Personalized or Standard Duration of Dual Antiplatelet Therapy After Percutaneous Coronary Intervention
Background: Dual antiplatelet therapy (DAPT) is recommended for patients undergoing percutaneous coronary intervention (PCI), although its optimal duration remains uncertain. Objectives: The authors performed a randomized trial comparing a personalized duration of DAPT, based on a risk score, for 3, 6, or 24 months with a standard duration of DAPT for 12 months after PCI. Methods: We randomly assigned 2,107 patients undergoing PCI to receive either a personalized or a standard DAPT. The primary endpoint was a net adverse clinical event (NACE) at 24 months, defined as the composite of all-cause death, myocardial infarction, stroke, urgent target vessel revascularization, or type 2, 3, or 5 bleeding according to the Bleeding Academic Research Consortium criteria. Results: At 24 months, NACE occurred in 196 of 1,055 patients (18.6%) in the personalized DAPT group and in 232 of 1,052 patients (22.2%) in the standard DAPT group (difference, 3.54 percentage points; 95% CI: −6.99 to −0.99; P = 0.040). This difference was mainly related to decreased rates of myocardial infarction (difference, −2.29 percentage points; 95% CI: −4.43 to −0.14) and urgent target vessel revascularization (difference, −1.30 percentage points; 95% CI: −2.55 to −0.05). Bleeding occurred at similar rates between the 2 groups (difference, −0.41 percentage points; 95% CI: −2.92 to 2.10). Conclusions: In patients undergoing PCI, a personalized DAPT duration from 3 to 24 months based on a clinical risk score led to a lowered risk of NACE than standard care consisting of 12 months of DAPT. A Comprehensive Genetic Study of Classical Hodgkin Lymphoma Using Circulating Tumor DNA
: This study analyzed the genetics of classical Hodgkin lymphoma (cHL) by using circulating tumor DNA (ctDNA). Two genetic subtypes were identified, differing in genetic instability mechanisms: one subtype (64% of cases) showed a higher mutation load and a higher fraction of mutations associated with activation-induced cytidine deaminase and microsatellite instability signatures, while the other subtype (36% of cases) exhibited chromosomal instability with more somatic copy number alterations. Whole-genome duplication was more common in cHL compared to other B-cell tumors and emerged as a prognostic biomarker for patients undergoing ABVD-based therapy. Non-coding regulatory mutations, similar to those in diffuse large B-cell lymphoma, were highly prevalent in 86% of cHL. A recurrent somatic expression quantitative trait locus (seQTL) involving the BCL6 gene was found in 30% of cases. The seQTL of BCL6 aligned with accessible chromatin and increased H3K27 acetylation in cHL, disrupted PRDM1 binding, and co-occurred with BCL6 expression in cHL cells. Weak to strong expression of BCL6 was observed in 68% of cases and BCL6 expression associated with gene repression similarly in cHL and germinal center B cells. After BCL6 degradation, the core set of genes directly bound and regulated by BCL6 was derepressed in cHL and proliferation was impaired. The number and clonality of neoantigens was associated with tumor microenvironment type and response to checkpoint blockade. Finally, ctDNA analysis was suggested as a tool to distinguish ambiguous PET/CT-positive lesions post-treatment
Il dialetto di Acerno tra dinamismo e conservazione
Nel settembre del 1924 Gerard Rohlfs condusse ad Acerno (SA) l’inchiesta AIS relativa al punto 724, avvalendosi di un informatore anziano con tratti linguistici molto conservativi. L'indagine di Rohlfs offre una preziosa testimonianza di una fase arcaica del dialetto acernese, già all’epoca interessato da forti correnti innovatrici. A un secolo di distanza, una nuova inchiesta sul campo – basata su dati di parlato spontaneo e questionario di traduzione e sulla base di un campione più ampio e diversificato – consente di analizzare le caratteristiche del dialetto attuale e di confrontarle sistematicamente con quelle documentate da Rohlfs. L’approccio adottato privilegia l’osservazione della variazione e del polimorfismo, con particolare attenzione ai livelli fonetico e morfologico. I risultati evidenziano un’evoluzione profonda del dialetto locale, facendo di Acerno un caso di studio esemplare per l’analisi delle dinamiche di cambiamento nei dialetti dell’Italia meridionale
Does National Culture Drive the Value Relevance of Nonfinancial Disclosure?
The aim of this paper is to investigate the association between national culture and the value relevance of nonfinancial disclosure
provided by listed firms in Germany, Spain, France, and Italy from 2018 to 2023. The results of Hierarchical Linear Model regressions
show that, in countries where the national culture is characterized by greater power distance, individualism, masculinity,
and indulgence, nonfinancial disclosure is less value-relevant
for market investors. Instead, in countries where the national culture
is characterized by greater uncertainty avoidance and long-term
orientation, nonfinancial disclosure is more value-relevant

Decentralized Continuification Control of Multi-Agent Systems via Distributed Density Estimation
This letter introduces a novel decentralized implementation of a continuification-based strategy to control the density of large-scale multi-agent systems. While continuification methods effectively address micro-to-macro control problems by reformulating ordinary/stochastic differential equations (ODEs/SDEs) agent-based models into more tractable partial differential equations (PDEs), they traditionally require centralized knowledge of macroscopic state observables. We overcome this limitation by developing a distributed density estimation framework that combines kernel density estimation with PI consensus dynamics. Our approach enables agents to compute local density estimates and derive local control actions using only information from neighboring agents in a communication network. Numerical validations across multiple scenarios—including regulation, tracking, and time-varying communication topologies—confirm the effectiveness of the proposed approach. They also convincingly demonstrate that our decentralized implementation achieves performance comparable to centralized approaches while enhancing reliability and practical applicability
Voxels for Finite Element Analysis of Cultural Heritage objects
The paper aims at presenting a study on the integration of thermographic data with 3D models for the predictive analysis of possible degradation of wooden furniture due to changing in temperature and humidity. The project is done in collaboration with the Royal Palace of Caserta, a UNESCO site that preserve a huge, majestic collection of ancient furniture. Combining thermographic images and 3D models to proceed with thermic analysis through Finite Element Analysis (FEA) software, the aim is to identify the influence of climate changes on ancient wood to predict their behavior or possible failure in the future. The paper presents the initial results obtained with a new methodological approach and has a fundamental importance in the conservation of ancient wooden objects. AI algorithms have been used to clean the 3D point cloud (denoising), create voxel, hence volumetric models for FEA and for the integration of all the data for the predictive analysis. The innovative part of the project lays in (i) creation of voxel from cleaned denoised 3D point cloud; (ii) accuracy analysis of the models obtained compared with the original point cloud and the 3D mesh; (iii) direct use of these volumetric data in the FEA software for thermic analysis, using thermographic data as boundary conditions
Potential targeting of urokinase-type plasminogen activator receptor–formyl peptide receptor signaling to prevent recurrence in posttransplant primary podocytopathies
: Primary podocytopathies are a group of disorders characterized by nephrotic syndrome and frequent progression to kidney failure, with high rates of posttransplant recurrence. Increased expression of the urokinase-type plasminogen activator receptor (uPAR) has been associated with podocyte dysfunction in primary podocytopathies. uPAR can interact with formyl peptide receptors (FPRs) in podocytes, but the relevance of this signaling pathway in these diseases remains unclear. We retrospectively evaluated uPAR-FPR expression in renal biopsies from patients with primary podocytopathies with recurrent disease after transplantation. Upregulation of uPAR-FPR signaling was consistently observed in podocytes from kidney biopsies of patients with primary podocytopathies and in cultured podocytes exposed to sera from patients with recurrent disease. Pharmacologic inhibition of uPAR-FPR interactions with UPARANT restored podocyte plasticity, reversing cytoskeletal alterations and protecting against damage. This treatment also led to significant functional recovery in an in vitro glomerular filtration barrier model. Finally, targeting uPAR-FPR crosstalk in a zebrafish model of podocytopathy reversed podocyte foot process effacement and reduced glomerular loss of high-molecular weight dextran, effectively alleviating the disease phenotype. Our data indicate that pharmacologic targeting of the uPAR-FPR axis protects podocytes from damage and may preserve renal function in primary podocytopathies, potentially reducing recurrence after transplantation
