Actionable driver gene alterations in early-stage non-small cell lung cancer: a review

    No full text
: Early-stage (I-III) non-small cell lung cancer (NSCLC) can harbor oncogenic driver mutations that have critical implications for patient management and outcomes. Historically, molecular testing in resected NSCLC was limited, often focusing only on EGFR mutations or ALK rearrangements due to the lack of approved targeted therapies in the adjuvant setting until recently. However, with the advent of next-generation sequencing (NGS) and emerging evidence of actionable mutations in early-stage tumors, comprehensive genomic profiling is becoming increasingly relevant in this setting. Identifying these alterations is clinically significant: the presence of specific mutations can directly influence adjuvant treatment planning and refine the role of immunotherapy. Beyond guiding therapy selection, molecular profiles also provide prognostic insight: certain driver subtypes have been associated with higher recurrence risk in early-stage patients, suggesting a need for intensified surveillance. The expanding role of NGS enables personalized postoperative strategies, including tailored follow-up intervals and potential circulating tumor DNA monitoring to detect minimal residual disease. In summary, incorporating broad molecular testing in early-stage NSCLC empowers clinicians to optimize adjuvant treatment decisions and surveillance strategies, ultimately aiming to improve patient outcomes through precision oncology

Commento all’edizione digitale della corrispondenza Bonatti-Gonzaga

    No full text

Developing a persona-driven, mHealth-enabled care model for young men with hypogonadism: a methods-based approach to early risk stratification and prevention

    No full text
Background: Early-onset hypogonadism is an emerging cardiometabolic risk marker in young men, yet current care pathways remain fragmented and insufficiently tailored to patients' multidimensional needs. Digital health technologies offer an opportunity to support early risk stratification, promote self-management, and facilitate long-term prevention. This study presents a methodological framework combining Blueprint Persona modeling and multidisciplinary co-design to support the development of tailored digital health interventions. Objective: To apply a persona-based, co-design methodology to develop a patient-centered, mHealth-enabled care model for young men with hypogonadism. Methods: We screened 800 males aged 15–35 years; 55 were diagnosed with hypogonadism. Using the European Commission “Blueprint Persona” methodology, we constructed a representative persona (“Luigi”) and validated it through a multidisciplinary Focus Group (11 experts). Unmet needs were identified and translated into key functional requirements for an mHealth-supported chronic care pathway. Results: The hypogonadal cohort demonstrated a higher metabolic burden compared to eugonadal peers and reported priorities centred on sexual health, mood and musculoskeletal well-being, and prevention of premature cardiovascular risk. Despite significant psychosocial vulnerabilities, digital readiness was high. The co-design process generated a structured, mHealth-enabled intervention concept integrating teleconsultations, guided diagnostic/therapeutic steps, behavioral monitoring, and adherence-support features. Conclusions: The Blueprint Persona methodology enabled the systematic translation of clinical, psychosocial, and behavioral insights into a digitally supported care model tailored to young men with hypogonadism. This Methods Article describes the development framework underlying the proposed intervention. A prospective evaluation will assess feasibility, adherence, and impact on cardiometabolic risk and quality of life

    No full text

Hemotropic mycoplasmas and Bartonella spp. in dogs from Southern Italy: prevalence and risk factors

: Canine vector-borne diseases (CVBDs) include several zoonoses and represent an important concern where veterinarians play a major role. Although usually responsible for asymptomatic infections in animals and humans, these agents may also have fatal consequences in immunocompromised patients. In this study, different categories of dogs from the Campania region (southern Italy) were investigated for three canine vector-borne bacteria (i.e., Candidatus Mycoplasma haematoparvum, Mycoplasma haemocanis, and Bartonella spp.) by using specific real-time PCR protocols. Out of 187 dogs, 45 (i.e., 24.1%) tested positive for at least one pathogen, with Candidatus Mycoplasma haematoparvum (14.4%) being the most prevalent, followed by Mycoplasma haemocanis (12.8%) and Bartonella spp. (1.6%). Coinfections with more than one agent were observed in 4.8% of the samples. The analysis of risk variables indicated province, activity, and lifestyle as risk factors associated with greater prevalence values. Non-hunting dogs (OR = 0.11) showed a significantly lower likelihood of being positive for at least one infection. Living outdoors (OR = 3.46), in Salerno province (OR = 13), had a higher likelihood of testing positive for at least one hemotropic species. These findings revealed a wide distribution of CBVDs in dog populations of the study area, clearly underlining the necessity for effective efforts to minimize ectoparasite activity

Real-world effectiveness and safety of guselkumab in adult patients with facial and/or genital psoriasis: a 52-week analysis from the Italian multicentric GULLIVER study

    No full text
Objectives: Facial and genital plaques are common manifestations of psoriasis, are challenging to treat, and significantly impact patients' quality of life (QoL). Methods: GULLIVER is a prospective, non-interventional study conducted in 2020-2023 in Italy, aimed at examining the effectiveness, safety and QoL impact of guselkumab through 52 weeks of treatment in patients with facial and/or genital psoriasis. The primary endpoint was the percentage of patients achieving a static Physician Global Assessment (sPGA) score of ≤1 and a minimum 2-grade improvement in sPGA score at Week 52. Results: Of 351 enrolled patients, 88.6% remained on guselkumab treatment at Week 52. The proportions of patients achieving the sPGA targets in the facial and genital groups, respectively, were 83.3% and 76.5% at week 12, increasing to 93.8% and 97.9% at Week 52. Mean Dermatology Life Quality Index score improved from 12.0 ± 7.5 at baseline to 1.1 ± 2.0 at Week 52 for patients with facial psoriasis (p-value <0.001) and from 12.0 ± 6.9 to 1.6 ± 3.5 for those with genital psoriasis (p-value <0.001). Guselkumab was well-tolerated and no new safety signals were identified. Conclusions: This Italian real-world study demonstrated the high effectiveness and a good safety profile of guselkumab in treating facial and genital psoriasis
