Approaches to Donor Kidney Quality Assessment for Transplantation: Scoping Review Protocol
Chronic kidney disease (CKD) is the deterioration in the function and anatomical structure of the kidney over 3 months with associated health implications. The irreversible nature of CKD leads thousands of patients to requiring dialysis and eventually transplantation. The numbers of patients requiring dialysis continues to grow every year, with this figure estimated to reach 143,000 patients by 2033.
Transplantation remains the definitive treatment for patients with end-stage renal disease, however meeting the demand for donor organs continues to pose a challenge for transplant communities across the globe. The rising demand has led to clinicians increasingly considering the use of Donation after Circulatory death (DCD) donors, as well as Extended Criteria Donors (ECD). These donors are often of older age, with more co-morbidities and hence require more complex decision making.
To ensure the safe and successful transplantation of donor kidneys, clinicians assess the quality of the kidney through visual inspection prior to transplantation to decide whether the kidney is suitable. This decision-making process is subjective and varies between clinicians depending on their individual experience. To aid this process, various tools and approaches have been developed to objectively evaluate the quality of a donor kidney. These approaches range from histological assessments to Artificial Intelligence (AI) enabled tools trained to predict patient outcomes based on donor and recipient characteristics.
This scoping review will aim to collate and summarise the various kidney quality assessment tools that have been developed to date. The review will map and categorise these methods according to the underlying approach and intended use of the tool, as well as reporting their strengths and limitations. The projected outcomes of this review will be a comprehensive synthesis of the range of existing tools and identify gaps in research. Ultimately, this review aims to support improvements in donor kidney assessments and utilisation by providing an overview of current kidney quality assessment methods for researchers and clinicians

The 17% Gap: Quantifying Epistemic Decay in AI-Assisted Survey Papers
Background: The adoption of Large Language Models (LLMs) in scientific writing promises efficiency but risks introducing informational entropy. While “hallucinated papers” are a known artifact, the systematic degradation of valid citation chains remains unquantified.
Methodology: We conducted a forensic audit of 50 recent survey papers in Artificial Intelligence (N = 5,514 citations) published between September 2024 and January 2026. We utilized a hybrid verification pipeline combining DOI resolution, Crossref metadata analysis, Semantic Scholar queries, and fuzzy text matching to distinguish between formatting errors (“Sloppiness”) and verifiable non-existence (“Phantoms”).
Results: We detect a persistent 17.0% Phantom Rate—citations that cannot be resolved to any digital object despite aggressive forensic recovery. Diagnostic categorization reveals three distinct failure modes: pure hallucinations (5.1%), hallucinated identifiers with valid titles (16.4%), and parsing-induced matching failures (78.5%). Longitudinal analysis reveals a flat trend (+0.07 pp/month), suggesting that high-entropy citation practices have stabilized as an endemic feature of the field.
Conclusion: The scientific citation graph in AI survey literature exhibits “link rot” at scale. This suggests a mechanism where AI tools act as “lazy research assistants,” retrieving correct titles but hallucinating metadata, thereby severing the digital chain of custody required for reproducible science

Experiences and Challenges of Women with Chronic Physical Health Conditions during the Transition to Motherhood: A Scoping Review Protocol
Objective:
The objective of this scoping review is to map and synthesise the evidence on how women with chronic physical health conditions experience the transition to motherhood during the perinatal and early postnatal period. The review will identify key challenges, adaptations, contextual influences, and gaps in the existing literature.
Introduction:
Women living with chronic physical health conditions may face distinct physical, emotional, and social challenges during the transition to motherhood. Although individual studies exist across different illness groups, no comprehensive review has examined how first-time mothers with chronic illness experience the perinatal and early postpartum period. A scoping review is needed to clarify what is known, how this topic has been studied, and where knowledge gaps remain.
Review question:
How do women with chronic physical health conditions experience the transition to motherhood during the perinatal and early postnatal period?
Inclusion criteria:
This review will include studies involving primiparous women with medically diagnosed chronic physical health conditions, capturing their experiences and outcomes — as reported through quantitative, qualitative, or mixed-methods approaches — from the final trimester of pregnancy through 12 months postpartum.
All geographic, cultural, and healthcare contexts will be eligible. Primary studies of any design, theses, and relevant grey literature will be included.
Methods:
A three-step search strategy will be undertaken across MEDLINE (via PubMed), CINAHL, PsycINFO, Scopus, Web of Science, and ProQuest Dissertations & Theses. This will be supplemented by targeted grey literature searches, including screening of Google Scholar, relevant organisational websites (e.g., WHO, AIHW), and citation chaining of key articles and reviews.
No date limits will be applied; only English-language sources will be included. Two reviewers will independently screen, extract, and chart data using a standardised form. Evidence will be mapped descriptively using frequency counts, narrative summaries, and qualitative content categorisation, presented in tables, narrative synthesis, and optional visual formats.
