    Global knowledge flows and the psychiatric encounter in Indonesia

    In this article, we examine the clinical encounters of people diagnosed with a severe mental illness (SMI). Drawing on more than 1-year of ethnographic research and interviews in Indonesia, we show that instances of moral self-reflection occurring in the process of acquiring and appropriating clinical insight emerge at the intersection of heterogeneous discursive regimes. When biomedical notions of health and illness dominate these discourses, they reimagine pre-existing notions about spirituality and religion. Furthermore, consenting to psychiatric notions of health and illness can create common ground and a sense of shared experience, leading to grassroots movements for the empowerment of the mentally ill, self-help groups, and other support structures. At the same time, these processes can increase uncertainty and be generative of a culture of blame, as individuals are caught in overlapping and at times contradictory moral systems that each have the potential to strip patients of their moral status

    High-rise syndrome in cats (part 2): injury patterns and survival rate

    Objectives This two-part study investigates the relationship between risk factors, injury patterns and survival rates in cats affected by high-rise syndrome (HRS). By identifying key prognostic and predictive factors, such as age, fall height and impact surface, we aim to enhance veterinarians’ ability to provide accurate prognoses and optimise patient management. In addition, this study seeks to address inconsistencies in the literature by re-evaluating the influence of these factors on injury severity and survival outcomes. Methods We analysed medical records and radiographs from 1125 cases of feline HRS treated at the Freie Universität Berlin’s small animal clinic between 2004 and 2013. All cases involved falls from a minimum height of 4 m. Data collected included sex, age, body weight, fall height, impact surface, injury details, prior treatment, time to veterinary presentation (>4 h or <4 h), radiographic findings and survival outcome. Results Among the 1125 HRS cases, circulatory shock was observed in 48.6%, craniocerebral injury in 2.8%, thoracic trauma in 58.3%, orofacial lesions in 51.1% and blunt abdominal trauma in 14.6%. Musculoskeletal injuries were common, with limb fractures occurring in 47.2% of cases and pelvic fractures in 11.1%. The overall survival rate was 87%, while 13.3% of cats died or were euthanased as a result of trauma severity or poor prognosis. Significant correlations were found between injury severity, age, impact surface and fall height. Conclusions and relevance This study includes the largest sample of HRS cases in cats to date, providing essential insights for veterinarians and pet owners regarding prognosis and treatment protocols. Although survival rates are high, euthanasia as a result of severe trauma and financial constraints remains a concern. The identified risk factors and injury patterns can help veterinarians provide more accurate and individualised prognoses, ultimately improving patient care and opening new avenues for future research, such as the importance of preventive measures at home and the role of treatment protocols for initial stabilisation before referral

    Katastrophenvorsorge ist unerlässlich, um Leben zu retten, Schäden zu begrenzen und wirtschaftliche Stabilität in Krisenzeiten zu sichern. Diese Arbeit fokussiert sich auf die Entwicklung eines resilienten, dezentralen IT-System (Informationstechnologie-System), welche die Vernetzung und Koordination zwischen den vielfältigen Akteuren im Katastrophenschutz erheblich verbessert. Während derzeit Informationen oft mühsam manuell gesammelt werden, ermöglicht diese Architektur einen effizienten, digitalen Austausch, der die Zusammenarbeit zwischen Einsatzkräften, Behörden und der Bevölkerung optimiert. Durch umfassende Anforderungsanalysen und die Anwendung moderner Entwicklungsmethoden wurde eine flexible Softwarearchitektur entworfen, die sich als Anwendungsfall zu Evaluierung des Systems auf die Koordination spontaner Freiwilligeneinsätze spezialisiert. Die Lösung umfasst ein webbasiertes Dashboard und mobile Apps, welche in einem realistischen Szenario getestet wurden. Die Ergebnisse zeigen eine hohe Akzeptanz und Zufriedenheit unter den Nutzenden, was die praktische Anwendbarkeit und den Nutzen des Systems unterstreicht. Dieses System bietet ein Modell, das auf verschiedene Szenarien und Stakeholder angepasst werden kann

    Can Reduced Hand Grip Strength Predict Fatigue in Primary Sjogren’s Syndrome? An Analysis of Associated Symptoms and Comparison to Chronic Fatigue Syndrome

    Hintergrund: Fatigue gehört mit einer Prävalenz von etwa 70% zu den häufigsten und gleichzeitig belastendsten Symptomen des primären Sjögren Syndroms (pSS). Sowohl das mangelnde Wissen über die Pathogenese dieses Symptoms als auch dessen fehlende Objektivierbarkeit stellen derzeit Hürden in der Therapie und Forschung dar. Ziel dieser Untersuchung war die Evaluation einer Handkraftmessung (HGS) als mögliches diagnostisches Instrument für Fatigue und die Suche nach mit einer höheren Fatigueschwere assoziierten Antikörpern und Charakteristika. Methoden: Bei 19 Patient:innen mit pSS wurden anhand verschiedener Fragebögen Daten in Bezug auf ihre Fatiguesymptome, Krankheitsaktivität, Depression, Angst, Schlafqualität, Alltagsaktivität und ihre autonomen Symptome erhoben. Zudem wurden die Handkraft, Knochendichte und Titer der a1- , a2- , b1- , b2-, M3- und M4-Rezeptor- Autoantikörper bestimmt. Die einzelnen Parameter wurden dann mit Fatigue korreliert. Für die HGS wurde eine ROC-Analyse durchgeführt. Ergebnisse: Parameter der HGS korrelierten eng mit Fatigue (p <0.05). Es konnte eine Patient:innengruppe mit hoher Fatigue identifiziert werden, bei der zudem die Prävalenz von Schmerzen (p=.045), depressiven Symptomen (p=.021), Schlafstörungen (p=.020) signifikant erhöht war. Diese Patient:innengruppe konnte anhand der mittleren Handkraft zuverlässig identifiziert werden. Des Weiteren waren die Titer der b1- , b2- und M4- Rezeptor-Autoantikörper erhöht und korrelierten signifikant mit der Krankheitsaktivität (p<.05). Zusammenfassung: Patient:innen mit pSS mit ausgeprägter Fatigue präsentierten sich mit einer höheren Symptomlast des pSS und mehr Komorbiditäten. Wiederholte HGS- Messungen können als objektive Untersuchung geeignet sein, diese vulnerable Gruppe zu identifizieren.Background: Fatigue being prevalent in 70% of patients is one of the most common and debilitating features in primary Sjogren’s Syndrome (pSS). Lacking understanding of its underlying mechanisms and failure to objectively quantify symptom load complicates treatment and further research. The aims of this study were to evaluate hand grip strength (HGS) as a diagnostic tool for fatigue assessment and to search for antibodies and characteristics associated with higher fatigue load. Methods: We assessed the level of fatigue, disease activity, depression, anxiety, sleep quality, physical activity and presence of autonomic symptoms in 19 patients with pSS via questionnaires. HGS, bone density and a1-, a2-, b1-, b2-, M3- and M4-receptorautoantibodies were measured. These parameters were then correlated with fatigue. An ROC-Analysis for HGS was performed. Results: HGS parameters strongly correlated with fatigue symptom severity in pSS (p<0.05). A cluster of patients with high fatigue load could be identified, that also had significantly higher incidence of pain (p=.045), depression (p=.021) and sleep disorder (p=.020). This patient cluster could be sufficiently identified using mean HGS. Furthermore, levels of b1-, b2- and M4-receptor-autoantibodies were elevated and correlated significantly with disease activity (p<0.05). Conclusions: Highly fatigued pSS patients presented with higher symptom load and more comorbidities. Repeat HGS assessment could be an objective measure to identify this vulnerable group of patients

    Systematic Review and Meta-analysis on Psychiatric Adverse Events in Tenofovir-Based HIV-Medication and Prophylaxis

    Einleitung: Die neuropsychiatrische Toxizität der antiretroviralen Therapie (ART) sowie deren Auswirkungen auf die psychologische Gesundheit von Personen, die mit HIV leben, ist wiederkehrender Gegenstand aktueller Forschung. Obwohl Tenofovir zu den am häufigsten kombinierten Wirkstoffen der HIV-Therapie und -Prophylaxe zählt, ist wenig über das Risiko Tenofovir-basierter Medikation bekannt, psychiatrische „adverse events“ (PAE) auszulösen. Methodik: Um diese Lücke zu schließen, wurde eine systematische Übersichtsarbeit mit Metaanalyse zu PAE-Berichten in zuvor antiretroviral unbehandelten Personen durchgeführt. Randomisierte, kontrollierte Studien zur ART oder Präexpositionsprophylaxe (PrEP) wurden inkludiert, wenn sie mindestens einen Interventionsarm zu Tenofovir Disoproxil Fumarat oder Tenofovir Alafenamid beinhalteten. Metaanalysen wurden durchgeführt, wenn in Studien zur ART ein direkter Vergleich vorlag von 1) verschiedenen Tenofovir-Wirkstoffen, 2) Tenofovir-basierten versus Abacavir/Lamivudin-basierten Hintergrundregimen, 3) Tenofovir-basierter ART mit und ohne Efavirenz und 4) im Falle von Placebo-kontrollierten Studien zur PrEP. Zur statistischen Auswertung wurden Odds Ratios (OR) mit einem 95% Konfidenzintervall (KI) mittels random-effects model berechnet. Ergebnisse: Diese systematische Übersichtsarbeit beinhaltete 59 Studien mit 23.623 Tenofovir-exponierten Personen. Unter den am häufigsten berichteten Symptomen waren insomnia, depression, anxiety und abnormal dreams mit einem höheren Auftreten in Studien zur ART als zur PrEP. Metaanalysen zu Vergleichen von Tenofovir Alafenamid versus Tenofovir Disoproxil Fumarat und zu Tenofovir/Emtricitabin-haltiger ART versus Abacavir/Lamivudin zeigten keine signifikanten Unterschiede im Auftreten von PAE. Gleiches galt für einen Vergleich von Tenofovir Disoproxil Fumarat-haltiger PrEP versus Placebo. Ein Vergleich Tenofovir Disoproxil Fumarat-basierter ART in Kombination mit Efavirenz zeigte signifikant höhere Risiken für somnolence (OR 2,91; 95% KI 2,05-4,13; p < 0,00001), abnormal dreams (OR 2,66; 95% KI 1,94-3,64; p < 0,00001), sleep disorder (OR 2,27; 95% KI 1,28-4,02; p = 0,005), decreased appetite (OR 1,61; 95% KI 1,07-2,44; p = 0,02) und anxiety (OR 1,56; 95% KI 1,18-2,06; p = 0,002) im Vergleich zu einer ART ohne Efavirenz. Diskussion: Unter Tenofovir-basierter Medikation kommt es, insbesondere im Kontext der antiretroviralen Therapie, zu einem häufigen Auftreten von PAE. Die Risiken variieren dabei je nach eingenommener Wirkstoffkombination. Obwohl ein kausaler Zusammenhang zwischen Tenofovir und psychiatrischen Ereignissen nicht festgestellt wurde, unterstreicht die hohe Prävalenz psychiatrischer AE unter der ART die Notwendigkeit einer gezielten Überwachung der psychischen Gesundheit während einer antiretroviralen Therapie.Introduction: There is an ongoing debate on the neuropsychiatric toxicity of antiretroviral therapy (ART) in people living with HIV regarding psychological health. Tenofovir is one of the most frequently combined agents of HIV therapy and prophylaxis. However, little is known about the risk of Tenofovir-based medication for psychiatric adverse events (PAE). Methods: A systematic review and meta-analysis was conducted on randomized, controlled trials in treatment-naïve adults reporting PAE. ART or pre-exposure prophylaxis (PrEP) studies containing tenofovir disoproxil fumarate or tenofovir alafenamide-based treatment arms were included. Meta-analyses were performed in case of ART-studies with direct comparisons of 1) different tenofovir formulations, 2) tenofovir-based versus abacavir/lamivudine-based background regimens, 3) tenofovir-based ART with and without efavirenz, and finally in placebo-controlled PrEP-studies. Pooled odds ratios (OR) and 95% confidence intervals (CI) were calculated using a random-effects model. Results: This study incorporated 59 trials including 23,623 patients taking tenofovir-based medication. Among the most commonly reported PAE were insomnia, depression, anxiety, and abnormal dreams with higher numbers reported in ART than in PrEP trials. Meta-analyses of trials comparing tenofovir alafenamide versus tenofovir disoproxil fumarate and tenofovir disoproxil fumarate/emtricitabine-based ART versus abacavir/lamivudine showed no significant risk difference for PAE, and neither did a comparison of tenofovir disoproxil fumarate-based PrEP with placebo. However, tenofovir disoproxil fumarate-based ART containing efavirenz yielded significantly higher risks for somnolence (OR 2.91; 95% CI 2.05-4.13; p < 0.00001), abnormal dreams (OR 2.66; 95% CI 1.94-3.64; p < 0.00001), sleep disorder (OR 2.27; 95% CI 1.28-4.02; p = 0.005), decreased appetite (OR 1.61; 95% CI 1.07-2.44; p = 0.02), and anxiety (OR 1.56; 95% CI 1.18-2.06; p = 0.002) when compared to tenofovir-based regimens without efavirenz. Discussion: PAE frequently occur in patients taking Tenofovir-based treatments, especially when used for antiretroviral therapy. Risks vary between different, combined regimens. Although a causal relationship between Tenofovir itself and the occurrence of PAEs was not detected, the high prevalence of psychiatric symptoms in patients receiving ART emphasizes a need for targeted monitoring of psychological health during treatment

    Thorium(IV) and Uranium(IV) Complexes with 2,6-Dipicolinoylbis(N,N-diethylthiourea) Ligands

    The reaction of thorium nitrate hydrate with 2,6-dipicolinoylbis(N,N-diethylthiourea), H2Lpic, results in the hydrolysis of the organic ligand and the formation of [Th(2,6-dipicolinolate)2(H2O)4] (1). Hydrolysis can be avoided by the use of [ThCl4(DME)2] (DME = 1,2-dimethoxyethane) as the starting material and the exclusion of water. The product, [Th(Lpic)3]2− (2), crystallizes as diammonium salt in form of yellow crystals in moderate yields. The thorium ion in the complex is nine-coordinate by the central O,N,O donor atoms of three deprotonated {Lpic}2− ligands. The sulfur atoms of the ligands do not bind to the actinide ion, but establish hydrogen bonds to the ammonium counter ions. A similar coordination sphere is also observed in the uranium(IV) complex [UAu2(Lpic)3}] (3), which was obtained from a reaction between H2Lpic, [U2I6(1,4-dioxane)3] and [AuCl(tht)] (tht = tetrahydrothiophene) in the presence of triethylamine. Charge compensation is established by the linear coordination of two Au+ ions between each two sulfur atoms of the ligands. The products have been studied by X-ray diffraction and IR spectroscopy. The actinide ions in both {Lpic}2− complexes have coordination number nine, but establish slightly different coordination spheres

    Does text generation improve learning from expository text? A conceptual replication attempt

    The aim of the present study was to test the replicability of the text generation effect for learning with expository texts while systematically varying contextual factors that—based on extant literature—can be assumed to affect the occurrence and magnitude of the text generation effect. Seven experiments were conducted in which participants either read (control condition) or unscrambled sentences (generation condition) in expository texts. The experiments varied systematically on intentionality of learning, learning time constraint, retention interval, and study design. Contrary to expectations, no text generation effect could be found. Instead, some of the experiments even revealed a learning disadvantage for text generation compared to the reading control condition. In only one experiment (Experiment 6) and for just one of the learning measures, learners performed better when they had generated the texts. In sum, the results indicate that a generation effect is most likely to occur when learning is intentional, when learning time is unrestricted, and for immediate testing. The findings suggest that the applications of text generation in educational contexts are rather limited
