    There and back again: local institutions, an Uruk expansion and the rejection of centralisation in the Sirwan/Upper Diyala region

    During the fourth millennium BC, public institutions developed at several large settlements across greater Mesopotamia. These are widely acknowledged as the first cities and states, yet surprisingly little is known about their emergence, functioning and demise. Here, the authors present new evidence of public institutions at the site of Shakhi Kora in the lower Sirwan/upper Diyala river valley of north-east Iraq. A sequence of four Late Chalcolithic institutional households precedes population dispersal and the apparent regional rejection of centralised social forms of organisation that were not then revisited for almost 1500 years

    Impact of hemoadsorption with CytoSorb® on meropenem and piperacillin exposure in critically ill patients in a post-CKRT setup: a single-center, retrospective data analysis

    Purpose CytoSorb® (CS) adsorbent is a hemoadsorption filter for extracorporeal blood purification often integrated into continuous kidney replacement therapy (CKRT). It is primarily used in critically ill patients with sepsis and related conditions, including cytokine storms and systemic inflammatory responses. Up to now, there is no evidence nor recommendation for the use of CS filters in sepsis (22). There is limited clinical data on the effect of CS on the plasma concentrations of beta-lactams. We aimed to evaluate the statistical and clinical impact of CS in a post-filter CKRT-CS setting on the plasma concentrations of the antibiotics meropenem and piperacillin in critically ill patients. Methods Patients admitted to the intensive care unit (ICU) who received a prolonged infusion of piperacillin or meropenem with CS-combined CKRT were included in this retrospective analysis. TDM (therapeutic drug monitoring) plasma blood samples were collected at three different points. The differences in antibiotic concentrations between Pre, Intra, and Post were statistically compared to evaluate the total and isolated contributions of CKRT and CS to antibiotic removal. CS, CKRT and combined clearance (CL) values were calculated. The hypothesis was that the CS filter would have no clinically relevant impact on antibiotic levels. Results 207 TDM samples were taken from 24 critically ill patients requiring beta-lactam antibiotics. Among these, 129 were meropenem samples, and 78 were piperacillin samples. A decrease in both antibiotic levels was observed between Pre and Intra, and Pre and Post, and the median relative difference between was >15% (meropenem: Pre–Intra 34.8%, Pre–Post 35.8%; piperacillin: Pre–Intra 41.1%, Pre–Post 34.7%), indicating a statistically and clinically significant effect of CKRT on both antibiotic exposures. No significant difference was observed between Intra and Post indicating no clinically relevant drug removal via the CS filter. Changes in CL attributed to CS were minimal, with combined CL differing by ≤8.60% compared to CKRT clearance. Conclusion The application of CS does not appear to significantly affect plasma concentrations of meropenem and piperacillin in critically ill patients

    Societal changes in Ancient Greece impacted terrestrial and marine environments

    The Aegean hosts some of the earliest cultural centers in European antiquity. To reconstruct the evolution of early anthropogenic impact in this region, we have examined lead (Pb) contents and vegetation dynamics on well-dated environmental archives extending to the early Holocene. We show that the impact of agropastoral societies on terrestrial ecosystems was locally confined during the Bronze and Iron Ages (5200–2750 years ago), although we record an onset of Pb pollution already at 5200 cal. years BP and thus about 1200 years earlier than previous archeological evidence. Our data demonstrate a marked increase in Pb pollution at 2150 cal. years BP that left an imprint across terrestrial and marine settings of the Aegean region. This first manifestation of marine pollution coincides with maximum deforestation and agricultural expansion, signaling pervasive human impact on ecosystems connected to the advanced monetized societies during the Hellenistic and Roman periods in Ancient Greece

    Towards Financial Autonomy: The Geopolitical Economy of EUR-Denominated Clearing Services Relocation

    This paper examines the factors influencing the relocation of EUR-denominated clearing services back to EU central counterparties (CCPs) by the European Commission. It adopts a novel security-centred approach to analyse this decision within the context of a fragmented geopolitical arena marked by uneven development in financial centres. Using the theory of Geopolitical Economy (GPE), this paper examines the implications and risks associated with financial dependencies and the pursuit of autonomy in the global financial landscape. It specifically addresses the risks posed by the dominance of UK CCPs, such as London Clearing House (LCH), in EUR-denominated clearing services and emphasises the EU’s need to bolster its financial autonomy and stability, mitigating these intricate risks in this process. Through this analysis, the paper contributes to the existing literature by offering a novel understanding of the geopolitical-economic factors influencing regulatory actions within financial services

    Der Einfluss von sozialen Faktoren und Armutsbedingungen wie Wohnungslosigkeit auf psychische Erkrankungen und ihre Behandlung

    Diese Habilitationsschrift widmet sich dem Einfluss von sozialen Faktoren auf psychische Erkrankungen und ihre Behandlung: Dabei untersucht die erste Arbeit den Zusammenhang von verschiedenen Wohnformen und Wohnungslosigkeit unter (teil)stationär-psychiatrischen Patient*innen mit dem Fokus auf sozidemografische und klinische Faktoren, sowie das Nutzungsverhalten des Krankenhauses sowie des ambulanten psychosozialen Hilfesystems abhängig von der Wohnform. In der Arbeit zeigt sich, dass ein beträchtlicher Teil der Patient*innen in keiner eigenen Wohnung, sondern entweder auf der Straße, in Wohnungslosen- oder Flüchtlingsheimen (13 %) oder in sozialtherapeutischen Einrichtungen (18,3 %) lebt. Charakterisiert waren wohnungslose Patient*innen vor allem durch ein jüngeres Alter, einem früheren psychiatrischen Erstkontakt, männliches Geschlecht, niedrigeres Bildungsniveau und das Vorliegen einer Substanzgebrauchsstörung und psychotischen Störung. Die Ergebnisse unterstreichen den Bedarf nach alternativen und niedrigschwelligen Wohnangeboten für Menschen mit schweren psychischen Störungen. Weiterhin zeigen die Ergebnisse, dass wohnungslose Menschen in stationär-psychiatrischer Behandlung signifikant kürzere Behandlungszeiten aufweisen, im Kontrast zu anderen europäischen und nordamerikanischen Ländern. In der zweiten Arbeit untersuchten wir im Rahmen der Wohin-Studie finanzielle Belastungen und Schulden bei psychiatrischen Patient*innen, wobei eine signifikante Anzahl von Patient*innen (55,1 %) Schulden, Kredite oder offene Rechnungen aufwies. Substanzabhängigkeit und ein jüngeres Alter waren signifikant assoziierte Faktoren für das Vorliegen von Schulden. Finanzielle Belastungen und Verschuldung bei Menschen mit psychischen Erkrankungen sollten daher im klinischen Kontext stärker berücksichtigt werden. Die dritte Arbeit der Habilitationsschrift untersuchte im Rahmen eines systematischen Reviews mit Metaanalyse problematisches Glücksspielverhalten unter wohnungslosen Menschen. Die Prävalenzraten für problematisches Spielverhalten und Spielsucht waren dabei deutlich erhöht gegenüber Zahlen in der Allgemeinbevölkerung. Modelle zur Früherkennung und Behandlung von problematischem Glücksspiel sollten daher zunehmend entwickelt und implementiert werden insbesondere für vulnerable Gruppen. Die vierte Arbeit widmet sich der Nutzung von Mobiltelefonen und Computern bei Patient*innen in (teil)stationärer psychiatrischer Behandlung. Die Ergebnisse zeigen, dass die Rate des Besitzes von Smartphones (59,3 %) und der regelmäßigen Nutzung von Computern (68,4 %) unter den psychiatrischen Patient*innen niedriger war als in der Allgemeinbevölkerung. Wohnungslosigkeit, die Diagnose einer psychotischen Erkrankung, höheres Alter und ein niedriger Bildungsgrad waren signifikant assoziierte Faktoren damit kein Mobiltelefon zu besitzen bzw. keinen Computer zu nutzen. Zur Verhinderung digitaler Barrieren sollten diese Ergebnisse bei der Planung der digitalen Umgestaltung der Versorgungsangebote als auch Entwicklung von E-Health-Lösungen berücksichtigt werden. Die fünfte Veröffentlichung dieser Habilitationsschrift befasst sich mit der Wohnzufriedenheit in verschiedenen Wohnformen psychiatrischer Patient*innen, wobei die Wohnzufriedenheit signifikant höher ausfiel bei Personen, die eine eigene Wohnung zur Verfügung hatten oder Wohngemeinschaften nutzen im Vergleich zu Personen, die in stärker institutionalisierten Settings oder in Einrichtungen für wohnungslose Menschen lebten. Dies unterstreicht den Bedarf an unabhängigen Wohnformen mit flexiblen Hilfen für Menschen mit schweren psychischen Erkrankungen in Wohnungslosigkeit oder erhöhtem Risiko

    Influence of surgical intervention at the level of the dorsal spinous processes on the biomechanics of the equine thoracolumbar spine

    Background Surgical treatment options for horses with overriding dorsal spinous processes include interspinous ligament desmotomy and partial spinous process ostectomy. The impact of spinal surgery on the three-dimensional biomechanics of the equine thoracolumbar spine and the epaxial musculature is unclear. Objectives To investigate the influence of interspinous ligament desmotomy and cranial wedge ostectomy on the biomechanics of the equine thoracolumbar spine and the paraspinal Musculi multifidi. Study design Ex-vivo experiments. Methods Twelve equine thoracolumbar spine specimens were mounted in a custom-made mechanical test rig. Based on computed tomographic imaging, distances between dorsal spinous processes and the spinal range of motion (lateral bending, axial rotation, flexion, extension) were compared before and after desmotomy and cranial wedge ostectomy performed at two or five surgical sites. Anatomical dissection was subsequently conducted to document surgical trauma to the Musculi multifidi following desmotomy. Results The distance between spinous processes in neutral position did not increase significantly after desmotomy (median preoperative = 7.2 mm, interquartile range [IQR] = 3.6 mm; median postoperative = 7.4 mm, IQR = 3.7 mm; p = 0.09), but increased significantly after ostectomy (median preoperative = 8.8 mm, IQR = 4.2 mm; median postoperative = 13 mm, IQR = 6.1 mm; p < 0.001). Both surgical procedures significantly increased the rotational spinal range of motion (p = 0.001), particularly at the level T14/T15 (median preoperative = 6.4°, IQR = 3.2°; median postoperative = 8.2°, IQR = 3.5°; increase = 28.1%; p = 0.02). Musculi multifidi injury was evident at all desmotomy sites. Main limitations Ex-vivo study with limited sample size. Conclusions Neither interspinous ligament desmotomy nor cranial wedge ostectomy resulted in an increased range of motion during flexion, extension or lateral bending but both procedures influenced the rotational component of the equine thoracolumbar spinal mobility
