar etkili olduğu ortaya koyulacaktır. Çalışma sırasında antik yazarların eserleri, imparatorlara sunulan övgü metinleri gibi konuyla ilgili birincil kaynak niteliği taşıyan eserler temel alınmış olup tetkik eserlerden de istifade edilmiştir. Anahtar Kelimeler: Diocletianus, Maximianus Herculius, Diarchia, Tetrarchia, Dominatus.The twenty-one-year rule of Roman emperor Diocletian between 284 and 305 is one of the important turning points in Roman history. The fifty-year crisis period, which started in 235 and caused the empire to enter a period of weakening, was stopped by the activities of Diocletian, who came to power in 284. At the time when Diocletian came to power, Roman soldiers had a unique influence on state administration, and he himself was a soldier trained in the Roman army. Although Diocletian was an emperor of military origin, when he came to power, he must have seen the difficulty of managing the empire with its wide borders on his own, so he changed the management model of the state. While he governed the state in a dual government model with his co-emperor between 286 and 293, he implemented the management model of four emperors known as Tetrarchy, a system not seen before in the Roman Empire, from 293 to 305. Diocletian himself was at the head of this management model, and the other augustus and caesars were the implementers of this model. By establishing a harmonious tetrarchy, they restored stability to the empire after half a century of political and military chaos. While Diocletian and his caesar Galerius ruled the Eastern part of the empire, his co-emperor Maximianus and his caesar Constantius Chlorus controlled the Western part of the empire. The empire was divided geographically for the first time in this period until the official division of the Roman Empire in 395, when the administration was shared between the four rulers. Diocletian attempted many reforms in order to save the empire, which was divided into two due to the areas of responsibility of his colleagues, from the effects of the past crisis and to re-stabilize it. Diocletian seems to have laid the groundwork for Constantine's strong rule by gaining superiority in political events, securing the borders of the empire, and making arrangements in many areas. In this thesis, it will be revealed to what extent Diocletian contributed to the Roman Empire in the political, administrative, military, economic, religious and law fields and how effective he was. During the thesis study, the works of ancient authors, texts of praise to the emperors, primary sources on the subject were taken as basis, and modern research works were also used.     Sex and age estimation with machine learning algorithms with parameters obtained from cone beam computed tomography images of maxillary first molar and canine teeth

    BackgroundThe aim of this study is to obtain a highly accurate and objective sex and age estimation by using the parameters of maxillary molar and canine teeth obtained from cone beam computed tomography images in the input of machine learning algorithms. Cone beam computed tomography images of 240 people aged between 25 and 54 were randomly selected from the archive systems of the hospital and transferred to Horos Medikal. 3D curved multiplanar reconstruction was applied to these images and a 3D image was obtained. The resulting image was brought to the orthogonal plane and the measurements were made by superimposing them.ResultsThe results were grouped in four different age groups (25-30, 31-36, 37-49, 50-54) and recorded. As a result of our study, the highest accuracy rate was found as 0.81 in sex estimation with ADA Boost Classifier algorithm, while in age estimation, the highest accuracy rate was found as 0.84 between 25-30 and 31-36 age groups with random forest algorithm, as 0.74 between 25-30 and 37-49 age groups with random forest and ADA Boost Classifier algorithms and as 0.85 between 25-30 and 50-54 age groups with random forest algorithm.ConclusionsOur study differs from other studies in two aspects; the first is the selection of a sensitive method such as cone beam computed tomography, and the second is the selection of machine learning algorithms. As a result of our study, the highest accuracy rate was found as 0.81 in sex estimation and as 0.85 in age estimation with parameters of maxillary canine and molar teeth

    Determination of distress, emotional eating and internalized weight bias levels of Turkish pregnant women

    This study was conducted to determine the stress, emotional eating and weight bias levels of Turkish pregnant women. The study sample was composed of 210 pregnant women, who met the research inclusion criteria and admitted to the obstetrics and gynaecology outpatient clinics of Bingol Hospital for Obstetrics and Gynaecology. Research data were collected between December 2018 and June 2019, using face-to-face interview technique. Personal Information Form, Tilburg Pregnancy Distress Scale (TPDS), Internalised Weight Bias Scale (IWBS), and Emotional Eating sub-scale items of the Netherlands Eating Behaviour Questionnaire used to collect data. In our study, 47.9% of pregnant women were found to be overweight or obese according to the pre-pregnancy body mass index (BMI) average. Pregnant women experience a moderate level of stress, emotional eating and weight bias. It was found that there was a statistically significant relationship between the weight bias score averages and the emotional eating and stress score averages of the pregnant women (p < .05). In our study, stress, emotional eating and weight bias score averages of pregnant women in the 3rd trimester were found to be higher than that of the pregnant women in the 2nd trimester (p < .05). It has been determined that nearly 1in 2 pregnant women was overweight or obese, when BMI level of the women increased, weight stigma and emotional eating of them also increased. IMPACT STATEMENT What is already known on this subject? To be overweight or obese pre-pregnancy is risk for pregnancy complications and adverse birth outcomes. What do the results of this study add? It is important to inform nurses about the relationship between stress, weight bias, eating disorders, and obesity; moreover, care should be providing with the awareness that pregnant women with obesity are at greater risk in terms of these factors. It is of great importance to provide the necessary training and counselling by nurses to ensure the psychological adaptation of pregnant women to childbirth and the postpartum period. Besides, any disadvantage or disparity between overweight and obese pregnant women in the care process should be eliminated, and all pregnant women, regardless of their body size, should have equal access to supportive prenatal and postnatal care. What are the implications of these findings for clinical practice and/or further research? It is of great importance to providing training and consultation by nurses on coping with stress and stigma and eating during pregnancy in order to ensure psychological adjustment of the pregnant women to childbirth and the postpartum period, which are at risk in terms of stress, emotional eating and weight bias factors

    Central nervous system thrombosis in pediatric acute lymphoblastic leukemia in Turkey: A multicenter study

    BackgroundIn patients with acute lymphoblastic leukemia (ALL), the risk of thromboembolism increases due to hemostatic changes secondary to the primary disease and due to treatment-related factors. In this multicenter study, we aimed to research the frequency of central nervous system (CNS) thrombosis occurring during treatment, hereditary and acquired risk factors, clinical and laboratory features of patients with thrombosis, treatment approaches, and thrombosis-related mortality and morbidity rates in pediatric ALL patients. ProcedurePediatric patients who developed CNS thrombosis during ALL treatment from 2010 to 2021 were analyzed retrospectively in 25 different Pediatric Hematology Oncology centers in Turkiye. The demographic characteristics of the patients, symptoms associated with thrombosis, the stage of the leukemia treatment during thrombosis, the anticoagulant therapy applied for thrombosis, and the final status of the patients recorded through electronic medical records were determined. ResultsData from 70 patients with CNS thrombosis during treatment, out of 3968 pediatric patients with ALL, were reviewed. The incidence of CNS thrombosis was 1.8% (venous: 1.5 %; arterial: 0.03%). Among patients with CNS thrombosis, 47 had the event in the first 2 months. Low molecular weight heparin (LMWH) was the most commonly used treatment with a median of 6 months (min-max: 3-28 months). No treatment-related complications occurred. Chronic thrombosis findings occurred in four patients (6%). In five (7%) patients who developed cerebral vein thrombosis, neurological sequelae (epilepsy and neurological deficit) remained. One patient died related to thrombosis, and the mortality rate was 1.4%. ConclusionCerebral venous thrombosis and, less frequently, cerebral arterial thrombosis may develop in patients with ALL. The incidence of CNS thrombosis is higher during induction therapy than during other courses of treatment. Therefore, patients receiving induction therapy should be monitored carefully for clinical findings suggestive of CNS thrombosis
