A relação entre política interna e política externa: o caso alemão 2017-2023
Versão corrigida e melhorada após a sua defesa públicaA política interna e a política externa estão intimamente ligadas, exigindo uma maior coordenação nas coligações em democracias parlamentares. Angela Merkel da CDU, esteve 16 anos à frente do executivo alemão, mas nem sempre sem controvérsia ou desalinhamento interno. A mudança de governo em 2021 com Olaf Scholz do SPD carregava largas expectativas sobre as mudanças que iriam ser efetuadas, principalmente
com os restantes integrantes da ‘Coligação Semáforo’, a Aliança 90/Os Verdes e o FDP. O início da guerra na Ucrânia veio desconstruir todas as expectativas para o novo executivo, levando a uma alteração do paradigma da Alemanha face à política externa, desde o final da 2ª Guerra Mundial. O presente trabalho procura perceber o papel dos partidos políticos nesta transformação, no que toca à posição da Rússia nas relações externas alemãs. Este relatório foi elaborado com base nas atividades desenvolvidas na Direção-Geral dos Assuntos Europeus (DGAE) do Ministério dos Negócios Estrangeiros, através de uma detalhada revisão bibliográfica e da análise de discursos políticos, manifestos partidários e políticas adotadas e concluiu que a mudança de partidos juniores da coligação em vigor e as suas preferências histórico-programáticas tiveram um impacto

in the Directorate-General for European Affairs (DGAE) of the Ministry of Foreign Affairs, through a detailed bibliographical review and analysis of political speeches, party manifestos and adopted policies, and concluded that the change of junior parties members of the current coalition and their historical-programmatic preferences, had a significant impact on Germany's internal and external structural changes from 2017 to 2023
The potential of information technologies in airport management
O presente relatório tem por desígnio expor as tarefas desenvolvidas durante o estágio curricular, para a obtenção do grau de mestre em Ordenamento do Território e Sistemas de Informação Geográfica. O estágio curricular foi realizado na Área de Desenvolvimento do Sistema de Informação Geográfica (SIG) do departamento de Gestão de Informação (GI), parte integrante da Gestão de Ativos (GA) na Direção de Engenharia e Gestão de Ativos (DEGA) da sede da ANA - Aeroportos de Portugal, S.A., em Lisboa.
Neste documento onde se aborda a importância que as TI desempenham no apoio às atividades realizadas na gestão aeroportuária, com um foco para o desenvolvimento e implementação dos Sistemas de Informação Geográfica (SIG) na ANA, é incontestável a relevância e a contribuição crescente que estas tecnologias apresentam para a compreensão e gestão eficaz da informação técnica georreferenciada de ativos e infraestruturas. A relação espacial dos elementos que se obtém pela consulta do SIG é uma ajuda inestimável para os diversos utilizadores, permitindo a análise, discussão e monitorização de diversas situações remotamente, sobre a mesma realidade. A implementação dos SIG na ANA tem possibilitado obter uma visão geral dos aeroportos geridos pelo grupo, simplificando a sua compreensão num todo pela representação interativa num WebSIG à distância de um clique e em tempo útil à decisão. Por outro lado, permite agilizar o tempo de resposta aos parceiros de atividade e às autoridades competentes com poder regulatório do setor da aviação civil, aumentando a confiabilidade de que as normas e os regulamentos são cumpridos. O WebSIG da ANA tem alargado a sua capacidade de apoio no acesso à informação de suporte a diversas atividades, fomentando a eficiência e a segurança operacional. No âmbito do estágio foi dado apoio à gestão corrente da plataforma SIG, desenvolvendo-se conteúdos e configurações que permitissem expandir a capacidade de resposta do software às atividades da ANA. Como tal, foram realizadas tarefas de gestão, atualização e exploração de informação geográfica, bem como a criação de geometrias georreferenciadas com associação alfanumérica, contribuindo para a gestão operacional e contínua implementação da plataforma SIG na empresa, fornecendo deste modo, o apoio à tomada de decisões relativas ao território aeroportuário continental e insular português. O projeto principal consistiu na criação das bases geográficas (referencial) do sistema APMS (Airport Pavement Management System) para as unidades de negócio da ANA (aeroportos nacionais), com recurso a um software SIG open source: o QGIS.The purpose of this report is to expose the tasks developed during the curricular internship, to obtain a Master's degree in Territorial Planning and Geographic Information Systems. The curricular internship was carried out in the Geographic Information System (GIS) Development Area of the Information Management (GI) department, an integral part of Asset Management (GA) in the Engineering and Asset Management Direction (DEGA) of the headquarters of the ANA - Aeroportos de Portugal, S.A., in Lisbon. In this document, which addresses the importance that IT plays in supporting activities carried out in airport management, with a focus on the development and implementation of Geographic Information Systems (GIS) at ANA, the relevance and growing contribution that these technologies present for the understanding and effective management of georeferenced technical information on assets and infrastructures is undeniable. The spatial relationship of the elements obtained by consulting the GIS is an invaluable help for different users, allowing the analysis, discussion and monitoring of different situations remotely, on the same reality. The implementation of GIS at ANA has made it possible to obtain an overview of the airports managed by the group, simplifying their understanding as a whole through interactive representation in a WebGIS just a click away and in a timely manner for decision-making. On the other hand, it allows faster response times to activity partners and competent authorities with regulatory power in the civil aviation sector, increasing the reliability that standards and regulations are complied with. ANA's WebGIS has expanded its capacity to support access to information supporting various activities, promoting efficiency and operational security. As part of the internship, support was given to the day-to-day management of the GIS platform, developing content and configurations that allowed the software's responsiveness to ANA's activities to be expanded. As such, management, updating and exploration of geographic information tasks were carried out, as well as the creation of georeferenced geometries with alphanumeric association, contributing to the operational management and continuous implementation of the GIS platform in the company, thus providing support for taking decisions relating to the Portuguese mainland and island airport territory. The main project consisted of creating the geographic bases (reference) of the APMS (Airport Pavement Management System) system for ANA's business units (national airports), using open source GIS software: QGIS

Searching for financial independence through the digital subscription model. Case study: increasing loyalty of Expresso’s readers and subscribers
As receitas de publicidade não são suficientes para o setor da imprensa ter um negócio sustentável na era digital. Ainda assim, os jornais devem ser lucrativos, para garantir a sua independência editorial. De forma a contribuir para a sustentabilidade financeira do jornalismo, este estudo propôs-se aprofundar o modelo de negócio baseado nas assinaturas digitais. Para este modelo funcionar, é necessário ter em consideração três fases do processo: captar novos assinantes, reter os atuais e recuperar ex-assinantes. Estas fases requerem estratégias de atuação diferentes, mas têm em comum um fator: a fidelização ao jornal. Assim, através de um inquérito com 6110 respostas, de 28 de outubro a 14 de novembro 2023, esta investigação identifica as melhores estratégias de captação, retenção e recuperação de assinantes do jornal Expresso, para atingir a eficácia do modelo de assinaturas. Os resultados demonstram que introduzir os leitores ao universo de conteúdos do Expresso, transmitir o value for money inerente à subscrição, manter a qualidade dos conteúdos exclusivos, criar uma relação de proximidade à redação e comunicar de forma eficaz as melhorias e iniciativas do jornal são essenciais para a captação e fidelização de assinantes, evitando o abandono da assinatura. Além disto, entrevistas a especialistas nos temas editoriais e de subscrição digital sugerem que é fundamental estimular o consumo do website e da app, através de push notifications, newsletters e podcasts regulares, que criam habituação. Por fim, mais de 90% dos inquiridos concorda que, para conseguir produzir jornalismo de qualidade, o jornal Expresso tem de ser financeiramente sustentável. Esta consciência generalizada é uma base sólida para apostar na consciencialização nos leitores de que o acesso pago à informação é uma garantia da qualidade e independência da informação, e consequentemente, garante de uma imprensa livre e sustentável num estado democrático que assegura a liberdade de imprensa.Revenue generated by online advertising does not provide sufficient income for news businesses such as newspapers to solely rely on it. However, news independence can only be preserved if news businesses are able to be self-sufficient. This study explores ways of maintaining newspapers’ financial independence through increasing online membership. In order to do so, it is necessary to understand: how to attract new subscribers, how to retain current subscribers, and how to win former subscribers back. While different strategies are needed to increase, retain or win back subscribers, these all share a common element: finding strategies to foster loyalty. This study analyses the responses of 6,110 participants to an online survey on Expresso’s digital newspaper, from de 28th of October to 14th of November 2023, in order to identify the best strategies to attract, retain and win back online subscribers to the Expresso newspaper. Responses indicate that the following would all contribute to increasing members and member loyalty, thus reducing churn: showing prospective members the breadth and variety of exclusive online content; communicating more clearly on value for money; maintaining the quality of exclusive content; creating closer links with the editorial team; and communicating more efficiently on improvements and initiatives. In addition, interviews with experts on editorial and digital subscriptions realms suggest that it is essential to make greater use of the Expresso website and app, through push notifications, newsletters and regular podcasts, as these all contribute to regular member engagement.
Lastly, 90% of respondents agree that, in order to produce quality news reporting, the Expresso newspaper must be financially independent. This generally accepted fact creates a solid starting point to convince readers and members that paid membership is a guarantee of news quality and independence, and therefore that paid memberships contribute to maintaining freedom of press
Metalloenzymes in microbial defence mechanisms against oxygen and nitric oxide
"Oxygen, nitric oxide and their derived species play key roles in life.
Whereas O2 is essential in many organisms to survive, others use NO as
defence mechanisms against pathogens. For example, the human
immune system releases NO and H2O2, leading to the inhibition of
multiple metabolic microbial processes. Flavodiiron proteins (FDPs)
constitute a widespread family of enzymes present in all life domains
with a crucial role in O2/ROS and/or NO detoxification, through the
reduction of these species either to H2O or N2O, respectively. Therefore,
flavodiiron proteins are key players in host-microbe interactions, from
a host-pathogen strife viewpoint, and for survival of the human

pathogens, in special Clostridia, the phylogenetic group with the larger

Developing orality in the teaching and learning of PLNM in middle and secondary school: activities, challenges and strategies
O presente relatório surge na sequência da realização de um estágio profissional no Agrupamento de Escolas do Restelo no âmbito da componente não letiva do Mestrado em Português como Língua Segunda e Estrangeira da Universidade NOVA de Lisboa. O trabalho apresenta o percurso da estagiária na instituição de ensino de acolhimento em prol da análise, elaboração, planificação e implementação de materiais e atividades
didáticas orientadas para o desenvolvimento da competência comunicativa e da oralidade no ensino-aprendizagem de PLNM no 3.º ciclo do ensino básico e no ensino secundário. Fundamentou-se, essencialmente, na averiguação do contexto do ensino-aprendizagem dos aprendentes de PLNM, do seu perfil cultural e sociolinguístico e das suas necessidades educativas específicas; na análise dos desafios
atuais ao ensino-aprendizagem de PLNM; no desempenho de funções pedagógicas inerentes ao ensino de PLNM; e na reflexão sobre a importância da oralidade no ensino-aprendizagem de PLNM. This paper summarises the trainee's journey at the host institution with the aim of analysing, designing, planning and implementing didactic materials and activities targeted at developing communicative competence and orality in the teaching and learning of PLNM in middle and secondary education. The main grounds for this report were to ascertain the teaching and learning context of PLNM learners, their cultural and sociolinguistic profile and their specific educational needs; to analyse the current challenges facing PLNM teaching and learning; to carry out pedagogical responsibilities inherent to PLNM teaching; and to reflect on the importance of orality in PLNM teaching and learning. This report also seeks to illustrate and highlight the fact that a significant part of the current challenges facing PLNM teaching stem, on the one hand, from the
difficulty schools have in implementing the current legal framework and, on the other, from the guidelines issued by the Ministry of Education. It also advocates the adoption of Communicative Language Teaching (CLT) as a viable pedagogical methodology that can be adapted to the real communicative needs of students
Lidl Portugal: Determining factors of the digital channel to create brand loyalty
A relevância da presença nas redes sociais por parte das marcas nunca foi tão mencionada. As redes sociais são capazes de conferir inúmeras vantagens às marcas, tais como, alcançar maior número de pessoas, comunicar de forma diferente única e mais próxima com os consumidores, diferenciar dos restantes players do mercado. Questões relativas à pertinência da participação nas redes sociais enquanto bom incentivo à Lealdade de Marca, têm surgido. Para uma marca ser bem sucedida, já não basta entregar produtos e serviços de boa qualidade, tem de envolver os seus consumidores e elevar toda a experiência que os mesmos têm consigo. É preciso rodeá-los em diferentes contextos e circunstâncias, procurando obter em todos os momentos de contacto, uma emoção positiva. O setor do Retalho para além do peso gigantesco a nível mundial, no que se refere à entrega de bens e serviços é também um dos que tem sofrido maiores mudanças: ora, os consumidores atuais são muito mais informados, sabem precisamente aquilo que procuram e quando o procurar, e tendem a comprar produtos e serviços de marcas com as quais não só se reveem, como também admiram. Marcas confiáveis, transparentes, com uma imagem positiva que acrescentam muito valor no seu dia-a-dia, e que, no fundo, os conquiste vezes e vezes sem conta. Podendo assim, alcançar a sua lealdade e o que a mesma tem para oferecer, no que diz respeito a um compromisso de longo prazo. De modo a analisar esta temática foi escolhido como caso de estudo o Lidl Portugal, uma vez que se destaca pelo seu posicionamento online e offline e tem a percepção de ser uma marca que a maioria dos portugueses conhece e gosta. O objetivo da presente investigação é o de propor um modelo concetual que permita estudar o Brand Loyalty, apoiado no engagement das marcas nas redes sociais e, em simultâneo, na experiência que as marcas passam no contacto direto com os seus consumidores. Outros moderadores deste modelo sugeridos são, Brand Image, Brand Love, Brand Trust e, por último, Customer Delight. Acredita-se que, seguindo este modelo, vai ser criada uma Lealdade à Marca muito profunda e verdadeiramente diferenciadora e, claro, poderosa. Para o efeito pretendido, elaborou-se um modelo conceptual com base em revisão de literatura que
visa analisar a relação entre as diferentes variáveis em estudo. Realizou-se também um estudo empírico que se baseou nos dados recolhidos de um questionário online. Posto isto, recorreu-se a uma metodologia quantitativa, aplicada a uma amostra de 606 indivíduos. Com o intuito de analisar os dados, usufruiu-se do software de análise estatística, Smart PLS. Confirmou-se positivamente o modelo conceptual proposto, em que a maioria das hipóteses foi corroborada. Conclui-se então que as marcas devem valorizar práticas que estabeleçam a comunicação e a relação com os consumidores em diferentes canais, pois têm elevadas repercussões positivas que podem alcançar o Brand Loyalty. Este estudo representa também um contributo positivo para a comunidade científica, através do desenvolvimento de um modelo conceptual fortemente
testado e validado.The importance of brands' presence on social networks has never been more emphasised. Social networks are capable of conferring numerous advantages on brands, such as reaching a greater number of people, communicating in a unique and closer way with consumers, and differentiating themselves from other market players. Questions have arisen about the relevance of participation in social networks as a good incentive for brand loyalty. For a brand to be successful, it's no longer enough to deliver good quality products and services, it has to involve its consumers and elevate the whole experience they have with it. You have to surround them in different contexts and circumstances, trying to achieve a positive emotion at every moment of
contact. The retail sector, in addition to its huge global weight in terms of delivering goods and services, is
also one of the sectors that has undergone the greatest changes: today's consumers are much more informed, they know precisely what they are looking for and when to look for it, and they tend to buy products and services from brands that they not only identify with, but also admire. Brands that are reliable, transparent, with a positive image that add a lot of value to their day-to-day lives, and that ultimately win them over time and time again. In this way, we can achieve their loyalty and what they have to offer in terms of long-term commitment.
In order to analyse this issue, Lidl Portugal was chosen as a case study, since it stands out for its on and off positioning and has the perception of being a brand that most Portuguese know and like. The aim of this research is to propose a robust Brand Loyalty model, based on brand engagement on social networks and, at the same time, on the experience that brands have in direct contact with their consumers. Other suggested moderators of this model are Brand Image, Brand Love, Brand Trust and, finally, Customer Delight. It is believed that following this model will create very deep, truly differentiating and, of course, powerful Brand Loyalty.
To this end, a conceptual model was drawn up based on a literature review to analyse the relationship between the different variables under study. An empirical study was also carried out, based on data collected from an online questionnaire. That said, a quantitative methodology was used, applied to a sample of 606 individuals. In order to analyse the data, Smart PLS statistical analysis software was used. The proposed conceptual model was positively confirmed, with most of the hypotheses being confirmed. The conclusion is that brands should value practices that establish communication and relationships with consumers in different channels, as they have high positive repercussions that can achieve Brand Loyalty. This study also makes a positive contribution to the scientific community by developing a conceptual model that has been thoroughly tested and validated
Long-term predictors of health-related quality of life in spondyloarthritis
Introduction
Spondyloarthritis (SpA) is a group of inflammatory rheumatic diseases that share common clinical
features, genetic susceptibility, and pathophysiological mechanisms. It includes ankylosing
spondylitis, psoriatic arthritis, reactive arthritis, arthritis associated with inflammatory bowel
disease, and undifferentiated SpA. Recently, SpA has also been classified into axial SpA (axSpA)
and peripheral SpA (pSpA), according to the cardinal manifestations of the disease.
SpA is a chronic condition with multi-dimensional burden on physical, mental, social, and
economic wellbeing, affecting global functioning and health (GH) and health-related quality of
life (HRQoL). Different SpA phenotypes may variably influence GH and the physical, social, and
mental HRQoL domains. Factors such as sociodemographic, lifestyle, clinical aspects, and
treatments have been associated to GH and HRQoL in patients with SpA but exhibit contradictory
findings, making integration challenging. Comparative HRQoL research, especially against chronic
low back pain (CLBP), is vital for evidence-based decision-making and patient-centered care.
Longitudinal studies are needed to understand the long-term effects on patients’ lives and the
fluctuating nature of HRQoL tied to lifestyle factors and therapy. Understanding the SpA
phenotypes’ distinct impacts on HRQoL domains is crucial for comprehensive management
approaches and improved wellbeing.
Objectives
The primary objective of this research project was to address the burden of SpA, focusing on
HRQoL and physical function, and identify factors associated with these health outcomes, along
with their long-term predictors. We aimed to (1) assess the burden of SpA on HRQoL and physical
function as compared to the general population; (2) compare HRQoL and physical function
between individuals with SpA and individuals with CLBP at a population level; (3) identify factors
associated with HRQoL and physical function in individuals with SpA, and CLBP, including
sociodemographic characteristics, modifiable lifestyle factors and disease and health
characteristics; (4) evaluate long-term HRQoL, and to identify and compare factors associated
with HRQoL and GH within the spectrum of spondyloarthritis, including sociodemographic
characteristics, modifiable lifestyle factors, clinical characteristics, and treatment-related factors; and (5) estimate the relationship between disease activity and quality of life utilities for economic
evaluation in individuals with axial spondyloarthritis.
Methodology
Data were analyzed from four cohorts: the Epidemiology of Chronic Diseases (EpiDoC) cohort,
the Assessment of Spondyloarthritis International Society (ASAS) peripheral involvement in SpA
(perSpA) cohort, the Rheumatic Diseases Portuguese Register (Reuma.pt) cohort, and the
SPondyloArthritis Caught Early (SPACE) cohort. The main studied outcomes were HRQoL,
assessed with the EuroQuality of life five dimensions (EQ-5D) score, the Medical Outcomes Study
Short-Form 36 Health Survey (SF-36) questionnaire, or the Ankylosing Spondylitis Quality of Life
(ASQoL) score, depending on the cohort; GH, assessed with the Assessment of SpondyloArthritis
international Society Health Index (ASAS-HI); and physical function, assessed with the Health
Assessment Questionnaire Disability Index (HAQ-DI). Sociodemographic, lifestyle, clinical, and
treatment-related variables were collected across the studied cohorts.
Results
In the EpiDoC cohort, we analyzed 92 patients with SpA, 1,376 individuals suffering from CLBP,
and 679 subjects without rheumatic diseases. Our study highlights the considerable impact of
SpA on the mental, physical, and social domains of HRQoL in Portugal, providing valuable insights
from a population-based perspective. By comparing SpA to CLBP, we shed light on their similar
burden on HRQoL. Additionally, we investigated the factors associated with HRQoL in both
conditions. In SpA patients, we found that multimorbidity and higher active disease were
associated with worse HRQoL. Among CLBP patients, multimorbidity, obesity, and low back pain
intensity were associated with worse HRQoL. Notably, regular physical exercise was significantly
associated with better HRQoL in both conditions. Furthermore, we confirmed the substantial
impact of SpA on physical functioning, assessed by the HAQ-DI. In comparison to CLBP, SpA
patients reported higher levels of disability. Factors associated with physical disability were also
explored. For SpA patients, higher levels of physical disability were associated with retirement
and multimorbidity. Conversely, in CLBP patients, higher physical disability was associated with
low back pain intensity, increasing age, obesity, multimorbidity, and retirement. Interestingly,
lower physical disability in CLBP was associated with alcohol consumption and being male.
Notably, regular physical exercise was associated with reduced physical disability in both
conditions. These two studies, collectively underscore the significant impact of SpA and CLBP on various domains of HRQoL and physical functioning when compared to the general population.
Furthermore, while SpA and CLBP patients reported similar HRQoL, SpA patients exhibited higher
levels of physical disability. The identification of factors associated with HRQoL and physical
disability by clinicians, may facilitate more targeted interventions to enhance HRQoL and physical
functioning.
Given that SpA phenotypes may impact HRQoL domains and GH in distinct ways, we conducted
an analysis of associated factors within the perSpA cohort comprising 4,185 SpA patients: 2,719
with axSpA, 433 with pSpA, and 1,033 with psoriatic arthritis. In axSpA, we found that higher
disease activity, lower physical function, female sex, and the presence of fibromyalgia were all
associated with worse HRQoL. Conversely, older age and having university education were
associated with a better outcome. For pSpA patients, higher disease activity and lower physical
function were associated with worse HRQoL. In the context of psoriatic arthritis, higher disease
activity, lower physical function, axial involvement, and female sex were all associated with worse
HRQoL. Meanwhile, with regards to GH assessments for axSpA patients, we observed that higher
disease activity, lower physical function, peripheral involvement, female sex, and the presence of
fibromyalgia were all associated with worse GH. Conversely, older age and university education
were associated with better GH outcomes. For pSpA patients, lower physical function, and female
sex were associated with worse GH, while a university education was associated with better GH.
Lastly, among psoriatic arthritis patients, higher disease activity, lower physical function, axial
involvement, female sex, and the presence of fibromyalgia were associated with worse GH, while
older age and university education were associated with a better outcome. This study shows the
critical role of disease activity and physical function as major determinants of HRQoL and GH
across the different SpA phenotypes. However, it’s important to recognize that clinical
characteristics and sociodemographic factors also play an important role, highlighting the need
for personalized, patient-centered approaches.
In the Reuma.pt cohort, we conducted an analysis of HRQoL in 342 patients with psoriatic
arthritis. After a 3-year follow-up, notable improvements were observed in both the EQ-5D score
and general perception of health. Predictors of poorer HRQoL encompassed older age, female
sex, higher disease activity, exposure to several biologic disease-modifying anti-rheumatic drugs
(bDMARDs), and switching between bDMARDs. This study provides valuable insights into the
long-term HRQoL in patients of psoriatic arthritis patients, shedding light on determinants of unfavorable outcomes, such as sociodemographic factors and exposure to multiple biologic
therapies, which is often indicative of more challenging-to-treat disease.
Furthermore, we evaluated long-term HRQoL in axSpA patients, and identified key factors
influencing HRQoL. We analyzed 709 patients from the Reuma.pt cohort. Among patients who
had never undergone biologic therapy, HRQoL remained stable throughout the entire follow-up
period. Conversely, patients already on biologic therapy at baseline, experienced progressive
improvement in HRQoL over the 3-year study duration. Predictors of poorer HRQoL included
advancing age, active disease, lower physical function, and symptoms of anxiety or depression.
Interestingly, patients with blue-collar jobs, but not those with white-collar jobs, reported worse
HRQoL compared to non-employed patients. Married patients exhibited better HRQoL. This study
provides insights into the long-term HRQoL of axSpA patients, shedding light on factors
contributing to poor outcomes, such as increasing age, blue-collar job type, and the presence of
anxiety and depressive symptoms.
In the study to estimate the relationship (“mapping”) between the Ankylosing Spondylitis Disease
Activity Index (ASDAS) instrument and the EQ-5D, we analyzed 5,000 observations from axSpA
patients in the Reuma.pt cohort. Subsequently, we tested the proposed model using 400
observations from the SPACE cohort. This study demonstrates the suitability of ASDAS as a
reliable tool for estimating quality of life, measured by the EQ-5D utility score, in clinical studies
involving individuals with axSpA.
Conclusions
Our analysis highlights the cumulative impact of CLBP on HRQoL and physical functioning, as well
as the influence of SpA on HRQoL, GH, and physical functioning. This research, enhances our
understanding of the disease burden across diverse HRQoL domains, as well as its impact on GH,
throughout the entire spectrum of SpA. This knowledge offers valuable guidance for development
of more targeted interventions (political, clinical) addressing modifiable factors. Through the
management of these factors, clinicians can improve wellbeing and enhance overall HRQoL and
GH by tailoring interventions applied to the various SpA phenotypes.
Lastly, grounded in the data we have gathered in our research, we present a stratified model for
health outcomes applicable to the entire spectrum of SpA, which can be subject to validation in
future studies.Introdução
As Espondilartrites (SpA), são um grupo de doenças reumáticas inflamatórias que partilham
características clínicas, suscetibilidade genética e mecanismos fisiopatológicos. Dentro deste
grupo incluem-se a espondilite anquilosante, a artrite psoriásica (AP), a artrite reativa, a artrite
associada à doença inflamatória intestinal e a SpA indiferenciada. Mais recentemente, as SpA
classificam-se em axial (axSpA) e periférica (pSpA), de acordo com o quadro clínico predominante.
As SpA são doenças crónicas que condicionam durante a sua evolução um impacto significativo a
nível do bem-estar físico, mental, social e económico, afetando a funcionalidade global e saúde
(SG) e a qualidade de vida relacionada com a saúde (QVRS). Os diferentes fenótipos de SpA
podem afetar de modo diferente a SG e as dimensões física, mental e social da QVRS. Fatores
sociodemográficos, de estilo de vida, aspetos clínicos e de tratamentos têm sido associados à SG
e à QVRS nos doentes com SpA, mas os resultados são contraditórios fazendo com que a sua
integração constitua um desafio na prática clínica. Estudos comparativos de QVRS,
nomeadamente com a lombalgia crónica, são essenciais para tomadas de decisão baseadas na
evidência e cuidados centrados no doente. Também os estudos longitudinais são importantes
para um melhor conhecimento do impacto a longo prazo na vida dos doentes e da flutuação
natural da QVRS ligada a aspetos de estilo de vida e da terapêutica. Para além disso,
compreensão do impacto dos diferentes fenótipos das SpA nas diversas dimensões da QVRS é
essencial para formular abordagens mais compreensivas e melhorar o bem-estar global.
Objetivos
O objetivo principal deste projeto foi abordar o impacto das SpA, focando a QVRS e a função
física, e identificar fatores associados com estes outcomes de saúde, bem como dos seus
preditores a longo-prazo. Especificamente, os objetivos foram: 1) Avaliar o impacto das SpA na
QVRS e identificar os preditores de QVRS a longo-prazo; 2) Comparar a QVRS e a função física,
entre indivíduos com SpA e indivíduos com lombalgia crónica a nível populacional; 3) Identificar
fatores associados á QVRS e à incapacidade física em indivíduos com SpA e em indivíduos com
lombalgia crónica, incluindo características sociodemográficas, fatores modificáveis de estilo de
vida e caraterísticas de saúde; 4) Avaliar a QVRS a longo-prazo e identificar fatores associados à QVRS e à SG em todo o espectro das SpA, incluindo características sociodemográficas, fatores
modificáveis de estilo de vida, caraterísticas da doença e relacionadas com o tratamento; e 5)
Estimar a relação entre a atividade da doença e as utilidades de qualidade de vida para avaliação
económica em indivíduos com axSpA.
Metodologia
Foram analisados dados de quatro coortes: a coorte Epidemiologia de Doenças Crónicas
(EpiDoC); a coorte Assessment of SpondyloArthritis international Society (ASAS) peripheral
involvement in SpA (perSpA); a coorte do Registo Português de Doenças Reumáticas (Reuma.pt);
e a coorte SPondyloArthritis Caught Early (SPACE). Os principais outcomes estudados foram: a
QVRS, avaliada pelo questionário EuroQuality of life five dimensions (EQ-5D), pelo questionário
Medical Outcomes Study Short-Form 36 Health Survey (SF-36) ou pelo questionário Ankylosing
Spondylitis Quality of Life (ASQoL), consoante a coorte em estudo; a SG, avaliada pelo Assessment
of SpondyloArthritis international Society Health Index (ASAS-HI); e a função física, avaliada pelo
Health Assessment Questionnaire Disability Index (HAQ-DI). Dados sociodemográficos, fatores
modificáveis de estilo de vida, variáveis clínicas e relacionadas com o tratamento foram
analisadas nas coortes estudadas.
Resultados
Na coorte EpiDoC, analisámos 92 indivíduos com SpA, 1376 indivíduos com lombalgia crónica e
679 indivíduos sem doenças reumáticas. Destacámos o impacto significativo das SpA nos
domínios físico, mental e social da QVRS em Portugal, de base populacional. Ao comparar os
indivíduos com SpA e com lombalgia crónica, demonstrámos um impacto semelhante na QVRS.
Explorámos os fatores associados à QVRS nas duas patologias. Nos indivíduos com SpA, a
presença de multimorbilidade e a atividade da doença associaram-se a uma pior QVRS. Nos
indivíduos com lombalgia crónica, uma pior QVRS associou-se com a intensidade da lombalgia, a
presença de multimorbilidade e de obesidade. Pelo contrário, o exercício físico regular associouse
a uma melhor QVRS nas duas patologias. Para além disso, demonstrámos o impacto
substancial das SpA na função física, avaliada pelo HAQ-DI. Em comparação com a lombalgia
crónica, os indivíduos com SpA reportaram uma maior incapacidade física. Os fatores associados
a maior incapacidade física foram explorados. Nos indivíduos com SpA, uma maior incapacidade
física associou-se com a reforma e a presença de multimorbilidade. Nos indivíduos com lombalgia
crónica uma maior incapacidade física associou-se com a intensidade da lombalgia, a idade mais avançada, a reforma, a obesidade e a multimorbilidade. Pelo contrário, uma melhor função física
associou-se ao sexo masculino e ao consumo regular de álcool. O exercício físico regular associouse
a uma melhor função física nas duas patologias. No seu conjunto, estes dois estudos
demonstram o impacto significativo das SpA e da lombalgia crónica nas várias dimensões da QVRS
e na função física, em comparação com a população em geral. Para além disso, os indivíduos com
SpA e com lombalgia crónica reportam uma QVRS semelhante, embora os indivíduos com SpA
apresentem uma maior incapacidade física. A identificação pelos médicos dos fatores associados
à QVRS e à função física, pode facilitar o planeamento de intervenções mais direcionadas para
melhorar a QVRS e a função física.
Dado que os fenótipos das SpA podem ter impactos diferentes nas várias dimensões da QVRS,
analisámos os fatores associados à QVRS em 4,185 doentes com SpA da coorte perSpA: 2719 com
axSpA, 433 com pSpA e 1033 com AP. Na axSpA, o sexo feminino, a maior atividade da doença, a
maior incapacidade física e a presença de fibromialgia associaram-se a pior QVRS. A idade e a
educação universitária a melhor QVRS. Nos doentes com pSpA, a maior atividade da doença e a
maior incapacidade física associaram-se a pior QVRS. No contexto da AP, o sexo feminino, a maior
atividade da doença, a maior incapacidade física e o envolvimento axial associaram-se a pior
QVRS. Na mesma coorte também analisámos os fatores associados à SG. Na axSpA, o sexo
feminino, a maior atividade da doença, a maior incapacidade física, o envolvimento periférico e
a presença de fibromialgia associaram-se a pior SG. A idade e a educação universitária a melhor
SG. Nos doentes com pSpA, o sexo feminino e a maior incapacidade física associaram-se a pior
SG. A educação universitária associou-se a melhor SG. No contexto da AP, o sexo feminino, a
maior atividade da doença, a maior incapacidade física, o envolvimento axial e a presença de
fibromialgia associaram-se a pior SG. A idade e a educação universitária associaram-se a melhor
SG. Com este estudo evidenciámos a importância da atividade da doença e da função física como
determinantes major para a QVRS e para a SG, nos diferentes fenótipos das SpA. No entanto,
destacámos que os fatores sociodemográficos e as características clínicas, devem ser tidos em
conta para uma abordagem individualizada e centrada no doente.
Na coorte do Reuma.pt, analisámos 342 doentes com AP. Observámos uma melhoria significativa,
tanto do EQ-5D como da perceção individual de saúde, ao longo de um período de três anos. Os
fatores preditores de pior QVRS incluíram a idade mais avançada, o sexo feminino, a maior
atividade da doença e a exposição a múltiplas terapêuticas biológicas ou ao switch destas terapêuticas Com este estudo, aumentámos o conhecimento sobre a QVRS a longo-prazo dos
doentes com AP e destacámos os fatores que contribuem para uma pior QVRS tais como a
influências dos aspetos sociodemográficos e a exposição a múltiplas terapêuticas biológicas,
muitas vezes indicativo de uma doença mais refratária ao tratamento .
Avaliámos também a QVRS a longo-prazo em doentes com axSpA. Analisámos 709 doentes com
axSpA da coorte Reuma.pt. Nos doentes que nunca efetuaram terapêutica biológica, a QVRS
manteve-se estável ao longo dos 3 anos de follow-up. Por outro lado, nos doentes que já se
encontravam sob terapêutica biológica, observou-se uma melhoria progressiva da QVRS. Os
fatores associados a pior QVRS incluíram uma idade mais avançada, a existência de doença ativa,
uma pior função física, e a existência de sintomas de ansiedade ou depressão. Os doentes com
profissões de “colarinho-azul”, reportaram pior QVRS em comparação com os doentes
reformados. Pelo contrário, os doentes casados exibiram uma melhor QVRS. Com este estudo,
obtivemos informação relevante sobre a QVRS a longo-prazo dos doentes com axSpA.
Destacámos alguns fatores que contribuem para QVRS, tais como a idade mais avançada,
profissões com maior sobrecarga física, e a presença de sintomas de ansiedade e depressão.
Para estimar a relação (mapping) entre o Ankylosing Spondylitis Disease Actitvy Index (ASDAS) e
o EQ-5D, analisámos 5,000 observações de doentes com axSpA da coorte Reuma.pt. O modelo
foi depois testado em 400 observações de doentes com axSpA da coorte SPACE. Com este estudo,
demonstrámos que o ASDAS constitui uma ferramenta adequada para estimar a qualidade de
vida avaliada pelo EQ-5D, em estudos clínicos que envolvem doentes com axSpA.
Conclusões
O nosso trabalho destaca o impacto da lombalgia crónica na QVRS e na função física, bem como
o impacto das SpA na QVRS, na SG e na função física. A nossa investigação aumentou
significativamente o conhecimento sobre impacto das SpA nas diferentes dimensões da QVRS e
da SG, em todo o espectro das SpA. A identificação e abordagem dos fatores modificáveis
identificados constitui uma orientação para o desenvolvimento de intervenções mais
direcionadas contribuindo para melhorar o bem-estar geral, a QVRS e a SG, através de
intervenções mais individualizadas e aplicáveis aos diferentes fenótipos das SpA.
Por fim, com base nos dados obtidos na nossa investigação, propomos um modelo estratificado
de outcomes em saúde, aplicável a todos os doentes com SpA, que poderá ser validado em
estudos futuros
The influence of the internet and digital media on television production and consumption: the case of Porto
Os meios digitais na produção e consumo da televisão, com foco no caso do Porto Canal. A ascensão da Internet e das tecnologias digitais transformou a paisagem mediática e impactou tanto a produção quanto o consumo de conteúdo televisivo. O Porto Canal, uma estação de televisão regional portuguesa, serve como estudo de caso relevante para compreender essas mudanças e as respetivas implicações. Numa era marcada pela
revolução digital, é necessário que as emissoras televisivas apostem na inovação através das oportunidades das plataformas online para fazer frente aos desafios que este cenário impõe. Deste modo, o propósito da reflexão é analisar as alterações que surgiram com a passagem para a era digital, bem como a forma como o Porto Canal está a inovar nos produtos, formatos e conteúdos digitais. Pretende-se ainda entender que estratégias estão a ser utilizadas para que o Porto Canal se adapte aos desafios da televisão digital, especificamente ao nível da comunicação com o público, e se as estratégias de inovação estão a ser bem-sucedidas. Para tal, a observação participante é complementada com resultados obtidos durante o período de estágio e com entrevistas a profissionais da Área de Planeamento e Estratégia Digital do Porto Canal. Ainda que o Porto Canal não seja um nativo digital, os resultados demonstram um esforço na exploração das
potencialidades oferecidas pelo ambiente digital.This internship report proposes a reflection on the influence of the Internet and digital media on television production and consumption, focusing on the case of Porto Canal. The rise of the Internet and digital technologies has transformed the media landscape and impacted both the production and consumption of television content. Porto Canal, a Portuguese regional television station, serves as a relevant case study for understanding these changes and their implications. In an era marked by the digital revolution, it is necessary for broadcasters to invest in innovation through the opportunities of online platforms in order to face the challenges that this scenario imposes. The purpose of this reflection is therefore to analyze the changes that have arisen with the transition to the digital age, as well as how Porto Canal is innovating in digital products, formats and content. It is also intended to understand what strategies are being used for Porto Canal to adapt to the challenges of digital television, specifically in terms of communication with the public, and whether the
innovation strategies are being successful. To this end, participant observation is complemented with results obtained during the internship period and interviews with professionals from Porto Canal's Digital Planning and Strategy Area. Although Porto Canal is not a digital native, the results demonstrate an effort to exploit the potential offered by the digital environment
Otimização de Recursos de Laboratório – Implementação de Novas Ferramentas e Métodos Analíticos Recorrendo a Técnicas Analíticas como HPLC e GC
Esta dissertação aborda a necessidade da Sovena Portugal Consumer Goods S.A. de laboratórios externos para análises fundamentais de controlo de qualidade dos seus produtos (óleos vegetais e azeites), resultando em custos elevados e atrasos na produção. O objetivo é avaliar a viabilidade da internalização destas análises para otimizar a gestão de recursos, reduzir custos e aumentar a eficiência operacional, apesar dos desafios técnicos relacionados à adaptação e validação de novos métodos laboratoriais.
A solução proposta envolve a reavaliação dos equipamentos e métodos analíticos disponíveis no laboratório de controlo de qualidade da unidade fabril da Sovena Barreiro. Propõe-se a utilização de equipamentos subutilizados, como o HPLC para a quantificação de hidrocarbonetos aromáticos policíclicos (PAH), o refratómetro na refinaria para diferenciação do tipo de óleo nos set-ups, e a cromatografia gasosa (GC) para a quantificação de ceras. Tanto o HPLC como o refratómetro foram testados com sucesso, confirmando a sua eficiência. Contudo, devido ao atraso na entrega do padrão interno (araquilato de laurilo), não foi possível quantificar as ceras; o método está, no entanto, pronto para implementação assim que o padrão chegar.
Os resultados mostram que a utilização do HPLC para quantificação de PAH pode reduzir os custos em 32,6%, enquanto o uso de GC para quantificação de ceras, ainda que não totalmente otimizado, já representa uma redução de 51,3%. A adoção destas propostas maximiza a utilização dos recursos internos, reduz custos operacionais e aumenta a autonomia no controlo de qualidade, fortalecendo a competitividade da Sovena ao garantir uma resposta mais ágil ao mercado e a consistência na qualidade dos produtos.This dissertation addresses Sovena Portugal Consumer Goods S.A.'s reliance on external laboratories for fundamental quality control analyses of its products (vegetable oils and olive oils), resulting in high costs and production delays. The objective is to assess the feasibility of internalizing these analyses to optimize resource management, reduce costs, and increase operational efficiency, despite the technical challenges related to the adaptation and validation of new laboratory methods.
The proposed solution involves re-evaluating the equipment and analytical methods available in the quality control laboratory at the Sovena Barreiro production unit. It suggests the use of underutilized equipment, such as HPLC for the quantification of polycyclic aromatic hydrocarbons (PAH), the refractometer in the refinery for differentiating oil types during set-ups, and gas chromatography (GC) for the quantification of waxes. Both the HPLC and refractometer were successfully tested, confirming their efficiency. However, due to a delay in the delivery of the internal standard (lauryl arachidonate), it was not possible to quantify the waxes; nevertheless, the method is ready for implementation as soon as the standard arrives.
The results show that using HPLC for PAH quantification can reduce costs by 32.6%, while the use of GC for wax quantification, although not fully optimized, already represents a 51.3% reduction. The adoption of these proposals maximizes the use of internal resources, reduces operational costs, and increases autonomy in quality control, strengthening Sovena's competitiveness by ensuring a more agile response to the market and consistency in product quality
