〈Originals〉Management of painful sensation with novel carbon dioxide laser : an open study
No full textpublisher〈Abstract〉The carbon dioxide (CO_2) laser is the prevailing treatment method in the management of skin tumors. However, there is a painful sensation when it is applied, causing anxiety to the patient. Recently, a novel CO_2 laser device was developed to relieve or reduce the painful sensation due to longer off-time (0.2 sec) during all cases repeat irradiation. We retrospectively evaluated the efficacy of the management of the painful sensation in 30 patients with small skin tumors who were treated by novel CO_2 laser without local anesthesia. In this study of 30 patients, the VAS (visual analogue scale) values's means was 25.4, and excellent improvement in 22(73.3%) and moderate improvement in 8(26.7%). Our results showed that the majority of the patients felt tolerable pain with excellent or moderate improvement. It was suggested that the novel CO_2 laser was effective in reducing the painful sensation and raising the effectveness

No full textpublisher[目次] 1. はじめに 2. 市町村 GRP の推計方法 3. 大阪府経済全体の概況 4. 市町村 GRP の推計結果 5. 分析事例 : 市町村間格差の検証 6. むすび[抄録]地域経済の現状を的確に捉えるためには, 客観的な経済統計の利用が欠かせない. なかでも域内総生産は当該地域の経済活動を包括的に捉えることの出来る経済統計である. しかし大阪府では, 大阪市を除いて府下市町村での域内総生産（市町村 GRP）の推計は行われていない. 本稿では, 他県での推計事例や先行研究を参考に, 独自に大阪府下の市町村 GRP の長期推計を行った. 推計結果から, 大阪府経済の長期低迷の主因は大阪市であること, 大阪市以外では, 府北東部の市は停滞しているが, 府南部の第二次産業比率の高い市町村が大阪経済を下支えしていたことがわかった. また推計結果を活用した分析事例として, タイル尺度により地域間格差を計測した. 大阪府の地域間格差は, 労働生産性でみると, 関西国際空港の開港に伴い急拡大したが, その後は縮小してきていることがわかった. [Abstract] In order to accurately grasp the current state of the regional economy, we need economic statistics. In particular, gross regional product（GRP）is economic statistics that can capture comprehensive economic activities in the region. However, in Osaka Prefecture, an estimate of gross regional product of the municipality has not been performed except Osaka City. In this paper, I estimate the long-term gross regional products of municipalities in Osaka Prefecture. According to the estimation results, it is found that a long-term downturn in Osaka economy is mainly attributed to economic stagnation of Osaka City. In addition, from the calculation results of the Theil index, regional inequality of Osaka Prefecture has been found smaller in recent years
Novel method to induce chondrio-regeneration using fine-cut, block-shaped microcartilage in vivo
No full textpublisher[抄録] 軟骨組織の再生修復能は極めて低く, かつ軟骨欠損部への軟骨片移植の臨床成績はいまだ不良である. 近年では幹細胞を用いた軟骨再生誘導法が開発され臨床応用されつつある. しかし幹細胞を単離する従来法にも手技上のコストや手技に伴う細胞障害等の諸問題が存在している. また以前の研究結果より, 幹細胞を含む軟骨細胞塊を用いた軟骨再生が可能であることが分かっている. そこで本研究では従来の方法を発展させ, より実際的な新規軟骨再生誘導法の開発を試みた. まず軟骨を低侵襲的に微細加工する装置を作製し, 再生誘導に適したマイクロ軟骨サイズの検討を行った. その結果, 細胞利用率が最も高いマイクロ軟骨の至適サイズは, 100～400μm前後であると推測された. 次に塩基性線維芽細胞増殖因子（basic fibroblast growth factor/bFGF）徐放化システムを軟骨移植に併用し, 本サイトカインが移植後軟骨片に及ぼす影響を評価した. その結果, 自家軟骨移植時にbFGF徐放化システムを併用することで, SOX5活性化を伴う軟骨細胞増殖が促進されることが示された. さらにマイクロ軟骨を吸収性足場材料に接着させ, マイクロ軟骨・足場材料・bFGF徐放化システムを組み合わせた移植を行い, 細胞培養行程を介さない新規軟骨組織再生誘導を試みた. その結果, bFGF徐放化システムによるbFGF拡散の程度はマイクロ軟骨のサイズに依存し, 比較的小型の100μm群においてSOX5発現亢進を介した軟骨再生が最も効率よく行われることが示唆された. 以上の結果より, 微細加工装置により作製したマイクロ軟骨にサイトカインを併用することで軟骨組織の分化増殖を促しうることが示され, 培養行程を介さない新規軟骨再生誘導法の可能性が示唆された

No full textpublisher[要約] がん医療, 緩和医療における心理職の役割は, まだ病院によって千差万別であると言わざるを得ない. チームのスタッフや病院の理解, 患者の状態によって介入方法や場面が変わってくるためである. また心理職は国家資格化されておらず, 法的な配置基準が設定されていないため, 守備範囲が明確ではない. そのような中, 様々な研修会が開催されており, 非精神医療専門職によるがん患者ヘの精神的支援技術や知識は向上している. 心理職だから出来ることはいったい何なのか. 筆者の専門性から, がん医療, 緩和医療領域における心理職の役割について, 若干の考察を行いたい
