    Bioaktives Adrenomedullin (Bio-ADM) und Proenkephalin A 119- 159 (PENK) als potenzielle neue prognostische Biomarker bei kardialer Amyloidose

    Background: Cardiac amyloidosis (CA), resulting from cardiac deposition of different misfolded precursor proteins as amyloid, is the main determinant of survival in systemic amyloidosis. Although the prognosis of CA is generally poor, it may range from a few months to several years, depending on the type and severity of CA. Consequently, methods to predict clinical outcome are essential, both to provide patients with a risk assessment and to guide therapeutic decision-making. Although the identification of several prognostic biomarkers, and the validation of multiparametric staging systems based on them, has already improved risk-stratification significantly, clinical experience indicates a remaining degree of inaccuracy. Thus, options facilitating further improvements are much sought after. Objectives: The aim of this study is to evaluate the potential role of Bioactive Adrenomedullin (Bio-ADM) and Proenkephalin A119-159 (PENK) as novel prognostic biomarkers in CA. Methods: 55 patients with transthyretin (ATTR-CA; n=40) or immunoglobulin light chain CA (AL-CA; n=15) were prospectively enrolled and followed for ≥12 months regarding the occurrence of major adverse cardiac events (MACE), including death, and a composite renal endpoint (CRE). Baseline Bio-ADM and PENK plasma concentrations were quantified using commercially available immunoluminometric assays. Correlations of Bio-ADM and PENK with clinical, laboratory, echocardiographic or electrocardiographic parameters were explored using Spearman’s rank correlation coefficient. The ability of Bio-ADM and PENK to predict the occurrence of the predefined endpoints within 1 year was assessed using Kaplan-Meier analyses and Cox regression models. Results: Bio-ADM correlated with cardiac and inflammatory biomarkers. Bio-ADM was significantly elevated in patients who died or experienced a MACE, but not in those who met the CRE. Accordingly, Bio-ADM was identified as a strong predictor of death and MACE within 1 year, outperforming all other tested biomarkers. Notably, Bio-ADM was also predictive of the CRE, although its prognostic power was marginal. PENK was generally higher among AL-CA patients and correlated with cardiac as well as renal biomarkers. PENK was significantly elevated among patients who experienced a CRE, but not in those who died or experienced a MACE. Accordingly, PENK was found to be a significant predictor of the CRE with superior prognostic power to that of all other tested biomarkers. PENK was also predictive of death, but its prognostic power was limited. By contrast, PENK did not predict MACE. Conclusions: The results of this study indicate that both Bio-ADM and PENK are promising new biomarkers in CA, differing in the outcomes they predict. While Bio-ADM is a strong predictor of MACE or death, PENK may be used to predict renal outcome. However, implementation of Bio-ADM or PENK in clinical routine care requires prior validation in larger cohorts.Hintergrund: Kardiale Amyloidose (CA), resultierend aus kardialer Ablagerung verschiedener fehlgefalteter Vorläuferproteine als Amyloid, ist die Hauptdeterminante des Überlebens bei systemischen Amyloidosen. Obwohl die Prognose der CA generell schlecht ist, variiert sie je nach Art und Schweregrad zwischen einigen Monaten und mehreren Jahren. Folglich sind Prädiktoren des klinischen Verlaufs für die Einschätzung des Risikos und therapeutische Entscheidungen notwendig. Die Identifizierung prognostischer Biomarker und die Validierung darauf basierender multiparametrischer Staging-Systeme verbesserten die Risikostratifizierung bereits erheblich. Gemäß klinischer Erfahrung verbleibt jedoch ein gewisses Maß an Ungenauigkeit. Daher wird stetig nach Methoden zur weiteren Verbesserung der Risikostratifizierung gesucht. Ziele: Ziel dieser Studie ist es, bioaktives Adrenomedullin (Bio-ADM) und Proenkephalin A119-159 (PENK) als mögliche prognostische Biomarker bei CA zu untersuchen. Methoden: 55 Patienten mit Transthyretin- (ATTR-CA; n=40) oder Immunglobulin-Leichtketten-CA (AL-CA; n=15) wurden prospektiv eingeschlossen und über ≥12 Monate hinsichtlich des Auftretens schwerwiegender unerwünschter kardialer Ereignisse (MACE), einschließlich Tod, und eines zusammengesetzten renalen Endpunkts (CRE) verfolgt. Bio-ADM- und PENK-Konzentrationen wurden mit immunoluminometrischen Assays quantifiziert. Korrelationen von Bio-ADM und PENK mit klinischen, labortechnischen, echo- oder elektrokardiografischen Parametern wurden mit Spearman's Rank Korrelationskoeffizienten untersucht. Mit Kaplan-Meier-Analysen und Cox-Regressionsmodellen wurde bewertet, ob Bio-ADM und PENK das Auftreten der vordefinierten Endpunkte binnen eines Jahres vorhersagen können. Ergebnisse: Bio-ADM korrelierte mit kardialen und inflammatorischen Biomarkern. Bio-ADM war bei Patienten, die verstarben oder MACE erlitten, signifikant erhöht, nicht aber bei Patienten mit CRE. Unter allen getesteten Biomarkern wurde Bio-ADM als stärkster Prädiktor für Tod und MACE identifiziert. Bio-ADM war auch ein Prädiktor für CRE, jedoch mit geringer Aussagekraft. PENK war bei AL-CA-Patienten erhöht und korrelierte mit kardialen und renalen Biomarkern. Bei Patienten mit CRE war PENK signifikant erhöht, nicht jedoch bei denen, die starben oder MACE erlitten. Entsprechend erwies sich PENK als signifikanter, allen anderen getesteten Biomarkern überlegener, Prädiktor für CRE. PENK war auch ein Prädiktor für Tod, jedoch mit begrenzter prognostischer Aussagekraft, aber kein Prädiktor für MACE. Schlussfolgerungen: Die Ergebnisse der Studie indizieren, dass Bio-ADM und PENK beide vielversprechende neue Biomarker für CA sind, sich aber in den Ergebnissen, die sie vorhersagen, unterscheiden. Während Bio-ADM ein starker Prädiktor für MACE oder Tod ist, dient PENK zur Vorhersage des renalen Verlaufs. Die Einführung beider Biomarker in die klinische Routineversorgung erfordert jedoch eine vorherige Validierung in größeren Kohorten

    A postcode lottery in education? Explaining regional inequality in multilevel systems

    Inequality is a challenge for societies around the globe. In addition to its socio-economic and political dimensions, inequality also has a territorial dimension. Focusing on educational inequality across regions, this article explores factors that shape regional differences in student enrolment and educational attainment in 14 OECD countries. The findings of the article suggest that a strong level of regional authority over education increases regional inequality in education. Further factors are regional differences in education expenditures and variation in the population size of regions. Low regional inequality in education seems to be explained by a low degree of regional authority or low differences in education spending. By examining regional inequality in education, the article enhances our understanding of the way multilevel governance influences the quality of life within regions. In particular, it highlights the impact of decentralization on territorial inequality

    Study to evaluate the reduction of agitation through the presence of a therapy dog in hospitalized patients with dementia

    Die Etablierung einer hundegestützten Intervention in einem Akutkrankenhaus trifft auch Jahrzehnte nach der initialen Idee von Hunden als Kotherapeuten in den meisten Fällen auf große Hürden. Dies ist nicht zuletzt der Tatsache geschuldet, dass die Studienlage zu den Effekten von Therapiehundeeinsätzen sehr divers und nicht einheitlich ist. Insbesondere als unterstützende therapeutische Maßnahme bei Patient*innen mit dementiellem Syndrom werden Besuchstherapiehunde in Pflegeeinrichtungen zunehmend eingesetzt und deren Effekte untersucht. Eine Lücke in der Datenlage besteht jedoch weiterhin im Einsatz im Akutkrankenhaus. Die hier präsentierte Arbeit widmet sich der Fragestellung, ob ein milieutherapeutisch eingesetzter Therapiehund auf einer gerontopsychiatrischen Station eines Akutkrankenhauses sich positiv auf die Agitationssymptomatik von Demenzpatient*innen auswirkt. Dazu wurden 26 Patient*innen mit einem Mini-Mental-Status-Test (MMST) < 21 Punkten und der Diagnose einer Demenz vom Alzheimer- oder vom Mischtyp, welche auf der gerontopsychiatrischen-geriatrischen Gemeinschaftsstation der Oberhavel Klinik Hennigsdorf behandelt wurden, in die Studie eingeschlossen. Untersucht wurde das Ausmaß der Agitation an einem Tag, an dem der Therapiehund anwesend war versus einen Tag, an welchem er abwesend war. Klinisch geschah die Evaluation der Agitation durch eine für die Anwesenheit des Hundes verblindete Raterin anhand der Overt Agitation Severity Scale (OASS). Als physiologischer Stressparameter wurde die Herzfrequenzvariabilität und als neurobiologischer Parameter das Serum Brain derived neurotrophic factor (BDNF) bestimmt. Die Patient*innen dienten somit als ihre eigene Kontrolle. Es konnte eine signifikant geringere Agitationssymptomatik mittels OASS gefunden werden, wenn der Hund anwesend war (p = 0.006). Die erhobenen Daten wiesen keinen signifikanten Unterschied in der Herzfrequenzvariabilitätsanalyse gemessen an mittlerer Herzfrequenz (p = 0.65), Root mean squared standard deviation (RMSSD) (p = 0.63), Hoch-frequenz-Band (HF) (p = 0.27) und in der Analyse des Serum BDNF (p= 0.42) auf. Aus den Ergebnissen der Studie lässt sich schließen, dass Therapiehunde als milieutherapeutische Settingvariable zur Reduktion von Agitation bei Patient*innen mit dementiellem Syndrom auch im Akutkrankenhaus eingesetzt werden können.The implementation of dog-assisted therapy in a hospital is a challenging process even after decades of the first successful attempts. The non-conclusive studies about dog therapy in patients with dementia are greatly to blame for this. Especially in care facilities, the use of dogs in a therapeutic setting for patients with dementia is common and has been studied increasingly in the last decade. Concerning the setting of a ward in an acute hospital there is still a significant lack of studies investigating the effect of therapy dogs on the behavior of patients with dementia. This study was designed to investigate the effects of a therapy dog on agitation of inpatients with dementia in a geropsychiatric ward. 26 patients with diagnosed Alzheimer or mix type dementia and Mini-Mental Status Examination score < 21 points were included into the study. The agitation on the intervention day, with the dog and his handler present, and the control day, without the dog’s presence, were compared for each patient. The assessment of agitation was conducted by a rater blinded for the presence of the dog via the Overt Agitation Severity Scale (OASS). Additionally, the measurements of the heart rate variability (HRV) as a physiological stress parameter and serum level of brain-derived neurotrophic factor (BDNF) as a neurobiological stress parameter were performed on both days. The OASS showed a significant reduction of agitation when the dog was present (p = 0.006). No differences were found for the HRV parameters mean heart rate (p = 0.65), root mean square of successive differences (p = 0.63), and high frequencies (p = 0.27) nor in serum BDNF concentrations (p = 0.42). The study allows to draw the conclusion that a therapy dog can be successfully implemented in the treatment of inpatients with dementia to reduce agitation

    Correlation of in vitro biofilm formation capacity with persistence of antibiotic-resistant Escherichia coli on gnotobiotic lamb’s lettuce

    Bacterial contamination of fresh produce is a growing concern for food safety, as apart from human pathogens, antibiotic-resistant bacteria (ARB) can persist on fresh leafy produce. A prominent persistence trait in bacteria is biofilm formation, as it provides increased tolerance to stressful conditions. We screened a comprehensive collection of 174 antibiotic-susceptible and -resistant Escherichia coli originating from fresh leafy produce and its production environment. We tested the ability of these strains to produce biofilms, ranging from none or weak to extreme biofilm-forming bacteria. Next, we tested the ability of selected antibiotic-resistant isolates to colonize gnotobiotic lamb’s lettuce (Valerianella locusta) plants. We hypothesized that a higher in vitro biofilm formation capacity correlates with increased colonization of gnotobiotic plant leaves. Despite a marked difference in the ability to form in vitro biofilms for a number of E. coli strains, in vitro biofilm formation was not associated with increased survival on gnotobiotic V. locusta leaf surfaces. However, all tested strains persisted for at least 21 days, highlighting potential food safety risks through unwanted ingestion of resistant bacteria. Population densities of biofilm-forming E. coli exhibited a complex pattern, with subpopulations more successful in colonizing gnotobiotic V. locusta leaves. These findings emphasize the complex behavior of ARB on leaf surfaces and their implications for human safety

    Transition-path probability depends non-monotonically on memory time for non-Markovian systems

    Defining low-dimensional reaction coordinates is crucial for analyzing the dynamics of complex systems and for comparison with experiments. The maximal value of the transition-path probability along the reaction coordinate x, p(TP|x), is a common estimator for reaction-coordinate quality by comparing to the theoretical maximal value of 1/2 in the overdamped Markovian limit. We show by analytical arguments and simulations that for non-Markovian dynamics p(TP|x) is non-monotonic as a function of the memory time and exceeds 1/2 for long memory time.     Neuartige präoperative Planungsmethoden mit Verwendung von nTMS und Diffusions-MRT zur Bewertung der Sprachfunktion bei Gliompatient*innen

    Background: Glioma resection poses a delicate balance between the imperative to achieve maximum tumor resection for extended survival and the need to avoid neurological impairment. Effective planning procedures are crucial for navigating this delicate decision-making process. Methods like tractography, a technique utilizing diffusion magnetic resonance imaging (dMRI) to visualize fiber bundles in the brain, and navigated transcranial magnetic stimulation (nTMS), a neurostimulation method for mapping brain regions, provide valuable insights non-invasively into the subcortical and cortical functionality of language. Methods: For study 1 and 2, we employed a novel nTMS language mapping protocol, in which tractography of the arcuate fasciculus (AF) was registered onto the patient’s T1 image and the cortical endings of the AF stimulated during mapping. The tractogram-based protocol was compared against a pre-existing protocol stimulating generally over the perisylvian cortex. In study 2, we took a linguistic approach and tested the impact of using transitive vs. intransitive verbs during nTMS language mapping. In study 3, we analysed preoperative diffusion data using TractSeg, an automated tract segmentation algorithm. After extracting the diffusion metrics using MRtrix3, a machine learning (ML)-based support vector machine (SVM) model was implemented to predict the occurrence of preoperative aphasia. Results: The implemented tractogram-based protocol in study 1 demonstrated proof-of-concept in the clinical workflow and significantly higher error rates (ER) when compared to the perisylvian protocol. Study 2 also stimulated endings of the AF with action naming and demonstrated a higher ERs for transitive verbs, especially in the temporal endings of the AF. In study 3, the model with the best performance utilized dMRI-based features, tumor grade and location, relative tract volumes, and patient demographics, achieving a maximum accuracy of 81% (AUC = 85%, specificity = 85%, sensitivity = 73%). Conclusion: Refinements to preoperative planning techniques play a crucial role in achieving optimal and individualized patient outcomes. This project involved the introduction of a novel targeted protocol, specifically stimulating the AF. Furthermore, we adapted the language mapping protocol in study 2 to evaluate the impact of linguistic stimuli on mapping outcomes and in study 3, employed ML-based methods to predict preoperative aphasia. Together, these initiatives aimed to enhance the assessment of language function in the preoperative setting and advance planning procedures for glioma patients.Hintergrund: Bei der Resektion von Gliomen ist die individuelle Abwägung zwischen maximaler Tumorresektion, um das Überleben des Patienten zu verlängern, und der Vermeidung zusätzlicher neurologischer Beeinträchtigungen durch die Beschädigung noch intakter funktioneller Strukturen eine zentrale Herausforderung. Daher sind effektive Planungsverfahren notwendig, die diesen anspruchsvollen Entscheidungsprozess unterstützen. Nichtinvasive Methoden wie die Traktographie, eine Technik, welche die Diffusionsbildgebung mittels Magnetresonanztomographie (dMRI) nutzt, um Faserbündel im Gehirn zu visualisieren, und die navigierte transkranielle Magnetstimulation (nTMS), eine Neurostimulationsmethode zur Kartierung von Hirnregionen, erlauben es, die subkortikale und kortikale Lokalisation der Sprache zu untersuchen. Methoden: In den Studien 1 und 2 haben wir ein neuartiges nTMS-Sprachkartierungsprotokoll verwendet, bei dem präoperativ ein Datensatz erstellt wurde, in dem die Traktographie des Fasciculus arcuatus (AF) auf das T1-MRT des Patienten registriert wurde, und die kortikalen Enden des AF während der Kartierung stimuliert wurden. Das gezielte, traktographiebasierte Protokoll wurde mit einem bereits bestehenden Protokoll verglichen, das ausschließlich über dem perisylvischen Kortex stimuliert. In Studie 2 haben wir einen linguistischen Ansatz untersucht, indem wir die Auswirkungen der Verwendung transitiver statt intransitiver Verben, während der nTMS-Sprachkartierung getestet haben. In Studie 3 haben wir präoperative Diffu-sionsdaten mit TractSeg, einem automatisierten Traktsegmentierungsalgorithmus, analysiert. Nach Extraktion der Diffusionsmetriken unter Verwendung von MRtrix3 wurde eine auf maschinellem Lernen (ML) basierende Support-Vector-Machine (SVM) implementiert, um das Auftreten präoperativer Aphasie bei Patienten vorherzusagen. Ergebnisse: Studie 1 demonstrierte einen Konzeptnachweis des traktographie-basierten Kartierungsprotokolls im klinischen Alltag und signifikant höhere Fehlerquoten im Vergleich zum perisylvischen Protokoll. Studie 2 stimulierte ebenfalls Enden des AFs mit Aktionsbenennungen und zeigte eine höhere Fehlerquoten für transitive Verben, insbesondere in den temporalen Enden des AF. In Studie 3 nutzte das Modell mit der besten Leistung dMRI-basierte Merkmale, Tumorgrad und -ort, relative Traktvolumina sowie demografische Daten, um eine maximale Genauigkeit von 81% zu erreichen (AUC = 85%, Spezifität = 85%, Sensitivität = 73%). Schlussfolgerung: Die individualisierte Optimierung präoperativer Planungstechniken spielt eine entscheidende Rolle für die Verbesserung der Patientenversorgung. In diesem Projekt wurde die Evaluation eines neuartigen, gezielten Protokolls, das speziell den AF stimuliert, demonstriert. Darüber hinaus haben wir das Sprachkartierungsprotokoll in Studie 2 angepasst, um die Auswirkungen von sprachlichen Reizen auf die Kartierungsergebnisse zu bewerten, und in Studie 3 ML-basierte Methoden zur Vorhersage von Aphasien eingesetzt. Durch die genauere Untersuchung der Sprachfunktionalität vor der Operation kann zusätzliche neurologische Beeinträchtigung verhindert und die Patientenversorgung verbessert werden
