    Can hypothyroidism be a protective factor for hepatocellular carcinoma in cirrhosis?

    WOS: 000525195500066PubMed: 32176089Despite many studies, the molecular mechanisms of hepatocellular carcinoma (HCC) development remain unclear. Thyroid hormone (TH) levels may vary in many chronic diseases including cirrhosis. the aim of this study was to evaluate TH status in patients with cirrhosis and HCC and to investigate the relationship between THs and HCC development. Five hundred seventy-seven patients with cirrhosis who applied to Demiroglu Bilim University, Faculty of Medicine, Gastroenterology Department between 2004 and 2019 were included the study. Three hundred sixty-seven patients who applied to Internal Medicine Unit for general health check-up were included in the study as healthy control group. Demographic, laboratory, and imaging findings of study groups were retrospectively reviewed and recorded from hospital information system. in the cirrhosis group, 252 patients had HCC (43.67%), and 325 patients had non-HCC cirrhosis (56.33%). Free thyroxine (FT4) levels were higher in the control group than in the cirrhotic group but there was no significant difference (P = .501). Thyroid-stimulating hormone (TSH) and FT4 levels were similar between groups, while free triiodothyronine (FT3) levels were significantly different between HCC group, non-HCC cirrhosis group, and control group (P = .299 for TSH, P = .263 for FT4, P < .001 for FT3). FT3 levels were significantly higher in HCC group than non-HCC cirrhosis group, but significantly lower than control group (P < .05). Our study confirmed the presence of hypothyroidism in cirrhosis patients and clearly demonstrated a strong relationship between FT3 levels and HCC development

    Beneficial Effects of Ibuprofen on Pentylenetetrazol-induced Convulsion

    WOS: 000553577100001PubMed: 32719978Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) that is commonly used as an anti-inflammatory, anti-pyretic, and analgesic. Although some studies have focused on the anti-inflammatory and anti-pyretic properties of ibuprofen during febrile convulsions, only one has investigated its antiepileptic effects. in the present study, we aimed to investigate the effects of ibuprofen in rats exposed to pentylenetetrazol (PTZ)-induced seizures. in total, 48 rats were randomly divided in two groups: Group A for electroencephalography (EEG) recordings and Group B for behavioral assessment. All EEG recordings and behavioral assessment protocols were performed. in addition, groups were compared in terms of prostaglandin F2 alfa (PGF2 alpha) levels in the brain. We demonstrated the beneficial effects of the administration of ibuprofen in PTZ-induced seizures in rats via the following findings: spike percentages and Racine convulsion scale values were significantly lower and first myoclonic jerk (FMJ) onset times were significantly higher in the ibuprofen-administered groups. Moreover, PGF2 alpha levels in the brain were significantly higher in the saline and PTZ 70 mg/kg group than in the control and PTZ 70 mg/kg and 400 mg/kg ibuprofen groups. Our study is the first to demonstrate the beneficial effects of ibuprofen on seizures through behavioral, EEG, and PGF2 alpha brain assessments. Ibuprofen can be used for epilepsy and febrile seizures safely and without inducing seizures. However, further experimental and clinical studies are needed to confirm our results

    Pharmacologically induced absence seizures versus kindling in Wistar rats

    WOS: 000514812200005PubMed: 32232200OBJECTIVE: This study aimed to investigate the effects of gamma-butyrolactone (GBL), a prodrug of gamma-Hydroxybutyric acid-induced absence seizures on the development of kindling in Wistar rats. METHODS: Three groups of adult male Wistar rats under anesthesia were implanted with bilateral cortical recording elec- trodes for the GBL group (GBL) and/or bipolar stimulation electrodes into the right basolateral amygdala for the Kindling group (KI) alone and Kindling plus GBL group (GBL+KI). Rats in the KI and GBL+KI groups were stimulated twice daily at the afterdischarge threshold until they reached Racine's stage 5 seizure state. the animals in the GBL + group had an i.p injection of GBL 20 minutes before each electrical stimulation, and the effects of GBL-induced seizures on the development of kindling were investigated. the animals in the GBL group were injected GBL twice daily i.p. for 15 days without receiving any electrical stimulation. RESULTS: the KI animals reached stage 5 seizure stage at 12th stimulations, whereas the GBL+KI rats reached at 27th stimulations. the mean numbers of stimulations needed for the development of the first stage 3, 4, or 5 generalized seizures were significantly higher in the GBL+KI group than the KI group.     Simultaneous use of FDG-18 and 68Ga-citrate PET/CT for the differential diagnosis of sarcoidosis and malignant disease

    A 67-year-old male presented with cutaneous rash, lassitude and fatigue of three weeks. Personal history included psoriasis and sarcoidosis. Physical examination revealed macular rash on the anterior chest wall. Laboratory results were within normal limits. Chest X-ray showed normal findings. Pulmonary function tests demonstrated a mild obstructive pattern and a mild decrease in DLCO/VA. Thorax CT revealed two nodules in the right upper and middle lobe. 68Ga-citrate PET/CT did not demonstrate any active inflammatory reaction associated with sarcoidosis while 18F-FDG PET/CT revealed increased FDG uptake in the right middle lobe, upper division bronchus and in the left lower abdominal quadrant. Histopathologic examination of the colon biopsy was compatible with adenocarcinoma and bronchoscopic biopsy of the lung lesions revealed nonspecific granulomatous inflammar tion. BAL cytology was normal while BAL culture did not grow any pathologic organisms. Simultaneous use of 18F-FDG and 68Ga-citrate PET/CT was the hallmark for the final diagnosis in our patient. While FDG/PET has detected the pulmonary and colonic malignant foci in our patient, 68Ga-citrate PET/CT excluded the presence of active granulomatous inflammation of sarcoidosis. Simultaneous utility of these two imaging modalities in patients with sarcoidosis is of great importance in terms of guiding the clinician towards the accurate diagnostic pathway which is the hallmark for final diagnosis, especially in the presence of concomitant malignant disease. ©Copyright: the Author(s), 202

    Lise Öğrencilerinin Siber Zorbalığa MaruzKalma Durumlarını Etkileyen Faktörler

    Amaç: Bu çalışmada amaç, lise öğrencilerinin siber zorbalığa maruz kalma durumlarını, etkileyen faktörlerin ve siber zorbalığın etkilerinin incelenmesidir.Yöntem: Bu araştırma, İstanbul ili, Şişli ilçesinde bulunan, rastgele örneklem ile seçilmiş üç lisede okuyan, araştırmaya katılmaya gönüllü 516 öğrenci ile yürütülmüştür. Örneklem seçim yöntemine gidilmemiş, ulaşılabilen tüm öğrenciler çalışmaya dâhil edilmiştir. Okulların toplam öğrenci sayısına göre ulaşılabilme oranı %41.8 olarak hesaplanmıştır. Veriler, hazırlanan veri toplama formu, Siber Mağduriyet Ölçeği ve Rosenberg Benlik Saygısı Ölçeği ile araştırmacı tarafından toplanmıştır. Veriler SPSS programında analiz edilmiştir. Frekans ve yüzde dağılımları, tablolar hâlinde gösterilmiş, ölçekler arası korelasyon testi uygulanmıştır. Bulgular: Araştırma kapsamına giren 516 öğrencinin siber mağduriyet puan ortalamaları 27.40+4.15, benlik saygısı puan ortalamaları ise 21.11+5.81 olarak belirlenmiştir. Anne babası sağ olan, birlikte yaşayan öğrencilerin siber mağduriyet puan ortalamaları düşük, tanımadığı kişilerle mesajlaşan, cep telefonuna şifre koyan ve tanımadığı kişilerle görüntülü konuşma yapan öğrencilerin mağduriyet puanları ise diğerlerine göre daha yüksek olarak belirlenmiştir. Öğrencilerin siber mağduriyet puanları ile benlik saygısı puanları arasında negatif yönde bir korelasyon olduğu görülmüştür (r = -0.136, p<.001).Sonuç: Ergenlerin ruh sağlıklarının korunması açısından, siber mağduriyete uğrayabilme riski yüksek, anne veya babası olmayan ya da ayrı olan öğrencilerin yakından gözlenmesi, tüm öğrencilere kendilerini bu saldırılardan koruyabilmeleri konusunda eğitim ve danışmanlık verilmesi önerilmektedir. Okul sağlığı hemşireleri bu görevi yerine getirecek uygun kişiler olabilirler

    Sacroiliitis in Systemic Lupus Erythematosus Revisited

    Objectives: This study aims to investigate the prevalence of inflammatory back pain (IBP) and sacroiliitis in a systemic lupus erythematosus (SLE)population as well as the association between IBP and the frequency of human leukocyte antigen B27 (HLA-B27).Patients and methods: The study included 281 SLE patients (16 males, 265 females; mean age 39.9±11.9 years; range, 20 to 69 years) and 100 healthycontrols (HCs) (2 males, 98 females; mean age 41.2±10.1 years; range, 19 to 64 years). Participants were administered a five-item Assessment ofSpondyloArthritis international Society-IBP questionnaire. Patients and controls with IBP underwent detailed clinical and laboratory examinationsto detect sacroiliitis. Radiographic evaluations were performed by a blinded rheumatologist and radiologist. Interobserver reliability was assessedwith Cohen’s kappa test.Results: According to the questionnaire, IBP was present in 46 SLE patients (46/281; 16.3%) whereas none of the HC had IBP (p<0.001). In radiologicalassessment, 22 SLE patients (7.8%) had sacroiliitis detected by conventional X-ray and/or magnetic resonance imaging. Only one SLE patient withsacroiliitis had HLA-B27.Conclusion: Our study showed that IBP is increased in SLE patients and IBP in SLE is not associated with HLA-B27

    Evaluation of Oral Care Practices and Oral Care Habits in Cancer Patients Undergoing Chemotherapy

    Amaç: Bu araştırma kemoterapi uygulanan kanser hastalarında ağız bakımı uygulamaları ve ağız bakım alışkanlıklarının değerlendirilmesi amacıyla gerçekleştirildi. Yöntem: Bu çalışma tanımlayıcı-kesitsel nitelikte bir araştırmadır. Örneklemi kemoterapi uygulanan 120 kanser hastası oluşturdu. Ağızda yara oluşumunu önlemeye ve şikayetleri azaltmaya yönelik kanser hastalarının kullandıkları yöntemler sorgulandı. Ağız bakım uygulamaları Ağızda Yara Oluşumunu Önlemeye ve Şikayetleri Azaltmaya Yönelik Uygulamaları Değerlendirme Formu ve Ağız Hijyeni Alışkanlıklarını Değerlendirme Formu kullanılarak değerlendirildi Bulgular: Hastaların yaşları 19-82 arasında değişmektedir ve çoğu meme kanseri (%23,8) tanısı almıştır. Çalışma grubunun en sık dil üstü ve yanları, dilaltı, damak, yanak içi, diş, diş eti ve dudaklarda oral mukoz membran bütünlüğü ile ilgili şikayetler bildirdi. Kanser hastaları ağız bakımında sırasıyla en sık kloroben, sodyum bikarbonat, tuzlu su, limonlu su ve bitkisel uygulamalardan yararlanmaktadır. Örneklemin çoğu dişlerini uygun sıklıkta ve doğru teknikle fırçalamadıklarını bildirdi. Hastaların uygun yumuşaklıkta diş fırça seçmedikleri ve diş fırçalarını önerilen sıklıkta değiştirmedikleri belirlendi. Sonuç: Sonuçlar kemoterapi süresince kanser hastalarının ağız-diş sağlığının korunması ve oral mukozitin önlenmesine yönelik kanıta dayalı ağız bakımı uygulamaları konularında eğitime gereksinimi olduğunu göstermektedir.Objective: The aim of this research was to assess the oral care practices and oral care habits of the cancer patients undergoing chemotherapy. Method: It is a descriptive cross-sectional study. Th sample consisted of 120 cancer patients. The patients were asked about the practices used to prevent oral sore formation and to reduce complaints when mouth sores. Oral care practices were assessed using the Care Practices Assessment Survey for Preventing Oral Sore Formation and Reducing Oral Complaints and Oral Hygiene Habits Assessment Survey. Results: The age of the patients ranged between 19-82 years and most of them (23.8%) were diagnosed with breast cancer. The research sample reported to have oral complaints that appear on tongue and sides, sublingual, palate, cheek, teeth, gums and lips. Cancer patients commonly used chloroben, sodium bicarbonate, saline water, lemon water and herbal applications for oral care, respectively. Most of the sample reported not to brush their teeth at the appropriate frequency using the proper technique. Patients did not prefer the right bristled toothbrush and did not change their toothbrushes as often as recommended. Conclusion: The results suggest that cancer patients need training for evidence-based oral care practices to protect oral health and prevent oral mucositis during chemotherap

    The Results of Arteriovenous Graft Placement in Hemodialysis Patients: Single Center Experience

    WOS:000560704400010Objective: Arteriovenous grafts (AVG) are the options of vascular access to be used in hemodialysis patients who do not have an arteriovenous fistula option in the upper extremity. In this study, we evaluated the graft survival and complication results of 39 patients who underwent AVG in our center. Methods: Between 2010 and 2013, a total of 55 patients underwent AVG. As the records of 16 patients could not be reached, 39 patients were included in the study. The cases were retrospectively evaluated in terms of demographic data, AVG sites, survival with or without revision, factors affecting graft survival, and reasons for graft termination. Results: The mean duration of graft use without revision was 9.9 +/- 8.5 months and the mean total graft use was 13.6 +/- 11.6 months.The total graft survival rates were 82.1%, 74.1%, and 35.3% at the end of the 2nd, 12th, and 24th months, respectively. The demographic data of the patients and the use of catheter or vascular access in the same side of the graft did not have a significant effect on graft survival or early complications. There was no significant difference in terms of survival time between the graft locations. The most common cause of AVG termination was thrombosis (43.6%). Conclusion: Although the rate of AVG which developed early functional loss was high in our study, first and second year survival rates in working grafts were similar with the literature. In conclusion, AVG is one of the important vascular access options with similar survival results with fistula in hemodialysispatients. Early graft loss can be reduced with preoperative venography
