Behcet's disease as a causative factor of cerebral venous sinus thrombosis: subgroup analysis of data from the VENOST study
WOS: 000465130500010PubMed ID: 29992235Objective This study was performed to determine the rate of cerebral venous sinus thrombosis (CVST) among cases of Behcet's disease (BD) included in a multicentre study of cerebral venous sinus thrombosis (VENOST). Methods VENOST was a retrospective and prospective national multicentre observational study that included 1144 patients with CVST. The patients were classified according to aetiologic factors, time of CVST symptom onset, sinus involvement, treatment approach and prognosis. Results BD was shown to be a causative factor of CVST in 108 (9.4%) of 1144 patients. The mean age of patients in the BD group was 35.27 years and 68.5% were men, whereas in the non-BD CVST group, the mean age was 40.57 years and 28.3% were men (P < 0.001). Among the aetiologic factors for patients aged 18-36 years, BD was predominant for men, and puerperium was predominant for women. The onset of symptoms in the BD group was consistent with the subacute form. The transverse sinuses were the most common sites of thrombosis, followed by the superior sagittal sinuses. The most common symptom was headache (96.2%), followed by visual field defects (38%). Conclusions BD was found in 9.4% of patients in our VENOST series. Patients with BD were younger and showed a male predominance. The functional outcome of CVST in patients with BD was good; only 12% of patients presenting with cranial nerve involvement and altered consciousness at the beginning had a poor outcome (modified Rankin Score 2)
Postoperative thrombotic effects of tranexamic acid in open heart surgery
Background: Following the administration of tranexamic acid, the occurrence of thromboembolic events is a controversial issue. Aims: In this retrospective cohort study, we aimed to determine the possible thromboembolic complications due to tranexamic acid as a prophylactic method in patients undergoing open heart surgery. Methods: The data of 172 adult patients undergoing open heart surgery were analyzed. All patients received tranexamic acid at a dose of 50 mg/kg. The patients were divided into 3 groups as multiple-valve surgery (group 1), coronary bypass alone (group 2), and coronary bypass with valve surgery (group 3). The amount of blood transfusion, bleeding in intra- and postoperative period, and the presence of thromboembolic events including myocardial infarction, stroke, pulmonary embolism, and deep vein thrombosis were investigated. Results: Patient demographics and duration of surgery were not significantly different in groups (p > 0.05). Hb, Htc, INR, and platelet levels of all groups did not differ significantly (p > 0.05). In total, 7 patients underwent reexploration. Postoperative DVT, stroke, and seizure were not seen at all. There was no statistically significant difference between groups in terms of the amount of blood transfusion, drainage, or peritoneal hematoma. The length of hospital stay and the mortality rate did not differ (p > 0.05). Conclusions: In patients receiving tranexamic acid infusion at 50 mg/kg dose, reexploration rates remained at 4.1% even after major cardiac surgeries. No thrombosis, stroke, or seizure were reported. Our findings support that tranexamic acid is a safe drug which has positive effect on reducing perioperative bleeding. © 2019, Royal Academy of Medicine in Ireland

Smoking stimulates the sympathetic nervous system, increases blood pressure and albuminuria, and decreases renal function. Smoking increases renal functional loss in patients with chronic kidney disease. Moreover, smoking accelerates the course of atherosclerosis, causing cardiovascular complications and premature death. Smoking decreases graft survival in kidney transplant patients. As a result, all physicians and health care providers should acknowledge all patients who smoke about the deleterious effects of smoking and continuously motivate them to stop smoking
Do Hemodialysis Patients Aware of the Importance of Using Home Sphygmomanometer?
WOS: 000477574800008Objective: We investigated the prevalence of patients owning a home sphygmomanometer among patients on hemodialysis, the type of home sphygmomanometer, the factors that affect owning a device, and the frequency of home blood pressure monitoring. Materials and Methods: The study involved 359 prevalent hemodialysis patients from five hemodialysis centers. The data were collected by means of a questionnaire. Results: A total of 272 (75.8%) patients owned a home sphygmomanometer. Most of them have been using an automated upper-arm device. The presence of diabetes and a high education level were the independent variables that affected owning of a home sphygmomanometer. Of the patients owning a home sphygmomanometer, 23.9% measured their blood pressure every day, whereas 14.7% stated that they measured it rarely or never. Of the 87 patients not having a home sphygmomanometer, 59.8% reported no reason for not buying a device. Conclusion: A significant proportion of patients on hemodialysis owns a home sphygmomanometer, the majority of which is an upper-arm device. The rate of owning the device is higher among patients with a higher education level and among patients with diabetes. The primary reason for the patients' not buying a device is lack of information about the importance of home blood pressure monitoring
The lower expression of parvalbumin in the primary somatosensory cortex of WAG/Rij rats may facilitate the occurrence of absence seizures
PubMed ID: 31153971Absence epilepsy (AE) is classified as a genetic generalized epilepsies. WAG/Rij strain of rats are regarded one of the most validated models of absence epilepsy. Studies point out the existence of hyperexcitable focus in somatosensory cortex of these rats, which has been attributed to the deficits in the GABAergic system. In the current study, we studied the changes of calcium binding proteins (CaBPs) in somatosensory cortex (S1) of the 2 and 8 month-old WAG/Rij rats and their age-matched Wistar Albino controls by investigating the expression levels of CaBPs (calbindin, calretinin and parvalbumin) in western blotting. Since WAG/Rij rats showed the low expression level of parvalbumin (PV) in western blots in comparison to Wistar Albino rats, we selectively investigated the number of PV positive neurons using the immunofluorescence staining method in order to confirm this decrement in the perioral region of somatosensory cortex (S1po). The most critical finding of this study was the age- independent reduction in the expression level of PV in the somatosensory cortex of epileptic rats as demonstrating western blotting. Nevertheless, no significant difference was found among numbers of PV + neuron in the S1po region by immunofluorescence staining concerning both of age and strain dependency. Transcervical Foley balloon catheter and vaginal prostaglandin E2 insert combination vs. vaginal prostaglandin E2 insert only for induction of labor at term: a randomized clinical trial
WOS: 000459146500016PubMed ID: 30536117PurposeTo analyze the effect of combined application of intravaginal PGE2 insert and intracervical Foley balloon catheter for induction of labor.MethodsPatients with unfavorable cervices who required induction of labor from August 2017 to December 2017 were evaluated for the study. Three hundred and ten participants were randomly assigned to study (n:155) and control group (n:155). Nine patients in study group and seven patients in control group were excluded, because they declined to participate in the study. Totally, 294 women analyzed in this prospective randomized study: Group 1 (control group): labor induction with intravaginal PgE2 vaginal insert alone (n=148) and Group 2 (study group): intracervical Foley balloon catheter insertion adjunct to the intravaginal PgE2 insert (n=146). The primary outcome of our study was the period from induction to delivery. The secondary outcome was the period from induction to active phase of labor.ResultsIn the analysis of primiparous pregnants, combination of intracervical Foley balloon catheter and intravaginal PgE2 insertion was shown to be associated with shorter duration from induction to active stage of labor (1000 vs. 585min, P0.05) and also to delivery (933.1 vs. 777.9, P>0.05, Table2) in subgroup of women with the previous vaginal delivery.ConclusionsCombined application of intracervical Foley balloon catheter and intravaginal PgE2 insert may result in a shorter time from labor induction to delivery without rising the risk of cesarean section in primiparous women with an unfavorable cervix
EFFECTS OF FLUORESCENT MARKED MAACKIA AMURENSIS-LECTIN-1 AND WHEAT GERM AGLUTIN ON THE CELL SURFACE GLYCAN PROFILES IN TWO DIFFERENT BREAST CANCER CELL LINES

In addition to all these tests and methods, the use of glycobiology has also increased significantly in cancer research. Differences in sugar residues observed on the membrane surfaces of breast cancer cell lines ER (+)-creating MCF-7 and ER (-) MDA-MB-231 were specifically marked with FITC-(Maackia Amurensis-Lectin-1) Ve FITC-(Wheat Germ Aglutinin) The intensity of radiation is discussed relatively. The aim of using this method is to clearly reveal that the differences in the sialic acid units in the membranes of breast cancer cell lines can be precisely separated in as little as two hours. Material and Method: MCF-7 and MDA-MB-231) cells were used in our study. [FITC-(Maackia Amurensis-Lectin-1) Ve FITC-(Wheat Germ Aglutin)] were purchased from the respective companies. Following this step, the FITC marked products were prepared and marked according to usage protocols. Results: FITC-labeled Maackia amurensis Lectin-1 was applied to MCF-7 and MDA-MB-231 breast cancer cell lines. Subsequently, the MDA-MB-231 cell line showed a relatively more intense radiation than the MCF-7. The FITC marked Wheat germ agglutinin was applied to the same cancer cell lines. As a result of this marking, radiation of similar intensity was detected in both cancer lines. Conclusion: These differences seen in the membranes of the MCF-7 and MDA-MB-231 cell lines used in the study are significant. Two different types of breast cancer cell types were rapidly separated using the two hour staining method applied to the cells. İnsülinoma INS-1 Hücrelerinde Hücre Çoğalması, Apoptoz ve Oksidatif Stres Üzerine Camellia sinensis Ekstraktının Etkileri
Objective: Green tea (Camellia sinensis) is one of the most consumed beverages in the world. C. sinensis consumption may prevent tumor and other diseases. Insulinomas are pancreatic islet cell tumors. In present study, the effects of the extract from C. sinensis were investigated in rat insulinoma INS-1 cells. Methods: C. sinensis leaves were collected in Rize, Turkey. The leaves were brewed in tap water for 20 and 40 minutes at 80°C. INS-1 cells were treated with different doses (1-5 mg/mL) of C. sinensis extract for 24 h. Cell viability, proliferation, death were examined. The malondialdehyde, glutathione and protein carbonyl levels were measured in INS-1 cells given C. sinensis extract. Results: The cell viability and the percentage of proliferating cell nuclear antigen immunopositive cells decreased, while the percentage of TUNEL positive cells increased in INS-1 cells treated with the extract from C. sinensis according to control group. While malondialdehyde and protein carbonyl levels in INS-1 cells treated with C. sinensis extract decreased, glutathione levels increased according to control group. Conclusion: The results indicate that the extracts from C. sinensis inhibited proliferation and caused apoptosis in INS-1 cell. C. sinensis may be natural agent for supporting the treatment of pancreatic tumorsAmaç: Yeşil çay (Camellia sinensis) dünyanın en çok tüketilen içeceklerinden biridir. C. sinensis tüketimi tümör ve diğer hastalıkları önleyebilir. İnsülinomlar pankreatik ada hücre tümörleridir. Bu çalışmada, sıçan insülinoma INS-1 hücrelerinde C. sinensis ekstraktının etkileri araştırılmıştır. Yöntemler: C. sinensis yaprakları Türkiye’de Rize’de toplandı. Yapraklar 80°C’de 20 ve 40 dakika musluk suyunda demlendi. INS-1 hücreleri, 24 saat boyunca C. sinensis ekstraktının farklı dozları (1-5 mg/mL) ile muamele edildi. Hücre canlılığı, çoğalması, ölümü incelendi. C. sinensis ekstraktı verilen INS-1 hücrelerinde malondialdehit, glutatyon ve protein karbonil seviyeleri ölçüldü. Bulgular: C. sinensis ekstraktı ile muamele edilen INS-1 hücrelerinde kontrol grubuna göre hücre canlılığı ve PCNA immünpozitif hücrelerin yüzdesi azalırken, TUNEL pozitif hücrelerin yüzdesi artmıştır. C. sinensis ekstraktı ile muamele edilen INS-1 hücrelerinde kontrol grubuna göre malondialdehit ve protein karbonil seviyeleri azalırken, glutatyon seviyeleri artmıştır. Sonuç: Sonuçlar INS-1 hücresinde C. sinensis ektraktının çoğalmayı inhibe ettiğini ve apoptoza neden olduğunu göstermektedir. C. sinensis, pankreas tümörlerinin tedavisini destekleyen doğal bir ajan olabilir
Tekirdağ İlindeki Ürologlar Üriner Sistem Taş Hastalığında Taş Analizi, Metabolik Değerlendirme ve Metafilaksi Yapabiliyor mu?
Objective: As the urolithiasis is an endemic situation in our country, the urologists are well-experienced about this situation. The prevention of urolithiasis recurrence is as important as the surgical treatment. For this reason metabolic evaluation and medical treatment of stone disease is extremely important. This study aimed to evaluate urological specialists' views of metabolic evaluation and medical treatment of urolithiasis. Thus, the main problems and the probable solutions about medical treatment and metabolic evaluation of urinary system stone disease can be evaluated. Methods: The urologists working at Tekirdag were invited to the study. A face-to-face questionnaire, which was composed of three parts, was performed to the participants. In the first section, the demographic characteristics of the participants were evaluated. In the second and third sections, the approach of participants to metabolic evaluation and medical treatment in urolithiasis were investigated, respectively. Results: A total of 29 urologist who routinely performed endourological surgery were included to the study. When we evaluated their endourological practice, 65.5% of the participants were performing 10-25 endourological surgery per month. When we asked the approach of participants about the metabolic evaluation, 34.5% reported that they did not perform metabolic evaluation to their adult patients and 65.5% did not perform to the children with urolithiasis. All participants believed the efficacy of medical treatment however, 66% of the participants were performing at less than half of their patients in clinical practice. Conclusions: The clinical practice about metabolic evaluation, stone analysis and metaphylaxis were lower than expected. Most of the participants were not able to make metabolic evaluation and stone analysis. Further studies are needed to resolve the problems associated with metabolic evaluation and metaphylaxis administration.Giriş: Üriner sistem taş hastalığı coğrafyamız açısından endemik bir durum olup, ülkemizdeki üroloji uzmanlarının üriner sistem taş hastalığı ile ilgili tecrübeleri oldukça yüksektir. Üriner sistem taş hastalığının cerrahisi kadar bu hastalığın nüksünü engellemek de bir o kadar önemlidir. Bu bağlamda, taş hastalığının metabolik değerlendirmesi ve medikal tedavisi son derece önem taşımaktadır. Bu çalışmada, Tekirdağ ilinde çalışan üroloji uzmanlarının, üriner sistem taş hastalığında metabolik değerlendirme ve medikal tedaviye bakış açılarının değerlendirmesi amaçlanmıştır. Yöntemler: Çalışmaya Tekirdağ ilindeki hastanelerde çalışan üroloji uzmanları davet edildi. Katılımcılara, toplam üç bölümden oluşan anket yüz-yüze uygulandı. Anketin ilk bölümde katılımcıların demografik özellikleri değerlendirildi. Anketin ikinci ve üçüncü bölümlerde ise sırasıyla üriner sistem taş hastalığında metabolik değerlendirme ve medikal tedaviye yönelik katılımcıların yaklaşımları incelendi. Bulgular: Klinik pratiğinde rutin endoürolojik cerrahi uygulayan toplam 29 üroloji uzmanı çalışmaya dahil edildi. Katılımcıların %65,5’i ayda ortalama 10-25 endoürolojik cerrahi uygulamaktaydı. Metabolik değerlendirme ile ilgili görüşleri sorgulandığında katılımcıların %34,5’i erişkinlerde ve %65,5’i çocuk taş hastalarında metabolik değerlendirme uygulamadığını belirtti. Tüm katılımcılar üriner sistem taş hastalığında medikal tedavinin etkinliğine inanmaktaydı. Ancak katılımcıların %66 ’sı klinik pratikte medikal tedaviyi hastaların yarısından azında uygulamaktaydı. Bunun sebebi sorulduğunda katılımcıların yarısına yakını, metabolik değerlendirme için olanaklarının olmadığını belirtti. Tartışma: Çalışmamızda, katılımcıların metabolik değerlendirme, taş analizi ve metafilaksi konusundaki uygulamalarının beklenilenden daha düşük olduğu görüldü. Katılımcıların çoğu metabolik değerlendirme ve taş analizi yapamadıklarını belirtti. Metabolik değerlendirme ve metafilaksi uygulanması ile ilgili problemlerin çözülmesine yönelik daha fazla çalışmaya ihtiyaç vardır
Ga-68 PSMA PET/MRG ile Tespit Edilen Primer Prostat Lezyonlarında SUVmaks ve Görünür Difüzyon Katsayısı Değerlerinin Korelasyonu ve Lenf Nodu Metastazlarında Önemi: Devam Eden Klinik Çalışmanın Ön Bulguları
Objectives: Gallium-68 (Ga-68) prostate specific membrane antigen (PSMA) positron emission tomography (PET) has been shown to be more accurate than multiparametric prostate magnetic resonance imaging (MRI) in detection of primary prostate lesions. Using hybrid PET/MRI we aim to detect the correlation between SUVmax and apparent diffusion coefficient (ADC) in primary prostate lesions and to assess their prognostic value in detection of lymph node (LN) metastasis. Methods: Twenty-six patients, who were diagnosed as having prostate cancer with biopsy and underwent Ga-68 PSMA PET/MRI together with biparametric prostate MRI (bpMRI) were included. SUVmax, SUVmean and ADC were recorded for index lesions drawing a region of interest (ROI) of 1 cm2 around the pixel with the highest SUVmax (ROI-1) and another ROI following borders of prostate tumor detected by bpMRI (ROI-2). Presence of LN metastasis was recorded according to PSMA PET/MRI. Results: SUVmax was inversely correlated with ADC (ROI-1: p=0.010; ROI-2: p=0.017 for b=800). SUVmax and SUVmeans were both higher in patients with LN metastasis and ADC was lower in patients with LN metastasis for ROI-1. SUVmax cut-off value of 19.8 for ROI-1 and 20.9 for ROI-2 had sensitivity and specificity of 77.8% and 76.5%, respectively for detection of LN metastasis, whereas ADC (b=800) cut-off value of 0.92x10-3 mm2/s had sensitivity and specificity of 87.5% and 76.5%, respectively. SUVmax/ADC (b=800) ratio increased the sensitivity and specificity to 100% and 82.4%, respectively. Conclusion: SUV and ADC values are inversely correlated in primary prostate lesions and the combined use of both values increases the diagnostic accuracy of hybrid PET/MRI in the detection of primary prostate lesions.Amaç: Galyum-68 (Ga-68) prostat spesifik membran antijeni (PSMA) pozitron emisyon tomografinin (PET) primer prostat lezyonunu saptamada multiparametrik prostat manyetik rezonans görüntülemeden (MRG) daha doğru olduğu gösterilmiştir. Hibrit PET/MRG kullanarak primer prostat lezyonlarında SUVmaks ve görünür difüzyon katsayısı (ADC) arasındaki korelasyonu tespit etmeyi ve lenf nodu (LN) metastazı tespitinde prognostik değerlerini saptamayı amaçlıyoruz. Yöntem: Biyopsi ile prostat kanseri tanısı almış, biparametrik prostat MRG (bpMRG) ile birlikte Ga-68 PSMA PET/MRG yapılmış 26 hasta çalışmaya alındı. İndeks lezyonların SUVmaks, SUVortalama ve ADC değerleri, hem en yüksek SUVmaks olan piksel etrafında 1 cm2 çapında ilgi alanı (ROI) çizerek (ROI-1) hem de bpMRI tarafından tespit edilen prostat tümörünün sınırlarını takip eden bir başka ROI çizerek (ROI-2) kaydedildi. LN metastazı varlığı PSMA PET/MRG’ye göre belirlendi. Bulgular: SUVmaks ve ADC arasında ters korelasyon saptandı (b=800 için ROI-1: p=0,010; ROI-2: p=0,017). SUVmaks ve SUVortalama değerleri LN metastazlı hastalarda daha yüksek bulundu ve ADC değeri ROI-1 için LN metastazlı hastalarda daha düşüktü. SUVmaks sınır değeri ROI-1 için 19,8 ve ROI-2 için 20,9 alındığında LN metastazının saptanmasında duyarlılık ve özgüllük sırasıyla %77,8 ve %76,5 olarak bulundu. Buna karşılık ADC (b=800) sınır değeri 0,92x10-3 mm2/s alındığında duyarlılık ve özgüllük sırasıyla %87,5 ve %76,5 olarak saptandı. SUVmaks/ADC (b=800) oranı, duyarlılığı ve özgüllüğü sırasıyla %100 ve %82,4’e yükseltti. Sonuç: SUV ve ADC değerleri primer prostat lezyonlarında ters korelasyon gösterir ve her iki değerin birlikte kullanımı primer prostat lezyonlarının tespitinde hibrit PET/MRG’nin tanısal doğruluğunu arttırır
