Kumar Oynama Motivasyonunun İç-Dış Kontrol Odağı İle İlişkisi
Amaç: Bu çalışmada iç-dış kontrol odağı ile kumar oynama motivasyonları arasındaki ilişkinin incelenmesihedeflenmiştir.Yöntem: Çalışmanın evrenini, İstanbul’da ikamet eden ve kumar oynayan 18 ile 55 yaş arasındaki bireyleroluşturmaktadır. Çalışmanın katılımcıları ise İstanbul’da ikamet eden ve kumar oynayan 121 bireydenmeydana gelmiştir. Çalışmada demografik bilgi formu, kumar oynama nedenleri ölçeği ve iç-dış kontrolodağı ölçeği kullanılmıştır.Bulgular: Analizler sonucu elde edilen bulgulara göre kumar oynama motivasyonun ve alt boyutları olaneğlenme/heyecanlanma, kazanma, sosyalleşme ve kaçınma) motivasyonlarının kontrol odağına göreistatiksel olarak anlamlı şekilde farklılaştığı görülmüştür. Ayrıca regresyon analizi sonucuna göre kontrolodağı, kumar oynama motivasyonunu (R²=0,327), eğlenme/heyecanlanma (R²=0,305), para kazanma(R²=0,205), sosyalleşme (R²=0,090) ve kaçınma (R²=0,154) motivasyonlarını anlamlı olarakyordamaktadır.Sonuç: Kontrol odağı ile kumar oynama motivasyonu arasında saptanan ilişki, kontrol odağının önemli birfaktör olduğunu göstermiştir. Ayrıca kumara başlama davranışının motivasyonu ile devam etmedavranışının altında daha farklı motivasyonlar olduğu anlaşılmıştır
Vertigo hastalarının psiko-sosyal faktörler açısından değerlendirilmesi
Amaç: Bu çalışmada periferik vertigo tanılı hastalar psikososyal faktörler açısından değerlendirildi.Hastalar ve Yöntemler: Nisan 2018 ve Haziran 2018tarihleri arasında başdönmesi yakınması ile kliniğimizebaşvuran ve periferik vertigo tanısı konulan 45 hasta(27 erkek, 18 kadın; ort. yaş 45.53 yıl; dağılım, 23-70 yıl)çalışmaya dahil edildi. Sonuçlar özgüven, algılanan stres,durumluk-süreklilik kaygı ve stresle başa çıkma ölçeklerikullanılarak değerlendirildi.Bulgular: Hastaların çoğunluğunun ekonomik düzeyi orta veortanın altında idi. Yaş değişkeni açısından bakıldığında 45yaş üstü hastalarda sosyal destek arama düzeyi daha yüksekidi. Evli katılımcıların sosyal destek arama boyutu puanları,bekar katılımcılara göre daha yüksek idi. Vertigo hastalarınınözgüveni arttıkça kaygı ve stres düzeyleri azaldı. Stresle başaçıkma becerileri arttıkça kaygı düzeyleri azaldı.Sonuç: Bu çalışma periferik vertigo hastalarında özgüven,stres ve kaygı düzeyleri ve stresle başa çıkma becerileriarasında anlamlı bir ilişki olduğunu gösterdi. Bu hastalaratedavi planlarının bir parçası olarak özgüven, kaygı ve streslebaşa çıkma açısından sosyal ve psikolojik destek verilmesiönerilmektedi
Single Center Experience on Screening Oncology Patients for Covid-19 Before Anti-Cancer Treatment
The most important factor in the transmission of the COVID-19 is asymptomatic carriers. We’ve tested all oncology patients , that receive anti-cancer therapy, for COVID-19. We aimed to determine the rate of asymptomatic carriers, and analyze the clinical and ra-diological findings of infected patients. Oncology patients who have indications of receiving anti-cancer treatment in the hospital were tested for COVID-19, two day prior to their treatment even if they were asymptomatic by collecting nasopharyngeal and oropharyngeal swab specimens for RT-PCR for viral RNA detection. Positive patients, underwent inspiratory phase of chest computed tomography examination. Infected patients were given the recommended treatment for COVID-19. PCR test was positive in 28 of 312 patients that we tested, and the positivity rate was 8.9%. Three patients (10.7%) had symptoms, 25 patients (89.3%) had no symptoms. Covid-19 testing before anti-cancer treatment may be recommended in order to continue their treatment without any problems and to prevent the risk of transmission due to the high rate of asymptomatics in infected patients
Relationship between ocular and computed tomography findings in patients with spina bifida
Objectives: Spina bifida is one of the most common congenital diseases in Turkey and around the world.Despite the developing diagnostic and therapeutic methods, abnormal ocular characteristicsin spina bifidapatients are still quite common. We investigated the ocular characteristics of spina bifida patients with andwithout hydrocephalus.Methods: We included 37 patients who were previously referred to the Istanbul Bilim University, Departmentof Ophthalmology and already had computed tomography (CT) scans. We retrospectively investigated thepatients’ ophthalmologic findings (refractive errors, strabismus, and optic disc characteristics) and used theirrecent CT images to measure the Evans ratios (ERs), which indirectly reflect the grade of hydrocephalus. Thepatients were divided into three groups according to their ERs (ER ? 0.3, 0.3-0.5, and ? 0.5), and then theocular characteristics of these groups were compared. In addition, the patients were divided into three groupsaccording to their ages (? 1 year, 1-3 years, and ? 3 years), and the ERs and rates of refraction defects in thesegroups were compared.Results: There was no relationship between specific ocular characteristics and ER or between age and ER.However, refraction errors were observed more frequently as patient age increased.Conclusions: The degree of hydrocephalus does not affect the ocular characteristics of patients with spinabifida, but emmetropization may be deteriorated in these patients
Ursodeoksikolik Asidin Streptozosin ile Diyabet Oluşturulmuş Farelerde Kardiyomyopatideki Antiinflamatuar ve Anti-Fibrotik Etkileri
INTRODUCTION: Diabetic cardiomyopathy is a consequenceof free fatty acid oxidation, dysfunction in mitochondria,oxidative stress and diffuse myocardial fibrosis. We aimed toinvestigate the anti-inflammatory and anti-fibrotic effect ofursodeoxycholic acid in streptozocin-induced diabetic ratmodel.METHODS: Male Sprague Dawley albino mature rats weredivided into 3 groups: Group 1 (n=10) control group; group 2(n=10) diabetic rats group; group 3 (n=10): diabetic ratstreated with ursodeoxycholic acid group. Diabetes mellitusmodel was established after injection of intraperitonealstreptozocin. Histopathological and biochemical examinationswere done after 4 weeks from heart tissues. Immunoexpressionlevels of fibronectin and TGF-? were obtained.Malondialdehyde levels were used to determine lipidperoxidation and pentraxin-3 levels were used to determineinflammation. Myocardial damage was also determined withtroponin-T and pro-BNP levels.RESULTS: Cardiac muscle cell thickness (hypertrophy), TGF? levels, fibronectin immunoexpression malondialdehyde,pentraxin-3, troponin-T and pro-BNP levels were increasedsignificantly in groups 2 and 3 when compared to controlgroup. Administration of ursodeoxycholic acid significantlyreduced inflammation and fibrosis in group 3 compared togroup 2.DISCUSSION AND CONCLUSION: In this experimentalstudy, we demonstrated the anti-inflammatory and anti-fibroticeffects of UDCA on diabetic rats and it can be a good drugcandidate for DM patientsGİRİŞ ve AMAÇ: Diabetik kardiyomyopati, serbest yağ aside oksidasyonu, mitokondrial disfonksiyon, oksidatif stress ve diffuz miyokardiyal fibrozise seconder olarak gelişmektedir. Bu deneysel çalışmada, ursodeoksikolik asidin streptozocin ile tetiklenmiş diabetik fare modelinde anti-inflamatuar ve antifibrotik etkilerini araştırmayı hedefledik. YÖNTEM ve GEREÇLER: Sprague Dawley albino 30 erişkin fare 3 gruba ayrıldı: Grup-1: kontrol grubu (n=10); Grup-2 (n=10) diabetik fare grubu; Grup-3 (n=10) ursodeoksikolik asit verilen diabetic fare grubu. Histopatolojik ve biyokimyasal değerlendirmeler 4 hafta sonra kalp dokusundan yapıldı. Fibronektin ve TGF-? immunekspresyonu, TGF-?, malondialdehid, pentraxin-3, pro-BNP ve troponin-T düzeyleri ölçüldü. BULGULAR: Fibronektin immunekspresyonu, TGF-?, pentraxin-3, troponin-t, pro-BNP ve malondialdehid düzeyleri diabetik farelerde control grubuna göre anlamlı olarak artmış saptandı. Ursodeoksikolik asidin inflamasyon belirteçlerini ve fibroz düzeyini anlamlı olarak azalttığı izlendi TARTIŞMA ve SONUÇ: Bu deneysel çalışmada, ursodeoksikolik asidin diabetik farelerde anti-inflammatuar ve anti-fibrotik etkilerini gösterdik. Diabetik hastalarda ursodeoksikolik asidin ilaç olarak kullanımı klinik olarak fayda gösterebilir

Evaluation of neurological examination, SEP results, MRI results, and lesion levels in patients who had been operated for myelomeningocele
PubMed: 33936304WOS:000613417100009Objective: Myelomeningocele is the most severe and the most frequent form of spina bifida. Most of the myelomeningocele patients undergo operations in new-born age. In terms of life quality and rehabilitation, follow-up's of these patients in the growth and development period after the operation is critical. In our study, our aim is to emphasize the correlation of SEP results with MRI results and clinical features of the myelomeningocele patients. Materials and Methods: In our study, we included 36 patients who had undergone myelomeningocele operation and have been followed-up in Istanbul Bilim University Florence Nightingale Hospital, Spina Bifida Research and Treatment Centre. Posterior tibial nerve SEP was performed on each patient and neurological examinations were done in the same session. Results were compared with clinical functional lesion levels, levels of fusion defect and ambulation levels. In order to evaluate SEP results, we used age-related reference values from Boor et al.'s study in 2008. Patients were grouped as normal, unilaterally prolonged, bilaterally prolonged, unilaterally lost, and bilaterally lost. Results: The correlations of posterior tibial nerve SEP results were significant with ambulation levels (r = 0.428, P < 0.01), clinical functional lesion levels (r = 0.477, P < 0.01) and fusion defect levels (r = -0.528 P < 0.05). The lumbar SEP results were only significantly correlated with functional lesion levels (r = 0.443 P < 0.05). Conclusions: Radiological studies are insufficient when evaluating the functionality of the central nervous system. To fully evaluate the functionality and watch the neurological development with accuracy, especially in operated patients, electrophysiological studies should be an indispensable part of myelomeningocele follow-ups
Effects of Nurse-Led Education on Quality of Life and Weight Loss in Patients Undergoing Bariatric Surgery
WOS:000489025600001Background: Although bariatric surgery is considered the most effective treatment for morbid obesity, patients' adherence to prescribed postoperative recommendations is important. Aims: To determine the effectiveness of nurse-led education with follow-up services on bariatric surgery patients' weight loss and overall quality of life. Materials and Methods: This study is a quasi-experimental research with a control group. The sample consisted of 102 patients undergoing bariatric surgery. Patients in the intervention group received education sessions as well as follow-up services by phone every 15 days after surgery for a period of 3 months. Bariatric surgery patients' overall quality of life, weight loss, and waist circumferences were measured. Results: In the intervention group, the average scores of the Short-Form Health Survey total and subdimensions were higher 3 months after the surgery, and the differences between the two groups were statistically significant (p < 0.05). Although the weight loss of patients 3 months after the surgery between the two groups was not significant, the decrease in the waist circumference of patients was significant between the two groups. Conclusions: Patient education and follow-ups resulted in significant improvements in patients' overall quality of life. However, the 3-month-long education and follow-up program did not have any effect on weight loss
