    Revision of the Australian Ozestheria Schwentner & Richter, 2015 (Crustacea: Branchiopoda: Spinicaudata) fauna, with the descriptions of 27 new species

    We provide an extensive and comprehensive revision of the Australian species of the genus Ozestheria based on an integrative taxonomic approach. In this integrative approach, molecular genetic analyses (COI, 16S and ITS-2) were combined with geometric morphometric analyses of carapace shape, ornamental features and traditional taxonomic assessments including head, thoracopod, telsonic and furcal features as well as carapace length. Initial species delimitation was based on genetic data. Morphometric differences in their carapace shape were then examined and overall morphological differences assessed. The genetic data yielded a high number of previously unknown and morphologically highly similar species. Deciding which of these correspond to the currently known and accepted species or to one of the previously synonymized species or varieties was challenging. Historic type specimens were thus assigned to the respective genetically delimited species with the aid of carapace shape variables as predictors of species identity. The taxonomic descriptions of all known and accepted species are updated based on the original type material (if available). Ozestheria dictyon is treated as a junior synonym of O. lutraria (the original description of O. dictyon was based on juveniles of O. lutraria), O. packardi is treated as a species inquirenda (the original description is inadequate and the types are lost), O. rufa is treated as a valid species (formerly treated as species inquirenda) and three former varieties of O. packardi are raised to full species status: O. cancellata comb. nov., O. minor comb. nov. and O. typica comb. nov. Furthermore, 27 species of Ozestheria new to science are formally described: O. barcaldinensis sp. nov., O. beleriandensis sp. nov., O. bourkensis sp. nov., O. carnegiensis sp. nov., O. christiani sp. nov., O. echidna sp. nov., O. frederikeae sp. nov., O. fuersichi sp. nov., O. gemina sp. nov., O. glabra sp. nov., O. henryae sp. nov., O. jiangi sp. nov., O. jonnae sp. nov., O. marthae sp. nov., O. matuwa sp. nov., O. ngamurru sp. nov., O. paralutraria sp. nov., O. pilbarensis sp. nov., O. quinlanae sp. nov., O. radiata sp. nov., O. richteri sp. nov., O. rincewindi sp. nov., O. selmae sp. nov., O. setifera sp. nov., O. sivesae sp. nov., O. timmsi sp. nov. and O. weeksi sp. nov. With now 38 Australian species of Ozestheria, our results further underline the exceptional diversity of the Australian spinicaudatan fauna, raising the total number of recognized spinicaudatan species to 97, nearly half of their worldwide known species diversity

    Using the ICH estimand framework to improve the interpretation of treatment effects in internet interventions

    The ICH E9(R1) Addendum on Estimands and Sensitivity Analysis provides a framework for defining the treatment effect a trial intends to estimate—the estimand. The addendum is widely adopted in pharmaceutical research. However, it remains underutilized in trials investigating internet-based interventions (IBIs). This manuscript introduces the addendum to IBI researchers. It concludes that estimands are essential to improve the interpretability, relevance, and validity of effect estimates derived in IBI trials

    Essays on Immigration, Population Aging, and Labor Markets

    This dissertation comprises four chapters that contribute to the literature on labor economics, with a particular focus on labor market challenges associated with population aging. As demographic change leads to growing labor shortages, two key strategies have emerged to mitigate its effects: increasing immigration and promoting higher labor force participation among older workers. The first two chapters focus on immigration as a potential response to workforce gaps in the elderly care sector, examining its effects on the career trajectories of native care workers, overall staffing levels, prices, and the availability of services. The third chapter examines the broader impact of immigration on worker health, with a focus on firm-level interactions between native and migrant workers. The final chapter explores how the design of unemployment insurance influences employment outcomes for older workers. Chapter 1 examines the effect of increased foreign staffing on the labor market outcomes of native workers in Germany’s long-term care sector. Drawing on administrative social security data and employing a shift-share instrument \citep{card2001immigrant}, the chapter shows that foreign staffing helps alleviate labor shortages but has diverging effects on native workers. For those employed in the sector just before a major immigration wave, the arrival of foreign workers is associated with transitions into jobs offering better working conditions, higher wages, and non-manual tasks. In contrast, it reduces re-employment prospects for natives with prior experience in long-term care who were unemployed at the time of the inflow. Chapter 2 analyzes how an increase in the inflow of working migrants affects prices and service provision in the long-term care sector. The identification strategy leverages a sharp rise in the number of foreign-born female residents in Germany by combining a shift-share instrument with a difference-in-differences design. Drawing on administrative data covering the universe of long-term care facilities, the chapter shows that immigration reduces the price of care across all levels of need, increases staffing levels, and expands the number of available places in nursing homes. This expansion in capacity is matched by a corresponding rise in take-up. The results can be plausibly attributed to a growing share of foreign workers in elderly care facilities and a reduction in average labor costs driven by lower wages. Chapter 3 evaluates the impact of immigration on the incidence of severe health shocks at the workplace level. Using rich linked employer-employee data from Germany, the chapter develops a firm-level instrumental variable based on policy variation to identify the causal effect of the relative size of the foreign workforce on rates of long-term sick leave. The analysis reveals that firms with a higher share of foreign workers exhibit lower rates of long-term sickness among employees. A decomposition by gender and worker origin reveals that these health improvements are concentrated entirely among native men, particularly in blue-collar sectors. In these firms, the share of native workers performing manual tasks declines, pointing to shifts in the allocation of physically demanding roles within the workforce. Chapter 4 investigates how the generosity of unemployment insurance (UI) affects labor market outcomes for older workers in Poland. Using administrative data covering the universe of unemployment benefit recipients and exploiting policy thresholds that create quasi-random variation in benefit levels and durations, the chapter applies regression discontinuity and difference-in-differences designs to estimate causal effects. Among the newly unemployed, more generous benefits lead to longer periods of benefit receipt and extended unemployment duration. For those still employed, increased generosity raises the likelihood of UI take-up, both through strategic re-timing of layoffs and through a sustained increase in unemployment entries over time

    Late Cenozoic Rise in Seawater δ11B Not Driven by Increasing Boron Adsorption

    The boron isotopic value of ancient seawater (δ11Bsw) is a prerequisite for the reconstruction of seawater pH and atmospheric CO2 concentrations. Available models and some proxy records suggest that δ11Bsw underwent a large increase during the last 45 million years. This increase has been attributed to an acceleration in sediment discharge into the ocean and the enhanced adsorption of boron on particle surfaces. However, whether global sedimentation rates have increased in the late Cenozoic is contested. Additionally, adsorption efficiency was likely modulated by secondary factors related to seawater chemistry and the sedimentary mineral content, that could have counteracted changes in sedimentation rates. Here we revisit the controls on boron adsorption over the last 100 million years. We found that changes in the seawater concentration of dissolved inorganic carbon (HCO3−, CO32−) and major ions (Ca2+, Mg2+, SO42−) had a negligible impact on boron adsorption. Instead, the sedimentary mineral assemblage and the acidity of seawater were important subordinate factors. By considering several possible sediment production scenarios, we propose that the ability of sediment to adsorb boron was lower in the Cretaceous but has remained similar to the present‐day since the Eocene. When these results are incorporated into a seawater model, δ11Bsw exhibits a step‐wise enrichment over the Cenozoic, that is, at times, 2‰ above previous model results. Our analysis precludes a dominant role of adsorption in the boron isotope cycle of the late Cenozoic, but nevertheless supports the view that δ11Bsw was lower than today for the last 60 million years

    Die nTMS gestützte Traktographie bei Patient:innen mit Tumoren des Hirnstamms

    Objective: Surgery on the brainstem level is associated with a high-risk postoperative morbidity due to the close spatial relation and density of vital neuronal structures. Recently, we have introduced the combination of navigated transcranial magnetic stimulation (nTMS) and diffusion tensor imaging (DTI) tractography as a new tool to define functionally relevant fiber tracts on the brainstem level to support operative planning and risk stratification in brainstem cavernomas. In this study we now aim to evaluate this method and prove its clinical impact for the surgery of intrinsic brainstem tumor and metastasis. Methods: Patients with brainstem tumors were examined preoperatively with nTMS and DTI tractography. The nTMS stimulation spots that elicited EMG responses on the cortical level were used as seed points for the DTI tractography. A fractional anisotropy (FA) value of 75% of the individual FA threshold (FAT) was used to track descending corticospinal (CST) and -bulbar tracts (CBT). The distance between the tumor and the somatotopic tracts (hand, leg, face) was defined and diffusion parameters (fractional anisotropy (FA), apparent diffusion coefficient (ADC)) were correlated to the patients’ outcome. Results: 17 patients were enrolled in this study, of which 10 patients underwent surgi-cal resection, 6 patients received a stereotactic biopsy and 1 patient received con-servative treatment. The tumor entities were mainly glioma (WHO I-II° 35.3%, III-IV°: 35.3%) followed by meningioma (17.7%) and metastasis (11.7%). In all patients nTMS mapping and somatotopic tractography were performed successfully. The FA values revealed a tract alteration by the tumor (p=0.049) and correlated with clinical symp-toms. This effect was even higher in infiltrative tumors (p=0.009). A tumor-tract distance (TTD) above 2mm was the critical limit to achieve a complete tumor resection. Conclusion: nTMS based DTI tractography combined with local diffusion analysis has a great potential to stratify the risk for postoperative morbidity and might therefore improve the outcome in patients with brainstem tumors. Our technique allows for an individually customized treatment strategy in these patients.Hintergrund: Aufgrund der hohen Dichte lebensnotwendiger neuronaler Strukturen be- steht bei neurochirurgischen Interventionen im Bereich des Hirnstammes ein erhöhtes Risiko für ein neues postoperatives Defizit. Die Faserbahndarstellung basierend auf der navigierten transkraniellen Magnetstimulation (nTMS) nimmt bereits eine wichtige Rolle in der präoperativen Planung bei Hirnstammkavernomen ein. Jedoch wurde diese Me- thode noch nicht bei Menschen mit Hirnstammtumoren untersucht. Unser Ziel war es, bei diesen Fällen mittels nTMS basierter Traktographie, basierend auf Diffusionstensor Bild- gebungen (DTI), eine verlässliche Darstellung der absteigenden motorischen Faserbah- nen des Hirnstammes zu generieren. Zusätzlich sollten die segmentalen Diffusionspara- meter und das klinische Potential der Methoden evaluiert werden. Methodik: Die Patient:innen wurden präoperativ mittels nTMS und DTI untersucht. Die Stimulationspunkte mit richtig-positiven EMG-Antworten wurden als Ausgangspunkte des standardisierten DTI Fiber Trackings verwendet. Zur Generierung wurde jeweils ein spe- zifischer Wert der fraktionellen Anisotropie (FA) errechnet. Diesen setzten wir bei 75% des individuellen FA Grenzwerts (FAT). Distanzberechnungen zwischen den Corticospi- nalen Faserbahnen (CST), den Corticobulbären Faserbahnen (CBT) und dem Tumor wurden durchgeführt. Die Diffusionsparameter, FA und der apparent diffusion coefficient (ADC) wurden mit dem Outcome der Patient:innen verglichen. Ergebnisse: 17 Fälle konnten in die Studie eingeschlossen werden, 10 erhielten eine chirurgische Resektion und 6 erhielten eine stereotaktische Biopsie. Die Diagnosen un- terteilten sich in Gliome (WHO I-II° 35,3%, III-IV°: 35,3%), Meningiome (17,7%) und Me- tastasen (11,7%). Das nTMS Mapping sowie die somatotopische Traktographie konnten bei allen Patient:innen erfolgreich durchgeführt werden. Die FA Werte des Hirnstamms zeigten eine Affektion durch den Tumor (p=0,049) und gingen mit den jeweiligen Symp- tomen einher. Infiltrativ wachsende Tumore wiesen eine höhere Affektion (p=0,009) und einen verschlechterten postoperativen Outcome auf. Bei Patient:innen mit einer Tumor- Trakt Distanz von unter 2mm konnte lediglich eine subtotale Resektion erzielt werden. Postoperativ zeigten sich keine neuaufgetretenen motorischen Defizite der Extremitäten. Conclusio: Die nTMS basierte DTI Traktographie kombiniert mit der lokalen Analyse der Diffusionsparameter hat ein großes Potential, die postoperative Morbidität und Outcomes bei Hirnstammtumoren zu stratifizieren. Unsere Technik spiegelt strukturelle Veränderun- gen wider und verbessert die chirurgische Planung bei Hirnstammtumoren und gewähr- leistet somit eine individuell zugeschnittene Therapiestrategie für diese Patient:innen

    New functions of the VEGF-R1 signalling pathway in chorioidal neovascularization

    Age-related macular degeneration (AMD) is one of the leading causes of blindness worldwide. Exudative AMD is treated with intravitreal injections of various anti-vascular endothelial growth factor (VEGF) agents. However, the anti-VEGF-A/anti-placental growth factor (PGF) strategy for the treatment of AMD has repeatedly shown advantages compared to monotherapy with anti-VEGF-A. The aim of this work was to find evidence for new functions of the VEGF-R1 signalling pathway and the role of PGF in choroidal neovascularisation (CNV). For this purpose, the interaction of monocytes (MO) and the PGF/VEGF receptor 1 (R1) signalling pathway was investigated with an additional focus on potential VEGF-R1-dependent as well as VEGF-R1-independent PGF signalling. Stimulation experiments were performed with isolated murine bone marrow MO. The cells were treated with antibodies (anti-PGF or anti-VEGF-R1) and stimulated in different milieus (VEGF-A, apolipoprotein E3 (ApoE3), PBS). The following components of PGF/VEGF-R1 signalling or mediators of the immune response were examined for potential dependence on PGF and/or VEGF-R1: PGF, VEGF-R1, VEGF-R2, interleukin (IL)-6, IL-1β2 and IL-4R. The expression of the target genes was measured using quantitative real-time polymerase chain reaction (real-time qPCR). In addition, immunocytochemistry (ICC) was used to obtain a visual indication of the specification of MO. In summary, we show that MO regulate the gene expression of messenger substances such as PGF, VEGF-R1 and -R2, under different environmental conditions in an autocrine manner. The gene expression analysis of IL-4R suggests that PGF activates a VEGF-R1 independent signalling pathway. IL-6 and IL-1β2 appear to be expressed independently of the PGF/VEGF-R1 signalling pathway. The heterogeneity of MO can be demonstrated by ICC.Die altersbedingte Makuladegeneration (AMD) ist eine der weltweit führenden Ursachen für Erblindung. Die exsudative AMD wird mit intravitrealen Injektionen verschiedener Wirkstoffe gegen den vaskulären endothelialen Wachstumsfaktor (VEGF) behandelt. Die anti-VEGF-A/anti-Plazenta-Wachstumsfaktor (PGF) -Strategie zur Behandlung der AMD konnte wiederholt Vorteile gegenüber einer Monotherapie mit anti-VEGF-A aufzeigen. Das Ziel dieser Arbeit war es, Hinweise auf neue Funktionen des VEGF-R1-Signalweges und die Rolle von PGF bei der chorioidale Neovaskularisation (CNV) zu finden. Hierfür wurde das Zusammenspiel von Monozyten (MO) und dem PGF/VEGF-Rezeptor 1 (R1) -Signalweg mit einem zusätzlichen Fokus auf ein potenziell VEGF-R1-abhängiges als auch VEGF-R1-unabhängiges PGF-Signalling untersucht. Es erfolgten Stimulationsexperimente an isolierten murinen Knochenmarksmonozyten. Die Zellen wurden mit Antikörpern (anti-PGF oder anti-VEGF-R1) behandelt und in verschiedenen Milieus (VEGF-A, Apolipoprotein E3 (ApoE3), PBS) stimuliert. Folgende Komponenten des PGF/VEGF-R1-Signalling bzw. Mediatoren der Immunantwort wurden auf eine potenzielle Abhängigkeit von PGF und/oder VEGF-R1 untersucht: PGF, VEGF-R1, VEGF-R2, Interleukin (IL) -6, IL-1β2 und IL-4R. Die Messung der Genexpression der Zielstrukturen erfolgte mittels quantitativer Echtzeit-Polymerase-Kettenreaktion (real-time qPCR). Ergänzend wurde mittels der Immunzytochemie (ICC) versucht einen visuellen Hinweis auf die Spezifizierung von MO zu gewinnen. Zusammenfassend konnte nachgewiesen werden, dass MO innerhalb verschiedener Umgebungsbedingungen über ein autoregulatives Stimulationssystem verfügen und die Genexpression von Botenstoffen wie PGF, VEGF-R1 und -R2 selbstständig modulieren. Die Genexpressionsanalyse des IL-4R weist auf einen weiteren Signalweg von PGF hin, der unabhängig von VEGF-R1 ist. IL-6 und IL-1β2 erscheinen unabhängig vom PGF/VEGF-R1-Signalweg exprimiert zu werden. Die Heterogenität der MO kann durch ICC aufgezeigt werden

    Urinary T cells as biomarker of ANCA-associated vasculitis

    Anti-Neutrophile cytoplasmatische Antikörper (ANCA) verursachen durch Bindung an und Aktivierung von neutrophilen Granulozyten und Monozyten eine nekrotisierende Inflammation der kleinen Blutgefäße. Dieses seltene Erkrankungsbild wird als ANCA-assoziierte Vaskulitis (AAV) bezeichnet, betrifft häufig die Nieren und kann zu einem raschen, lebensbedrohlichen Verlust der Organfunktion führen. Aufgrund des unbehandelt tödlichen und schubartigen Verlaufs ist eine starke, langfristige immunsuppressive Therapie erforderlich. Diese ist jedoch mit potenziell schweren Nebenwirkungen, insbesondere Infektionen, verbunden und ist hauptverantwortlich für Morbidität und Mortalität. Bisher fehlen nicht-invasive Biomarker, die die Krankheitsaktivität verlässlich und seriell bestimmen, um den individuellen Therapiebedarf festzulegen. Die durchflusszytometrische Analyse von Urin-Leukozyten findet zunehmend Verwendung in der Untersuchung inflammatorischer Nierenerkrankungen, ist jedoch bei AAV bisher nicht etabliert. Unter der zentralen Hypothese, dass Urin-T Zellen das renal inflammatorische Milieu reflektieren und einen verlässlichen Biomarker für Krankheitsaktivität darstellen, wurden über einen Zeitraum von 2 Jahren an 4 internationalen Zentren 87 AAV-Patient:innen eingeschlossen und T Zell Populationen durchflusszytometrisch in Blut und Urin analysiert. Diese Ergebnisse wurden mit Parametern der klinischen Routine, histopathologischen Befunden und Daten aus der Langzeitbeobachtung verglichen. Es zeigte sich, dass die Anzahl von Urin-T Zellen, insbesondere CD3+ und Treg, bei renal aktiver Vaskulitis signifikant erhöht ist und die Krankheitsaktivität mit verlässlichen Biomarkereigenschaften bestimmen kann. Diese zeigten bessere Sensitivitäts- und Spezifitätswerte als traditionelle und neuartige Marker. Darüber hinaus fand sich eine Diskordanz zwischen den T Zell Subpopulationen in Blut und Urin sowie ein Zusammenhang zwischen Urin Zellzahl und histopathologischem Aktivitätsgrad. In der Langzeitbeobachtung war anhand der Treg und TH17 Zellzahl bei akut Erkrankten die Prädiktion des klinischen Ansprechens, bzw. bei stabiler Erkrankung das Auftreten von erneuten Krankheitsschüben möglich. Zusammengefasst bieten Urin T Zellen verlässliche Biomarkereigenschaften für die Bestimmung der renalen Krankheitsaktivität und haben dabei prognostischen Wert. Diskordante T Zell Populationen in Blut und Urin sowie die Assoziation zwischen histopathologischem Aktivitätsgrad und Urin T Zellzahl legen nahe, dass Urin-T Zellen die renale Inflammation spezifisch reflektieren. Daher stellt die durchflusszytometrische Quantifizierung von Urin-T Zellen einen vielversprechenden nicht-invasiven Biomarker für die renale Krankheitsaktivität bei AAV dar. Zur Validierung dieser Ergebnisse und als erster Schritt in Richtung einer Verwendung bei der Diagnosestellung und Prädiktion von renalen Schüben folgte die prospektive PRE-FLARED Studie, deren Ergebnisse aktuell noch ausstehen.Anti-neutrophil cytoplasmic antibodies (ANCA) cause necrotizing inflammation of small blood vessels through binding to and consequent activation of neutrophils and monocytes. This rare disease, known as ANCA-associated vasculitis (AAV), often affects the kidneys, leading to a rapid, life-threatening loss of organ function. As the disease is fatal if left untreated, strong immunosuppressive therapy is required. However, AAV often shows a relapsing course and, therefore, requires prolonged treatment. This long-term immunosuppression is associated with severe side effects, especially infections which are mainly responsible for morbidity and mortality. Non-invasive biomarkers to reliably monitor disease activity, establish individual need for treatment and enable individually tailored treatment are still lacking. Flow cytometric analysis of urinary leukocytes is increasingly used in the investigation of inflammatory kidney diseases but has not yet been described in detail for AAV. Therefore, the hypothesis of this study was that urinary T cells reflect the renal inflammatory milieu and represent a reliable biomarker for disease activity. Over a period of 2 years, 87 AAV patients from 4 international centers were recruited and T cell populations in blood and urine were analyzed by flow cytometry. These results were compared with routine clinical parameters, histopathologic findings, and data from long-term follow-up. Patients with active renal vasculitis showed significantly increased urinary T cells, especially CD3+ and Treg. These could reliably determine disease activity and showed better biomarker characteristics than traditional and novel markers. Furthermore, there were discordant proportions of T cell subpopulations in blood and urine as well as an association between urinary cell count and histopathologic level of activity. Lastly, long-term observation showed that Treg and TH17 cell count allowed prediction of clinical response in active patients and the occurrence of new relapses in patients with stable disease. In summary, urinary T cells offer reliable biomarker properties for the determination of renal disease activity and have prognostic value. Discordant T cell populations in blood and urine and the association between urinary T cell count and vasculitis activity on histopathologic analysis indicate, that T cells in urine reflect renal inflammation. Thus, flow cytometric quantification of urinary T cells represents a promising non-invasive biomarker for renal disease activity in AAV. The prospective PRE-FLARED follow-up study investigating patients in stable remission could represent a first step towards validation of these results and establishment of this marker for diagnosis and prediction of renal relapses

    GPs’ role in prevention and care for sexually transmitted infections in women – a qualitative interview study with general practitioners in Berlin

    Sexuell übertragbare Infektionen (STI) sind weltweit häufig und verlaufen meist asymptomatisch. Frauen sind von Komplikationen überproportional betroffen und Diagnosen werden später gestellt, Personen mit Migrationserfahrung und LBTQI-Personen werden von der Versorgung schlechter erreicht. Die deutsche ambulante Versorgungslandschaft ist segmentiert, Sexualanamnesen werden hier selten erho-ben und STI sind teils stigmatisiert. Das Gynäkologie-basierte System ist spezifisch für Deutschland und Polen, Hausärzt:innen (HÄ) könnten niedrigschwellige, gruppenübergreifende Versorgung leisten. In dieser Promotion wird daher die Rollenwahrnehmung von HÄ in der STI-Versorgung von Frauen untersucht, und ergänzend strukturelle und persönliche Bedingungen für variierende Versorgungsstrategien analysiert. Es wurden 19 qualitative, teilstandardisierte Leitfadeninterviews mit HÄ in Berlin geführt. Zwischen 10/2020 und 09/2021 wurden mit verschiedenen Samplingstrategien 75 Praxen kontaktiert, eingeschlossen wurden auch 6 Hausärzt:innen in infektiologischen Schwerpunktpraxen (HÄiS). Die Auswertung der Ergebnisse erfolgte mit verschiedenen inhaltsanalytischen Techniken. Zuständigkeiten, Abrechnungsmöglichkeiten und diagnostische Optionen waren oft unklar, zugleich standen viele der befragten HÄ unter Zeitdruck. Innerhalb erschwerter struktureller Bedingungen zeigten sich drei Typen der hausärztlichen STI-Versorgung: die Gruppe der sich abgrenzenden HÄ überwies Patient:innen sofort an andere Stellen, zeigte stigmatisierende Wahrnehmungen, hatte keine Fort- oder Weiterbildungen zum Thema besucht, sah keinen Bedarf für hausärztliche STI-Versorgung und fühlte sich nicht verantwortlich. Eine weitere Gruppe von HÄ leistete selektive Versorgung in Einzelfällen, zeigte ambivalentere Einstellungen und sah sich als erste Ansprechpartner:in, hatte aber ebenfalls keine Fort- oder Weiterbildungen besucht. Sie sahen selten und eher bei Männern Bedarf für hausärztliche STI-Versorgung und zeigten ein intermediäres Verantwortungsgefühl. Dagegen zeigten HÄ, die bereits STI-Primärversorgung auch für Frauen leisteten offene Einstellungen zu Sexualität und STI, hatten relevante Fortbildungen besucht, nahmen einen Bedarf für Versorgung wahr und sahen sich in der Verantwortung, diesen abzudecken. Die konkreten Versorgungsstrategien variierten entsprechend erheblich. Im eigentlichen Sinne zuständig für STI-Versorgung von Frauen fühlte sich niemand, die HÄ sahen sich jedoch stark in der Rolle der ersten Ansprechpartner:innen und äußerten differenzierte Wünsche für die Verbesserung der Versorgung. HÄ können und wollen niedrigschwellige STI-Versorgung leisten; auch von Patient:innen wird dies gewünscht. Dem stehen strukturelle Schwierigkeiten und fehlende Kenntnisse, aber auch unzureichend reflektierte, persönliche Einstellungen der HÄ im Wege. Durch angepasste Leitlinien und Weiterbildungen könnten diese Aspekte adressiert werden.Sexually transmitted infections (STIs) are common worldwide and often asymptomatic. Women are disproportionately affected by complications and late diagnoses, people with experiences of migration and LBTQI people are poorly reached by STI care. German ambulatory STI care provision is fragmented, sexual histories are rarely taken and STIs are sometimes stigmatised. The gynaecology-based system is specific to Germany and Poland; general practitioners (GPs) could offer accessible care across groups. This dissertation therefore investigates the role perception of GPs in the STI care of women and analyses structural and personal conditions leading to different care strategies. With GPs in Berlin, 19 qualitative, semi-structured guide-assisted interviews were conducted. Between 10/2020 and 09/2021, 75 practices were contacted using different sampling strategies, 6 GPs with an additional specialisation in infectious diseases were included. The results were analysed using various content analysis techniques. Responsibilities, remuneration and diagnostic options were often unclear, while at the same time many of the GPs surveyed were under time pressure. Within difficult structural conditions, three types of GP STI care provision emerged: the group of GPs distancing themselves immediately referred patients to other services, showed stigmatising perceptions, had not sought any further training on the topic, saw no need for GP STI care and did not feel responsible. Another group of GPs provided selective care in individual cases, showed more ambivalent attitudes, and saw themselves as the first point of contact but had also not attended any further training. They rarely saw a need for GP STI care but were more likely to see it in men and showed an intermediary sense of responsibility. In contrast, GPs who already provided STI primary care for women showed open attitudes towards sexuality and STIs, had attended relevant training courses, perceived a need for care and felt responsible for providing it. The specific care strategies varied considerably accordingly. No one felt formally responsible for STI care for women, but the GPs perceived themselves strongly in the role of first contact person and expressed nuanced wishes for improving care. GPs can and want to provide accessible STI care; this is also requested by patients. This is hindered by structural difficulties and a lack of knowledge, but also by insufficiently reflected personal attitudes on the part of GPs.     Entwicklung und Charakterisierung von neuen Krankheitsmodellen für Familiäre Zerebrale Arteriovenöse Malformationen

    Brain arteriovenous malformations (bAVMs) are fragile tangles of dysplastic blood vessels in the brain that pose a significant lifetime risk of hemorrhagic stroke. Familial bAVMs are commonly associated with hereditary hemorrhagic telangiectasia (HHT), a multisystem vascular disorder caused by mutations in the genes ENG (endoglin, HHT type I), ACVRL1 (activin A receptor like kinase 1 or ALK1, HHT type II), and SMAD4 (SMAD family member 4). The current lack of effective medical treatment to either prevent the formation of bAVMs or halt their progression poses a significant therapeutic gap. This gap is further widened by challenges in developing robust disease models, which has impeded advances in effective treatment. This research addresses the challenges of existing disease models for bAVMs by introducing two novel experimental mouse models for HHT type I and type II, with the overarching aim of advancing our comprehension of bAVM pathobiology and creating avenues for translational drug discovery. For the HHT type I model, Eng-conditional knockout mice were treated systemically with tamoxifen to induce bAVMs. For the HHT type II model, a novel experimental approach was developed to deliver 4-hydroxytamoxifen stereotactically into the brains of Alk1-conditional knockout mice. Both models showed a high incidence of bAVM formation (88% in Eng mice and 73% in Alk1 mice) upon visualization with systemic vascular latex dye perfusion. Comprehensive histologic analyses revealed that the bAVMs closely simulate human disease characteristics, including microhemorrhages, neuroinflammation, and aberrant blood vessel morphology. Notably, inducing the deletion of genes that cause HHT during the first two postnatal weeks significantly increased bAVM formation efficiency, correlating with increased angiogenic activity, as indicated by the increased expression of VEGFR2 (vascular endothelial growth factor receptor 2) during early postnatal development. Although the systemic Eng gene deletion resulted in death after arteriovenous malformations developed in internal organs, the targeted Alk1 gene deletion yielded a robust longitudinal bAVM model, with nidal stability observed at up to 9 months on serial magnetic resonance imaging. The development and comprehensive characterization of these two novel HHT models thus offer valuable insights into the pathobiology of bAVMs, particularly highlighting the role of angiogenesis in their development. Furthermore, these models provide a powerful discovery platform for advancing our understanding of disease mechanisms and for accelerating therapeutic discovery.Zerebrale arteriovenöse Malformationen (bAVMs) sind fragile Fehlbildungen dysplastischer Blutgefäße im Gehirn, die ein signifikantes Lebenszeitrisiko für hämorrhagischen Schlaganfall darstellen. Familiäre bAVMs sind häufig mit hereditärer hämorrhagischer Teleangiektasie (HHT) assoziiert, einer multisystemischen Gefäßerkrankung, die durch Mutationen in den Genen ENG (Endoglin, HHT Typ I), ACVRL1 (Aktivin-A-Rezeptor-ähnliche Kinase 1 oder ALK1, HHT Typ II) und SMAD4 (SMAD-Familienmitglied 4) verursacht wird. Der derzeitige Mangel an wirksamen medizinischen Behandlungen, um entweder die Bildung von bAVMs zu verhindern oder ihr Fortschreiten zu stoppen, stellt eine bedeutende therapeutische Lücke dar. Diese Lücke wird durch Herausforderungen bei der Entwicklung robuster Krankheitsmodelle weiter vergrößert. Diese Forschung adressiert die Herausforderungen bestehender Krankheitsmodelle für bAVMs durch die Entwicklung von zwei neuen experimentellen Mausmodellen für HHT Typ I und Typ II, mit dem übergeordneten Ziel, unser Verständnis der Pathobiologie von bAVMs zu erweitern und Wege für die translationale Arzneimittelforschung zu bahnen. Um bAVMs im HHT-Typ-IModell zu induzieren, wurden konditionale Eng-Knockout-Mäuse systemisch mit Tamoxifen behandelt. Für das HHT-Typ-II-Modell wurde ein neuer experimenteller Ansatz entwickelt, bei dem 4-Hydroxytamoxifen stereotaktisch in die Gehirne von konditionalen Alk1-Knockout-Mäusen injiziert wurde. Beide Modelle zeigten eine hohe Inzidenz der bAVM-Bildung (88% bei Eng-Mäusen und 73% bei Alk1-Mäusen) bei der Visualisierung mit systemischer vaskulärer Latexfarbstoffperfusion. Umfassende histologische Analysen ergaben, dass die bAVMs die Charakteristika der menschlichen Krankheit aufweisen, einschließlich Mikroblutungen, Neuroinflammation und abnormer Blutgefäßmorphologie. Bemerkenswerterweise führte die Induktion der Deletion von HHT-Genen in den ersten beiden postnatalen Wochen verstärkt zur bAVM-Bildung, was mit einer erhöhten angiogenen Aktivität korrelierte, wie die erhöhte Expression von VEGFR2 (vaskulärer endothelialer Wachstumsfaktorrezeptor 2) während der frühen postnatalen Entwicklung zeigt. Obwohl die systemische Eng-Gendeletion aufgrund der AVM-Entwicklung in inneren Organen zum Tode führte, ergab die lokale Alk1-Gendeletion ein robustes longitudinales bAVM-Modell mit Stabilität der Nidi bis zu 9 Monaten bei serieller Magnetresonanztomographie. Die Entwicklung und umfassende Charakterisierung dieser beiden neuen HHT-Modelle bieten somit wertvolle Einblicke in die Pathobiologie von bAVMs und heben insbesondere die Rolle der Angiogenese in ihrer Entwicklung hervor. Darüber hinaus bieten diese Modelle eine leistungsstarke Entdeckungsplattform, um unser Verständnis der Krankheitsmechanismen zu vertiefen und die therapeutische Entdeckung zu beschleunigen
