    Impact of Triglyceride Levels on the Long-term Clinical Outcomes in Patients With Acute Myocardial Infarction

    Background/Aim: Hypertriglyceridemia is a known cardiovascular risk factor. However, the relationship between serum triglyceride (TG) levels and the clinical outcomes in patients with acute myocardial infarction (AMI) is unclear. Patients and Methods: We conducted a single-center, retrospective observational study involving 538 consecutive patients with AMI who underwent emergent percutaneous coronary intervention within 12 hours of onset. Patients were categorized into three groups based on their serum TG levels at admission as follows: T1 group (TG <78 mg/dl, n=172), T2 group (78≤TG<141 mg/dl, n=177), and T3 group (141 mg/dl ≤TG, n=176). The primary endpoint was major adverse cardiovascular events (MACEs) defined as a composite of cardiovascular death, non-fatal MI, and non-fatal stroke. The median follow-up period was 2.4 (1.5-4.2) years. Results: Patients in the T1 group were older, had a higher proportion of females, and had fewer cardiovascular risk factors. However, they also had a higher prevalence of multi-vessel coronary artery disease and severely calcified culprit lesions. The T1 group had a significantly higher rate of MACEs (20.4% in T1, 12.4% in T2 and 8.5% in T3, p<0.05 by Log-rank test, respectively). Multivariate analysis revealed that T1 was an independent predictor of MACEs (hazard ratio=2.19, 95% confidence interval=1.16-4.14, p<0.05).     Choroid plexus enlargement in mild cognitive impairment on MRI: a large cohort study

    Objectives Previous studies have shown possible choroid plexus (CP) dysfunction in Alzheimer’s disease (AD) and highlighted CP enlargement on magnetic resonance imaging (MRI) as a predictive factor of AD. However, few studies have assessed the relationship between CP volume (CPV) and mild cognitive impairment (MCI). In this large elderly population study, we investigated the changes in CPV in patients with MCI using MRI above 65 years. Methods This cross-sectional study included 2144 participants (median age, 69 years; 60.9% females) who underwent 3T MRI; they were grouped as 218 MCI participants and 1904 cognitively healthy controls. The total intracranial volume (ICV), total brain volume (TBV), CPV, hippocampal volume (HV), and lateral ventricle volume (LVV) were calculated. Results CPV/ICV was a significant independent predictor of MCI (p < 0.01) after adjusting for potential confounders (age, sex, hypertension, hyperlipidemia, diabetes, and education level). The CPV/ICV ratio was also a significant independent predictor of MCI after adjusting for the TBV/ICV ratio (p = 0.022) or HV/ICV ratio (p = 0.017), in addition to potential confounders. The CPV was significantly correlated with the LVV (r = 0.97, p < 0.01). Conclusion We identified a relationship between CPV and MCI, which could not be explained by the degree of brain atrophy. Our results support CP dysfunction in MCI. Clinical relevance statement Choroid plexus volume measurement may serve as a valuable imaging biomarker for diagnosing and monitoring mild cognitive impairment. The enlargement of the choroid plexus, independent of brain atrophy, suggests its potential role in mild cognitive impairment pathology.     Improved Assessment of Juxtacortical Lesions in Multiple Sclerosis Using Highly-accelerated High-resolution Double Inversion Recovery MR Imaging with Deep Learning-based Reconstruction

    Purpose: Recently, a novel deep learning (DL)-based technique for reconstructing highly undersampled MR data (DL-Speed, DLS) has been developed, which demonstrated superior performance over compressed sensing. This study aimed to achieve high-resolution double inversion recovery (DIR) imaging using DLS (DLS-DIR) and compare its diagnostic performance in the detection of juxtacortical multiple sclerosis (MS) lesions with that of conventional DIR (C-DIR). Methods: We retrospectively analyzed MRI data from 25 patients with MS who underwent a comprehensive imaging protocol, including 3D fluid-attenuated inversion recovery (FLAIR), C-DIR, and DLS-DIR. A voxel size of 1.3 × 1.3 × 1.4 mm3 with a scan duration of 3 mins 55s were used for C-DIR, and isotropic 0.7 mm voxels with a scan time of 4 mins 23s were employed for DLS-DIR. Two neuroradiologists assessed the juxtacortical MS lesions during 2 separate reading sessions (one with C-DIR and the other with DLS-DIR). Lesions were categorized as subcortical white matter lesions, intracortical lesions, or mixed lesions involving both subcortical white and gray matter. The lesion counts per region were compared between the imaging techniques using the Wilcoxon signed-rank test. Results: DLS-DIR detected a significantly higher number of juxtacortical MS lesions compared to C-DIR (Radiologist A: 211 lesions vs. 164 lesions; Radiologist B: 209 lesions vs. 157 lesions, P < 0.05). DLS-DIR also identified more intracortical lesions (Radiologist A: 22 additional lesions, Radiologist B: 34 additional lesions, P < 0.05) and more mixed lesions (Radiologist A: 46 additional lesions, Radiologist B: 42 additional lesions, P < 0.05). Conclusion: The DLS technology enables high-resolution, whole-brain DLS-DIR imaging within a 5 mins acquisition time, which can be seamlessly incorporated into routine clinical workflows.     Prognostic impact of eligibility for adjuvant immunotherapy in locally advanced urothelial cancer

    Abstract Objective: To evaluate the effect of postoperative pathological findings related to the eligibility of adjuvant immunotherapy on oncologic outcomes in patients with localized and locally advanced muscle-invasive bladder carcinoma (MIBC) and upper tract urothelial carcinoma (UTUC). Patients and methods: We retrospectively evaluated 1082 patients treated with radical cystectomy (n = 597) and nephroureterectomy (n = 485) between January 2000 and April 2021. Patients were divided into two groups: pT3-4 or pN+ without neoadjuvant chemotherapy and ypT2-4 or pN+ treated with neoadjuvant chemotherapy (trial-eligible group) or others (trial-ineligible group). The primary outcome was the effect of trial eligibility for adjuvant immunotherapy on disease-free survival (DFS) and overall survival (OS). Secondary outcomes included the additional effect of lymphovascular invasion (LVI) status to the clinical trial criteria on prognosis and a risk model development. Results: The median ages of the patients were 69 and 72 years in the MIBC and UTUC groups, respectively. Fifty-two percent of patients met the trial inclusion criteria. Trial eligibility was significantly associated with poor DFS and OS among patients with MIBC and UTUC. LVI-positive status was significantly associated with poor prognosis among patients in the trial-eligible group. A very high risk (LVI+ or pN+ among the pT3-4 or ypT2-4) was significantly associated with poor prognosis. Conclusion: A total of 52% of patients were eligible for adjuvant immunotherapy. Trial eligibility was significantly associated with a poor prognosis. LVI+ and pN+ may play a key role in candidate selection for adjuvant immunotherapy.弘前大学博士(医学

    Expression of ISG60 is induced by TLR3 signaling in BEAS-2B bronchial epithelial cells: Possible involvement in CXCL10 expression

    Viral infections in the respiratory tract are common, and, in recent years, severe acute respiratory syndrome coronavirus 2 outbreaks have highlighted the effect of viral infections on antiviral innate immune and inflammatory reactions. Specific treatments for numerous viral respiratory infections have not yet been established and they are mainly treated symptomatically. Therefore, understanding the details of the innate immune system underlying the airway epithelium is crucial for the development of new therapies. The present study aimed to investigate the function and expression of interferon (IFN)-stimulated gene (ISG)60 in non-cancerous bronchial epithelial BEAS-2B cells exposed to a Toll-like receptor 3 agonist. BEAS-2B cells were treated with a synthetic TLR3 ligand, polyinosinic-polycytidylic acid (poly IC). The mRNA and protein expression levels of ISG60 were analyzed using reverse transcription-quantitative PCR and western blotting, respectively. The levels of C-X-C motif chemokine ligand 10 (CXCL10) were examined using an enzyme-linked immunosorbent assay, and the effects of knockdown of IFN-β, ISG60 and ISG56 were examined using specific small interfering RNAs. Notably, ISG60 expression was increased in proportion to poly IC concentration, and recombinant human IFN-β also induced ISG60 expression. By contrast, knockdown of IFN-β and ISG56 decreased ISG60 expression, and ISG60 knockdown reduced CXCL10 and ISG56 expression. These findings suggested that ISG60 is partly implicated in CXCL10 expression and that ISG60 may serve a role in the innate immune response of bronchial epithelial cells.
