Nupharanin, the first ellagitannin with 1,4-dehydrohexahydroxydiphenoyl-α-d-glucopyranose from Nuphar japonicum
Nupharanin (1), the first ellagitannin with dehydrohexahydroxydiphenoyl esters at the 1,4-positions of a-D-glucose, was isolated from the fresh rhizome of Nuphar japonicum. Based on spectroscopic evidence,the structure was determined to be 1,4-(S)-dehydrohexahydroxydiphenoyl-2-galloyl-3,6-(R)-hexahydroxydiphenoyl-
a-D-glucopyranose. The structure was supported by spectroscopic studies of acetonyl derivatives of 1 and its partial hydrolysate. In addition, unusual oxidative cleavage of the hydrated cyclohexenetrione ring of 1 was observed on moderate heating at 50 C in pH 6 buffer
Neglected subluxation of the hip after trauma: A case report
Introduction: In children, the pelvis contains a large amount of cartilage components; therefore, when traumatic hip dislocation spontaneously reduces,it can be impossible to see on X-ray or computed tomography (CT) images in some cases, which can delay its detection. Case presentation:
We report the case of a 10-year-old boy who was injured by being hit by a car while walking. Upon diagnosis of pelvic ring fracture, the patient received conservative treatment. Seven months after injury, the patient was referred to our department with the chief complaint of limping.Diagnosis: Marked limitation was observed in the left hip with extension of −40°, abduction of 10°, and internal rotation of 20°.
X-ray revealed narrowing of the left hip joint space, with deformity of the femoral head, obturator foramen narrowing, and the break in the Shenton line.CT revealed proximal dislocation of the posterior acetabular wall and posterior subluxation of the femoral head. Magnetic resonance imaging (MRI) revealed necrosis of the femoral head. Intervention: Operation was performed with soft tissue dissection, varus-extension-internal rotation femoral osteotomy, greater trochanteric epiphysiodesis, and pelvic osteotomy (incomplete periacetabular osteotomy: IPO). After operation, complete paralysis of the sciatic nerve was observed. Outcomes: At 1 year after operation, the patient's limited range of motion (ROM) and
femoral head necrosis had improved. The sciatic nerve paralysis had fully recovered.
Conclusion: If hip extension, abduction, and internal rotation are limited and X-ray reveals a break in the Shenton line.,
subluxation of the hip should be suspected
Human T-cell lymphotropic virus type-1 infection associated with sarcopenia: community-based cross-sectional study in Goto, Japan
Sarcopenia is characterized by a progressive skeletal muscle disorder that involves the loss of muscle mass and low muscle
strength, which contributes to increased adverse outcomes. Few studies have investigated the association between chronic
infection and sarcopenia. This study aimed to examine the association between human T-cell lymphotropic virus type-1 (HTLV-1)
and sarcopenia. We conducted a cross-sectional study and enrolled 2,811 participants aged ≥ 40 years from a prospective cohort study in Japanese community dwellers during 2017-2019. Sarcopenia was defined as low appendicular skeletal muscle mass and low handgrip strength. The association between HTLV-1 seropositivity and sarcopenia was assessed using multivariable logistic regression. Odds ratio (OR) and 95% confidence interval (CI) of sarcopenia were analysed using HTLV-1 seropositivity.We adjusted for age, sex, body mass index, physical activity, systolic blood pressure, glycated haemoglobin, low-density lipoprotein cholesterol, and smoking and drinking status. Of 2,811 participants, 484 (17.2%) HTLV-1 infected participants were detected. HTLV-1 infection was significantly associated with sarcopenia (adjusted OR 1.46, 95% CI 1.03-2.07, P = 0.034). HTLV-1 was associated with sarcopenia among community-dwelling adults.Active surveillance and early detection of asymptomatic HTLV-1 infection might be beneficial to reinforce countermeasures
to inhibit the progress of HTLV infection-associated sarcopenia
Test on Development of Simple Seismic Retrofit by Knee Braces for Steel Building
The authors have developed a new method of seismic retrofit for steel structures by adding knee braces which are jointed
to the existing structures by the vices made of high hardness metal screw bolts. The strength and behavior of the connection
by the vice had been already studied in our previous papers. The features of the connection by the vice are easy set up and
providing the strength as much as that by the normal bolts joint. An experimental study has been conducted for the
specimens of steel frames retrofitted by the knee braces. The knee braces were jointed to the specimen by the vices. Two
types of failure modes were investigated. The former was slipping behavior of the connection and the later was bucking at
the knee braces. The strengths of the specimens were estimated by simple calculations for the two types of failure modes
Study on the damage location identification method of bridges using the rate of change of the deflection
The purpose of this study is to present a damage location identification method using the rate of change of the flexure influence line and the flexure curve obtained from bridge displacement measurement, and to verify its effectiveness.
First, using a truss model, a damage location identification method was presented using the rate of change of the deflection curve and the deflection influence line. Then, the method was applied to the RC beam loading test in the laboratory and the actual RC bridge loading test, and their usefulness was examined
