The effectiveness of workplace smoking cessation programmes: a meta-analysis of recent studies.
OBJECTIVE: Using meta-analytic procedures, we compare the effectiveness of recent controlled trials of worksite smoking cessation during the 1990s with a previous meta-analysis of programmes conducted in the 1980s. DATA SOURCES: ABI/Inform, BRS, CHID, Dissertation Abstracts International, ERIC, Medline, Occupational Health and Safety Database, PsycInfo, Smoking and Health Database, SSCI, and Sociological Abstracts. STUDY SELECTION: Controlled smoking cessation interventions at the workplace with at least six months follow up published from 1989 to 2001 and reporting quit rates (QRs). DATA EXTRACTION: Two reviewers independently scanned titles/abstracts of relevant reports, and we reached consensus regarding inclusion/exclusion of the full text reports by negotiation. A third reviewer resolved disagreements. Two reviewers extracted data according to a coding manual. Consensus was again reached through negotiation and the use of a third reviewer. DATA SYNTHESIS: 19 journal articles were found reporting studies conforming to the study's inclusion criteria. Interventions included self help manuals, physician advice, health education, cessation groups, incentives, and competitions. A total of 4960 control subjects were compared with 4618 intervention subjects. The adjusted random effects odds ratio was 2.03 (95% confidence interval 1.42 to 2.90) at six months follow up, 1.56 (95% CI 1.17 to 2.07) at 12 months, and 1.33 (95% CI 0.95 to 1.87) at more than 12 months follow up. Funnel plots were consistent with strong publication bias at the first two follow ups but not the third. In Fisher et al's 1990 study, the corresponding ORs were 1.18, 1.66, and 1.18. CONCLUSIONS: Smoking cessation interventions at the worksite showed initial effectiveness, but the effect seemed to decrease over time and was not present beyond 12 months. Compared to the Fisher (1990) analysis, the effectiveness was higher for the six month follow up. Disappointingly, we found methodological inadequacies and insufficient reporting of key variables that were similar to those found in the earlier meta-analysis. This prevented us from determining much about the most effective components of interventions. It is advisable for researchers conducting studies in the future to report data on attrition and retention rates of participants who quit, because these variables can affect QRs
Oppsummering av landsomfattende tilsyn med fødeinstitusjoner
NORSK SAMMENDRAG: Helsetilsynet i fylkene utførte landsomfattende tilsyn med 26 av landets ca. 60 fødeinstitusjoner i landets fem helseregioner. Det ble undersøkt om helseforetakene gjennom systematisk styring og kontinuering forbedring sikrer at fødeinstitusjonene er tilrettelagt slik at de oppfyller krav til faglig forsvarlighet. Tilsynet undersøkte fire hovedtema:\ud

seleksjon av fødende til ulike typer fødeinstitusjoner \ud
overvåking og oppfølging av noen utvalgte tilstander \ud
håndtering av akutte situasjoner \ud
læring og forbedring\ud
Selv om Norge er et av de tryggeste landene å føde i, har dette tilsynet vist at det er rom for forbedringer. Både når det gjelder seleksjon til rett nivå, overvåking og oppfølging under fødsel har tilsynet påpekt områder som kan forbedres. Det er ikke pålitelige nasjonale tall på forekomst av ulike tilstander og resultater ved fødselshjelp. Alle helseforetakene har ikke lagt til rette for nødvendig kommunikasjon og samarbeid mellom leger og jordmødre, og ansvarsforhold er ikke avklart alle steder. Også ledelse og styring kan forbedres.\ud

Det tilrådes at det blir utarbeidet et nasjonalt sett av data som skal registreres ved alle fødeinstitusjoner som både kan brukes til internt forbedringsarbeid og til nasjonale oversikter. For noen komplikasjoner bør forekomsten søkes redusert. Helseforetakene må utvikle hensiktsmessige samhandlingsarenaer for fødselshjelpere, og der det er uklare styringslinjer bør disse avklares. Ledelsen av virksomhetene må også sørge for å bruke egne resultat til systematisk evaluering, forbedring og styring av virksomheten.SUMMARY IN ENGLISH: The Norwegian Board of Health in the counties carried out countrywide supervision of 26 of the 60 maternity units in the country’s five health authorities. The investigation looked at whether the health trusts, through systematic management and continuous improvements, ensure that maternity units are organized in such a way that they meet the requirements for sound professional standards. Four main areas were investigated: \ud

monitoring and follow-up of a selection of conditions \ud
dealing with acute emergencies \ud
learning and improvement\ud
Although Norway is one of the safest countries in which to give birth, supervision of the services has shown that there is room for improvement. Both in relation to selection of women to the most appropriate level of care, and monitoring and follow-up during delivery, areas have been identified in which improvements can be made. Reliable national figures on the incidence of different conditions and the results of maternity services are not available. Not all the health trusts have organized their services in such a way as to ensure that necessary communication and cooperation between physicians and midwives take place, and allocation of responsibility is not clearly defined in all places. Leadership and management can also be improved.\ud

It is recommended that a national data set should be developed, based on data that shall be registered in all the maternity units. This data set can be used both for internal quality improvement and to provide a country-wide overview. Attempts should be made to reduce the incidence of some birth complications. The health trusts must develop appropriate arenas for the development of cooperation. Unclear lines of management should be clarified. The administrators must also ensure that results are used for systematic evaluation, improvement and management of the units
Grading quality of evidence and strength of recommendations.
Users of clinical practice guidelines and other recommendations need to know how much confidence they can place in the recommendations. Systematic and explicit methods of making judgments can reduce errors and improve communication. We have developed a system for grading the quality of evidence and the strength of recommendations that can be applied across a wide range of interventions and contexts. In this article we present a summary of our approach from the perspective of a guideline user. Judgments about the strength of a recommendation require consideration of the balance between benefits and harms, the quality of the evidence, translation of the evidence into specific circumstances, and the certainty of the baseline risk. It is also important to consider costs (resource utilisation) before making a recommendation. Inconsistencies among systems for grading the quality of evidence and the strength of recommendations reduce their potential to facilitate critical appraisal and improve communication of these judgments. Our system for guiding these complex judgments balances the need for simplicity with the need for full and transparent consideration of all important issues

This report describes the results of a survey of capacity1 in departments of internal medicine in 2003 and the trend from 1999-2003. The survey in 2003 shows that there has been an increase in the number of excess patients and the number of patients ready to be discharged in the country as a whole since 2002. The prevalence of these types of patients was at about the same level as in 1999.\ud

In the survey week in 2003, the occupancy rate was over 100 per cent in ten health trusts, and there were 50 or more excess patients per 1000 hospital beds in eight health trusts. In five health trusts there was both a high occupancy rate and a high number of excess patients. There is reason to look more closely at the situation in these health trusts. An important issue for the regional health authorities and the health trusts is whether a high prevalence of excess patients is associated with too low capacity.\ud

1 The surveys described in the report investigated capacity in departments of internal medicine by measuring the prevalence of “excess patients” and “patients ready to be discharged” in the survey week. The Norwegian term that is used for excess patients is “korridorpasient”, which means literally “corridor patient”.\ud

“Excess patients” are patients who are placed in a bed in a hospital corridor, a bathroom, a day-room, a treatment room or any other type of room that is not intended to be used as a patient room, because all the ordinary hospital beds are occupied.\ud

“Patients who are ready to be discharged” are patients who have finished their treatment, who could already have been discharged from hospital, but who are still in hospital because they are waiting to be offered a place by the municipality in, for example, a nursing home or sheltered accommodation

NORSK SAMMENDRAG: Rapporten viser at antall meldinger om pasientskade i 2001 og 2002 er redusert i forhold til tidligere år. Reduksjonen antas i hovedsak å ha sammenheng med at meldeplikten nå omfatter forhold som har medført eller kan medføre betydelig pasientskade, mens det tidligere også ble meldt om nesten-uhell og hendelser uten pasientskade.\ud

Helsetilsynet finner, på bakgrunn at de erfaringer som er gjort gjennom tilsyn, at det er stor forskjell på hvor langt norske helseinstitusjoner har kommet når det gjelder systematisk kvalitetsarbeid, og at institusjonene i liten grad synes å bruke meldingene i sitt kvalitetsarbeid. Meldingene peker imidlertid på noen problemområder, og selv om bildet ikke er dekkende er det ingen grunn til å redusere kravet om at helsetjenesten må ha økt fokus på kvalitetsarbeid.\ud

Det er Helsetilsynets oppfatning at det eksisterer en underrapportering av hendelser som medfører fare for pasientskade. Bekymringen gjelder i særlig grad at underrapporteringen kan indikere en mangel på systematisk kvalitetsarbeid i helseforetakenes håndtering av uønskede hendelser.SUMMARY IN ENGLISH: This report shows that the number of reports of injuries to patients in 2001 and 2002 was less than in previous years. The reduction is believed to be mainly associated with the fact that the duty to report now encompasses events that led to, or could have led to, substantial injury to patients. Previously, near-misses (sentinel events) and events that occurred, but that did not result in injury to patients, were also reported.\ud

Based on the experience gained from supervision of health services, the Norwegian Board of Health finds that there are large differences with regard to how far Norwegian health institutions have come in their work with systematic quality improvement, and that the institutions appear to use reports of adverse events only to a limited degree in this work. However, the reports indicate certain problem areas, and even though the picture is not complete, there is no reason to reduce the requirement for health services to pay more attention to their work with quality improvement.\ud

The Norwegian Board of Health believes that many adverse events that could have led to substantial injury to patients are not reported. We are particularly concerned that this under-reporting may reflect a lack of systematic work with quality improvement, specifically in relation to the way in which health trusts deal with adverse events
Kosmetisk kirurgiske inngrep i Norge

Den ene handler om omfanget av slike inngrep i 2003, med særlig vekt på inngrep på personer under 18 år. Den viser at av de 8000-9000 inngrepene som ble foretatt, ble 38 utført på personer under 18 år, og samtlige av disse inngrepene ble ansett å være medisinsk indiserte. Helsetilsynets er derfor av den oppfatning at kosmetisk kirurgiske inngrep på personer under 18 år er av et svært lite omfang. Ni av ti inngrep utføres på kvinner, og både blant menn og kvinner er aldersgruppen 30-49 år sterkest representert.\ud

Den andre kartleggingen handler om markedsføring av tjenestene. Det ble gjort en gjennomgang av markedsføringsmateriale i aviser og blader/magasiner, hvor det ble funnet lite markedsføringsmateriale, og på Internett, hvor det ble funnet mye. En del av markedsføringsmaterialet ble vurdert å bevege seg på kanten av hva som er lovlig. På bakgrunn av funnene anbefaler Helsetilsynet at det utarbeides en forskrift om markedsføring av kosmetisk kirurgi. Samtidig pekes det på at behovet for nøytral informasjon til publikum ikke er tilfredsstillende dekket. \ud

I en sammenfatning peker Helsetilsynet på at regulering av bransjen med bruk av lov og forskrift bare i liten grad kan påvirke etterspørsel og tilbud. Kroppsidealer, seksualitetens kår og individualisering er stikkord. For Helsetilsynet er klager er en viktig kilde til informasjon om kvaliteten på helsetjenester, men for kosmetiske inngrep er det forholdsvis få klager.SUMMARY IN ENGLISH: In this report, the results of two surveys of cosmetic surgery in Norway are presented.\ud

The first survey investigated the number of cosmetic surgical procedures carried out in 2003, with a specific focus on persons under 18 years of age. The results show that 8 000-9 000 cosmetic surgical procedures were carried out. Nine out of ten procedures were performed on women, and both for women and men the age group 30-49 was most strongly represented. Only 38 of all the reported procedures were performed on persons under 18 years of age, and all of them were considered to be medically indicated. The Norwegian Board of Health thus believes that very few cosmetic surgery procedures are performed on persons under 18 years of age. \ud

The second survey investigated marketing of cosmetic surgery services. Advertisements in newspapers and magazines were reviewed. Little material was found in such sources. Advertisements on the Internet were also reviewed. Much material was found here. Some of the advertisements were found to be on the border of what is legal. Based on these findings, the Norwegian Board of Health recommends that regulations on marketing of cosmetic surgery services should be drawn up. At the same time, it is pointed out that there is insufficient unbiased information available to the public.\ud

In the summary, the Norwegian Board of Health point out that regulation of the branch by legislative means can influence supply and demand only to a limited degree. Ideal body image, sexuality and individualization are key words in this respect. For the Norwegian Board of Health, complaints are an important source of information about the quality of health services, but there are few complaints about cosmetic surgery

Helsetilsynet har i 2003 utredet området tannhelsetjenester med et særlig fokus på om de prioriterte gruppene får de tannhelsetjenester de har krav på.\ud

I henhold til tannhelsetjenesteloven er det fem prioriterte grupper. Det er barn og ungdom 0-18 år (gruppe a), psykisk utviklingshemmede voksne (gruppe b), eldre, langtidssyke og uføre i institusjon og hjemmesykepleie (gruppe c) og ungdom 19-20 år (gruppe d). Fylkeskommunen kan i tillegg vedta å prioritere andre grupper (gruppe e). Fylkeskommunen kan også yte tjenester til voksent betalende klientell dersom behovet til de prioriterte grupper er dekket.\ud

According to the Dental Health Services Act, there are five priority groups. These are children and young people aged 0-18 (Group A), mentally handicapped adults (Group B), elderly people, people with long-standing illness and people with a disability pension who are either living in an institution or receiving home nursing care (Group C), and young people aged 19 20 (Group D). In addition, the county authorities can decide to give priority to other groups (Group E). The county authorities can also provide dental services for adults, who pay for their treatment, if the needs of people in the priority groups have been met.\ud

The sources for the data are Statistics Norway (KOSTRA: Municipality-State-Reporting and population statistics) and the Norwegian Board of Health (annual reports for the public dental services).\ud

This report shows that the proportion of people under supervision of the public dental services shows great variation, both for the different priority groups, and for the different counties.\ud

The Norwegian Board of Health believes that the probability that people in the priority groups do not receive an adequate service is greatest in the counties where the proportion of people under supervision is below the mean for the whole country.\ud

The statistics clearly show that manpower level is not the only factor that that can explain the differences between the counties in the level of dental services offered to people in the priority groups. Other factors, such as organizational factors and geographical factors, also influence service provision
Tilsyn med kodepraksis
NORSK SAMMENDRAG: Statens helsetilsyn mottok 17. februar 2003 informasjon som kunne tyde på feil i forbindelse med koding av diagnoser hos pasienter behandlet ved Sørlandet sykehus HF i Arendal. \ud

Helsetilsynet gjennomførte et særskilt tilsyn med øre-nese-hals (ønh) -avdelingen og nevrologisk avdeling, Sørlandet sykehus HF Arendal i september 2003. Nevrologisk avdeling ble valgt som referanseavdeling. Helsetilsynet besluttet å føre tilsvarende tilsyn med flere sykehus for å få et mer helhetlig bilde av kodepraksis. Helsetilsynet gjennomførte derfor tilsyn med ønh-avdelingen og nevrologisk avdeling ved Helse Nordmøre og Romsdal HF Molde sjukehus og ved Sykehuset Østfold HF Fredrikstad.\ud

Gjennom tilsynene avdekket Helsetilsynet:\ud
at flere pasienter ved ønh-avdelingen, Sørlandet sykehus HF Arendal, har fått diagnosen søvnapné uten at hovedsymptomet fremgikk verken av henvisningsskriv eller journal. I tillegg ble 20 % av de gjennomgåtte journalene ved samme avdeling vurdert som mangelfulle i forhold til journalføringsreglene. \ud
at alle helseforetakene kodet feil i forhold til gjeldende koderegler for sykehusopphold. De nevrologiske avdelingene hadde betydelig større forekomst av feil enn ønh-avdelingene, helt opp til nesten 70 % av de undersøkte oppholdene. \ud
at det er betydelige uoverensstemmelser mellom diagnose- og prosedyrekodene som var nedtegnet i pasientens journal og kodene som var sendt Norsk Pasientregister. Dette gjaldt for ønh-avdelingene og de nevrologiske avdelingene ved Sørlandet sykehus HF Arendal og Helse Nordmøre og Romsdal HF Molde sjukehus.\ud

Etter Helsetilsynets vurdering tilsier disse funnene en mangelfull kontroll med grunnlagsmaterialet for beregning av inntekter til sykehusene.SUMMARY IN ENGLISH: On 17 February 2003, the Norwegian Board of Health received information that indicated that there was a possibility that diagnoses for patients treated at the hospital Sørlandet sykehus HF in Arendal had been coded incorrectly.\ud

In September 2003, the Norwegian Board of Health carried out special supervision of the ear, nose and throat (ENT) department and the neurological department in Sørlandet sykehus HF Arendal. The neurological department was chosen as a reference department. It was decided that similar supervision of several other hospitals should be carried out, in order to obtain a broader picture of coding practice. The Norwegian Board of Health thus carried out supervision of the ENT departments and the neurological departments in the following hospitals: Helse Nordmøre og Romsdal HF Molde sjukehus and Sykehuset Østfold HF Fredrikstad.\ud

Through supervision, the Norwegian Board of Health identified the following:\ud
Several patients in the ENT department Sørlandet sykehus HF Arendal had been given the diagnosis sleep apnoea, although the main symptom of this disorder was neither recorded in the referral letter nor in the patient record. In addition, 20 % of the records that were checked in the same department were inadequate according to the regulations for patient records. \ud
All the health trusts coded incorrectly in relation to the current coding regulations for hospital stay. The neurological departments had a higher frequency of incorrect coding than the ENT departments, in some cases as high as 70 %. \ud
There were great inconsistencies between the diagnosis and procedure codes that were recorded in the patient records, and the codes that were reported to the Norwegian Patient Register. This was the case for the ENT departments and the neurological departments in Sørlandet sykehus HF Arendal and Helse Nordmøre og Romsdal HF Molde sjukehus.\ud
In the view of the Norwegian Board of Health, this evaluation indicates inadequate quality control of the basic data that is used for calculating hospital revenue
