Reduced fetal movements and cerebroplacental ratio: evidence for worsening fetal hypoxemia
Objective: To investigate the fetal cerebroplacental ratio (CPR) in women presenting with reduced fetal movements (RFM). Methods: This was a retrospective cohort study of data collected over an 8-year period at a fetal medicine unit at a tertiary referral center. The cohort comprised 4500 singleton pregnancies presenting with RFM at or after 36 weeks' gestation and 1527 control pregnancies at a similar gestational age without RFM. Fetal biometry and Doppler parameters were recorded and converted into centiles and multiples of the median (MoM). CPR was defined as the ratio between the fetal middle cerebral artery (MCA) pulsatility index (PI) and the umbilical artery (UA) PI. Subgroup analysis for fetal size and for single vs multiple episodes of RFM was performed. Results: Compared with controls, pregnancies with RFM had lower MCA-PI MoM (median, 0.95 vs 0.97; P < 0.001) and CPR MoM (median, 0.97 vs 0.99; P = 0.018). Compared with women presenting with single episodes of RFM, pregnancies with multiple episodes (≥ 2 episodes) had lower CPR MoM (median, 0.94 vs 0.98; P = 0.003). On subgroup analysis for fetal size, compared with controls, appropriate-for-gestational-age fetuses in the RFM group had lower MCA-PI MoM (median, 0.96 vs 0.97; P = 0.003) and higher rate of CPR below the 5th centile (5.3% vs 3.6%; P = 0.015). Logistic regression analysis demonstrated an association of risk of recurrent RFM with maternal age (OR, 0.96; 95% CI, 0.93-0.99), non-Caucasian ethnicity (OR, 0.72; 95% CI, 0.53-0.97), estimated fetal weight centile (OR, 1.01; 95% CI, 1.00-1.02) and CPR MoM (OR, 0.24; 95% CI, 0.12-0.47). Conclusion: Pregnancies complicated by multiple episodes of RFM show significantly lower CPR MoM and MCA-PI MoM compared with those with single episodes and controls. This is likely to be due to worsening fetal hypoxemia in women presenting with recurrent RFM
Introducing TED Talks as a Pedagogical Resource in Sport Management Education through YouTube and LinkedIn
The COVID-19 pandemic has affected the educational sector, pushing it towards teaching-learning methodologies where the online aspect takes on special importance. In this sense, social media are tools that facilitate the creation of meaningful and sustainable learning environments. This study shares an educational experience where TED (Technology, Entertainment, Design) Talks are introduced as a pedagogical resource through YouTube and LinkedIn in a sport management course. A total of five TED Talks are viewed and discussed by the students in order to develop their professional profile, assisted by a set of initial questions posed by faculty. To assess the impact of the experience, a new scale is created and then validated. The analyses performed reflected the unidimensional nature of this scale, explaining the 64.36% of the variance, and presenting good psychometric properties (α = 0.95). Furthermore, the results obtained reflect the educational potential of TED Talks in the context of sport management, with increases in the averages of all the items from the scale, with significant (p < 0.05) increases in six of them. Both the new validated instrument and the shared pedagogical proposal can be valuable to guide and evaluate future educational experiences which introduce TED Talks as a pedagogical resource in sport management educatio
Vulnerabilidad y criminalidad: un análisis criminológico de los menores extranjeros no acompañados en España
RESUMEN: Este artículo explora los aspectos criminológicos relacionados con los Menores Extranjeros No Acompañados en España, un grupo particularmente vulnerable que enfrenta múltiples desafíos en los países de acogida. Se examinan los factores de riesgo que contribuyen a la implicación de estos menores en actividades delictivas, incluyendo la marginación social, la falta de apoyo familiar y las limitaciones en el acceso a recursos educativos y laborales. Además, se analiza el rol de las políticas migratorias y de protección infantil, destacando tanto sus deficiencias como sus potencialidades para mitigar la criminalidad entre estos menores. A través de un enfoque interdisciplinario que combina la criminología, la psicología, la sociología y el derecho, se presentan estrategias de intervención que buscan no solo prevenir la delincuencia, sino también promover la integración social y el desarrollo positivo de estos menores. Finalmente, el artículo ofrece recomendaciones para la formulación de políticas públicas que aborden de manera efectiva los problemas identificados, con el objetivo de mejorar las condiciones de vida y las oportunidades para los menores, reduciendo así su vulnerabilidad frente al crimen
Poètica del desig en l'obra de vicent andrés estellés: comparativa amb ausiàs march
El vincle entre les obres d'Ausiàs March i de Vicent Andrés Estellés és una de les constants que ha caracteritzat la poètica i l'estudi sobretot del darrer, perquè permet entendre les particularitats de la creació literària estellesiana. En primer lloc, l'article analitza la presència de March en els textos d'Estellés des del context històric i de les dades textuals, alhora que revisa la diversitat d'investigacions que han tractat aquesta relació entre dos referents de la poesia catalana d'èpoques diverses. En segon lloc, des d'una perspectiva comparada, es presenta com apareix el desig en les respectives obres i autors, però sobretot en l'obra d'Estellés. El desig en l'obra d'Ausiàs March es vincula a una pugna entre l'amor eròtic, carnal, i l'amor espiritual, d'elevació vers una puresa intel·lectual, pròpia del neoplatonisme que havia modificat els paràmetres del desig inicial ¿mutu, recíproc¿ de l'amor cortés medieval. En principi, en el cas d'Estellés, el desig s'associa a la impossibilitat, a l'ambigüitat i, fins i tot, a una força violenta, com s'exemplifica tot revisant alguns poemes representatius de la seua obra, com ara Llibre de meravelles, estretament vinculat a Ausiàs March i a altres referents medievals. Hi ha elements contemporanis en la seua poesia com el cinema, el context de repressió social i sexual, la consciència de les dificultats del llenguatge per a l'expressió poètica o la relació amb els espais comuns de la ciutat de València, que constitueixen ara el marc en què es desenvolupa la poesia d'Estellés. Gilles Deleuze o Marta Segarra han analitzat de manera especial com el desig esdevé un motor de la literatura, investigacions que fem servir a l'article com a referents teòrics. Arran del treball, es mostra, per tant, un jo poètic estellesià en crisi, en tensió pel desig de l'alteritat propi del subjecte contemporani. Així mateix, es conclou que la relació de March amb Estellés es dona també arran de l'espai compartit, que forma part del rizoma de desig que constitueix la poesia estellesiana sobre la matèria eroticoamorosa: un objecte concret que constitueix un tot que es desitja més enllà del propi objecte fins i tot. Aquest itinerari de desig esdevé també nacional, tal com prendrà cos en l'obra marcada per la voluntat de canvi col·lectiu que és Mural del País Valencià. English The presence of Ausiàs March in Estellés' poems is analyzed in the historical context and the textual data. From a comparative perspective, we can see how desire appears in the respective works, especially in Estellés. Desire in March's poems represents a struggle between erotic love and spiritual love. In the case of Estellés, desire is associated with impossibility, ambiguity and a violent force. Contemporary elements such as cinema, the context of social and sexual repression, the awareness of the difficulties of language for poetic expression or the relationship with the spaces of the city of Valencia, constitute the framework in which the poetic desire develops in Estellés. The Estellesian poetic self is shown to be in crisis, in tension due to the desire for alterity typical of the contemporary subject. March's relationship with Estellés is also based on the shared space, part of the rhizome of desire that constitutes Estellés' poetry. The itinerary of desire also becomes national, as in the work marked by the desire for collective change that is Mural del País Valencià
Experimental Study following Inactive Implantation of an Auditory Brain Stem Implant in Nonhuman Primates
We report changes in the cochlear nuclei (CNs) after 3 months of bilateral auditory deafferentation and simultaneous unilateral implantation of a dummy auditory brain stem implant (ABI) in 6 nonhuman primates (Macaca fascicularis). These specimens were compared to CNs of 9 controls and 7 bilaterally deafferented animals without implantation. The ABI array consists of 3 platinum electrodes mounted on a silicone pad with the back side covered with Dacron. No migration of the ABI was observed. All deafferented animals showed astrocytic reorganization in the CNs. Histologic changes consisted of superficial reactions around the implant, with formation of fibrillar bundles of fusiform cells, and the presence of giant cells close to the Dacron. Other findings were related to surgical trauma. The dummy ABI did not itself provoke serious adverse reactions in the CNs. Our observations support the possibility of ABI reimplantation surgery
Agreement in the assessment of metastatic spine disease using scoring systems
Purpose: To assess variability in the use of Tomita and modified Bauer scores in spine metastases. Materials and methods: Clinical data and imaging from 90 patients with biopsy-proven spinal metastases, were provided to 83 specialists from 44 hospitals. Spinal levels involved and the Tomita and modified Bauer scores for each case were determined twice by each clinician, with a minimum of 6-week interval. Clinicians were blinded to every evaluation. Kappa statistic was used to assess intra and inter-observer agreement. Subgroup analyses were performed according to clinicians' specialty (medical oncology, neurosurgery, radiology, orthopedic surgery and radiation oncology), years of experience (⩽7, 8-13, ⩾14), and type of hospital (four levels). Results: For metastases identification, intra-observer agreement was 'substantial' (0.600.80) at the other levels. Inter-observer agreement was 'almost perfect' at lumbar spine, and 'substantial' at the other levels. Intra-observer agreement for the Tomita and Bauer scores was almost perfect. Inter-observer agreement was almost perfect for the Tomita score and substantial for the Bauer one. Results were similar across specialties, years of experience and type of hospital. Conclusion: Agreement in the assessment of metastatic spine disease is high. These scoring systems can improve communication among clinicians involved in oncology care
Impact of solutions and storage time on the chemical and mechanical properties of human dentin
Considering the importance of standardization of the pH of control solutions and its impact on the chemical composition and mechanical properties of dentin during storage over a specified period, this study aimed to analyze the pH of control solutions and how it affects the chemical composition and mechanical properties of dentin stored over a given period.Six control solutions—coconut water, mineral water, distilled water, deionized water, artificial tears, and saline—were kept in a bacteriological incubator at 37°C, with their pH measured using a pH meter (mPA-210 from MS Tecnopon ®) for seven days: T0 (initial), T1 (2 hours), T2 (24 hours), T3 (48 hours), and T4 (7 days). In the second phase, the two solutions with the most stable pH in the first phase were selected and aligned with the critical pH of dentin (pH ≥ 6.5). Human third molars were sectioned and divided into two groups (n=13): distilled water and deionized water. The pH of the solutions, the chemical composition, and the microhardness of the dentin were evaluated by pH meter, Fourier transform infrared spectroscopy (FTIR), and Knoop microhardness (KH), respectively, at the aforementioned time points. The values were analyzed by two-way ANOVA (storage time and solution), followed by the Tukey test.Both solutions presented pH incompatible with the dentin until T2, and the samples presented a 40% reduction in microhardness at T4. Additionally, a reduction in carbonate and an increase in amides were observed in the dentin, indicating changes in the mineral and organic phases.It is concluded that both the solutions and the storage time negatively affect the chemical composition and microhardness of the dentin, highlighting the importance of carefully selecting the control solutions and the storage time in in vitro studies
What makes female commuters 'unhappy'? Harassment, fear of crime, and unsought travel behavioral adaptations in public transport against life satisfaction
Introduction While promoting public transport is undoubtedly essential to meet current sustainability goals, numerous challenges persist among daily travelers, either remaining active or receiving limited attention. A prime example is the exposure to sexual harassment, fear, and forced travel behavioral adaptations primarily affecting women, especially daily commuters constantly exposed to these issues. Moreover, the extent to which it affects their life satisfaction remains somewhat unaddressed in the literature. Aim The core aim of this study was to assess the influence of sexual harassment experiences, fear of crime, and unwanted travel behavior adaptations on the life satisfaction of female commuters using public transport. Methods This cross-sectional study used the data provided by 720 public transport commuting women in Spanish cities, aged M = 29 years, spending about M = 74 min a day commuting. Participants responded to an electronic questionnaire addressing everyday traveling issues. Results The relationships among experiences of sexual harassment, fear of crime, and responsive patterns of travel behavioral change were consistently positive and significant. Moreover, behavioral adaptations were more prevalent (although not exclusively) among women 'in touch' with sexual harassment on the move. Furthermore, structural analyses suggest that life satisfaction rates are impaired to a great extent by harassment experiences and fear of non-sexual crime but through the full statistical mediation of the often inconvenient, undesired travel adaptations made by commuting women. Conclusion These outcomes support the idea that the overall life satisfaction of female daily public transport travelers may be significantly affected by negative commuting experiences and inconvenient responsive behavioral demands. At a practical level, the current study provides valuable insights into the still challenging dynamics involving sexual harassment, fear, and commuting constraints in women. On this basis, conducting contextually aware crime and fear-facing actions might help to improve their travel experiences, overall quality of life, and access to opportunitie

Influence of the hardness of insoles on the abductor hallucis muscle in Baxter's entrapment. A cross-over randomized study
Background: The etiology and diagnosis of heel pain are complex and multifactorial, and it has been reported that Baxter´s entrapment is responsible for up to 20% of foot disorders. The most conservative treatment of Baxter's entrapment has been the use of custom insoles. Electromyography was considered an effective test to assess muscle activity. The aim of this study was to test the use of insoles of different hardness on muscle activity of the abductor hallucis muscle in subjects with entrapment of the lateral branch of the external plantar nerve. Methods: 18 subjects (7 women and 11 men) diagnosed with nerve entrapment of the first branch of the lateral plantar nerve were recruited. Muscle activity of the adductor hallucis muscle was analyzed with insoles of different hardness in static and dynamic situations using electromyographic evaluation
