    Fortsatt press på plassene. Kartlegging av belegg i psykiatriske akuttavdelinger 2003 og utvikling 2002-2003

    NORSK SAMMENDRAG: Denne rapporten presenterer resultatene fra en kartlegging av belegg i psykiatriske akuttavdelinger i 2003, og beskriver utviklingen fra 2002 til 2003. Bare akuttavdelinger med plikt til å yte øyeblikkelig hjelp ble inkludert i kartleggingen.\ud \ud Beleggsprosenten i kartleggingsuken var 99 prosent i 2002 og 96 prosent i 2003. Dette varierte i 2003 fra 79 prosent i Helse Midt-Norge til 112 prosent i Helse Vest. Beleggsprosenten var høyere enn 100 prosent i åtte helseforetak og lavere enn 90 prosent i ti helseforetak.\ud \ud På landsbasis var hele 13 prosent av inneliggende pasienter utskrivningsklare i 2002 og 14 prosent i 2003. Dette varierte i 2003 fra 8 prosent i Helse Midt-Norge til 21 prosent i Helse Sør.\ud \ud En betydelig andel av enhetene hadde i løpet av kartleggingsuken plassert pasienter andre steder enn i pasientrom. Trettito prosent av enhetene i 2003 og 40 prosent av enhetene i 2002 svarte bekreftende på spørsmål om det forekommer pasienter som deler rom med andre pasienter, men som egentlig skulle hatt enerom.\ud \ud Konklusjonen er at det fortsatt er stort press på plassene i akuttavdelingene. Når institusjonene utnytter plassen slik kartleggingen har vist, er det grunn til å stille spørsmål om forutsetningene for bruk av tvang er tilstede.SUMMARY IN ENGLISH: This report presents the results of a survey of capacity 1 in acute departments of psychiatry in 2003, and describes the trend from 2002 to 2003. Only acute departments with responsibility for providing emergency services were included in the survey.\ud \ud The occupancy rate in the survey week was 99 per cent in 2002 and 96 per cent in 2003. In 2003 this varied from 79 per cent in Mid Norway Health Authority to 112 per cent in Western Norway Health Authority. The occupancy rate was over 100 per cent in eight health trusts and lower than 90 per cent in ten health trusts.\ud \ud For the country as a whole, as many as 13 per cent of patients were ready to be discharged in 2002 and 14 per cent in 2003. This varied from 8 per cent in Mid Norway Health Authority to 21 per cent in Southern Norway Health Authority.\ud \ud During the survey week, a substantial proportion of units had patients who were placed in rooms other than patient rooms. Thirty-two per cent of units in 2002 and 40 per cent of units in 2003 reported that patients who should have had a single room had to share a room.\ud \ud The conclusion is that there are still not enough places in acute departments. When the institutions utilize available space in the way that has been illustrated by the survey, there is reason to question whether the conditions for use of compulsory treatment are met.\ud \ud 1The surveys described in the report investigated capacity in acute departments of psychiatry by measuring the total number of patients in the departments and the number of “patients ready to be discharged” in the survey week.\ud \ud “Patients ready to be discharged” are patients who have finished their treatment, who could already have been discharged from hospital, but who are still in hospital because they are waiting to be offered a place, for example, in a district psychiatric centre or in sheltered accommodation

    NORSK SAMMENDRAG: Helsetilsynet i fylkene gjennomførte i 2003 landsomfattende tilsyn ved 50 somatiske poliklinikker i landets fem helseregioner, for å undersøke hvordan pasientrettigheter som er vesentlige ved henvisning til spesialisthelsetjenesten, ivaretas:\ud Sikrer spesialisthelsetjenesten at pasientene får den vurderingen de har krav på? \ud Hvordan følges resultatet av vurderingen opp? \ud Sikrer spesialisthelsetjenesten at pasienter som har rett til helsetjenester, får informasjon om hvilken helsehjelp de kan forvente, og om ventetider og sykehusvalg?\ud Tilsynet viste at pasientenes rettigheter på flere områder ikke blir godt nok ivaretatt. Mange somatiske poliklinikker har liten kjennskap til hvilke krav som gjelder for vurdering, prioritering og oppfølging av henvisninger, og for håndtering av ventelister og fritt sykehusvalg. Tilsynet viste at det er mange utfordringer å ta tak i før befolkningen kan sies å få lik tilgang til spesialisthelsetjenester. Stortingets uttrykk for hvilke pasientgrupper som bør prioriteres, sikres ikke godt nok slik prioriteringen praktiseres i dag.\ud \ud Det må iverksettes tiltak for å gjøre kravene kjent og tatt i bruk i spesialisthelsetjenesten slik at formålet med pasientrettighetsloven skal kunne nås. Ledelsesnivåene i spesialisthelsetjenesten må sørge for at kravene integreres i måten oppgaver organiseres, utføres og følges opp.SUMMARY IN ENGLISH: In 2003 the Norwegian Board of Health in the counties carried out a nationwide supervision of 50 somatic out-patient clinics in the country’s five health authorities, in order to study whether patients’ rights, that are important when patients are referred to specialist health services, are met:\ud \ud Do specialist health services ensure that patients receive the professional assessment that they have the right to receive? \ud How are the results of professional assessment followed up? \ud Do specialist health services ensure that patients who have a right to receive health services receive information about the type of health service that they can expect, about waiting lists and about free choice of hospital?\ud The supervision showed that there are several areas in which patients’ rights are not adequately met. Many somatic out-patient clinics have limited knowledge about the requirements for professional assessment, prioritization and follow-up of referrals, and for dealing with waiting lists and free choice of hospital. The supervision showed that there are many challenges to be met before everyone in the population has equal access to specialist health services. In the way prioritization is practised today, the requirements laid down by the Storting (the Norwegian parliament) regarding the patient groups that should be given priority, have not been adequately met.\ud \ud Measures must be implemented to make the requirements known and to ensure that they are implemented in specialist health services, so that the aims of the Patients’ Rights Act can be achieved. Leadership in specialist health services must ensure that these requirements are taken into account in the system for organizing, carrying out and following up tasks

    NORSK SAMMENDRAG: Rapporten retter søkelys mot møtet mellom klienter med innvandrerbakgrunn og ansatte i ulike tiltak for rusmiddelmisbrukere i Norge. Utvalget besto av 23 klienter over 18 år med forskjellig ikke-norsk etnisk bakgrunn og 61 ansatte fra 16 forskjellige offentlige rusmiddeltiltak og instanser som sosialkontor, avrusnings-, utrednings- og behandlingsinstitusjoner.\ud \ud Hovedkonklusjonen er at klientene og de ansatte formidler en dyp uenighet om hva behandling innebærer. Deres forskjellige og motsetningsfulle forestillinger og forventninger til hva behandling er forhindrer dem i å etablere kontakt og utvikle et samarbeidsforhold. Måten de forholder seg, eller ikke forholder seg, til uenigheten bidrar til å forsterke de vanskelige samhandlingssituasjonene. Det at de ikke forhandler seg frem til en gjensidig situasjonsdefinisjon fører samtalene ut i en monolog. En monolog der ansatte i stor grad fremstår som aktive deltagere og klientene som passive tilskuere

    NORSK SAMMENDRAG: I rapporten oppsummeres erfaringer fra landsomfattende tilsyn med helsetjenester til barn og unge med psykiske problemer. Helsetilsynet har vurdert funnene og kommet med tilrådinger. Både vurderingene og tilrådingene kan leses i rapporten. \ud \ud Det ble gjennomført tilsyn i 83 kommuner og ved 41 barne- og ungdomspsykiatriske poliklinikker. Hovedfunnene var svikt i legeundersøkelsen ved helsestasjon og i skolehelsetjenesten, sviktende samarbeid mellom nivåene i helsetjenesten og nesten fullstendig mangel på individuelle planer. Retten til vurdering og prioritering i spesialisthelsetjenesten synes å være ivaretatt.\ud \ud Helsetilsynet konkluderer i rapporten at det er et stort forbedringspotensial innen helsetjenester til barn og unge med psykiske problemer, særlig gjelder det legenes rolle og deltakelse både i kommunen og ved poliklinikkene og arbeidet med individuell plan på begge nivåer. Helsetilsynet mener det er en utfordring for Helsedepartementet å avklare ansvarsforhold knyttet til individuell plan slik at lovkravet kan bli ivaretatt. Helsetilsynet stiller spørsmål ved om ikke veiledningsplikten bør legges på foretaksnivå.SUMMARY IN ENGLISH: In this report, experience from nationwide supervision of health services for children and young people with mental problems is summarized. The Norwegian Board of Health also presents an evaluation of the findings and gives recommendations for the future.\ud \ud Supervision of mental health services was carried out in 41 out-patient psychiatric clinics for children and young people and in 83 municipalities. The main findings were as follows:\ud \ud health centres and school health services do not offer adequate medical examinations of the patients in question \ud cooperation between different levels in the health services is inadequate \ud there is an almost total lack of individual patient plans. Such plans are a legal requirement \ud the right to be assessed and to be given priority by the specialist health services seems to be met.\ud The Norwegian Board of Health concludes that there is great potential for improvement of health services for children and young people with mental problems. This applies particularly to the physician’s role and participation in municipal health services and in out-patient services, and to work with individual patient plans. The Norwegian Board of Health believes that it is a challenge for the Ministry of Health to clarify who has responsibility for individual patient plans, in order that legislative requirements are met. The Norwegian Board of Health questions whether the duty to give guidance should be placed at the level of the health trusts

    NORSK SAMMENDRAG: I rapporten redegjør Helsetilsynet for hvilke tiltak Helsetilsynet sentralt og i fylkene har satt i verk for å få oversikt over omfang og konsekvenser av smitteutbruddet med bakterien Pseudomonas aeruginosa som rammet 27 sykehus vinteren og våren 2002. \ud \ud I alt 231 sykehuspasienter fikk påvist smitte som skyldtes bakterier fra munnpenselen.\ud \ud Helsetilsynets konklusjon er at verken helsepersonell eller helsetjenesten har handlet faglig uforsvarlig ved å anskaffe og anvende Dent-O-Sept munnpensler til munnstell.\ud \ud Saken har imidlertid avdekket betydelige mangler på flere områder i helsetjenesten.\ud \ud Det gjelder særlig manglende rutiner for oppdatering av kunnskap om munnstell, manglende rutiner for bruk av engangsutstyr og for innkjøp av denne typen medisinsk utstyr. \ud \ud På bakgrunn av erfaringene fra Dent-O-Sept saken anbefaler Helsetilsynet at tiltak settes i verk på flere områder for å forbedre sikkerheten i helsetjenesten. Forslagene er beskrevet i rapporten.SUMMARY IN ENGLISH: In this report, the Norwegian Board of Health describes the measures that the Norwegian Board of Health centrally and the Norwegian Board of Health in the counties have initiated in order to obtain an overview of the extent of the problem and the consequences of the outbreak of infection with the bacterium Pseudomonas aeruginosa, in 27 hospitals in the winter and spring of 2002.\ud \ud Altogether 231 hospital patients were identified as having been infected with bacteria from infected mouth swabs used for oral care. \ud \ud The conclusion of the Norwegian Board of Health is that neither health care personnel nor the health services acted in a professionally unacceptable manner by purchasing or using Dent-O-Sept mouth swabs for oral care.\ud \ud However, the incident has revealed serious deficiencies within several areas of the health services, such as lack of routines for updating information about oral care, and lack of routines for purchase and use of disposable equipment.\ud \ud Based on the experience from the Dent-O-Sept incident, the Norwegian Board of Health recommends that measures should be initiated in several areas to improve safety in the health services. The recommendations are presented in the report

    Hvilken nytte, for hvem og til hvilken kostnad? En prospektiv studie av stoffmisbrukere i behandling

    NORSK SAMMENDRAG:Rapportens formål er å gi et bedre kunnskapsgrunnlag for å kunne vurdere nytten i ulike behandlingstiltak i forhold til kostnadene. Informasjon fra 482 klienter ved 20 ulike behandlingstiltak ble samlet inn.\ud \ud Klientene rapporterte om en betydelig reduksjon i rusmiddelmisbruk og antall dager med kriminalitet etter behandling. Disse endringene viste seg å være stabile. Forbedringen i psykisk helse og arbeidsituasjon var mindre markant. \ud \ud Dersom tallene for endringer og kostnader blir sett i forhold til hverandre, viser det seg at ingen behandling er klart best på alle områder. Forskerne konkluderer med at man bør være svært varsom med å generalisere tallene utover å være en ren beskrivelse av kostnader og endringer for en gruppe klienter i et relativt kort tidsrom. Bare en ny oppfølging vil kunne vise om endringene holder seg stabile også over lengre tid

    Performance on the dementia rating scale in Parkinson's disease with dementia and dementia with Lewy bodies: comparison with progressive supranuclear palsy and Alzheimer's disease.

    BACKGROUND: The relation between dementia with Lewy bodies (DLB) and Parkinson's disease with dementia (PDD) is unknown. OBJECTIVES: To compare the cognitive profiles of patients with DLB and PDD, and compare those with the performance of patients with a subcortical dementia (progressive supranuclear palsy) and a cortical dementia (Alzheimer's disease). DESIGN: Survey of cognitive features. SETTING: General community in Rogaland county, Norway, and a university dementia and movement disorder research centre in the USA. PATIENTS: 60 patients with DLB, 35 with PDD, 49 with progressive supranuclear palsy, and 29 with Alzheimer's disease, diagnosed by either standardised clinical procedures and criteria (all PDD and Alzheimer cases and 76% of cases of progressive supranuclear palsy), or necropsy (all DLB cases and 24% of cases of progressive supranuclear palsy). Level of dementia severity was matched using the total score on the dementia rating scale adjusted for age and education. MAIN OUTCOME MEASURES: Dementia rating scale subscores corrected for age. RESULTS: No significant differences between the dementia rating scale subscores in the PDD and DLB groups were found in the severely demented patients; in patients with mild to moderate dementia the conceptualisation subscore was higher in PDD than in DLB (p = 0.03). Compared with Alzheimer's disease, PDD and DLB had higher memory subscores (p < 0.001) but lower initiation and perseveration (p = 0.008 and p=0.021) and construction subscores (p = 0.009 and p = 0.001). DLB patients had a lower conceptualisation subscore (p = 0.004). Compared with progressive supranuclear palsy, PDD and DLB patients had lower memory subscores (p < 0.001). CONCLUSIONS: The cognitive profiles of patients with DLB and PDD were similar, but they differed from those of patients with Alzheimer's disease and progressive supranuclear palsy. The cognitive pattern in DLB and PDD probably reflects the superimposition of subcortical deficits upon deficits typically associated with Alzheimer's disease

    NORSK SAMMENDRAG: I rapporten beskrives utfallet av en telefonundersøkelse til 432 kommuner. Hensikten var å finne ut om kommunene hadde et system for å sikre tilgjengelighet til lege på dagtid. Norsk Gallup utførte intervjuene på oppdrag av Statens helsetilsyn. \ud \ud I 184 kommuner oppnådde intervjueren kontakt med et legekontor som hadde vakt.\ud \ud \ud I 114 kommuner ble den som ringte henvist til egen fastlege eller annen lege og i 78 kommuner ble det ikke oppnådd kontakt. I 68 kommuner ble henvendelsen besvart av akuttmedisinsk nødmeldetjeneste.\ud \ud Kartleggingen var metodisk komplisert og resultatene må tolkes med varsomhet.\ud \ud Helsetilsynet mener likevel at de samlet sett gir et rimelig godt bilde av den situasjonen en pasient kan møte ved å benytte telefonkatalogens nummeropplysning ”Når hjelpen kan vente litt” for å oppnå kontakt med vakthavende lege på dagtid.SUMMARY IN ENGLISH: In this report, the results of a telephone survey encompassing 432 municipalities are presented. The aim was to find out whether the municipalities had a system for ensuring that a doctor on call is available during the day. The market research company TNS Gallup was commissioned by the Norwegian Board of Health to carry out the interviews.\ud \ud The interviewers managed to make contact with doctors’ surgeries with on-call duty in 184 municipalities.\ud \ud In 114 municipalities, the person who rang was referred to their own regular general medical practitioner or another physician, and in 78 municipalities contact with the doctor was not attained. In 68 municipalities the telephone call was answered by the emergency medical service.\ud \ud The methodology for the survey was complicated and the results must be interpreted with caution.\ud \ud However, the Norwegian Board of Health believes that the results as a whole provide a reasonably good picture of the situation that patients can experience when using the number given in the telephone directory for contacting a doctor on call during the day “when help is not urgent”
