    THE PROCESS OF BUILDING AND IMPLEMENTING A QUALITY-INNOVATION MODEL

    This paper describes the construction and application of a relationship model between the quality and innovation variables in the context of the agricultural machinery industry in Mexico. The model allows the degree of maturity of each of the variables to be weighted, assigning a value to each component and element to analyze their relationship. Thus, the construction process is presented in four stages and its application in an agricultural machinery manufacturing company located in Mexico. It shows that quality positively but indirectly affects innovation, through the strengthening of the intermediate variable that drives innovation. Furthermore, results suggest that quality promotes incremental rather than radical innovation, by driving rational innovation, in which companies can ensure commercial viability. Finally, the limitations of the work are exposed in the work and its validation through the application in other industries and contexts is suggested

    Structural challenges in the search for mental health services among military police officers: an integrative review

    Introduction: Military police officers daily face stressful and potentially traumatic situations, resulting in a high prevalence of occupational stress and mental comorbidities within this group. Despite this, the pursuit of mental health services by these professionals encounters structural challenges that are still insufficiently discussed. Objective: To determine the factors that most influence the search for, provision of, and utilization of mental health services by police officers through an integrative literature review. Materials and Methods: The search for articles was conducted using the following databases: Virtual Health Library (VHL), MEDLINE, Scopus, and Embase. After the selection process, 23 articles were included for analysis in this review. Results: The main findings indicate that social stigma, lack of knowledge on the subject, and organizational deficiencies are critical aspects that hinder the pursuit of psychological well-being services in the police field.Discussion: Despite the many barriers to police officers seeking mental health care, it was highlighted that family support and professional development are positively related to help-seeking behavior. Conclusion: Further research is needed to evaluate mental health programs and better understand the difficulties in implementing these services. Promoting the search for psychological help is essential to improving police officers' health and population's safety

    Perception of the ethical climate among healthcare professionals in an emergency room from southern Brazil

    Introduction: The ethical climate is defined as the shared perception among healthcare professionals of what is ethically correct behavior and how to deal with ethical issues.Objective: To evaluate the perception of the ethical climate among health professionals working in an emergency room. Materials and Methods: Cross-sectional study carried out with healthcare professionals working in the emergency room of a university hospital in southern Brazil. Collection took place in January 2024 through the Positive Ethical Climate Promotion Platform, through the Hospital Ethical Climate Survey-Brazilian Version instrument. The data was organized and analyzed on the Positive Ethical Climate Promotion Platform, applying descriptive statistics. Results: 71 health professionals participated in the research, in which the general ethical climate was classified as positive (M=3.73; SD=0.60). The factors were evaluated as positive, except for the medical factor (M=3.23; SD=1.16), classified as negative. Descriptively, female professionals (p=0.063), with a partner (p=0.508), aged over 42 years (p=0.047) and with training time over 17 years (p=0.072) presented high averages for positive ethical climate, however, only age showed a significant association.Discussion: Despite the positive assessment of the general ethical climate, the importance of self-reflection and self-awareness when making ethical decisions in care is understood.     Anxiety, depression, and quality of life of caregivers of palliative care patients with cancer

    Introduction: Palliative care has recently gained importance in the context of life-threatening diseases, such as cancer, that affect the mental health of patients and their caregivers. Psychological symptoms, such as anxiety and depression, are the most prevalent in this population. Objective: To determine the association between anxiety, depression, and quality of life of caregivers of palliative care patients with cancer. Materials and Methods: A quantitative, descriptive, and correlational design was used. A total of 190 primary caregivers recruited from Colombian hospitals and home care programs participated. The Quality of Life in Life-Threatening Illness-Family Carer (QOLLTI-FT) questionnaire, the Beck Depression Inventory-II (BDI-II), and the Beck Anxiety Inventory (BAI) were used. The analysis was performed with SPSS Statistics 26.0, using descriptive and inferential statistics.Results: The predominant sex was female, and the level of education was high school. The mean age of the caregivers was 44.15 years, and the mean age of the patients was 64.51 years. The average time as a caregiver was 33.79 ± 64.77 months. The results show correlations between anxiety and caregiver status (p≤0.000), anxiety and Information and Communication Technology (ICT) use (p≤0.006). There were also correlations between anxiety, quality of life, and caregiver status (p≤0.000) and between depression and total quality of life (p≤0.001).Discussion: Correlations were also found between depression and quality of life and between hours of daily care and the level of dependency of the cancer patient. This entails the need to manage the psychological symptoms as soon as the family member is diagnosed to prevent alterations in their quality of life that could affect their well-being. Conclusion: Caregivers with moderate or severe depression were more likely to report symptoms of sadness, punishment feelings, self-dislike, suicidal thoughts or wishes, indecisiveness, irritability, changes in appetite, concentration difficulty, and tiredness or fatigue. Among caregivers with mild depression, loss of interest in sex, agitation, and past failure were identified. Strategies for psycho-emotional counseling, education, and support for caregivers are needed

    La Sociedad Americana del Cáncer informa que el cáncer de mama es la neoplasia maligna más común y la principal causa de muerte por cáncer en mujeres. Utilizando Scopus, PubMed, ResearchGate y Google Scholar, entre otras bases de datos prestigiosas, recopilé varias publicaciones sobre cáncer de mama, algunas originales y otras de revisión. No se incluyeron fotografías clínicas, informes de casos ni series de casos. Las estrategias cruciales de manejo en países de ingresos bajos y medios aún giran en torno al diagnóstico temprano y la disponibilidad de la terapia adecuada. La atención exitosa de las pacientes depende de la detección y el tratamiento tempranos del cáncer de mama. El manejo eficaz de la enfermedad requiere un diagnóstico y tratamiento oportunos. Mejorar la atención a largo plazo de las pacientes y reducir la tasa de mortalidad relacionada se puede lograr mediante consideraciones de manejo basadas en diversos estándares para el cáncer de mama. Las prácticas locales se basan en diversas recomendaciones internacionales

    DIAGNÓSTICO Y TRATAMIENTO QUIRÚRGICO DE METÁSTASIS AISLADAS EN PÁNCREAS: A PROPÓSITO DE CUATRO CASOS

    Las metástasis en la glándula pancreática de otros tumores primarios son poco frecuentes, el origen puede ser renal, colorrectal, pulmonar, ginecológico, mamario o sarcomatoide. Se diagnostican durante el seguimiento del tumor primario, y en casos de metástasis únicas metacrónicas, sin más enfermedad a distancia, puede estar indicado el tratamiento quirúrgico. Presentamos cuatro casos de pacientes intervenidos de metástasis en páncreas en la Fundación Instituto Valenciano de Oncología, con datos epidemiológicos y de seguimiento. En tres de los casos el tumor primario fue renal, y en uno, pulmonar. En los cuatro pacientes se realizó una resección de cuerpo y/o cola de páncreas con márgenes libres. Uno de los pacientes tuvo una recaída de la enfermedad a los 95 meses de la cirugía pancreática, y los otros tres están libres de enfermedad. En la revisión bibliográfica, las metástasis de origen renal e intervalo libre de enfermedad prolongado desde la cirugía del primario, son las que tienen mejor pronóstico

    TRANSFORMACIÓN MALIGNA DE QUISTE TIROGLOSO A PROPÓSITO DE UN CASO

    OBJETIVO: Los quistes del conducto tirogloso son anomalías cervicales congénitas frecuentes. Con una incidencia de transformación maligna que varía del 0,7 % al 1 %, hasta la fecha se han descrito aproximadamente 300 casos de tumores malignos en quiste del conducto tirogloso desde que Bretano identificara el primer caso en 1911. CASO CLÍNICO: Paciente masculino 38 años quien inicia enfermedad actual 8 meses previo a su ingreso al presentar aumento de volumen en región lateral izquierda de cuello, acude a esta institución se solicitan estudios con hallazgo en adenopatía mediante punción aspiración con aguja fina de carcinoma metastásico por lo que se realiza intervención quirúrgica DISCUSIÓN: Estas neoplasias son infrecuentes y no se ha establecido una estadificación o estandarización del tratamiento, pero se refiere que la operación de Sistrunk se considera adecuada, sin embargo, de detectarse anomalías en la glándula tiroides mediante palpación o ecografía, debe considerarse la posibilidad de realizar una tiroidectomía
