Myotonic Dystrophy
Myotonic dystrophy (MD) is a rare, progressive muscle disease, characterized by autosomal dominant heredity and multisystem involvement. The muscle involvement's being progressive in MD patients leads to negative effects such as dependency of the patients in their daily activities. For this reason, medical treatment and rehabilitation of these patients are very important. We considered 3 patients with MD diagnosis in this series of cases. We made pre-treatment and post-treatment evaluations of 3 patients who were hospitalized in our clinic by giving them a 4 week conventional exercise program and functional electrical stimulation (FES) treatment. The rehabilitation program we applied to the patients contained joint's range of motion exercises, breathing exercises, standing training, gait training, balance coordination exercises, and FES application to ankle and wrist dorsiflexors. We achieved positive changes in the clinical findings that we documented with pre-treatment and post-treatment assessment values
A Numerical Approach Technique for Solving Generalized Delay Integro-Differential Equations with Functional Bounds by Means of Dickson Polynomials
In this study, we have considered the linear classes of differential-(difference), integro-differential-(difference) and integral equations by constituting a generalized form, which contains proportional delay, difference, differentiable difference or delay. To solve the generalized form numerically, we use the efficient matrix technique based on Dickson polynomials with the parameter-a along with the collocation points. We also encode the useful computer program for susceptibility of the technique. The residual error analysis is implemented by using the residual function. The consistency of the technique is analyzed. Also, the numerical results illustrated in tables and figures are compared
MHD mixed convection of nanofluid filled partially heated triangular enclosure with a rotating adiabatic cylinder
MHD mixed convection of Cu-water nanofluid filled triangular enclosure with a rotating cylinder is investigated numerically. A partial heater is added on the left vertical wall of the cavity and the right inclined wall is kept at constant temperature. Other walls of the triangular cavity and cylinder surface are assumed to be adiabatic. The governing equations are solved using the finite element method. The effects of the Grashof number, Hartmann number, angular rotational speed of the cylinder and volume fraction of the nanoparticle on fluid flow and heat transfer are investigated numerically. The second law of thermodynamics is also applied to the flow and heat transfer corresponding to different combinations of parameters. It is observed that with increasing the Hartmann number the total entropy generation, local and averaged heat transfer decrease. Averaged Nusselt number increases with the Grashof number. Averaged heat transfer and total entropy generation increase with increase in the angular rotational speed of the cylinder. 50.4% and 37.4% of heat transfer enhancements are obtained for omega = 20 and omega = -20 compared to motionless cylinder omega = 0. Heat transfer and total entropy generation increase as the solid volume fraction of nanoparticle increases. (C) 2014 Taiwan Institute of Chemical Engineers. Published by Elsevier B.V. Retinol and α-tocopherol concentrations in breast milk of Turkish lactating mothers under different socio-economic status
Retinol and alpha-tocopherol levels in breast milk of Turkish mothers under different socio-economic status were investigated. Mature milk samples were collected from 92 lactating mothers living in Izmir and in Manisa, cities of Turkey, who were at 60-90 days of the lactating period. Socio-economic, anthropometric, and dietary data were collected by means of a questionnaire. The body mass index was used to determine the nutritional status. The retinol and alpha-tocopherol contents of breast milk were determined by high-performance liquid chromatography; the concentration of alpha-tocopherol was 9.84 mu g/ml whereas retinol levels were 81.5 mu g/100 ml. The questionnaire survey was used to determine the level of these vitamins in the daily ration of the women. No significant differences were found in terms of milk retinol and alpha-tocopherol levels for the variables income, educational level and mothers' body mass index
Dynamic dispatching priority setting in customer-oriented manufacturing environments
In today's competitive environment, customer-oriented view is essential in gaining sustainable competitive advantage. This study aims to reflect the customer-oriented view to production planning and control decisions. To this aim, a simulation optimization-based approach is developed for job shop systems with dynamic order arrivals. Product-type-based lot splitting is applied in order to improve the flow time, and machine-based dispatching rules are utilized for sublot scheduling to realize dynamic scheduling. Multiple customer segments with different importance weights and their expectations and penalties on order completion rate on due date, tardiness, and earliness are considered. A customer satisfaction-based objective function is defined. Customer-oriented dispatching rules are proposed in this study to ensure the prioritization of orders from the key customers in order fulfilling. In order to prevent customer losses by providing a balanced structure between the customer segments in terms of the satisfaction levels, weight setting functions that dynamically compute the weights in the proposed dispatching rules are proposed. It is aimed to determine the near-optimal values of the segment-based parameters of the related weight setting functions. To this aim, a differential evolution algorithm-based simulation optimization approach is proposed. To confirm its viability, the proposed approach is applied to a realistic job shop system
Malignant tumor of outer root sheath epithelium, trichilemmal carcinoma: Clinical presentations, treatments and outcomes
Objectives: To emphasize different clinical features of tumor that can be misdiagnosed clinically. Methods: A total of 8 cases operated between September 2009 and 2016 at the Celal Bayar University, Faculty of Medicine were included in the study. Patients' clinicopathological features, type of surgery and follow up information were evaluated. Results: Six patients were male. The average age was 75.50. The lesions were located on the head and neck, and chest wall. Six patients had a history of the rapid growth of lesion. There was no metastasis at the time of diagnosis. None of the patients needed adjuvant therapy. Mean follow up time was 19.37 months. None of the patients developed recurrence or metastasis. Conclusion: This tumor resembles basal or squamous cell carcinoma. The histopathological evaluation may lead to misdiagnosis. Regional or distant metastasis is very rare. There is no consensus about adjuvant therapy. Screening for metastasis and close follow up are mandatory
N-BISHOP DARBOUX VECTOR OF THE SPACELIKE CURVE WITH SPACELIKE BINORMAL
In this article, the N-Bishop frame in Minkowski space is investigated for space like curves with a spacelike binormial. Some features of the normal expansion are proven via for the spacelike curve. Then, a new Darboux frame called by the N-Bishop Darboux frame is introduced at first time. Furthermore, some geometrical properties of the N-Bishop Darboux frame are proven. As a result, the N-Bishop Darboux axis and momentum rotation vector are calculated
A rare cause of persistent atelectasis in childhood: Mucoepidermoid carcinoma
A 12-year-old boy presented to our clinic with recurrent lower respiratory tract symptoms that are wheezing, cough, bronchopneumonia, and fever and with a diagnosis of bacterial endocarditis. Physical examination revealed that breath sounds diminished in the left lower lobe. Because cardiac sounds and echocardiography were normal bacterial endocarditis was excluded. Chest radiograph and computerized tomography scan showed a left lower lobe atelectasis and consolidation. Despite medical therapy including antibiotics, bronchodilator etc, atelectasis persisted. Fiberoptic flexible bronchoscopy (FFB) revealed a tumor that totally obstructed the left lower lobe bronchus. Open lung biopsy revealed a low-grade mucoepidermoid carcinoma. Left lower lung lobectomy was performed. The patient is in good condition 12 months after the operation. We proposed that FFB should be performed earlier in patients with recurrent lower respiratory symptoms who have persistent atelectasis or unifocal infiltration
Effects of interferon-α-2b and octreotide on healing of esophageal corrosive burns
Objective. The aim of this study was to investigate the effect of interferon-alpha to 2b and octreotide on the treatment of esophagus corrosive burns. Method: The experimental study was performed on 63 rabbits. Burn was obtained by treating a segment, isolated in cervical esophagus, with NAOH. The rabbits were allocated into three groups (control, interferon, and octreotide). On the fourth, eighth, and 20th days, 7 rabbits randomly selected from each group were killed. Comparisons among the groups were made by taking histopathologic findings, stenosis index, and hydroxyproline production into consideration. Cross-tabulation and analysis of variance tests were used for statistical analysis. Results: There were no statistically significant differences among the groups in terms of epithelial changes, extension of inflammation, and severity of fibrosis. In terms of severity of inflammation, there was a statistically Significant difference between the control and the other two groups. Also, a significant difference was found between the control group and the other two groups in terms of extension of fibrosis on the 20th day. The difference between the control and the two treatment groups was significant on the eighth day and the 20th day measurements. Comparison of the hydroxyproline values showed a statistically significant difference between the octreotide group and the other two groups on the eighth and 20th days. Conclusion: Histopathologic and biochemical findings indicate that, by hindering fibrosis progress, octreotide (and interferon) might be new and promising alternatives in the treatment of corrosive burns of the esophagus
Axillary Schwannoma Mimicking Lymphadenopathy: Two Different Case Reports
Schwannomas are essentially benign tumors originating from schwann cells of peripheral, cranial or sympathetic nerves. They are not aggressive, encapsulated and grow slowly. Malignant transformation can rarely be seen. Even though they can be localized in any where in the body, cutaneous nerves of the head-neck region and flexor parts of the extremities are most commonly involved. Schwannomas are usually solitary, however multiple schwannomas can rarely be seen in the peripheral nervous system including cranial nerves, spinal nerve roots, brachial-lumbosacral plexus and peripheral nerves. In these two cases, we aimed to report ultrasonography (US), magnetic resonance imaging (MRI) and pathology findings of different axillary schwannomas mimicking lymphadenopathy, and discuss the differential diagnosisin the light of the literature. In these patients, the lesions were noted to be in direct continuity with a cord-like structure resembling a nerve. US and MRI findings were compatible with the literature. Radiologic findings were confirmed by pathologic examination. As seen in our cases, a detailed differential diagnosis should be considered in every axillary lesion including frequently seen axillary lymphadenopathies
