At-home psychotherapy for older adults with depression and long-term care needs: Experiences of Psychotherapists in Germany
Objective: This study provides insight into the experience of psychotherapists using at-home cognitive-behavioral therapy with older adults (aged 65 and above) who have long-term care needs and depression.
Methods: Grounded theory methodology was employed to develop a hierarchically organized system of categories was developed based on semi-structured interviews with 16 female psychotherapists (ranging in age from 26 to 70) and ten associated case studies.
Results: Participants experience dilemmas in at-home therapy with older adults with long-term care needs as triggers for their own professional development. These dilemmas included (1) pushing for change vs. acknowledging limitation, (2) providing help vs. maintaining boundaries, and (3) being a guest in the patients' home vs. being a host the psychotherapeutic setting (Demmerle et al., 2023). These dilemmas are exacerbated by the lack of a shared understanding of therapy between psychotherapist and patient and by the immediate confrontation with existential suffering. Participants integrate elements initially perceived as contradictory and mutually exclusive into their therapeutic approach in a balanced manner. Building on this, they adapt their professional self-image and therapeutic approach. The participants experience additional time investment, financial losses, logistical challenges, and uncertainties about legal frameworks as major barriers to translating this work into mainstream care. Adapted structures, specific qualifications, and expanded opportunities for exchange and reflection are seen as necessary.
Conclusion: Based on the results, it appears advisable to prepare psychotherapists for working with older adults in need of care and the potential experience of dilemma situations through training and further education, as well as to incorporate this into supervision processes. Structural adjustments are necessary to enable comprehensive care for this essential and growing target group.Zielsetzung: Die Studie gibt einen Einblick in die lebendige Erfahrung von Psychotherapeut:innen mit aufsuchender kognitiver Verhaltenstherapie (KVT) bei pflegebedürftigen, älteren Erwachsenen (+65) mit Depression, die zu Hause leben.
Methode: Unter Anwendung der Methodik der Grounded Theory wurde ein hierarchisch organisiertes Kategoriensystem entwickelt, das auf halbstrukturierten Interviews mit 16 Psychotherapeutinnen (zwischen 26 und 70 Jahren) und zehn zugehörigen Fallstudien basiert.
Ergebnisse: Die Teilnehmerinnen erleben Dilemmata in der aufsuchenden Arbeit mit pflegebedürftigen Älteren als Auslöser für ihre eigene berufliche Entwicklung. Zu den Dilemmata gehören das (1) Drängen auf Veränderung vs. Anerkennen von Begrenzung, (2) Leisten von Hilfe vs. Wahren von Grenzen und (3) Gast im Zuhause der Patient:innen vs. Gastgeber:in des psychotherapeutischen Rahmens (Demmerle et al., 2023). Die Dilemmata werden durch das Fehlen eines gemeinsamen Therapieverständnisses zwischen Psychotherapeut:in und Patient:in sowie durch die unmittelbare Konfrontation mit existenziellem Leid verstärkt. Die Teilnehmerinnen integrieren die anfänglich als gegensätzlich und sich ausschließend wahrgenommenen Elemente balanciert in ihre therapeutische Haltung. Darauf aufbauend passen sie ihr professionelles Selbstverständnis und ihre therapeutische Herangehensweise an. Sie erleben zusätzliche zeitliche Investitionen und finanzielle Verluste, logistische Heraus¬forderungen und Unsicherheiten hinsichtlich der rechtlichen Rahmenbedingungen als wesentliche Hindernisse bei der Übersetzung dieser Arbeit in die Regelversorgung. Angepasste Strukturen, spezifische Qualifizierungen sowie erweiterte Austausch- und Reflexionsmöglichkeiten werden als notwendig erachtet.
Schlussfolgerung: Vor dem Hintergrund der Ergebnisse erscheint es geboten, Psychotherapeut:innen auf die Arbeit mit älteren, pflegebedürftigen Erwachsenen und das potenzielle Erleben von Dilemma-Situationen in Aus- und Weiterbildung vorzubereiten und dies in Supervisionsprozessen zu reflektieren. Strukturelle Anpassungen sind notwendig, um eine flächendeckende Versorgung dieser wesentlichen und wachsenden Zielgruppe zu ermöglichen
Mackinawite partial oxidation to green rust produces a large, abiotic uranium isotope fractionation
Low-oxygen settings and transition zones between strictly anoxic and oxic conditions may have characterised large expanses of Precambrian continental margins, where oxygen-breathing, complex life emerged and diversified. Accurate reconstructions of oxygen levels in such conditions are therefore required, but current geochemical proxies fail in identifying transitional redox conditions. Uranium isotopes are an emerging palaeoredox proxy, as large isotope fractionations are recorded during the reduction of uranium into anoxic sediments. Their potential application to transitional conditions is, however, unclear, because the redox regulating mineralogy of such environments and the associated isotope fractionations are poorly constrained. Here, we explore the mineralogical transformations occurring during the partial oxidation of mackinawite. We show that green rust, a key mineral of Precambrian oceans, forms as a by-product of mackinawite oxidation, along with uraninite and polysulphide. We also demonstrate that this mechanism records a large abiotic uranium isotope fractionation during its reaction with aqueous iron sulphide
Analyzing homelessness and its correlates among psychiatric in-patients in Berlin, Germany: a 13-year study of routine data
Wohnungslosigkeit unter psychiatrisch stationären Patient*innen ist ein Thema, das bisher nur wenig erforscht wurde, wodurch ein Mangel an Verständnis für die damit verbundenen Faktoren besteht. Vergleichbare Studien in somatischen Abteilungen haben gezeigt, dass wohnungslose Patient*innen mit einer Reihe von Herausforderungen konfrontiert sind, die ihre Gesundheit und ihre medizinische Versorgung beeinträchtigen. Diese retrospektive Studie zielt darauf ab, Veränderung des Anteils wohnungsloser Personen unter stationär-psychiatrischen Behandlungsfällen über einen Zeitraum von 13 Jahren in einer psychiatrischen Universitätsklinik in Berlin, Deutschland, zu untersuchen sowie mit Wohnungslosigkeit in Verbindung stehende Faktoren zu identifizieren. Insgesamt wurden hierzu 1205 elektronische Patientenakten ausgewählt und analysiert. Die Ergebnisse zeigten eine signifikante Zunahme von 14,6 % in der Rate wohnungsloser psychiatrischer stationärer Patient*innen im Untersuchungszeitraum. Es wurden klinische Faktoren (Vorliegen einer Substanzgebrauchsstörung, psychotischen Störung, Traumafolgestörung und Persönlichkeitsstörung) und soziodemographische Faktoren (Geschlecht, Geburtsland und Vorliegen einer ambulanten Behandlung) identifiziert, die signifikant mit Wohnungslosigkeit assoziiert waren. Darüber hinaus konnte gezeigt werden, dass der Übergang von wohnungslosen Patient*innen in therapeutische Einrichtungen nur äußerst selten direkt aus dem klinischen Setting erfolgt. Diese Ergebnisse unterstreichen die wachsende Zahl von Patient*innen, die in der psychiatrischen Versorgung mit prekären sozialen Situationen konfrontiert sind, und die Notwendigkeit, diesen Trend bei der Ressourcenallokation im Gesundheitswesen zu berücksichtigen. Die Umsetzung individualisierter Nachsorgelösungen in Verbindung mit unterstützenden Wohninitiativen und begleitender therapeutischer Begleitung könnte eine effektive Maßnahme sein, um diesem Problem entgegenzuwirken. Ein interdisziplinärer Forschungsansatz, der auch somatische Erkrankungen der Betroffenen aber auch Gesundheitssystemforschung zu Barrieren und Lösungen einbezieht, kann ebenfalls dazu beitragen, die Behandlung von wohnungslosen Personen zu verbessern und ihre ganzheitlichen Bedürfnisse besser zu adressieren.Homelessness among psychiatric inpatients is an understudied issue, leading to a lack of understanding of the associated factors. Comparable studies in somatic departments have shown that homeless patients face a variety of challenges that affect their health and medical care. This retrospective study aims to examine the changes in the number of homeless psychiatric in-patients over a 13-year period in a university psychiatric hos-pital in Berlin, Germany. A total of 1205 electronic patient files were analyzed to explore the sociodemographic and clinical factors related to homelessness. The results revealed a significant 14.6% increase in the rate of homeless psychiatric in-patients over the study period. Clinical factors (presence of a substance use disorder, psychotic disorder, trauma-related disorder, and personality disorder) and sociodemographic factors (gen-der, country of birth, and presence of outpatient treatment) were identified to be signifi-cantly associated with homelessness. Additionally, it was shown that the transfer of homeless patients to therapeutic facilities rarely occurs directly from the clinical setting. These findings underscore the growing number of patients confronted with precarious social situations in psychiatric care and the necessity to consider this trend in healthcare resource allocation. Implementing individualized aftercare solutions in conjunction with supportive housing initiatives and accompanying therapeutic support could be an effec-tive measure to counteract this issue. An interdisciplinary research approach, which also includes somatic diseases of the affected individuals as well as health system research on barriers and solutions, can also contribute to improving the treatment of homeless individuals and better addressing their holistic needs
Optical coherence tomography-based assessment of coronary plaque calcium and its implications on inflammatory activity in acute coronary syndrome
Hintergrund: Die koronare Herzerkrankung (KHK) und deren verheerendste Manifestation, das akute Koronarsyndrom (ACS) tragen einen großen Teil zur weltweiten Mortalität und Morbidität bei. Die optische Kohärenztomographie (OCT) ermöglicht eine hochauflösende Darstellung intravaskulärer Plaquestrukturen im ACS in vivo. Die OP-TICO-ACS Studie ist eine OCT-basierte Beobachtungsstudie, in der Zusammenhänge zwischen bildmorphologischen Plaquestrukturen, klinischen Patientencharakteristika und lokalen Entzündungsmechanismen im ACS untersucht wurden. Die vorliegende Arbeit untersucht diese Zusammenhänge mit besonderem Fokus auf kleine, fleckige Kalzifizierungen (spotty calcium deposit – SCD), die ein Merkmal vulnerabler Plaques sind.
Methoden: Im Rahmen der Herzkatheteruntersuchung erfolgte bei 155 ACS-Patient:innen eine intrakoronare Bilderfassung der ACS-auslösenden Läsion (culprit lesion – CL) mittels OCT. Simultan wurde lokales Blut aus der unmittelbaren Plaqueumgebung (LOK) und der systemischen Blutzirkulation (SYS) entnommen. Nach Probengewinn erfolgte die Immuncharakterisierung der Proben mittels Durchflusszytometrie und Zytokinanalyse. Alle CLs wurden auf das Vorkommen vordefinierter Plaquestrukturen untersucht (insbesondere ACS-auslösende Pathophysiologie, Lipid- und Kalkanteile). Die Studienkohorte wurde in Patient:innen mit - und ohne SCD unterteilt (SCD+ und SCD-). SCD waren definiert als Kalzifizierungen mit einer maximalen Länge von 4 mm und einem maximalen Querschnittswinkel von 90°, während alle anderen Kalzifizierungen als Makrokalzifizierungen definiert wurden.
Ergebnisse: In der Patientenkohorte hatten 78,1 % aller Patient:innen eine oder mehrere Kalzifizierungen an der CL. 45,2% aller Patient:innen hatten mindestens eine SCD an der CL. SCD+ Läsionen waren assoziiert mit anderen Hochrisikostrukturen, wie z.B. Fibroatheromata mit dünner Kappe (86,6 vs. 72,8%; p<0,05). Es zeigte sich außerdem ein größerer Myokardschaden während des ACS (CK max 1485,1 vs. 1273,2 U/l; p<0,05) in SCD+ Patient:innen im Vergleich zu SCD- Patient:innen. Die Immuncharakterisierung ergab vor allem eine Anreicherung von Interleukin (IL)-8 (2,04 vs. 1,37; p<0,05), Tumor Nekrose Faktor (TNF)-α (1,17 vs. 1,06; p<0,05) und Neutrophilen (0,96 vs. 0,91; p<0,05) an SCD+ Läsionen. In SCD+ Patient:innen zeigte sich zudem nach 12 Monaten Nachbeobachtung ein erhöhtes Vorkommen uner-wünschter, kardialer Ereignisse (16,4% vs. 5,3%; p<0,05) im Vergleich zu SCD- Patient:innen.
Schlussfolgerung: Im Rahmen der OPTICO-ACS Studie konnten spezifische, in-flammatorische Vorgänge an der CL mit klar abzugrenzenden Plaquestrukturen assoziiert werden. In der vorliegenden Sub-Analyse zeigte sich vor allem eine vermehrte Anreicherung von Zellen und Zytokinen der angeborenen Immunantwort in SCD+ Patienti:nnen, sowie eine schlechtere kardiovaskuläre Gesamtprognose.Background: Coronary heart disease (CAD) and its most detrimental manifestation, acute
coronary syndrome (ACS) contribute largely to mortality and morbidity worldwide. Optical coherence
tomography (OCT) allows for detailed, intracoronary assessment of plaque structures
in vivo and has a superior resolution compared to conventional imaging modalities.
Within the observational OPTICO-ACS study the relationship between morphological plaque
features in patients with ACS, clinical characteristics and local inflammatory mechanisms
were investigated via a translational approach. Within the present sub-analysis these mechanisms
were compared between patients with – and without spotty calcium deposits (SCD),
which represent a high risk plaque feature at ACS-causing culprit lesions (CLs).
Methods: During angiography, 155 patients underwent intracoronary imaging via OCT. Simultaneously,
local blood (LOC) directly from the culprit lesion (CL) as well as systemic blood
(SYS) from the periphery were collected. After blood collection, samples were immediately
analyzed by fluorescence assorted cell sorting and cytokine bead array technique. CLs were
also analyzed via OCT for pre-defined plaque structures (especially ACS-causing pathophysiology,
lipid- and calcium content). SCD were defined as within 4mm length and within a maximum
angle of 90°. All other calcifications were defined as macrocalcifications. The study
cohort was divided into patients with- and without SCD (SCD+ and SCD-). Inflammatory cells
and cytokines were compared between the two groups.
Results: Within the present cohort, 78.1% of all patients had at least one calcification at the
CL. 45.2% of all CL contained one or more SCD. SCD+ CLs were associated with other highrisk
plaque features, as for example thin cap fibroatheroma (86.6 vs. 72.8%; p<0.05). Moreover,
SCD+ patients had a larger myocardial injury during ACS (peak creatinkinase 1485.1
U/l vs. 1273.2 U/l vs.; p<0.05) Furthermore, CLs containing SCD were characterized by increased
levels of interleukin (IL)-8 (2.04 vs. 1.37; p<0.05), tumor necrosis factor (TNF)-α (1.17
vs. 1.06; p<0.05) and neutrophils (0.96 vs. 0.91; p<0.05) as opposed to SCD- lesions. Further,
rates of major adverse cardiac events within 12 months of follow-up were increased in
SCD+ patients as compared to SCD- patients (16.4 vs. 5.3%; p<0.05).
Conclusion:. Within the OPTICO-ACS study, multiple distinct inflammatory mechanisms
were associated with specific plaque structures. Within the present sub-analysis, SCD were
associated with a pro-inflammatory milieu at SCD+ CL, dominated by cells and cytokines of
the innate immune system. Further, SCD+ patients had a worse cardiovascular prognosis

Salutogenesis and religion/spirituality: A meta-analysis of the relationship between sense of coherence and measures of religion and spirituality
Background: Meta-analyses show that there is a weak positive correlation between religion/spirituality (R/S) and mental health. Various theoretical approaches and mechanisms of action have been put forward to explain this relationship. There have also been attempts to bring the mechanisms together in a general effect model. The coherence hypothesis, inspired by Antonovsky's salutogenesis model, plays a central role in these explanatory attempts and states that the positive association between R/S and mental health can be explained by sense of coherence (SOC). However, a systematic review of the literature and an analysis of the empirical basis for the assumption that R/S and SOC are positively related are still lacking.
Objective: The aim of the doctoral thesis was to present an initial systematic review of English and German-language publications that report a statistical correlation between R/S and SOC. The meta-analysis of these studies aimed to identify factors that influence this association.
Methods: In March 2023, 89 studies were included in the meta-analysis that used original SOC questionnaires and an R/S scale that corresponded to a heuristic definition of R/S after qualitative content analysis. In May 2024, the database search was repeated using the same search algorithm and a further 5 studies were identified. The content analysis of the R/S scales provided a taxonomy that showed which aspects of R/S were primarily measured. Subgroup analyses were conducted according to this taxonomy and other study and sample characteristics.
Results: Looking at the 89 studies, positive R/S scales were found to correlate positively (r+ = .120, 95% CI [.092, .149]) and negative R/S scales negatively (r+ = -.405, 95% CI [-.476, -.333]) with SOC. The results were confirmed by the 5 additional studies: The effect sizes between positive R/S scales and SOC ranged from r = .01 to .36. The positive correlation is consistent with the hypothesis and would have been stronger if certain preliminary methodological decisions (e.g., inclusion of the extrinsic religiosity scales) had been made differently. The spirituality-SOC association was, as assumed, stronger than the religion-SOC association, which was attributed to the fact that spiritual systems of world interpretation gain social plausibility. Emotional aspects of R/S were more strongly associated with SOC than behavioral or cognitive aspects. Moreover, the strength of the association was not only influenced by individual differences (age, gender, health, religious affiliation), but above all by the cultural-spatial context. As expected, studies with a good fit to the research question and studies published in journals reported higher effect sizes.Hintergrund: Meta-Analysen belegen, dass zwischen Religion/Spiritualität (R/S) und psychischer Gesundheit ein schwacher positiver Zusammenhang besteht. Zusammenhang zu erklären, wurden unterschiedliche theoretische Ansätze und
Wirkmechanismen ins Feld geführt. Auch gibt es Versuche, die Mechanismen in einem allgemeinen Wirkmodell zusammenzuführen. Die durch Antonovskys Salutogenesemodell inspirierte Kohärenzhypothese spielt in diesen Erklärungsversuchen eine zentrale Rolle und besagt, dass die positive Assoziation von R/S und psychischer Gesundheit durch das Kohärenzgefühl (sense of coherence, SOC) erklärt werden kann. Die Meta-Analyse dieser Studien zielte darauf ab, Faktoren zu identifizieren, die diese Assoziation beeinflussen.
Methodik: Im März 2023 wurden 89 Studien in die Meta-Analyse eingeschlossen, die originäre SOC-Fragebögen verwendeten sowie eine R/S-Skala nutzten, die nach qualitativer Inhaltsanalyse einer heuristischen Definition von R/S entsprachen. Im Mai 2024 wurde die Datenbankrecherche mit demselben Suchalgorithmus wiederholt und es konnten weitere 5 Studien identifiziert werden. Die Inhaltsanalyse der R/S-Skalen lieferte eine Taxonomie, die aufzeigte, welche R/S-Aspekte vorrangig gemessen wurden. Gemäß dieser Taxonomie und weiterer Studien- und Stichprobenmerkmale wurden Subgruppen-Analysen durchgeführt.
Ergebnisse: Mit Blick auf die 89 Studien wurde festgestellt, dass positive R/S-Skalen positiv (r+ = 0,120; 95% CI [0,092; 0,149]) und negative R/S-Skalen negativ (r+ = -0,405, 95% CI [-0,476; -0,333]) mit SOC korrelierten. Das Ergebnisbild konnte von den 5 zusätzlichen Studien bestätigt werden: Die Effektstärken zwischen positiven R/S-Skalen und SOC reichten von r = 0,01 bis 0,36. Der positive Zusammenhang ist hypothesenkonform und wäre stärker ausgefallen, wenn bestimmte methodische Vorentscheidungen (z.B. Einschluss der Skalen zur extrinsischen Religiosität) anders getroffen worden wären. Die Spiritualität-SOC-Assoziation fiel, wie angenommen, deutlicher aus als die Religion-SOC-Assoziation, was darauf zurückgeführt wurde, dass spirituelle Weltdeutungssysteme an sozialer Plausibilität gewinnen. Emotionale Aspekte von R/S standen stärker mit SOC im Zusammenhang als behaviorale oder kognitive. Die Stärke der Assoziation wurde nicht nur von individuellen Unterschieden (Alter, Geschlecht, Gesundheit, Religionszugehörigkeit) beeinflusst, sondern vor allem auch vom kulturräumlichen Kontext. Erwartungsgemäß berichteten Studien, die eine gute Passung zur Forschungsfrage aufwiesen und Studien, die in Zeitschriften publiziert wurden, höhere Effektstärken

mRNA decay is a key process controlling cellular protein levels and homeostasis. Cells tune mRNA stability by selective degradation, a process traditionally thought to rely on recognition of mRNA sequence features by proteins or complementary RNAs. By contrast, recent work showed how mRNA translation by the ribosome can direct selective mRNA decay. The ribosome reads out the codon composition and produces the nascent protein, both of which can be used to trigger decay via the CCR4-NOT complex
A grammar of Sherbro
Synopsis:
This is the first modern grammar and dictionary of Sherbro, an endangered Mel language spoken by ca. 50,000 people in Sierra Leone. The language faces significant pressure being abandoned in favor of the lingua francas Mende, Themne, Krio, and English. The previous grammar, while competently done, was written in 1921 as a handbook for missionaries. The autonym for the language is Bolom; however, speakers of the language while speaking Krio or English use “Sherbro” to refer to themselves and their language, so that practice is followed here. The grammar and dictionary are based on field data collected as part of a four-year research project (2016-2020), “Documenting the Sherbro Language and Culture,” funded by the Hans Rausing Endangered Languages Documentation Programme. An archive of the project is permanently housed by the Endangered Languages Archive (ELAR), www.elar-archive.org including the FLEx database, more than 30 glossed transcriptions and more than 150 recordings. Trade-offs among restored ecosystem functions are context-dependent in Mediterranean-type regions
Global biodiversity hotspots, including Mediterranean-type ecosystems worldwide, are highly threatened by global change that alters biodiversity, ecosystem functions, and services. Some restoration activities enhance ecosystem functions by reintroducing plant species based on known relationships between plant traits and ecosystem processes. Achieving multiple functions across different site conditions, however, requires understanding how abiotic factors like climate and soil, along with plant assemblages, influence ecosystem functions, including their trade-offs and synergies. We used the ModEST ecosystem simulation model, which integrates carbon, water, and nutrient processes with plant traits, to assess the relationships between restored plant assemblages and ecosystem functions in Mediterranean-type climates and soils. We investigated whether maximised carbon increment, water use efficiency, and nitrogen use efficiency, along with their trade-offs and synergies, varied across different abiotic contexts. Further, we asked whether assemblages that maximised functions varied across environments and among these functions. We found that maximised ecosystem carbon increment and nitrogen use efficiency occurred under moist, warm conditions, while water use efficiency peaked under drier conditions. Generally, the assemblage that maximised one function differed from those for other maximised functions. Synergies were rare, except between water and nitrogen use efficiencies in loam soils across most climates. Trade-offs among maximised functions were common, varying in strength with abiotic context and plant assemblages, and were more pronounced in sandy loam soils compared to clay-rich soils. Our findings suggest that due to variation in abiotic conditions within and across Mediterranean-type regions at the global scale, site-specific plant assemblages are required to maximise ecosystem functions. Thus, lessons from a single site cannot be transferred to another site, even where the same plant functional types are available for restoration.
