Glacier Lake Inventory and Lake Outburst Flood Hazard Assessment in Kanchenjunga Conservation Area, Nepal
This study presents a comprehensive assessment of glacial lakes and land surface temperature (LST) dynamics within the Kanchenjunga Conservation Area (KCA) from 2010 to 2023. The KCA holds significant ecological importance, serving as a critical habitat for diverse flora and fauna, while also playing a vital role in regional hydrological systems. Additionally, its stunning landscapes attract tourists and mountaineers from around the world, further emphasizing the need for understanding environmental changes within the area. Utilizing medium to high-resolution satellite imagery from PlanetScope for glacial mapping and Landsat for LST analysis, the research aimed to achieve two primary objectives: creating an updated inventory of glacial lakes and conducting a thorough assessment of Glacial Lake Outburst Flood (GLOF) hazards associated with these lakes. Focusing on a 200-meter buffer around Randolph Glacier Inventory (RGI) boundaries ensured precise analysis within the glacial zone.
In 2023, a total of 140 glacial lakes covering 3.37 km² were identified, revealing their spatial distribution and characteristics. Of these, 76 were supra-glacial lakes and 27 were proglacial lakes, highlighting their dominance in the region. Analysis of elevation profiles highlighted the prevalence of lakes at higher elevations, while size distribution analysis showed dominance of small to medium-sized lakes, with larger classes contributing significantly to total lake area. Furthermore, the study assessed the trends in land surface temperature (LST), revealing mean temperatures of -8.73°C in 2010, -7.28°C in 2015, and -7.09°C in 2023. These temperature variations have significant implications for glacial melt rates, vegetation dynamics, and local climate patterns.
Additionally, hazard assessment of glacial lakes identified varying levels of GLOF susceptibility, with one lake categorized as \u27Very High,\u27 one as \u27High,\u27 five as \u27Low,\u27 and three as \u27Very Low\u27 hazard levels. This assessment was conducted using the Analytic Hierarchy Process (AHP). Overall, this study contributes to understanding the complex dynamics of glacial lakes and LST in the KCA, highlighting the importance of continued monitoring and assessment for informed decision-making and mitigation strategies in the face of changing environmental conditions
Detailing the Effects of Cannabidiol on Exosome Release in Ewing’s Sarcoma
Ewing’s sarcoma (ES) is a highly aggressive pediatric cancer found within bone that carries a low five-year survival rate within its patients. This is due, in part, to the cancer’s high malignancy properties as well as the development of resistance to the chemotherapies used to combat this cancer. This project aims to determine if cannabidiol (CBD) has an effect on either exosome release or the protein, signal transducer and activator of transcription 3 (STAT3) within ES cells. Exosomes are vesicles that can carry proteins and other signaling molecules which are released by the cell and allow for cell-to-cell communication. Within cancer cells, these exosomes are hijacked and begin to communicate cancer proteins to other cancer cells or even healthy cells. Because of this, exosomes can allow for cancer migration, invasion, and immune escape which leads to making the cancer harder to treat. STAT3 is a protein that normally acts as a transcription activator but has abnormal functionality in 70% of all cancers, assisting in tumor-induced immunosuppression. This protein is associated with exosome release as it has an impact on exosome production as well as being able to be packaged into exosomes to be sent to other cells, causing more mutations and more cancer cells to be created. The effects of CBD on both mechanisms have been observed within other cancer types such as prostate cancer and hepatocellular carcinoma, showing that an increase in CBD concentration leads to a decrease in both cellular exosome release as well as STAT3 expression. CBD was found to have no statistical impact on ES cell viability allowing for exosome quantification. This data encourages further investigation of CBD as a therapeutic for both mechanisms of tumor progression

NUTRIC: Enhancing Surgical Intensive Care Unit Patient Nutrition Consultations
Background: Daily nutritional screenings in the critical care population can identify patients in need of nutritional modifications during hospital admission. Inadequate nutritional assessments of critically ill patients lead to poor outcomes. Daily nutritional assessments result in the early identification of patients at risk for nutritional compromise compared to weekly assessments.
Purpose: The purpose of this project was to incorporate a daily nutritional assessment using the Nutrition Risk in the Critically Ill (NUTRIC) scores for patients in a surgical intensive care unit to identify patients in need of a referral to a registered dietician.
Methods: The method for this project used secondary data from the surgical intensive care patient to calculate daily NUTRIC scores. Patients scoring five or greater were considered high risk for unmet nutritional needs. These patients required nutritional consultations by the registered dietitian.
Results: The current policy identified 0.29 patients per day or one patient every 3.5 days requiring a nutrition consult by the registered dietitian. Using NUTRIC scores, the number of patients requiring consultations by the registered dietitian increased to 0.57 patients per day or a patient every 1.75 days. Overall, NUTRIC scoring resulted in an increase in dietary referrals of 96.6% in the surgical intensive care patient.
Conclusions: Daily NUTRIC scores identified more at-risk for unmet nutritional needs patients than the facilities’ weekly nutritional assessments in the critical care population thereby increasing registered dietitian consultations.

Implementing a Comprehensive Tobacco-Cessation Program for Patients with Tobacco Use Disorder
Background: Individuals who use tobacco are at an increased risk for the development of heart disease, congestive heart failure, and chronic obstructive pulmonary disease. Tobacco use is one of the most preventable causes of death in the world.
Purpose: The focus of the quality improvement project was to improve access to tobacco cessation resources in patients with tobacco use disorder at a primary care clinic.
Methods: Weekly stakeholder meetings were held to ensure the validity of the project. The plan, do, study, act (PDSA) model of continuous quality improvement guided the project. A comprehensive tobacco-cessation program including informational materials and cards with resource referrals for patients was developed. Nurse practitioners offered informational cards to patients whose electronic health records indicated a diagnosis of tobacco use disorder. Cards included referral information on national, state, and community tobacco-cessation resources.
Results: Out of the 127 patients total, 43 patients (34%) had a diagnosis of tobacco use disorder on their electronic medical record. Of those, 29 accepted and 14 declined the informational cards.
Conclusion: The community clinic plays a vital role in resource allocation for tobacco users. Nurse practitioners should continue to provide patients with continuing education and access to resources.
