Clinical outcomes of posterior lumbar interbody fusion in chronic renal failure patients on hemodialysis
Chronic kidney disease (CKD) and its treatment with hemodialysis (HD) pose unique challenges for spinal surgery due to complications such as destructive spondyloarthropathy (DSA). This study retrospectively compared the surgical outcomes of posterior lumbar interbody fusion (PLIF) between 48 HD patients and 57 non-HD controls. Patients with tumors, infections, prior spinal surgery, or severe osteoporosis were excluded. HD patients had a mean dialysis duration of 16.2 years, while controls were treated for degenerative lumbar conditions. HD patients exhibited significantly higher intraoperative blood loss (415.8 ± 231.7 mL vs 293.4 ± 57.3 mL, P < 0.001) and lower 2-year bony fusion rates (72.9% vs 94.7%, P = 0.008). Pseudoarthrosis and adjacent segment disease (ASD) were more common in the HD group, necessitating reoperation in five cases versus one in controls. Neurological recovery at 2 years was worse in the HD group, with a mean Japanese Orthopaedic Association score of 19.6 ± 4.3 compared to 26.5 ± 2.2 in controls (P < 0.01). Despite facilitating initial neurological recovery, PLIF outcomes in HD patients were compromised by greater complication rates, including pseudoarthrosis and ASD. Thus, PLIF facilitates early neurological improvement in HD patients, but long-term functional outcomes are compromised due to higher rates of pseudoarthrosis and ASD, necessitating careful long-term management. A case of primary cutaneous diffuse large B-cell lymphoma, leg type with MYC rearrangement and high BCL2 protein expression due to trisomy 18
We report the case of an 80-year-old woman with a medial thigh nodule who was diagnosed with primary cutaneous diffuse large B-cell lymphoma, leg type. Further examination identified it as doubleexpressor lymphoma with elevated expression of both MYC and BCL2. This elevated expression has been linked to MYC translocation and is likely associated with trisomy 18. Our findings suggest that trisomy 18 plays a significant role in the development of double-expressor lymphoma. While cases of doubleexpressor lymphoma caused by extra copies of BCL2 have been reported, we could not find any case of double-expressor lymphoma caused by trisomy 18. Therefore, this is an unique case of double-expressor lymphoma linked to MYC rearrangement and trisomy 18. Double-expressor lymphoma typically has a poor prognosis and is often resistant to standard treatments. Small, obligately anaerobic strains 13CB8CT, 13CB11CT, 13CB18CT and 13GAM1GT were isolated from a faecal sample in a patient with Parkinson’s disease with a history of duodenal resection. After conducting a comprehensive polyphasic taxonomic analysis including genomic analysis, we propose the establishment of one new genus and four new species. The novel bacteria are Desulfovibrio falkowii sp. nov. (type strain JCM 36128T = DSM 116810T), Porphyromonas miyakawae sp. nov. (type strain JCM 36129T = DSM 116947T), Mediterraneibacter flintii sp. nov. (type strain JCM 36130T = DSM 116866T) and Owariibacterium komagatae gen. nov. sp. nov. Typical emotional verbs are said to be like state verbs in that the suru form can express an internal event in the present. On the other hand, as with action verbs, there are a small number of emotional verbs whose suru forms express only internal events in the future (non-expressive emotional verbs). The tense-aspectual meaning in the suru form of non-expressive emotional verbs is related to the personhood of the emotional subject. When judging nature as a potential event, an unspecified general person is the subject, while when describing an actual event, there are emotional verbs that take firstperson subjects, such as tanosimu, kōkai-suru and ai-suru, and emotional verbs that take third-person subjects, such as yorokobu and kanasimu. The kōkai-suru kind of suru form basically cannot co-occur with an adverb indicating a specific time in the future, but with a conditional clause, it hypothetically expresses a one-time future event by superimposing what is originally a general condition on the situation. When a direct expression that combines the speech scene with the concrete reality in front of us appears in the sentence together with the conditional clause, the meaning of predicting a future situation becomes easier to read. In addition, verbs of the ai-suru kind, which express long-term feelings, usually express the speaker's attitude. On the other hand, verbs of the tanosimu kind, which have strong volitional and action characteristics, can co-occur with temporal adverbs that refer to the future and express their psychology in the future. The sita form has two main meanings: the basic meaning “one-time past” and the derivative meaning “experience”. In addition, person bias appears to be mitigated compared to the suru form. The sita form of non-expressive emotional verbs, like action verbs, generally describes past internal situations as factual confirmation when a temporal adverb indicating the past appears in the sentence. On the other hand, about derivational meaning, action verbs always express the meaning of “perfect (completion, experience)” when they are associated with “anymore,” but when non-expressive emotional verbs are used with adverbs that indicate “completion,” such as “anymore,” they can be somewhat uncomfortable. Manetho, an Egyptian priest believed to have served under the Early Ptolemaic Dynasty, wrote Aegyptiaca, which marked the first attempt to depict a comprehensive dynastic history of Egypt through historical narrative. Aegyptiaca survives only in fragments preserved in later references and citations by Jewish and Christian authors. The original text is lost, and the information these fragments provide often does not correspond to the Egyptian history reconstructed archaeologically. Nonetheless, Aegyptiaca remains a crucial source for understanding how Manetho, in his unique position as a local priest serving under the Early Ptolemaic Dynasty, perceived and articulated Egyptian dynastic history. By translating descriptions related to the Early Dynasties, which have not received sufficient studies, this paper tries to explore how Manetho interpreted the establishment of Egyptian dynasties. Based on this examination, we reconsider why Manetho wrote Aegyptiaca. The following can be said from fragments related to the early dynasties. First, Manetho likely perceived Egyptian history as characterized by a certain pattern of repetition. In addition, as exemplified by the descriptions of the first two kings of the Second Dynasty, the annotations attached to each king were inserted in a way that had some relationship and could function as lessons for the newly arrived Greek elites of the Ptolemaic period. However, these points do not imply that Manetho freely altered the history of Egypt. As indicated by the descriptions of Menes, Manetho did not seem to present Egyptian history as particularly glorious. This suggests that he aimed to reconstruct the history of Egyptian dynasties in a manner faithful to the sources he used. Nevertheless, it is certain that Manetho reinterpreted these sources to match the circumstances of the Ptolemaic period. In particular, the story that a woman received the honor of kingship during the reign of Binothris of the Second Dynasty implies a deep connection with the policies under the reign of Ptolemy II. Based on the analyses, it can be concluded that Manetho's Aegyptiaca had a highly multifaceted purpose. The creation of Aegyptiaca essentially involved compiling the temple records into a dynastic history, following the traditional Egyptian format of king lists. However, by adding narratives, Aegyptiaca became a more practical and instructive work for the Greek ruling class. Furthermore, Manetho attempted to historicize and legitimize the innovative policies under the reign of Ptolemy II by incorporating them into the history of past dynasties.departmental bulletin pape

Since 2022, issues related to generative AI have surged on a global scale. Image-generating AI, in particular, has been the subject of controversy on the part of developers and users and on the part of creators. This is happening because developers and users of generative AI are downplaying, ignoring, or not understanding the problems claimed by creators. I have investigated why this is so, based on interviews with creators and the results of a survey I conducted in the early days of generative AI. As a result, I found that if there is any malice on the part of the developers, it is that they are very selfish. They are rejecting what is inconvenient for their own opinions. If there was no malice on the part of the developers, it was that they thought they understood even though they did not.
