Patterns and Processes of Diel Streamflow Cycles Along the Longitudinal Profile of an Alpine Headwater Stream
In high‐alpine catchments, diel streamflow cycles are typically controlled by snow and ice melt during days without precipitation. Potentially co‐occurring evapotranspiration (ET) controlled streamflow cycles are of opposite phase and can often not be identified in such environments. This study investigates the spatial and temporal differences in diel streamflow cycles along the longitudinal river profile of the Fundusbach catchment (13 km 2 ; 1620–3095 m a.s.l.) in the Ötztal Alps, Austria. We investigated variations in the shape characteristics of diel hydrographs from June to October 2022 using hourly streamflow from four stations along the river. We identified the main processes controlling the diel streamflow cycles, and tried to quantify the volumes of water added or removed from streamflow due to melt processes or ET. Results show that the amplitude of diel streamflow cycles decreases with increasing catchment area and over the second half of the year, indicating a damping effect of the catchment and decreasing importance of meltwater processes along the river. Despite the dominance of meltwater‐driven diel cycles, we show that ET occurs simultaneously and its influence becomes more pronounced in the lower reaches of the river. By synchronising the diel cycles and deducting the meltwater influence from upstream stations, we visualised the ET‐controlled streamflow cycles at downstream stations. However, the quantification of ET volumes from daily hydrographs is subject to large uncertainty. Our study highlights the importance of considering both meltwater and ET processes to understand diel streamflow cycles in high alpine catchments, especially given the likely increasing relevance of ET in these areas in a warmer climate
Sulfonium-based polymethacrylamides for antimicrobial use: influence of the structure and composition†
We are facing a shortage of new antibiotics to fight against increasingly resistant bacteria. As an alternative to conventional small molecule antibiotics, antimicrobial polymers (AMPs) have great potential. These polymers contain cationic and hydrophobic groups and disrupt bacterial cell membranes through a combination of electrostatic and hydrophobic interactions. While most examples focus on ammonium-based cations, sulfonium groups are recently emerging to broaden the scope of polymeric therapeutics. Here, main-chain sulfonium polymers exhibit good antimicrobial activity. In contrast, the potential of side-chain sulfonium polymers remains less explored with structure–activity relationships still being limited. To address this limitation, we thoroughly investigated key factors influencing antimicrobial activity in side-chain sulfonium-based AMPs. For this, we combined sulfonium cations with different hydrophobic (aliphatic/aromatic) and hydrophilic polyethylene glycol (PEG) groups to create a library of polymers with comparable chain lengths. For all compositions, we additionally examined the position of cationic and hydrophobic groups on the polymer backbone, i.e. , we systematically compared same center and different center structures. Bactericidal tests against Gram-positive and Gram-negative bacteria suggest that same center polymers are more active than different center polymers of similar clog P . Ultimately, sulfonium-based AMPs show superior bactericidal activity and selectivity when compared to their quaternary ammonium cationic analogues
Zelluläre und humorale Immunität in Antwort auf eine SARS-CoV-2 Infektion und Impfung bei Patienten mit primären Antikörpermangelerkrankungen
At the onset of the COVID-19 pandemic, caused by severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2), which evoked in Wuhan, China in 2019, patients with inborn errors of immunity were considered as high-risk group prone to more severe disease. Primary antibody deficiencies (PAD) represent the largest group of these immunodeficiencies. In addition to quantitative immunoglobulin deficiency, a qualitative impairment of humoral immunity occurs, that is characterized by a lack of specific antibody (Ab) response after infection or vaccination. An initial observational clinical study during early pandemic stage confirmed the increased incidence of morbidity and mortality due to COVID-19 in PAD. At the beginning of this doctoral thesis, mechanisms of cellular and humoral immune responses to SARS-CoV-2 infection and vaccination in PAD remained elusive. The conducted experimental studies involved the analysis of polyfunctional T cell responses by flow cytometry along with Ab status and functional B cell memory determined by ELISA and ELISpot techniques. Study 1 was the first among literature to demonstrate T cell immunity to human endemic coronaviruses and their potential for SARS-CoV-2 cross-reactivity in 7/11 PAD. However, compared with uninfected and convalescent healthy individuals, detection frequency and polyfunctional capacity were lower in the patient group. In order to better understand the importance of specific COVID-19 Ab response, study 2 examined T cell and innate immune responses of severe COVID-19 PAD cases without seroconversion. Although all PAD investigated had intact polyfunctional T cell immunity and no impaired type I interferon response, frequent SARS-CoV-2 viremia and persistent viral shedding up to 127 days were observed. Study 3 examined COVID-19 vaccination response in PAD patients. Here, specific Ab responses in PAD showed significantly reduced avidity compared with healthy individuals. In addition, the majority of seroconverted PAD failed to from a functional B-cell memory. T cell response was intact in 100% of patients with, and in 83% of patients without SARS-CoV-2 Ab responses. Reactive follicular T helper cells were increased in Ab-positive PAD. In summary, the studies indicate normal T cell responses in PAD subsequent to SARS-CoV-2 infection or vaccination. Unvaccinated PAD without a SARS-CoV-2 Ab response showed more severe disease progression and prolonged mucosal viral persistence. While clinical data support the value of prophylactic vaccine-induced T cell immunity, qualitative differences in the avidity and functional memory formation of humoral immunity in vaccinated PAD highlight the need for a more nuanced assessment beyond the binary classification of seronegative and –positive patients. The long term value of therapeutic measures, such as the use of monoclonal Ab and the significance of IgG replacement therapy from immunized donors, has not yet been sufficiently clarified in this patient group.Zu Beginn der COVID-19 Pandemie, ausgelöst durch das 2019 in Wuhan, China entdeckte Virus Schweres Akutes Respiratorisches Syndrom-Coronavirus-2 (SARS-CoV-2), wurden Patienten mit angeborenen Immundefekten mit hohem Risiko für schwere Krankheitsverläufe eingestuft. Primäre Antikörperdefizienzen (PAD) stellen die größte Gruppe dieser Immundefekte dar. Zusätzlich zum quantitativen Immunglobulinmangel, tritt eine qualitative Einschränkung der humoralen Immunität auf, gekennzeichnet durch fehlende spezifische Antikörper (Ak)-Antwort nach Infektion oder Impfung. Eine erste klinische Beobachtungstudie bestätigte die erhöhte COVID-19 Morbidität und Mortalität für PAD. Zu Beginn dieser Dissertation waren die Mechanismen der zellulären und humoralen Immunantworten bei SARS-CoV-2-Infektion und Impfung für PAD unbekannt. In den experimentellen Studien wurden polyfunktionale T-Zell-Antworten durchflusszytometrisch analysiert und der Ak-Status, sowie das funktionelle B-Zell-Gedächtnis mittels ELISA und ELISpot-Techniken bestimmt. Studie 1 wies erstmals eine T-Zell-Immunität gegen endemische Coronaviren und deren Potential zur SARS-CoV-2-Kreuzreaktivität bei 7/11 PAD nach. Im Vergleich zu unifizierten und rekonvaleszenten Gesunden war die Erkennungshäufigkeit und Polyfunktionalität bei PAD geringer. Um die Bedeutung der spezifischen COVID-19 Ak-Antwort besser zu verstehen, wurden in Studie 2 T-zelluläre und innate Immunantworten schwerer COVID-19 Verläufe bei PAD ohne Serokonversion untersucht. Obwohl alle untersuchten PAD eine intakte polyfunktionale T-Zell-Immunität aufwiesen und keine gestörte Typ-I-Interferonantwort vorlag, wurden gehäuft SARS-CoV-2 Virämien und persistierende virale Ausscheidungen bis zu 127 Tage beobachtet. Studie 3 untersuchte die COVID-19 Impfantwort bei PAD. Dabei zeigten spezifische Ak-Antworten bei PAD im Vergleich zu Gesunden eine deutlich verminderte Avidität. Zudem konnte die Mehrzahl der serokonvertierten PAD kein funktionelles B-Zell-Gedächtnis aufbauen. Die T-Zell-Antwort war bei 100% der Patienten mit, und bei 83% der Patienten ohne Ak-Antwort intakt. Reaktive follikuläre T-Helferzellen waren bei Ak-positiven PAD erhöht. Zusammenfassend zeigten die Studien normale T-Zell-Antworten bei PAD nach SARS-CoV-2-Infektion oder Impfung. Ungeimpfte PAD ohne spezifische SARS-CoV-2 Ak-Antwort zeigten schwerere Krankheitsverlaufe und prolongierte mukosale Viruspersistenzen. Während klinische Daten den Stellenwert einer prophylaktischen Vakzin-induzierten T-Zell-Immunität untermauern, zeigen qualitative Unterschiede in der Avidität und funktionellen Gedächtnisformation der humoralen Immunität bei geimpften PAD die Notwendigkeit einer differenzierteren Bewertung jenseits der binären Einteilung in seronegativ und -positiv. Die langfristige Bedeutung therapeutischer Maßnahmen, wie der Einsatz monoklonaler Ak und der Stellenwert der IgG-Ersatztherapie von immunisierten Spendern, ist in dieser Patientengruppe noch nicht hinreichend geklärt
Bayes'scher Ansatz zur Quantifizierung der aleatorischen und epistemischen Unsicherheit von quantitativen Parametern: Anwendung bei der DCE-MRT der Leber
Background. Advances in medical imaging play a major role in accurate diagnosis of cancer and treatment planning. Dynamic contrast enhanced (DCE)-MR imaging for example can be used to quantitatively analyse tumor and vascular structures in clinical oncology. The recent advances in Deep-Learning (DL) techniques had shown great progress in quantitative parameter estimation. However, quantitative analysis of DCE-MR images is challenged by the lack of high-quality training data, the need of techniques robust to noise in the data, and difficulty in interpretation of the quality of the obtained quantitative parameters. In the DL-process, the intrinsic ambiguity of the data propagates to the estimated quantitative parameters. Moreover, the training data of the DL-network may mismatch the application data. These factors lead to uncertain parameter estimates.
Aim. The aim of this work is to develop a DL-framework for a joint estimation of quantitative parameters and their respective uncertainties.
Methods. The work was composed of three tasks: i) Simulation of time-concentration curves by applying quantitative parameters and arterial input functions extracted from in-vivo data to a tracer-kinetic model (i.e., extended Tofts (eTofts) model); ii) Training of Bayesian Neural network (BNN) using the simulated data; and iii) Inference of the parameters and uncertainties by application of in-vivo DCE-MR images. The images were acquired for five male patients (56±8 years and, 88±11kg) with hepatic metastases. The estimated quantitative parameters by the proposed framework were compared with the reference method (i.e., non-linear-least-squares fit (NLLS fit)).
Results.The BNN provided more accurate quantitative parameter estimates (for simulated data) and provided parameter estimates more robust to noise of the input concentration curves (for in-vivo data) as compared to the NLLS fitting. The RMSE for the BNN were smaller than the NLLS fit by 33%±1.9%, 22%±6%, 89%±5% for k_trans,〖 v〗_e and v_p on overage for 0% to 15% noise levels, respectively. The AIC values for the BNN were smaller by 28.5%±7.5%, 12.7%±6%, 12.2%±2.7% than the NLLS fit for k_trans, v_e and v_p on overage for 0% to 15% noise levels, respectively. The quantitative parameters yielded increased aleatoric uncertainties when the noise level in the time-concentration curves was increased. The epistemic uncertainty increased when there was a mismatch between the training and application data or less training data.
Conclusion. The proposed framework provided more accurate quantitative parameter estimates than the NLLS fit, and uncertainty estimates, which explained the intrinsic ambiguity of the data (aleatoric uncertainty) and inadequacy of the trained DL-network to characterize the in-vivo data (epistemic uncertainty).Hintergrund: Fortschritte in der medizinischen Bildgebung spielen eine wichtige Rolle in der genauen Diagnose von Krebs und in der Behandlungsplanung. So kann die dynamische kontrastmittelbasierte MRT (DCE-MRT) beispielsweise zur quantitativen Analyse von Tumor- und Gefäßstrukturen in der klinischen Onkologie eingesetzt werden. Jüngste Fortschritte bei den Deep Learning (DL)-Methoden haben zu großen Fortschritten bei der quantitativen Parameterschätzung geführt. Die quantitative Analyse von DCE-MRT-Aufnahmen wird jedoch durch den Mangel an qualitativ hochwertigen Trainingsdaten, die Notwendigkeit von Techniken, die gegenüber Rauschen in Daten robust sind, und die Schwierigkeiten bei der Interpretation der Qualität der erhaltenen quantitativen Parameter erschwert. Bei der DL-Methode überträgt sich die inhärente Mehrdeutigkeit der Daten auf die geschätzten quantitativen Parameter. Außerdem kann es vorkommen, dass die Trainingsdaten nicht mit den Anwendungsdaten übereinstimmen. Diese Faktoren führen zu unsicheren Parameter¬schätzungen.
Ziel: Ziel dieser Arbeit ist es, ein DL-Framework für die gemeinsame Schätzung quantitativer Parameter und deren Unsicherheiten zu entwickeln.
Verfahren: Die Arbeit bestand aus drei Aufgaben: i) Simulation von Zeit-Konzentrations-Kurven, indem quantitative Parameter und arterielle Input-Funktionen, die aus in-vivo-Daten extrahiert wurden, auf ein Tracer-kinetisches Modell (d. h. ein erweitertes Tofts (eTofts)-Modell) angewendet wurden; ii) Training eines Bayes'schen Neuronalen Netzwerks (BNN) unter Verwendung der simulierten Daten; und iii) Inferenz der Parameter und Unsicherheiten mithilfe von in-vivo DCE-MRT-Aufnahmen. Die Aufnahmen wurden von fünf männlichen Patienten (56±8 Jahre und 88±11 kg) mit Lebermetastasen gemacht. Die mittels des vorgeschlagenen Frameworks geschätzten quantitativen Parameter wurden mit der Referenzmethode (d. h. Anpassung durch nichtlineare kleinste Quadrate (NLLS-Fit)) verglichen.
Ergebnisse: Das binäre neuronale Netzwerk lieferte genauere quantitative Parameterschätzungen (für simulierte Daten) und robustere Parameterschätzungen in Bezug auf das Rauschen der Input-Konzentrationskurven (für In-vivo-Daten) im Vergleich zum NLLS-Fit. Die mittlere quadratische Abweichung (RMSE) für das BNN war um 33 %±1,9 %, 22 %±6 %, 89 %±5 % kleiner als das NLLS-Fit durchschnittlich für 0 % bis 15 % Rauschpegel jeweils klei ner als die das NLLS-Fit für k_trans,〖 v〗_e und v_p. Die AIC-Werte für das BNN waren um 28,5 %±7,5 %, 12,7 %±6 %, 12,2 %±2,7 % kleiner als das NLLS-Fit für k_trans, v_e und v_p durchschnittlich für 0 % bis 15 % Rauschpegel. Die aleatorische Unsicherheit der quantitativen Parameter nahm mit steigendem Rauschpegel zu, während die epistemische Unsicherheit zunahm, wenn es eine Diskrepanz zwischen Trainings- und Anwendungsdaten gab oder weniger Trainingsdaten vorhanden waren.
Fazit: Das vorgeschlagene Framework lieferte genauere quantitative Parameterschätzungen als das NLLS-Fit sowie Unsicherheitsschätzungen, die eine Erklärung für die inhärente Mehrdeutigkeit der Daten (aleatorische Unsicherheit) und die Unzulänglichkeit des trainierten DL-Netzwerks zur Charakterisierung der in vivo-Daten (epistemische Unsicherheit) lieferten
Midterm results of a stemless anatomical shoulder arthroplasty for patients with primary osteoarthritis
Hintergrund: Die schaftlose Schulterendoprothese mit vier offenen, rippenförmigen Press-fit-Verankerungen zeigt hervorragende klinische und radiologischen Zwei-Jahres- Ergebnisse. In der vorliegenden prospektiven, multizentrischen Studie werden die klinischen und radiologischen Fünf-Jahres-Ergebnisse der schaftlosen Schulterendoprothese bei primärer Omarthrose evaluiert.
Methodik: Im Zeitraum zwischen November 2012 und Dezember 2015 wurden 100 Pa- tient:innen mit primärer Omarthrose mit dem Sidus Stem-Free Shoulder System an sieben europäischen Kliniken behandelt. Es erfolgten eine klinische (Constant-Murley- Score [CS], American Shoulder and Elbow Standardized Shoulder Assessment Form Score [ASES], Subjektiver Schulterwert [SSV], Abduktionskraft, Bewegungsumfang) und eine radiologische Evaluation der Schulter (in drei Ebenen, dabei wurden den Fragen nach Osteolyse, Implantatlockerung, heterotoper Ossifikation, radiologischen Lockerungslinien [RLL], Komponentenmigration, humeraler Knochenresorption nachgegangen) vor sowie nach ein, zwei und fünf Jahren nach der Operation. Zusätzlich zu einer Kaplan-Meier-Überlebensanalyse wurden vergleichende Subgruppenanalysen der Fünf-Jahres-Ergebnisse zwischen Patient:innen mit Total- (TEP) und Hemiendoprothetik (HEP) sowie mit einer konzentrischen und exzentrischen Glenoidpatholgie durchgeführt.
Ergebnisse: Einundsiebzig Patient:innen (36 Frauen) (Durchschnittsalter von 63,8 Jahren; Spanne: 47–79 Jahre) wurden im Rahmen der fünfjährigen, klinische und röntgenologische Nachuntersuchung evaluiert (Spanne: 52–79 Monate). Im Vergleich zu den präoperativen Ausgangswerten war ein signifikanter Anstieg (p < 0,0001) in allen Funktionsparametern nach ein, zwei und fünf Jahren zu verzeichnen. Patient:innen mit einer TEP (n = 48) erzielten signifikant bessere funktionelle Ergebnisse im CS, ASES und SSV mit mehr Abduktionskraft sowie einen größeren Bewegungsumfang in Flexion und Außenrotation (p ≤ 0.004) als Patient:innen mit einer HEP (n = 23). Es zeigten sich keine Unterschiede in der Funktion zwischen Patient:innen mit exzentrischer (n = 30) oder konzentrischer (n = 31) Glenoidpathologie. Radiologisch gab es mittelfristig keine Anzeichen von Osteolyse oder humeraler Komponentenlockerung, jedoch Fälle von hete- rotoper Ossifikation (1,4 %), RLL um die humerale (1,4 %) und glenoidale Komponente (15 %) mit Migration der Pfanne (2,1 %) sowie inferiore Osteophytenbildung (1,4 %) oder humeraler Knochenresorption (9,9 %). Die Fünf-Jahres-Überlebensrate betrug 94 %.
Schlussfolgerung: Es zeigen sich sehr gute klinische und radiologische Ergebnisse der schaftlosen Schulterendoprothese bei Patient:innen mit primärer Omarthrose nach ein, zwei und fünf Jahren mit geringen Komplikationsraten und ohne Anzeichen einer humeralen Implantatlockerung. Die Versorgung nach TEP erreichte nach fünf Jahren signifikant bessere Ergebnisse als die nach HEP; der exzentrische Glenoidverbrauch zeigte keinen Einfluss auf die mittelfristige Funktion.Background: Stemless shoulder arthroplasty using four open-fin press-fit anchors has been showing promising short-term clinical and radiographic results for patients primary osteoarthritis. This prospective, multicenter study presents five year postoperative clinical and radiological outcomes of a stemless shoulder arthroplasty for primary osteoarthritis. Methods: Between November 2012 and December 2015, 100 patients were treated for primary osteoarthritis with Sidus Stem-Free Shoulder System at seven European centers. All patients were evaluated clinically and radiographically at baseline, one, two and five years post-surgery. Clinical assessment included Constant-Murley Score (CS), American Shoulder and Elbow Standardized Shoulder Assessment Form Score (ASES), Subjective Shoulder Value (SSV), range of motion (ROM) and abduction strength. Radiographically, osteolysis, glenoid and humerus loosening, heterotopic ossification, radiolucent lines, component migration and humeral bone resorption were assessed. In addition to a Kaplan-Meier survival analysis, comparative analyses between hemi (HSA) and total shoulder arthroplasty (TSA) such as eccentric and concentric glenoid defects were performed.
Results: Seventy-one patients (36 females) with a mean age of 63.8 years (range: 47– 79 years) were available for the five-year clinical and radiographic follow-up (range: 52– 79 months). There was a significant increase (p < 0.0001) in all outcome scores after one, two and five years compared to baseline values. Patients with TSA (n = 48) achieved significantly better functional outcome than patients with shoulder HSA (n = 23) with regards to CS, ASES and SSV as well as abduction strength and ROM in forward elevation and external rotation (p ≤ 0.004). There were no differences between patients with eccentric (n = 30) or concentric (n = 31) glenoid wear. At five years postoperatively, there were no cases of osteolysis or humeral loosening but some cases of heterotopic ossification (1.4 %), radiolucency around the humerus (1.4 %) or glenoid (15 %), glenoid migration (2.1 %) as well as inferior osteophytes (1.4 %) or humeral bone resorption (9.9 %). Five year survival was 94 %.
Conclusion: Patients treated with this stemless shoulder arthroplasty for primary osteoarthritis achieve very good clinical results at one, two and five years postoperatively without any signs of aseptic humeral implant loosening. Patients with TSA show better results after five years than those with HSA. Eccentric glenoid defects do not show inferior results at midterm follow-up examinations
Greenhouse Gas Disclosure: Evidence from Private Firms
Existing literature on greenhouse gas (GHG) emissions disclosure has paid little attention to private firms, despite the fact that this type of firm is responsible for significant GHG emissions. This study empirically analyzes the GHG disclosure of German private firms. The results suggest that more pronounced information asymmetries due to a more dispersed ownership structure and/or multiple bank relationships are associated with more extensive GHG disclosure. This aligns with arguments from agency and stakeholder theory. While this result is not new for public firms, it is for private firms. Given the specific characteristics of this type of firms (no separation of ownership and control, private communication channels, close bank–borrower relationships), it is not a straightforward assumption that observations from public firms can be transferred to private firms one-to-one. Moreover, higher levels of actual GHG emissions are also associated with more GHG disclosure, indicating that legitimacy theory arguments hold for private firms as well
Acceptance of a brief narrative intervention in primary care for patients with post-traumatic stress disorder – a qualitative study
A significant number of patients in primary care showing post-traumatic stress symptoms are not currently receiving adequate care. In the multicentred, randomised-controlled PICTURE trial, patients with post-traumatic stress disorder (PTSD) following intensive care treatment received a short-term intervention based on narrative exposure therapy (NET) by their general practitioner (GP). To explore its future implementation in general practice, we investigated the acceptance of the intervention by GPs.
We conducted semi-structured telephone interviews with a sample of the GPs who had performed a NET-based intervention (NET-GP) as part of the PICTURE trial. The interview guide was developed based on the Theoretical Framework of Acceptance from Sekhon et al. The interviews were recorded, transcribed and analysed using qualitative content analysis according to Mayring.
We analysed 15 telephone interviews averaging 51 minutes (34 to 74 minutes) in length. The interviews revealed a range of perceptions among GPs of the prevalence and gravity of PTSD. Those who felt they had a part to play in treating of these patients were ready to implement the intervention, despite the high organisational and time effort required. Challenges in implementation arose from GPs’ lack of experience with psychotherapeutic interventions, including dealing with strong emotional reactions from patients, and from the difficulty of scheduling longer consultations into routine practice. Suggested support included validated screening tools in form of questionnaires, training opportunities and a psychotherapeutic supervision for challenging situations during therapy. Furthermore, NET-GP should only be offered by GPs who opt in to giving it.
From the perspective of the GPs interviewed the investigated intervention is feasible and could be adapted for use in general practice. The challenges raised by interviewees could be met by intensified training and supervision for GPs and suitable remuneration for delivering the treatment.In der hausärztlichen Versorgung zeigen zwischen 2 % und 32 % der Patient*innen posttraumatische

posttraumatischer Belastungsstörung (PTBS) nach intensivmedizinischer Behandlung eine
hausärztliche Gesprächsintervention basierend auf der Narrativen Expositionstherapie (NET). Im Sinne
der begleitenden Implementierungsforschung wurden Hausärzt*innen befragt, inwieweit diese
Intervention zukünftig in den hausärztlichen Praxisalltag auf andere Patient*innengruppen übertragen
und in die Regelversorgung integriert werden könnte.
Die qualitative Studie fand durch semistrukturierte Interviews mit Hausärzt*innen statt, die im Rahmen
der Studie die Gesprächsintervention anwendeten. Der Interviewleitfaden wurde theoriegeleitet auf
Basis eines Frameworks der Akzeptanzforschung nach Sekhon et al. entwickelt, die Interviews
audiodigital aufgenommen, im Wortlaut transkribiert und mit qualitativer Inhaltsanalyse nach Mayring
ausgewertet.
Es wurden 15 Telefoninterviews über durchschnittlich 51 Minuten (minimal 34, maximal 74 Minuten)
geführt. Ergebnisse zeigten eine unterschiedlich ausgeprägte Wahrnehmung der Häufigkeit und der
Relevanz einer PTBS in der hausärztlichen Praxis. Dementsprechend ergaben sich unterschiedliche
Einschätzungen des notwendigen Betreuungs- und Behandlungsbedarfs. Vor allem bei Hausärzt*innen,
die einen hohen Bedarf wahrnahmen, war die Bereitschaft zur Umsetzung der NET hoch, trotz des
organisatorischen und zeitlichen Aufwands. Von diesen wurde die erlernte Gesprächsintervention als
gut nutzbares Instrument eingeschätzt. Herausforderungen in der Umsetzung entstanden aus Sicht der
Hausärzt*innen durch wenig eigene Erfahrung mit psychotherapeutischen Vorgehensweisen, wie bei
stark emotionalen Reaktionen von Patient*innen während der therapeutischen Exposition. Auch die
Terminplanung für längere Gespräche wurde im Praxisalltag als problematisch betrachtet.
Unterstützung solle in Form validierter diagnostischer Mittel durch Fragebögen, Schulungen und eines
psychotherapeutischen Netzwerkes für kritische Therapiesituationen angeboten werden. Darüber
hinaus solle die NET nur von Hausärzt*innen mit spezifischem Interesse in diesem Gebiet durchgeführt
werden.
Die untersuchte Intervention ist aus Sicht der befragten Hausärzt*innen umsetzbar, im Sinne der
hausärztlichen Anwendbarkeit könnte diese weiterentwickelt werden. Den benannten
Herausforderungen könnten durch verbesserte und intensivere Schulungsangebote und eine adäquate
Finanzierung der zeitaufwändigen Intervention begegnet werden
Harnessing macrophage-drug conjugates for allogeneic cell-based therapy of solid tumors via the TRAIN mechanism
Treatment of solid tumors remains challenging and therapeutic strategies require continuous development. Tumor-infiltrating macrophages play a pivotal role in tumor dynamics. Here, we present a Macrophage-Drug Conjugate (MDC) platform technology that enables loading macrophages with ferritin-drug complexes. We first show that macrophages actively take up human heavy chain ferritin (HFt) in vitro via macrophage scavenger receptor 1 (MSR1). We further manifest that drug-loaded macrophages transfer ferritin to adjacent cancer cells through a process termed ‘TRAnsfer of Iron-binding protein’ (TRAIN). The TRAIN process requires direct cell-to-cell contact and an immune synapse-like structure. At last, MDCs with various anti-cancer drugs are formulated with their safety and anti-tumor efficacy validated in multiple syngeneic mice and orthotopic human tumor models via different routes of administration. Importantly, MDCs can be prepared in advance and used as thawed products, supporting their clinical applicability. This MDC approach thus represents a promising advancement in the therapeutic landscape for solid tumors
Investigation of the association between polymorphisms of the glucocorticoid receptor gene and quality of life in patients with Cushing's syndrome
CONTEXT: Chronic hypercortisolism in patients with endogenous Cushing's syndrome (CS) predisposes them to psychiatric and neurocognitive disorders leading to impaired quality of life. Since quality of life does not significantly improve after remission in some patients, there are likely to be other influencing factors besides cortisol excess. In our study, we investigated the single nucleotide polymorphisms (BclI, N363S, ER22/23EK, and A3669G) of the glucocorticoid receptor gene because their receptor protein variants have been shown to differ in their sensitivity to cortisol. Thus, we aimed to analyze whether there is an association between the glucocorticoid receptor polymorphisms, quality of life, and recovery after remission in patients with CS.
METHODS: For the cross-sectional study of this multicenter investigation, we were able to include 295 patients with endogenous CS (81 of them with active CS and 214 in remission) from three sites of the German Cushing Registry. The longitudinal analysis includes 120 patients from the cross-sectional study who were examined at registry entry and after 1.5±0.9 years of follow-up. Two Cushing-specific questionnaires (CushingQoL and Tuebingen CD-25) and a general QoL questionnaire (SF-36) have been used to assess quality of life. Genotyping of the four single nucleotide polymorphisms was performed after prior DNA extraction from peripheral blood leukocytes.
RESULTS: In two subcategories of the SF-36 questionnaire, carriers of the mutant Bcll allele showed significant improvement in vitality (p=0.038) and mental health (p=0.013) in longitudinal analysis compared to wild type.
In cross-sectional analysis, no differences in quality of life were observed between minor allele and wild-type carriers for all GR polymorphisms tested.
CONCLUSION: The single nucleotide polymorphism Bcll, is associated with increased sensitivity to cortisol, carriers of this mutant allele recovered better from impaired quality of life than wild type. The results may partially explain the inter-individual differences in the severity of QOL impairment after remission in patients with CS. Screening for such genetic alterations (SNPs) could therefore be useful for early treatment of psychiatric disorders and for learning and strengthening coping strategies.EINLEITUNG: Der chronische Hypercortisolismus bei Patienten mit endogenem Cushing Syndrom (CS) prädisponiert diese für psychiatrische und neurokognitive Störungen, die zu einer eingeschränkten Lebensqualität führen. Da sich die Lebensqualität nach Remission bei einigen Patienten nicht signifikant bessert, wird es wahrscheinlich neben dem Cortisolüberschuss noch weitere Einflussfaktoren geben. Wir haben in unserer Studie die Einzelnukleotid-Polymorphismen (BclI, N363S, ER22/23EK und A3669G) des Glucocorticoidrezeptor-Gens untersucht, weil deren Rezeptorproteinvarianten sich nachweislich in ihrer Sensitivität auf Cortisol unterscheiden. Somit wollten wir analysieren, ob es eine Assoziation zwischen den Glucocorticoidrezeptor-Polymorphismen, der Lebensqualität und der Genesung nach Remission in Patienten mit CS gibt.
METHODIK: In die Querschnittsstudie dieser multizentrischen Untersuchung konnten wir 295 Patienten mit endogenem CS (81 davon mit floridem CS und 214 in Remission) von drei Standorten des Deutschen Cushing Registers einschließen. Die longitudinale Betrachtung schließt 120 Patienten der Querschnittsstudie ein, die bei Registereintritt und nach 1.5 ± 0.9 Jahren beim Follow-Up untersucht wurden. Um die Lebensqualität zu erfassen sind zwei Cushing-spezifische Fragebögen (CushingQoL und Tuebingen CD-25) und ein allgemeiner Lebensqualitätsfragebogen (SF-36) verwendet worden. Die Genotypisierung der vier Einzelnukleotid-Polymorphismen erfolgte nach vorheriger DNA-Extraktion aus peripheren Blutleukozyten.
ERGEBNISSE: In zwei Subkategorien des SF-36 Fragebogens zeigten Träger des mutierten Bcll-Allels in der longitudinalen Analyse eine signifikante Verbesserung in „vitality“ (p=0.038) und „mental health“ (p=0.013) im Vergleich zum Wildtyp.
In der Querschnittsanalyse wurden keine Unterschiede in der Lebensqualität zwischen Minor-Allel- und Wildtyp-Trägern für alle getesteten GR-Polymorphismen festgestellt.
DISKUSSION: Der Einzelnukleotid-Polymorphismus Bcll wird mit einer erhöhten Empfindlichkeit gegenüber Cortisol in Verbindung gebracht. Träger dieses mutierten Allels erholten sich demnach besser von einer beeinträchtigten Lebensqualität als der Wildtyp. Die Ergebnisse könnten teilweise die inter-individuellen Unterschiede in der Schwere der Beeinträchtigung der Lebensqualität nach Remission in Patienten mit CS erklären
Depression, anxiety and stress in pregnancies with breech presentation – A prospective study on the association between clinical parameters and maternal outcomes
Die Beckenendlage als fetale Lageanomalie betrifft etwa 5% der Schwangerschaften. Aufgrund der assoziierten Risiken wie Geburtsstillstand, Asphyxie und Hüftdysplasie kann die Diagnose eine erhebliche psychische Belastung für die werdende Mutter darstellen. Die oft späte Feststellung erfordert eine zügige Entscheidungsfindung bezüglich möglicher Interventionen wie einer äußeren Wendung sowie der Anpassung des Geburtsmodus hinsichtlich vaginaler Geburt oder Sectio cesarea. Das Zusammenwirken aus zu erwartenden Risiken und dem auf die Schwangere ausgeübten Entscheidungsdruck führt möglicherweise zu einer pathologischen Ausprägung psychischer Symptome. Im Rahmen der BREXIT-Studie sollen Interventionsmöglichkeiten bei BEL sowie Auswirkungen auf die mentale Gesundheit der Patientinnen untersucht werden. Die Arbeit befasst sich mit der Frage, inwieweit eine BEL ein unabhängiger Risikofaktor für die Entwicklung einer psychischen Belastung ist und welche Faktoren die mentale Gesundheit beeinflussen können.
Im Rahmen der Sprechstunde für Beckenendlage an der Charité Campus Mitte wurden 379 Patientinnen mit Beckenendlage ab 36+0 Schwangerschaftswochen rekrutiert. Eine Kontrollgruppe aus 128 Frauen mit Schädellage aus der Schwangerenberatung diente zum Vergleich. Mithilfe des psychologischen Fragebogens „Depression Anxiety Stress Scales“ (DASS-21) wurden Symptome von Depression, Angst und Stress erfragt. Neben der Feststellung der Prävalenz psychischer Symptome sollten demografische und klinische Risikofaktoren mittels einer logistischen Regressionsanalyse identifiziert werden.
Eine mindestens moderate Ausprägung von Symptomen von Depression, Angst und Stress konnte bei 5,8%, 14,5% und 11,9% der Patientinnen in der Studiengruppe festgestellt werden. Frauen mit Schädellage erzielten jedoch eine signifikant höhere Punktzahl für Angst (p = 0,035). Die Hypothese der psychischen Mehrbelastung von Patientinnen mit Beckenendlage im Vergleich zu Schädellage bestätigte sich nicht. Als Risikofaktoren für psychische Symptome konnten das maternale Alter und das Vorhandensein von Vorerkrankungen sowie von mindestens zwei zurückliegenden Geburten identifiziert werden. Für Mehrgebärende mit Beckenendlage im Vergleich zu Erstgebärenden wurde ein 8-faches Risiko für mindestens moderate Symptome von Depression, ein 3,4-faches Risiko für Angst sowie ein 4,5-faches Risiko für Stress ermittelt.
Im Regressionsmodell zeigte sich, dass die Kindslage ungeeignet zur Vorhersage mütterlicher psychischer Belastung ist. Trotzdem konnten Patientinnen mit Beckenendlage mit pathologisch erhöhten Punktzahlen im DASS-21 identifiziert werden. Die Ermittlung von Multiparität und Vorerkrankungen als Risikofaktoren für psychische Belastung sowie die Nutzung psychologischer Fragebögen als Screening im pränatalen Kontext ermöglichen es, Patientinnen mit Behandlungsbedarf besser zu identifizieren und gezielt Interventionsmöglichkeiten anzubieten.Breech presentation as a fetal lie anomaly affects about 5% of pregnancies. Due to the associated risks such as arrest of labour, asphyxia and hip dysplasia the diagnosis can be a considerable psychological burden for the expectant mother. The often late detection requires intensive decision-making regarding possible interventions such as external cephalic version and adjustment of the mode of delivery with regard to vaginal birth or cesarean section. The combination of expected risks and the demands on the pregnant woman to make a decision may lead to a pathological development of psychological symptoms. The BREXIT study aims to investigate possible interventions for breech as well as the impact on the mental health of the expectant mothers. The study is dedicated to the question to what extent breech is an independent risk factor for the onset of a mental disease and which factors may cause pathological distress.
The study group, which included 379 patients from 36+0 weeks of pregnancy, was recruited within the framework of the consultation for breech at the Charité Campus Mitte. A control group consisting of 128 patients with fetal cephalic presentation served as a comparison. The psychological questionnaire "Depression Anxiety Stress Scales" (DASS-21) was used to evaluate symptoms of depression, anxiety and stress. In addition to determining the prevalence of psychological distress, demographic and clinical risk factors were identified using a binary logistic regression analysis.
An at least moderate expression of symptoms of depression, anxiety, and stress was found in 5.8%, 14.5%, and 11.9% of patients in the study group, respectively. However, women with cephalic presentation scored significantly higher for anxiety (p = 0.035). The hypothesis of increased psychological distress in patients with breech presentation compared to cephalic presentation was not confirmed. Maternal age, the presence of pre-existing diseases and at least two previous pregnancies were identified as risk factors for psychological symptoms. For multiparous women with breech compared to nulliparous women, an 8-fold risk for at least moderate symptoms of depression, a 3.4-fold risk for anxiety, and a 4.5-fold risk for stress was found.
In the regression model, fetal position was shown to be inappropriate for predicting maternal psychological distress. Nevertheless, patients with breech with pathologically elevated scores on the DASS-21 were identified. The detection of multiparity and pre-existing conditions as risk factors for psychological distress, as well as the use of psychological questionnaires as screening in the prenatal context, allow for better identification of patients in need of treatment and targeted intervention options
