Nurses’ experiences of practicing motivational interviewing : A literature review
Bakgrund: Livsstilsrelaterade sjukdomar är en växande orsak till sjuklighet och förtida dödsfall globalt. I sjukvården sker prevention av livsstilsrelaterad ohälsa ofta i form av rådgivande samtal och stöd till hälsosamma levnadsvanor. Motiverande samtal är en samarbetsinriktad metod som används brett i sjukvården för att motivera och stödja patienter till beteendeförändring. Syfte: Syftet var att undersöka sjuksköterskors upplevelser av att använda motiverande samtal i kliniskt patientarbete. Metod: En allmän litteraturöversikt baserad på sju vetenskapliga artiklar som analyserades med kvalitativ innehållsanalys. Resultat: Analysen genererade sex övergripande teman och tjugo underliggande kategorier. Teman som beskrevs var Personcentrerad vård, Patientens delaktighet, Förutsättningar för MI, Utmaningar med MI, Patientens respons och Fördelar i sjuksköterskans arbete. Slutsatser: Motiverande samtal uppfattades ha fördelar för både patienter och sjuksköterskor. För att lära in metoden behövdes kontinuerlig träning, och sjuksköterskor upplevde tidsbegränsningar och brist på stöd från arbetsplatsen som hinder för att tillämpa metoden kliniskt. Fortsatt forskning bör fokusera på förbättringsarbete för att upprätthålla och stödja sjuksköterskors färdigheter i motiverande samtal.Background: Non-communicable diseases related to lifestyle is a growing cause of morbidity and mortality globally. Prevention of lifestyle-related issues in clinical care is often focused on counselling and promotion of healthy lifestyle choices. Motivational interviewing is a collaborative counselling method used widely in healthcare to motivate and support patients’ behaviour change efforts. Aim: The purpose of this study was to examine nurses’ experiences of using motivational interviewing in clinical patient care. Methods: A general literature review based on seven research articles, analysed using qualitative content analysis. Results: The analysis generated six main themes and twenty underlying categories. Themes described were Person-centered care, The patient’s participation, Prerequisites for using MI, Challenges in using MI, The patient’s response, and Advantages to nurses’ work. Conclusions: Motivational interviewing was perceived to be advantageous for both patients and nurses. Learning to apply the method required persistent practice, and nurses experienced lack of workplace support and time restraints as barriers for clinical implementation. Future studies should investigate improvement efforts to maintain and support nurses’ use of motivational interviewing
Measurement of Mental Health Among Adolescents at the Population Level: A Multicountry Protocol for Adaptation and Validation of Mental Health Measures
Purpose: Mental disorders are among the leading causes of disability among adolescents aged 10-19 years. However, data on prevalence of mental health conditions are extremely sparse across low- and middle-income countries, even though most adolescents live in these settings. This data gap is further exacerbated because few brief instruments for adolescent mental health are validated in these settings, making population-level measurement of adolescent mental health especially cumbersome to carry out. In response, the UNICEF has undertaken the Measurement of Mental Health Among Adolescents at the Population Level (MMAP) initiative, validating open-access brief measures and encouraging data collection in this area. Methods: This protocol presents the MMAP mixed-methods approach for cultural adaptation and clinical validation of adolescent mental health data collection tools across settings. Qualitative activities include an initial translation and adaptation, review by mental health experts, focus-group discussions with adolescents, cognitive interviews, synthesis of findings, and back-translation. An enriched sample of adolescents with mental health problems is then interviewed with the adapted tool, followed by gold-standard semistructured diagnostic interviews. Results: The study protocol is being implemented in Belize, Kenya, Nepal, and South Africa and includes measures for anxiety, depression, functional limitations, suicidality, care-seeking, and connectedness. Analyses, including psychometrics, will be conducted individually by country and combined across settings to assess the MMAP methodological process. Discussion:This protocol contributes to closing the data gap on adolescent mental health conditions by providing a rigorous process of cross-cultural adaptation and validation of data collection approaches
Voices of women in homelessness during the outbreak of the COVID-19 pandemic : a co-created qualitative study
Background: Women in homelessness face extreme health- and social inequities. It could be postulated that during societal crises, they become even more vulnerable. Thus, the aim was to explore experiences related to the COVID-19pandemic among women in homelessness. Methods: Ten interviews were conducted with women in homelessness, in Stockholm, Sweden, using researcher-driven photo elicitation. The data analysis was guided by the DEPICT model for collaborative data analysis and a qualitative content analysis was performed. A collaborative reference group of women with lived experience of homelessness contributed to the research process through designing the data collection, performing the data analysis, and providing feedback during report writing. Results: For women in homelessness, the COVID-19 pandemic was adding insult to injury, as it significantly affected everyday life and permeated most aspects of existence, leading to diminished interactions with others and reduced societal support. Thus, in an already dire situation, the virus amplified health- and social issues to another level. The women strived to find their balance on the shifting sands of guidelines and restrictions due to the pandemic. Adheringto the new social distancing rules and guidelines in line with the rest of society, was simply impossible when experiencing homelessness. However, for some women the pandemic was nothing but a storm in a teacup. The harsh reality continued irrespectively, living one day at a time and prioritizing provision for basic human needs. Conclusions: The COVID-19 pandemic and homelessness can be viewed as two intersecting crises. However, the women’s aggregated experiences were greater than the sum of experiencing homelessness and meeting the threatof the virus. Gender, exposure to violence, poverty, social isolation, and substance use were additional factors that further marginalized the women during the pandemic. To rebuild a better and more sustainable post-pandemic future for all, global commitment to ending homelessness is crucial. In addition, addressing social determinants of health must be the number one health intervention
Nurses' experience of working with patient participation in forensic psychiatric care : A qualitative interview study
Bakgrund: Patientens delaktighet är reglerat i flertalet lagar och är en rättighet för alla patienter inom svensk hälso- och sjukvård. Inom tvångsvården har tidigare forskning visat att delaktigheten begränsas då det finns vårigheter i att få en patient som vårdas under tvång att känna sig delaktig. För att öka förståelsen för hur arbetet kring patientdelaktighet inom den rättspsykiatriska kontexten går till valde författarna att undersöka detta. Syfte: Syftet var att beskriva sjuksköterskors erfarenhet av att arbeta med patientdelaktighet inom rättspsykiatrisk vård. Metod: En kvalitativ studie med semistrukturerade intervjuer som analyserades enligt kvalitativ innehållsanalys med induktiv ansats. Sex sjuksköterskor med minst två års erfarenhet av att arbeta inom rättspsykiatrisk vård intervjuades. Resultat: Resultatet presenterades i tre olika teman. Vikten av kommunikation beskriver betydelsen av tydlig kommunikation och att patienten får den information som krävs för att kunna vara delaktig. Patientens engagemang i sin vård tar upp sjuksköterskornas syn på delaktighet, deras erfarenhet av att motivera till delaktighet samt att alla patienter har olika nivåer av förmåga och insikt. Förutsättningar för patientdelaktighet beskriver de utmaningar sjuksköterskan möter i arbetet kring delaktighet och vad det finns för resurser som främjar delaktighet. Slutsats: Denna studie undersökte sjuksköterskors erfarenheter av att arbeta med patientdelaktighet inom rättspsykiatrisk vård. Resultaten visade att patientens vårdplan och kommunikation var centrala för att främja delaktighet. Anpassning, motivation och kompetens var viktiga faktorer. A caring touch : Nurses’ experiences of physical touch in a caring setting
Bakgrund: Beröring är en form av icke-verbal kommunikation som sjuksköterskan ofta stöter på i vårdandet av andra. Beröring kan användas av sjuksköterskan för att exempelvis förmedla tröst eller förståelse. Man kan dela upp fysisk beröring i två former, instrumental och expressiv beröring, men dessa går inte att helt skiljas åt. Sjuksköterskan behöver ta hänsyn till att alla patienter inte kommer vara bekväma med beröring samt hålla deras integritet i åtanke. Syfte: Syftet med denna studie var att belysa hur sjuksköterskan beskriver sina erfarenheter och upplevelser av fysisk beröring i en omvårdnadskontext. Metod: En litteraturstudie med kvalitativ ansats som följde Polit och Becks niostegsmodell. 12 artiklar inkluderades och analyserades med tematisk analys enligt Braun och Clarke. Resultat: Två teman identifierades: Vårdande möten och När beröring inte är självklart. Inom första temat identifierades tre subteman: Beröring är essentiellt för omvårdnad, En väg till patienten och För att lindra lidande. Inom andra temat identifierades två subteman: Ta hänsyn till patientens integritet och förutsättningar samt Osäkerhet hos män att använda beröring. Slutsats: Beröring är en essentiell del av sjuksköterskans arbete och förmågan att implementera det på ett gott sätt påverkas av faktorer som kön, integritet, religion och kultur. Den personliga preferensen hos sjuksköterskan påverkar hur de använder sig av beröring. Background: Touch is a form of non-verbal communication that nurses often come across when caring for others. The nurse can use touch to convey comfort, understanding or to build a rapport with the patient. Physical touch can be divided into two categories, instrumental and expressive touch, but these two cannot be fully separated from each other. The nurse needs to consider that not all patients will be comfortable with being touched and to keep the patient’s integrity in mind. Aim: The aim of this study was to illustrate how nurses describe their experiences of using physical touch in a caring setting. Method: A literature review with a qualitative approach that followed Polit and Beck’s nine step model. 12 articles were included and analyzed using Braun and Clarke’s method of thematic analysis. Results: Two themes were identified: Caring meetings and When touch isn’t a given. The first theme contains three subthemes: Touch is essential for nursing, A way to the patient and To ease suffering. The second theme contains two subthemes: Taking patient integrity and conditions into account and Insecurity in men in using touch. Conclusions: Touch is an essential part of the nurses’ work, the ability to implement it well is affected by factors like gender, integrity, religion and culture. The nurse’s personal preferences influence how they use touch.
Barn som överlevt tortyr : En systematisk litteraturöversiktom screening, dokumentation ochbehandling av tortyrskadade barn
Syfte: Att syntetisera forskning och grå litteratur gällande screening, dokumentation och tortyrskadevård av tortyrskadade barn. Metod: En systematisk litteraturstudie genomfördes. Studier genomsöktes i vetenskapliga databaser. Totalt screenades 4 795 titlar och/eller abstrakt varav 276 artiklar lästes i fulltext. Dessutom screenades 81 rapporter, riktlinjer och vårdprogram varav 12 lästes i fulltext. Totalt inkluderades 80 vetenskapliga artiklar och sex rapporter, riktlinjer och vårdprogram. Resultat: Screening för tortyrutsatthet utgjordes i regel av frågor som ingick i skattningsformulär om traumatiska livshändelser, ofta kopplade till krig. Frågor om aktörer bakom de traumatiska händelserna saknades i mer än hälften av studierna. I 89 procent av studierna screenades det för psykiska och/eller psykosociala symtom medan endast 21 procent screenade för fysiska skador/symtom. Samtliga studier som utfört tortyrdokumentation rapporterade fysiska tortyrskador medan endast 43 procent av studierna rapporterade psykiska symtom. Traumafokuserad kognitiv beteendeterapi (TF-KBT) och Narrative exposure therapy (NET) minskade PTSD signifikant från tre månader och upp till ett år efter avslutad behandling hos barn varibland vissa överlevt tortyr. Behandlingar med individ- och gruppbaserade format såväl som de med normal- och högintensiva upplägg visade sig ha effekt på PTSD. Rekommendationer för klinik och forskning: Utveckling av ett generiskt screeninginstrument som kan underlätta identifiering av barn som överlevt tortyr, för samhällsaktörer som kommer i kontakt med målgruppen. Traumabehandling i gruppformat såväl som i en intensifierad form skulle kunna implementeras i klinik och även utvärderas ytterligare i forskning för den specifika målgruppen.Aim: To synthesize research and grey literature regarding screening, documentation and torture injury treatment of torture-injured children. Method: A systematic literature review was conducted. Studies were searched in scientific databases. A total of 4795 titles and/or abstracts were screened, of which 276 articles were read in full text. In addition, 81 reports, guidelines and care programs were screened, of which 12 were read in full text. A total of 80 scientific articles and six reports, guidelines and care programs were included. Results: Screening for torture exposure usually consisted of questions that were included in questionnaires about traumatic life events, often linked to war. Questions about the actors behind the traumatic events were missing from more than half of the studies. In 89 percent of the studies screening for mental and/or psychosocial symptoms were done while only 21 percent screened for physicalinjuries/symptoms. All studies that performed torture documentation reported physical torture injuries, while only 43 percent of the studies reported psychological symptoms. Trauma focused cognitive behavioural therapy (TF-CBT) and Narrative exposure therapy (NET) significantly reduced PTSD from three months and up to one year after the end of treatment in children of whom some had survived torture. Treatments with individual and group-based formats as wellas those with normal and high-intensity approaches were found to have an effect on PTSD. Recommendations for clinic and research: Development of ageneric screening instrument that can facilitate the identification of torture survivors of children in various social services that encounterthe target group. Women with endometriosis and their perception of treatment by health professionals : A qualitative literature review
Bakgrund: Tio procent av kvinnor och flickor i fertil ålder drabbas av endometrios. Vid endometrios har vävnad eller endometrioshärdar växt till sig utanför livmodern, vilket resulterar i inflammation och tillväxt av nervändar. Laparoskopi och laparotomi används för att säkerställa eller utesluta en endometriosdiagnos. Analgetika används som om den basala smärtbehandlingen mot dysmenorré, det främsta symtomet. Syfte: Syftet var att beskriva kvinnor med endometrios upplevelser av hälso- och sjukvårdens bemötande. Metod: En kvalitativ litteraturöversikt genomfördes med en tematisk analys varav 13 artiklar analyserades. Resultat: Ett tema och fyra subteman identifierades. Tema: Bemötande påverkar kvinnors självbild. Subteman: Invalidering av känslor och upplevelser, Normalisering av symtom, Hopplöshet relaterat till inadekvat vård orsakad av inkompetens, samt Bristande informationsgivning och vikten av lyhördhet. Slutsats: Kunskapsbristen bidrog till invalidering av kvinnors känslor och upplevelser, normalisering av symtom, samt bristande lyhördhet och informationsgivning. Detta tillsammans med okunskap bidrog till både fel- och sen diagnostisering. Detta bidrog till negativa upplevelser av hälso- och sjukvården. Resultatet visade att ett fortsatt behov av vidare forskning inom syftet behövs.Background: Ten percent of women and girls of childbearing age are affected by endometriosis. In endometriosis, tissue or endometrial lesions have grown outside the uterus, resulting in inflammation and growth of nerve endings. Laparoscopy and laparotomy are used to confirm or exclude a diagnosis of endometriosis. Analgesics are used as basic pain management for dysmenorrhea, the main symptom. Purpose: The aim was to describe the perception of health care treatment among women with endometriosis. Method: A qualitative literature review was conducted with a thematic analysis of which 13 articles were analyzed. Results: One theme and four sub-themes were identified. Theme: Treatment affects women's self-image. Sub-themes: Invalidation of feelings and experiences, Normalization of symptoms, Hopelessness related to inadequate care caused by incompetence, and Lack of information and the importance of attentiveness. Conclusion: The lack of knowledge contributed to the invalidation of women's feelings and experiences, normalization of symptoms, and lack of sensitivity and information provision. This, together with the lack of knowledge, contributed to both misdiagnosis and late diagnosis. This contributed to negative experiences of healthcare. The results show that there is a continued need for further research within the research question
Nurses' experiences of caring for Covid-19 patients during the Covid-19 pandemic : A qualitative review
Background: The Covid-19 pandemic came suddenly and despite our efforts to limit the spread of the virus many people lost their lives. Nurses, the largest group of health care workers, played a crucial role in the caretaking of patients. They cared, informed, screened, organized and managed teams, making them central to patients´ care in that crisis. Aim: The aim is to explore nurses´ experiences of caring for Covid-19 patients at hospitals during the Covid-19 pandemic. Method: A qualitative review based on ten scientific articles. They were analyzed with a thematic inductive analysis following the Braun and Clarks model. Results: Two themes were identified: struggling with new challenges and coping with new challenges. With the respective subthemes: Personal Protective Equipment, fear of Covid-19, feeling lost, abandonment and growing as a nurse and finding strength within the team. Conclusion: This research shows that nurses faced many difficulties while caring for Covid-19 patients, whether it was the lack of PPE, the constant changes in protocols or the fear of being infected. More studies are needed to better prepare nurses if a similar situation occur, which would ensure good and safe nursing care for patients
Relative’s experiences from palliative care : Care meeting between relative’s and nurses
Bakgrund: Miljontals människor världen över är i behov av palliativ vård. Cirka en sjundedel av de som behöver palliativ vård erhåller det. Den palliativa vården syftar till förhöja livskvalitén och lindra lidande. Det palliativa vårdandet ska uppmärksamma både patienten och den närståendes behov. Närstående associeras ofta med känslor som sorg. Syfte: Syftet var att beskriva närståendes erfarenheter av vårdmötet med sjuksköterskor inom palliativ vård. Metod: Med en litteraturöversikt med kvalitativ ansats har studiens syfte besvarats. För litteraturöversikten användes Polit och Becks Niostegsmodell. Artiklar är hämtade från databaserna Cinahl och PubMed. Resultat: Resultatet baseras på tretton artiklar. Det framkom två kategorier och fyra subkategorier identifierades. De två kategorierna var Att få stöd i sorgeprocessen och Sjuksköterskors professionalism. Den förstnämndas två identifierade subkategorier var erfarenheter av stöd från sjuksköterskor och upplevelser av vårdmiljön. Den andra kategorins subkategorier identifierades som Upplevelse av sjuksköterskors kunskap och Erfarenheter av informationsutbytet. Slutsats: Det framkommer att sjuksköterskors förmåga att synliggöra närstående spelar en viktig del i deras upplevelse av vårdmötet. Det finns en osäkerhet bland sjuksköterskor i att bemöta närstående i sorg då de känslor närstående upplever kan vara svåra att bemöta. The patient’s experiences of self-care in heart failure : A literature study
Bakgrund: Hjärtsvikt är en global sjukdom där patienten är i behov av egenvård som ett komplement till konventionell behandling. Hur patienten engagerar sig i egenvård är relaterat till livssituationen, men även den fysiska samt psykiska förmåga. Patienters erfarenheter av egenvård behöver beaktas för ett kunna ge effektivt stöd för deras sjukdom samt egenvård. Syfte: Syftet med denna studie var att undersöka patientens erfarenheter av egenvård vid hjärtsvikt. Metod: En litteraturöversikt med kvalitativ ansats baserad på 14 artiklar, där en tematisk analys användes för att analysera data. Resultat: Två teman med två respektive subteman presenterades i resultatet. Det första temat är "Att övervinna utmaningarna som medföljer hjärtsvikten” som har subteman “Navigera känslomässiga utmaningar och acceptera förändring” och ”Förändrad förmåga”. Det andra temat som presenterades är “Att upprätthålla egenvård” som har subteman “Behov av stöd” och “Behov av kunskap och information”. Slutsats: Egenvård är viktigt för att hantera hjärtsvikt, men erfarenheterna varierar och påverkar hälsa och livskvalitet. Stöd från vårdgivare, familjemedlemmar samt relevant information är nyckeln till att uppnå förtroende till sina förmågor och därmed uppnå självständighet i sin egenvård.Background: Heart failure is a global disease that necessitates self-care management. How patients engage in self-care is related to their life situation, but also their physical and mental abilities. Patients experience of self-care needs to be taken into account for effective support in their illness and self-care. Aim: The aim of this study was to explore patients experience of self-care in heart failure. Method: A literature review with a qualitative approach based on 14 articles, where a thematic analysis was used to analyze the data. Results: The study identified two themes with two sub-themes each. The first theme was “Overcoming the challenges that come with heart-failure” which has the sub-theme “Navigating emotional challenges and accepting change” and “Changed ability”. The second theme was "To maintain self-care" which has the sub-themes "The need of support" and "The need of knowledge and information". Conclusions: Self-care is essential in heart failure management, but experiences vary and affect health and quality of life. Support from healthcare providers and family members, and relevant information and resources are key to achieving confidence in their abilities and therefore independence in self-care management.
