Diffuse Pulmonary Lesions Due to Invasive Fungal Infection in an Immune-capable Male: A Case Report

    No full text
Introduction: Invasive fungal pulmonary infection is a fungal illness caused by many species of fungi, including Aspergillus, generally presenting as life-threatening hazards to people with compromised immune systems. Although instances in immunecompetent individuals are infrequent, a recently published report detailed active lung infection within an immune-competent patient at postmortem. We report a rare instance of invasive fungal infection (IFI) in an immune-capable male, exhibiting diffuse systemic lesions. Case Report: A 33-year-old male approached our facility, complaining of chest discomfort and dyspnea lasting for 3 months. Upon clinical examination, he looked emaciated and presented with sinus symptoms. A radiographic examination showed indistinct lesions in the lower left lobe of the lung. Bronchoscopy as well as bronchoalveolar lavage (BAL) were conducted, before the commencement of oral antibiotic treatment. The lavage findings were negative for staining techniques (e.g., acid-fast bacilli (AFB)), and culture demonstrated many septate hyphae on the fungal screen. Histopathological analysis of lung tissue (via bronchoscopy) revealed persistent granulomatous inflammation with apical fungal hyphae, diagnostic of aspergillosis. Further cultures confirmed the presence of Aspergillus fumigatus, leading us to commence voriconazole treatment. The patient had a notable recovery, evidenced by gaining weight and a recovered appetite within a brief timeframe. His symptoms improved within three months of medication, allowing him to return to almost normal life. Conclusion: This case underscores the identification of IFI in an immune-capable patient exhibiting extensive nodular lesions throughout the respiratory tract, mediastinum, and belly. Clinicians must uphold a heightened concern for aspergillosis in persistent pneumonia and diffuse nodular lesions, despite patients having no conventional risk factors

    No full text
Background: Primary rib tumors are relatively rare and more difficult to diagnose compared to other tumors, leading to a higher risk of misdiagnosis. Case: The patient was a 23-year-old male who presented with intermittent right-sided chest pain for six months. Six months prior, he developed right chest wall pain without an obvious cause, characterized by dull pain and occasional stabbing sensations, often accompanied by coughing and white mucus production, but no fever, night sweats, nausea, or vomiting. Symptoms would resolve on their own after about a week. CT 3D reconstruction technology and AI recognition suggested a rib tumor, requiring surgical removal. The surgery was successful, and the patient recovered well with no recurrence. Conclusion: Primary rib tumors are rare. Using CT 3D reconstruction and AI recognition technology can improve preoperative diagnosis and assist in surgical planning

    No full text
Polycystic ovary syndrome (PCOS) represents one of the most common endocrine disorders affecting women of reproductive age, serving as a major contributor to infertility on a global scale, with an estimated prevalence varying from 6 to 20% worldwide. PCOS is a complex syndrome defined by hyperandrogenism, irregular ovulation, and the presence of polycystic ovarian morphology. This syndrome is often associated with metabolic complications, including obesity, dyslipidemia, and an elevated risk of insulin resistance and developing type 2 diabetes. Symptoms include menstrual irregularities, weight gain, hirsutism, acne, and infertility. The presently available conventional treatment options for PCOS include hormonal contraceptives, insulin sensitizers, anti-androgens, and ovulation-inducing agents. While these treatments may offer symptomatic relief, they often entail potential side effects and do not adequately address the underlying cause of this syndrome. Herbal remedies have been used for centuries across cultures to manage various disorders due to their cost efficiency, availability, and safe nature. Several studies have demonstrated the efficiency of herbal remedies in the treatment of female reproductive disorders, including PCOS. This review comprises an extensive survey of the latest animal and clinical studies concerning plants and phytochemicals that have demonstrated effectiveness in alleviating symptoms associated with PCOS, as well as treating the underlying mechanisms involved in their efficac

Child-free in the Perspective of Positive Law in Indonesia

    No full text
The decline in birth rates in Indonesia, reflected in the decreasing Total Fertility Rate and the increasing percentage of women choosing to be child-free, raises concerns about the country’s demographic structure. The child-free phenomenon is influenced by external factors such as economic and social conditions, as well as internal factors like decision-making maturity and family experiences, significantly impacting Indonesia’s population dynamics. From an Islamic legal perspective, the decision not to have children or to be child-free is recognized and permitted, considering the underlying motives. According to Imam Al-Ghazali, the act of rejecting the existence of children, if done before the sperm enters the woman’s womb, is legitimate and permissible. Good motives, such as financial or health reasons, are acceptable, while incorrect motives render the child-free decision impermissible. Legally, the choice to be child-free is protected by the Indonesian constitution and laws that guarantee human rights, personal freedom, and gender equality. Health policies and family planning also support the right of individuals or couples to decide on the number of children or to choose not to have children. Although there are no regulations prohibiting being child-free, the state still views the purpose of marriage as having good and healthy offspring, while respecting individual rights and freedoms. The state is expected to create policies that support individual choices while maintaining population balance to prevent potential sex recession and ensure the continuity of future generations
