    European bio-naive spondyloarthritis patients initiating TNF inhibitor: time trends in baseline characteristics, treatment retention and response

    Objectives To investigate time trends in baseline characteristics and retention, remission and response rates in bio-naive axial spondyloarthritis (axSpA) and psoriatic arthritis (PsA) patients initiating TNF inhibitor (TNFi) treatment. Methods Prospectively collected data on bio-naive axSpA and PsA patients from routine care in 15 European countries were pooled. Three cohorts were defined according to year of TNFi initiation: A (1999-2008), B (2009-2014) and C (2015-2018). Retention, remission and response rates were assessed at 6, 12 and 24 months. Results In total, 27 149 axSpA and 17 446 PsA patients were included. Cohort A patients had longer disease duration compared with B and C. In axSpA, cohort A had the largest proportion of male and HLA-B27 positive patients. In PsA, baseline disease activity was highest in cohort A. Retention rates in axSpA/PsA were highest in cohort A and differed only slightly between B and C. For all cohorts, disease activity decreased markedly from 0 to 6 months. In axSpA, disease activity at 24 months was highest in cohort A, where also remission and response rates were lowest. In PsA, remission rates at 6 and 12 months tended to be lowest in cohort A. Response rates were at all time points comparable across cohorts, and less between-cohort disease activity differences were seen at 24 months. Conclusion Our findings indicate that over the past decades, clinicians have implemented more aggressive treatment strategies in spondyloarthritis. This was illustrated by shorter disease duration at treatment initiation, decreased retention rates and higher remission rates during recent years

    Borderline ovarian tumors: A contemporary review of clinicopathological characteristics, diagnostic methods and therapeutic options

    Borderline ovarian tumors (BOTs) differ from the epithelial ovarian malignancies with their excellent prognosis, curability with surgery, and being seen in relatively young ages. Thus, fertility sparing and conservative surgical approaches are currently recommended. Preoperative diagnosis of BOTs can be challenging because, clinical and ultrasonographic features might overlap with invasive carcinomas and sometimes with benign adnexal masses. Certain characteristics such as stage at diagnosis, age of the patient and histologic features are important while deciding the extensiveness and the type of surgery. Detailed evaluation of the entire abdominal cavity and sampling all suspected areas are imperative during operation. Frozen section is essential for the intraoperative diagnosis, despite the fact that the diagnostic value of frozen section is not as high as in invasive ovarian carcinomas. Routine appendectomy and/or contralateral ovarian biopsy in cases of isolated tumor with normal appearing appendix and/or contralateral ovary are not recommended. Conservative approach might improve the recurrence rate without worsening the overall survival. The exact role of laparoscopic surgery with its advantages and disadvantages in the treatment of BOTs needs to be confirmed with further studies

    Inflammatory myopathies with mitochondrial pathology and protein aggregates

    Objectives: To compare the clinical course and Muscle biopsy features of polymyositis with mitochondrial pathology (PM-Mito) to inclusion body myositis (IBM) and steroid-responsive inflammatory myopathies (polymyositis). Methods: We compared clinical, laboratory and myopathologic features in a retrospective study of patients with PM-Mito (23), IBM (26) and polymyositis (12). Results: Selective weakness in the quadriceps or finger flexors was common in PM-Mito (62%) and IBM (87%). Weakness progressed more slowly in PM-Mito than in IBM. PM-Mito patients with more rapidly progressive weakness had more cytochrome oxidase negative Muscle fibers. There was no history of benefit from corticosteroid treatment in any PM-Mito or IBM patients. B-cell foci were absent in IBM and PM-Mito. LC3, an autophagy marker, and alpha B-crystallin were common in aggregates in PM-Mito and IBM, but not polymyositis. SMI-31 and TDP-43 positive aggregates were common in IBM but not in PM-Mito or polymyositis. beta-amyloid showed no differences in aggregates among the three groups. Conclusions: PM-Mito and IBM may be part of the same disease spectrum. PM-Mito has more slowly progressive weakness than IBM and rarely has TDP-43 or SMI-31 staining aggregates in muscle fibers. The most frequent proteins in aggregates in both PM-Mito and IBM are LC3, an autophagy marker, and alpha B-crystallin. Alterations in autophagic degradation pathways may be a common pathogenic mechanism in PM-Mito and IBM. In pathologically typical polymyositis, staining for mitochondrial enzyme activity, aggregates and B-cells helps to distinguish PM-Mito from inflammatory myopathy syndromes that are more likely to respond to corticosteroid treatment.     Investigation of microstructural and mechanical properties of hot forged 31Mn4 dual phase steel with computer-aided simulations

    In this study, it is aimed to realise the hot forging process design and prototype production of the steel yielding support connection clamp, the critical safety element of underground mining ground support systems, by using 31Mn4 dual phase steel. In this context, at the end of the forging and deburring processes, air cooling was designed, simulated and cooling rates were obtained. The cooling curve was integrated into the continuous cooling transformation diagram and phase formation was predicted. The distribution of pearlitic, ferritic, bainitic structures and formation rates, hardness distributions, strength data were also obtained. The hardness and tensile strength calculations, microstructural examinations and SEM analysis were carried out for validation. It has been determined that pearlitic-ferritic microstructures are formed in regions where the cooling rate is slow. In thinner sections where the cooling rate is around 1.2 degrees C/sec, ferritic structures become smaller and bainite phase was observed. As a result, the average tensile strength in 1st and 2nd regions were recorded as 684.5, 690 MPa in simulation, while these values were recorded as 688.45 and 694.15 MPa in tensile tests. It has been determined that this result corresponds to the 99.4% accuracy rate of the prototype obtained by simulation-supported and real productions

    Effect of polyethylene glycol 4000 on adhesion formation following thyroid surgery in rats

    Background & objectives: Difficulties in dissection encountered during. reoperative thyroid surgery could be minimised by preventing formation of adhesions in the,operative field. We studied the prevention of adhesion formation in rats after thyroid surgery by using polyethylene glycol 4000 (PEG 4000). Methods: Twenty two rats in two equal groups were studied. Right hemithyroidectomy was performed in both groups with 0.9 per cent saline being instilled in. the operative field in the control group and PEG 4000 solution in the study group. After 10 days, all rats were re-explored and the remaining thyroid tissue of the right lobe was excised. The specimens were scored according to the thickness of the connective tissue over the remainder of the thyroid gland. Results: The connective tissue thickness over the remaining tissue of the thyroid gland in the control and study groups was 272.04 +/- 77.10 and 172.90 +/- 48.92 mum respectively. The decrease of the connective tissue thickness over, the remainder of thyroid gland in the study group Was significant (P<0.05). Interpretation & conclusion: In this animal model although application of PEG 4000 did not completely prevent adhesion formation, a significantly decreased amount of adhesions was found

    Analysis of Emotional Intelligence and Personality Traits of Students In Faculty of Sports Sciences

    This study it was aimed to examine the emotional intelligence and personality traits of the students who study in different departments of the Faculty of Sport Sciences at Ege University and actively do sports in different sports branches. In total 549 students participated in the study voluntarily. To evaluate the variables of the study, the Schutte Emotional Intelligence Scale developed by Schutte et al. (1998) and adapted into Turkish by Tatar et al. (2011), and to evaluate personality traits, five-factor personality inventory developed by Somer, Tatar, and Korkmaz (2004) and whose short form was validated and reliable by Tatar (2005) were used. In this study, the data set was SPSS 22.0, and it was completed by using frequency tables, reliability analysis, independent sample t-test, one-way variance analysis, Tukey test, and correlation analysis in analyzes. There is both a significant (p0.05) difference between emotional intelligence and subscales of personality according to the department, type of education, average grade, class, gender, age and branches. There is no significant difference between emotional intelligence and personality (p> 0.05). They are independent of each other. As a result, as the concepts of emotional intelligence and personality affect individuals deeply, the main purpose of this study is to find the necessary information with the necessary studies to better understand the students in the sports science faculty. This study is to measure the emotional intelligence and personality traits of the students studying at the Faculty of Sport Sciences at Ege University and to examine the relationship between emotional intelligence and personality. We think that it is an important study as it will make a serious contribution to the studies by repeating similar studies and applying them to large sample groups in different fields in different universities and different branches

    Energetic performance analysis of a solar photovoltaic cell (PV) assisted closed loop earth-to-air heat exchanger for solar greenhouse cooling: An experimental study for low energy architecture in Aegean Region

    An experimental system was developed and tested in order to investigate the energetic performance of a solar photovoltaic system (PV) assisted earth-to-air heat exchanger (underground air tunnel) that is used for greenhouse cooling at the Solar Energy Institute, Ege University, Izmir, Turkey. Average value of temperature differences between inlet and outlet of earth-to-air heat exchanger (EAHE) was observed 8.29 degrees C at experimental measurements. The average heat discharge rate (cooling load) was realized as 5.02 kW by using 0.7 kW fan. System was operated about 11 h/day. As a result, total electricity energy consumption of the system was measured to be 8.10 kWh and 34.55% of this energy demand was provided from photovoltaic cells. Furthermore, 65.45% of the electricity energy demand was provided from grid connection. Results are discussed and interpreted in the paper for various performance metrics. (C) 2012 Elsevier Ltd.     Evaluation of the Effects of Different Anesthetic Techniques on Neonatal Bilirubin Levels

    Objectives: The aim of the present study was to determine whether different anesthetic techniques applied for vaginal delivery and cesarean section affect neonatal bilirubin levels in the first 24 hours of life. Materials and Methods: A total of 511 neonates delivered by vaginal route or cesarean section were included in the study. The neonates were classified according to method of delivery and anesthetic agents as group A (cesarean section / general anesthesia with sevoflurane), group B (cesarean section / spinal anesthesia with bupivacaine hydrochloride), group C (vaginal delivery with episiotomy / local anesthesia with prilocaine hydrochloride) and group D (vaginal delivery/ no anesthesia). The levels of neonatal serum bilirubin in the groups were compared. Results: There was no difference between group A and group B in terms of neonatal bilirubin levels (p = 0.98). Depending on the use of prilocaine hydrochloride as local anesthetic agent in the vaginal delivery, there was no significant difference between the groups C and D, in terms of the neonatal bilirubin levels (p = 0.99). The serum levels of bilirubin in cesarean section groups were significantly higher than those of the vaginal delivery groups (p < 0.001). Conclusion: Prilocaine hydrochloride used for episiotomy did not exert any effects on neonatal hyperbilirubinemia. However, cesarean section with sevoflurane and bupivacaine hydrochloride seemed to result in increased bilirubin levels
