Development of hemodialysis simulator system using a simulated extracorporeal circulation system and its application to education
No full text九州保健福祉大学博士(保健科学)平成27年度The improper operation of the hemodialysis apparatus during by the hemodialysis treatment has a serious influence on the patient’s life. Since the clinical staffs could not have opportunities of the training to operate the apparatus and dealt withproblems except for the clinical situation, some accidents due to their immaturity occurred.Therefore, the authoraimed at the development of the simulator system which could improve such situation.
The developed system has the functions to generate alarms (decreased venous pulse pressure, elevation venous pressure and blood leak) from the hemodialysis apparatusfor five kinds of problems selected by taking the occurrence frequency and the level of severity into accounts. The functions and operating characteristics of the system were verified for the five kinds of main apparatus manufactured by four major makers and used in Japan.
In all five hemodialysis apparatus models, all five kinds of problems were reproduced. Moreover, the operation training of the hemodialysis apparatus was performed by using thissystem. The number of subjects of the training was four (a male and three females) and they have had license of nurse or clinical engineer. Their age was 35.5±6.6 years old, and they had 1 to 19 years of experience. The averaged required times of the two trainings were 117 seconds (the first time) and 91 seconds (the second time) respectively. It means that their skills have been improved by the repetitive training. In the cases that the alarms do not lead to five kinds of problems directly, the required time of the operation is longer than the other cases, because they have to confirm several points related to the problems.
The viewpoint movements of an expert and a beginner during the hemodialysis apparatus operation were quantified with the aim to apply the system to education and the training. The expert is familiar with the operating procedure empirically and the sequence of the operation emerges as the movement ofthe viewpoint. The analytical results of the movement of the viewpoint have a possibility to optimize the training.
From the above, there is a possibility that the training by using the system improves the user’s skills to specify the factors of the problem and fix them. The experiences of some simulated troubles would assist in understanding the operation procedure. The simulator system that enables the repetitive training of the hemodialysis apparatus operation could be a useful tool for the clinical stuff such as nurse and clinical engineer.doctoral thesi
Access to Medical Services and Proper Drug Use among Residents in Mountainous Northern Areas of Miyazaki Prefecture
No full text九州保健福祉大学博士（医療薬学）平成27年度Residents in the mountainous northern areas of Miyazaki prefecture find it difficult to access medical institutions and care facilities, and they have little contact with healthcare professionals. In this study, we investigated community access to medical services and proper drug use among residents in areas affected by depopulation and aging. We conducted canvassing interviews with elderly people (aged 65 years and older) in an urban district and four mountainous districts. When problems were identified, we gave instructions for proper drug use, and encouraged medical consultations. Although the number of residents who had access to a family physician differed between districts, many residents had a family physician. However, the number of residents who had a family pharmacy was lower than the number of residents who had a family physician in all districts. The role of pharmacies in proper drug use may not have been recognized in some districts. We found that some residents had side effects by multiple prescriptions and there was a lack of understanding about the meaning of medication in mountainous areas. There were problems regarding over-the-counter and household drugs in districts with a high rate of having these drugs, and we also discovered double medications of prescription and over-the-counter drugs. We conclude that the current situation of community access to medicine has become more difficult for residents in mountainous areas. Nevertheless, pharmaceutical interventions are useful for the promotion of proper drug use for residents in mountainous areas.doctoral thesi

Present situation of community support by managers of care support specialists in home care support offices : The recognition of managers of care support specialists and care support specialists
No full textP(論文)本研究では,居宅介護支援事業所の主任介護支援専門員が果たす「事業所外(地域)Jでの役割,課題について,主任介護支援専門員と介護支援専門員の二者の認識を調査した。
調査票は210ヶ所に配布し,主任介護支援専門員の有効回答は119名(56.6%),介護支援専門員の有効回答は98名(46.6%)であった。
主任介護支援専門員,介護支援専門員ともに「社会資源の開発」などの「事業所外(地域)」における役割は主任介護支援専門員にとって重要であると回答した。
このことから主任介護支援専門員が「事業所外(地域)Jを基盤に支援を展開しようとする認識が明らかになった。
今後,地域包括ケアシステムを推進する上で「事業所外(地域)」における「社会資源の開発」などの活動は重要性を増しており,その役割は介護支援専門員にも求められている。
介護支援専門員がその力量を高めるためにも,彼らの身近に存在する居宅介護支援事業所の主任介護支援専門員による指導・助言が不可欠である。ln this study, recognition on the role and tasks required for Managers of Care Support Specialists of Home Care Support Office in "the outside of office (community)" was researched among Managers of Care Support Specialists and Care Support Specialists.
The survey slips were distributed to 210 spots, and the number of valid responses from Managers of Care Support Specialists was 119 (56.6%), while that from Care Support Specialists was 98(46.6%).
Both Specialists answered that they thought the role such as "developing social resource" in "the outside of office (community)" was important for Managers of Care Support Specialists.
From the above, it was made clear that the Managers of Care Support Specialists recognized expanding the support based on "the outside of office (community)" would be their challenge.
In the future, activities like "developing social resource" in "the outside of office (community)" will be more important to promote the integrated community care system, and Care Support Specialits are also required to play the role.

On the Interpretation of the “Personality” in the Education of Service and Receiving Visitors

of service and receiving visitors, clarifying the essential meaning based on its etymology and historical
sources. The “personality” is a Japanese word showing one’ s personality and dignity and has been used
in character evaluations since Heian classic literature was written. In modern times, its analysis was
made in the field of education in particular to contribute to the development of human resources. In
recent times, too, it is clear that it is an indicator for character evaluations in the jobs which ask for
service and receiving visitors.
