Cervantes' El licenciado vidriera und die Neugier

    No full text

A Comparative Analysis of Carlevale IOL Versus Artisan IOL Implantation Using a Scleral Tunnel Incision Technique

    No full text
Background: The aim of this retrospective study was to compare the surgical and refractive outcomes using the Carlevale IOL (FIL SSF; SOLEKO) with those of the retropupillary-fixated Artisan IOL (Aphakia Model 205; OPHTEC), implanted through a 6 mm sclerocorneal tunnel incision in both groups. Methods: This study included 51 consecutive eyes (25 Carlevale and 26 Artisan IOLs). Due to complex preoperative conditions (e.g., dislocated polymethylmethacrylat IOL, luxated Cataracta rubra), all patients underwent lens explantation using a standardized 6 mm sclerocorneal tunnel incision and a 23 G or 25 G pars plana vitrectomy. Visual acuity (VA), spherical equivalent, refractive prediction error (PE), incision-suture time, and complication rates were recorded preoperatively and during the follow-up period. Results: The average follow-up period was 40.9 ± 5.7 days. VA improved by 0.28 ± 0.39 logMAR (p < 0.0001) in the Carlevale group and by 0.36 ± 0.47 logMAR (p < 0.0001) in the Artisan group. The improvement was comparable between both groups (p = 0.921). The deviation of the PE was −0.67 ± 0.56 in the Carlevale group and 0.34 ± 0.71 in the Artisan group (p < 0.0001). The mean incision-suture time was 42.5 ± 5.8 min in the Carlevale group and 28.2 ± 6.4 min in the Artisan group. Anterior chamber and vitreous hemorrhages were the most common complications, occurring in 12% in the Carlevale group and 17.2% in the Artisan group. Conclusions: The use of the Carlevale IOL, implanted using a sclerocorneal tunnel technique, presents a valid option for treating complex lens dislocations. The scleral fixation of the Carlevale IOL minimizes risks associated with iris fixation, such as chronic inflammation and pupil distortion, making it particularly suitable for patients with damaged irises

Adaptive Photochemical Nonlinearities for Optical Neural Networks

    No full text
Optical neural networks (ONNs) hold great potential for faster and more energy-efficient information processing in coherent photonic circuits. To realize ONNs, linear combinations and nonlinear activation functions have to be implemented in an optical fashion. Optical nonlinearities are, however, still difficult to achieve, and existing designs are usually too inflexible to offer different activation functions as used in artificial neural networks. Herein, the nonlinear properties of the large and highly adaptive class of photoswitchable chemical compounds is made accessible as activation functions in ONNs by employing photo-induced isomerization in azobenzenes to steer activation behavior through nonlinear modulation of an information-carrying optical signal. The strength of the nonlinearity can be controlled by the chemical concentration while a physically motivated model describes the experimental data for systematically varied photoswitching parameters, resulting in a tunable yet interpretable activation function. Employing such an activation function in a neural network then allows to gauge its strength and perform established classification tasks. The work combines recent advances with photoswitchable chemical compounds and optical neural networks to enable control over the design of nonlinear activation functions, thus opening exciting perspectives for explaining the emergence of intelligent behavior in neural networks

Diskurse des Feudalismus und des gerechten Krieges im Don Quijote

    No full text

Effect of Prior Transurethral Prostate Resection (TURP) or Laser Enucleation (ThuLEP) on Radiotherapy-Induced Toxicity and Quality of Life in Prostate Cancer Patients Undergoing Definitive Radiotherapy

    No full text
In our study, the post-radiotherapy quality of life of prostate cancer patients who previously underwent transurethral resection of the prostate (TURP) is compared to those who had thulium laser enucleation of the prostate (ThuLEP) and those who had no prior surgery. It also aims to identify and assess risk factors affecting therapy tolerance in this patient group. We analyzed 132 patients with localized prostate cancer treated with definitive radiotherapy (RT), including 23 who had prior TURP and 19 who previously underwent ThuLEP. A total of 62% of patients underwent irradiation within 12 months after surgery. We included only patients treated with radiotherapy using the IMRT technique. Changes in patient-reported urinary toxicity were evaluated using the International Prostate Syndrome Score (IPSS) and the quality of life index of the World Health Organization (QoL/WHO-PSS) over a three-year post-radiotherapy period. Patients with prior TURP experienced significant deterioration in QoL and IPSS immediately after irradiation (p < 0.001), whereas those without previous surgery showed both less significant differences in IPSS and QoL scores. In conclusion, patients with previous TURP/ThuLEP differ from those without previous surgery in urinary quality of life and acute and chronic urinary symptom profiles after RT. The surgical technique (ThuLEP vs. TURP) and the time interval to irradiation are crucial factors affecting RT tolerance in acute and late settings. The previously operated patient group reported a significantly longer period of increased symptom burden

Excitatory stimulation of the ventromedial prefrontal cortex reduces cognitive gambling biases via improved feedback learning

    No full text
Humans are subject to a variety of cognitive biases, such as the framing-effect or the gambler's fallacy, that lead to decisions unfitting of a purely rational agent. Previous studies have shown that the ventromedial prefrontal cortex (vmPFC) plays a key role in making rational decisions and that stronger vmPFC activity is associated with attenuated cognitive biases. Accordingly, dysfunctions of the vmPFC are associated with impulsive decisions and pathological gambling. By applying a gambling paradigm in a between-subjects design with 33 healthy adults, we demonstrate that vmPFC excitation via transcranial direct current stimulation (tDCS) reduces the framing-effect and the gambler's fallacy compared to sham stimulation. Corresponding magnetoencephalographic data suggest improved inhibition of maladaptive options after excitatory vmPFC-tDCS. Our analyses suggest that the underlying mechanism might be improved reinforcement learning, as effects only emerge over time. These findings encourage further investigations of whether excitatory vmPFC-tDCS has clinical utility in treating pathological gambling or other behavioral addictions

Gemeinnützige Stiftungen in Landshut:Stiftungskritik und die regionale Realität

    No full text
Research regarding foundations focuses mainly on the most wealthy organizations. Therefore it is difficult to achieve a holistic and accurate understanding of foundations in society. Ultimately, it is not only the few mega-foundations that bring socially desirable added value, but also the many small personal charitable commitments. These foundations help where the state and businesses do not take action. Some charitable foundations in the city and district of Landshut have been pursuing their purposes for centuries. Foundations in the Landshut region have played a relevant role for the common good of society and positive urban development since the ear-liest days of the Wittelsbacher. The Landshut foundations work on almost all socially relevant challenges of our time according to the sustainable development goals of the United Nations. However, they work in the background and with very few resources. This dissertation confronts the common criticism of foundations with the regional reality of foundation work. The main charitable activities of the Lower Bavarian capital are specified from the founding of the city with the construction of Trausnitz Castle in 1204 to the present. Also, it is explained how little the region's current foundations have in common with the general criticism of foundations

Hedgehog is relayed through dynamic heparan sulfate interactions to shape its gradient

    No full text
Cellular differentiation is directly determined by concentration gradients of morphogens. As a central model for gradient formation during development, Hedgehog (Hh) morphogens spread away from their source to direct growth and pattern formation in Drosophila wing and eye discs. What is not known is how extracellular Hh spread is achieved and how it translates into precise gradients. Here we show that two separate binding areas located on opposite sides of the Hh molecule can interact directly and simultaneously with two heparan sulfate (HS) chains to temporarily cross-link the chains. Mutated Hh lacking one fully functional binding site still binds HS but shows reduced HS cross-linking. This, in turn, impairs Hhs ability to switch between both chains in vitro and results in striking Hh gradient hypomorphs in vivo. The speed and propensity of direct Hh switching between HS therefore shapes the Hh gradient, revealing a scalable design principle in morphogen-patterned tissues
