Pleomorphic adenoma originated from the inferior nasal turbinate
Although pleomorphic adenoma is the most common benign neoplasm of the salivary glands, it has also been reported to be present in the neck, ear, mediastinum, external nose and nasal cavity. Intranasal localization of this lesion is very rare and mainly originates from the nasal septum. From wherever the lesion originates, the main treatment modality should be surgical. We presented a very rare case of intranasal pleomorphic adenoma originated from the inferior nasal turbinate. Due to the expansile nature of the lesion, a midfacial degloving approach was preferred. Essential Oil Composition and Antioxidant Activities of Alkanet (Alkanna tinctoria subsp. tinctoria)
This study is designed to examine the chemical composition and in vitro antioxidant activity of the hydrodistillated essential oil and the various extracts of alkanet (Alkanna tinctoria subsp. tinctoria). Gas chromatography (GC) and GC-mass spectrometry (GC-MS) analysis of the essential oil were resulted in the determination of 27 different compounds, representing 93.32% of the total oil. The major compounds detected in the oil, were pulegone (22.27%), 1,8-cineole (13.03%), alpha-terpinyl acetate (6.87%), and isophytol (6.83%), respectively. Antioxidant activities of thy samples were determined by 4 different test systems namely beta-carotene/linoleic acid, 1,1-diphenyl-2-picrylhydrazyl (DPPH), reducing power, and chelating effect. In all systems, essential oil showed the weakest activity profile. On the other hand, ethyl acetate and water extracts exhibited excellent antioxidant activities. As well as the antioxidant activities of the extracts, they were evaluated in terms of their total phenolic and flavonoid contents. In parallel to the experiments, ethyl acetate and water extracts were found to be rich-in these phytochemicals
Antimicrobial susceptibility and resistance mechanisms of methicillin resistant Staphylococcus aureus isolated from 12 Hospitals in Turkey
Introduction: Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most important nosocomial pathogens and is also emerging in Turkish hospitals. The aim of this study was to determine the antimicrobial susceptibility profiles of MRSA isolated from Turkish hospitals. Materials and methods: A total of 397 MRSA strains isolated from 12 hospitals in Turkey were included to present study. Antimicrobial susceptibilities were tested using agar dilution method. Presence of ermA, ermB, ermC, msrA, tetM, tetK, linA and aac-aph genes were studied by PCR. Results: All strains were susceptible to vancomycin and linezolid. The susceptibility rates for fusidic acid, lincomycin, erythromycin, tetracyclin, gentamycin, kanamycin, and, ciprofloxacin were 91.9%, 41.1%, 27.2%, 11.8%, 8.5%, 8.3% and 6.8%, respectively. Lincomycin inactivation was positive for 3 isolates. Of 225 erythromycin resistant isolates 48 had ermA, 20 had ermC, and 128 had ermA-C. PCR was negative for 15 strains. Of 3 isolates with lincomycin inactivation one had linA and msrA. Of 358 gentamycin resistant isolates 334 had aac-aph and 24 were negatives. Among 350 tetracyclin resistant isolates 314 had tetM. Of 36 tetM negative isolates 10 had tetK. Conclusion: MRSA isolates from Turkish hospitals were multiresistant to antimicrobials. Quinolone and gentamycin resistance levels were high and macrolide and lincosamide resistance were relatively low. Susceptibility rates for fusidic asid were high. Linezolide and vancomycin resistance are not emerged. The most common resistance genes were ermA, tetM and aac-aph. Evolution of antimicrobial susceptibilities and resistance genes profiles of MRSA isolates should be surveyed at regional and national level for accurate treatment of patients and to control dissemination of resistance genes
Coexisting acromegaly and primary empty sella syndrome
The normal functions of the pituitary gland may be suppressed when the gland is compressed onto the sella floor by arachnoid tissue extending through an impaired sella diaphragm. Interestingly, pituitary hormone hypo- and hypersecretion, including acromegaly, have been observed in patients with an 'empty sella' (1-4). This 'empty sella syndrome' has been classified into a primary form, in which no inciting factor (pituitary irradiation or surgery for a pituitary tumor) is present, and a secondary form, in which the empty sella occurs after pituitary procedures. In this report we describe a patient who presented with clinical and biochemical features of acromegaly and who had an empty sella on pituitary magnetic resonance imaging (MRI)
Psychometric Properties of the Turkish Version of the University o Virginia Parent Low Blood Sugar Survey
Objective: The aim of this study was to produce and validate a Turkish version of the University of Virginia Parent Low Blood Sugar Survey (P-LBSS). The P-LBSS is used to assess parental fear of their diabetic children's hypoglycemia. Methods: Linguistic, content and face validity of the translated P-LBSS was tested. Afterwards, explanatory and confirmatory factor analyses were conducted in order to evaluate construct validity. Results: The sample included 390 parents of type 1 diabetic adolescents aged 12-17 years. Results of the factor analysis showed that the Turkish P-LBSS had 2 subscales (behavior and worry) as in the original. The Cronbach's alpha coefficient of the Turkish version of the total P-LBSS was found to be 0.803, and the value was 0.865 for the behavior and 0.790 for the worry subscales. Psychometric investigation of the Turkish version of P-LBSS indicated high reliability and good retestability, content and construct validity. Conclusion: The Turkish P-LBSS is a valid and reliable instrument to measure the fear of hypoglycemia experienced by parents of diabetic adolescents aged between 12-17 years in the TUrkish population
A Novel Cooling System by Surface Corrugation and Nanofluid Utilization for the Performance Improvement of Photovoltaic Module Coupled with Thermoelectric Generator and Efficient Computations by Using Artificial Neural Network-Based Hybrid Scheme
For a photovoltaic module coupled with thermoelectric generator, a unique wavy cooling channel is proposed, and its performance is numerically assessed by using three-dimensional computations. The cooling channel uses nanofluid of alumina-water with various shaped nanoparticles (spherical, cylindrical and brick). Numerical simulations are performed for a range of parameters for the corrugation amplitude (0 \text {o}\end{document}C, while frequency has little impact on average cell temperature and its uniformity. The best-performing particles are those with cylindrical shapes, and the drop-in average photovoltaic temperature with solid volume fraction is essentially linear. As utilizing cylindrical-shaped particles, the average temperature of corrugated cooling channels decreases by around 1.9 o\documentclass[12pt]{minimal} \usepackage{amsmath} \usepackage{wasysym} \usepackage{amsfonts} \usepackage{amssymb} \usepackage{amsbsy} \usepackage{mathrsfs} \usepackage{upgreek} \setlength{\oddsidemargin}{-69pt} \begin{document}\text {o}\end{document}C as compared to flat cooling channels with base fluid at the greatest solid volume fraction. As compared to un-cooled photovoltaic, cell temperature drops by around 43.2 o\documentclass[12pt]{minimal} \usepackage{amsmath} \usepackage{wasysym} \usepackage{amsfonts} \usepackage{amssymb} \usepackage{amsbsy} \usepackage{mathrsfs} \usepackage{upgreek} \setlength{\oddsidemargin}{-69pt} \begin{document}\text {o}\end{document}C when employing thermoelectric generator. However, temperature drop value of 59.8 o\documentclass[12pt]{minimal} \usepackage{amsmath} \usepackage{wasysym} \usepackage{amsfonts} \usepackage{amssymb} \usepackage{amsbsy} \usepackage{mathrsfs} \usepackage{upgreek} \setlength{\oddsidemargin}{-69pt} \begin{document}\text {o}\end{document}C can be obtained by using thermoelectric generator and nano-enhanced wavy cooling channel utilizing cylindrical-shaped nanoparticles. An hybrid computational strategy for the fully coupled system of photovoltaic with cooling system is provided, which reduces the computational time by a factor of 75
Best Practice Recommendations for Dysphagia Management in Intensive Care Unit (ICU): A Delphi Study from Multidisciplinary Experts in Turkey
There is no study about all aspects of oropharyngoesophageal (OPE) dysphagia from diagnosis to follow-up in a multidisciplinary manner in the world. In order to close this gap, we aimed to create a recommendation study that can be used in clinical practice, addressing all aspects of dysphagia in the ICU in detail with the opinion of experienced multidisciplinary experts. This recommendation paper was generated by a multidisciplinary team, using the seven-step process and a three-modified Delphi round via e-mail. Firstly, 15 open-ended questions were created, and then detailed recommendations including general principles, management, diagnosis, rehabilitation, and follow-up were created with the answers from these questions, Each recommendation item was voted on by the experts as overall consensus (strong recommendation), approaching consensus (weak recommendation), and divergent consensus (not recommended). In the first Delphi round, a questionnaire consisting of 413 items evaluated with a scale of 0-10 was prepared from the opinions and suggestions given to 15 open-ended questions. In the second Delphi round, 55.4% were accepted and revised suggestions were created. At the end of the third Delphi round, the revised suggestion form was approved again and the final proposals containing 133 items were created. This study includes comprehensive and detailed recommendations, including a broad perspective from diagnosis to treatment and follow-up, as detailed as possible, for management of dysphagia in patients with both oropharyngeal- and esophageal-dysphagia in ICU

Serum angiotensin-converting enzyme and plasma atrial natriuretic peptide levels in hyperthyroid and hypothyroid rabbits
Background. It is known that serum angiotensin-converting enzyme (ACE) and plasma atrial natriuretic peptide (p-ANP) levels increase in hyperthyroidism. However, the precise mechanism of the effects of thyroid hormone on ANP release remains to be clarified. No study investigating serum ACE together with p-ANP levels has been performed in experimental hyperthyroid and hypothyroid rabbits. The present study was designed in order to provide additional evidence of increased ANP production and secretion in hyperthyroidism and to investigate the relationships between ANP, ACE and thyroid hormones. Methods. Male New Zealand white rabbits (2.3-3.4 kg) were used throughout the study. Hyperthyroidism was induced by daily intraperitoneal administration of L-thyroxin (50 mu g/100 g). Hypothyroidism was induced by daily intraperitoneal injection of propylthiouracil (2 mg/100 g body weight). Twelve days after the end of treatment, animals were sacrified under anesthesia and blood samples were obtained from the aorta for serum ACE and thyroid hormone and p-ANP determinations. Results. Serum ACE, plasma renin activity (PRA) and p-ANP were higher in hyperthyroid rabbits and lower in hypothyroid rabbits than in euthyroid rabbits. ANP concentration in atria was lower in hyperthyroid rabbits and higher in hypothyroid rabbits than in euthyroid rabbits. p-ANP, PRA and serum ACE levels were positively correlated with serum thyroxin levels. Inverse correlation was found between serum thyroxin and ANP concentration in atria (a-ANP), and between p-ANP and a-ANP. Conclusions. Our results indicate that not only p-ANP but also serum ACE activity was markedly increased in experimental hyperthyroid rabbits. It was thought that there were both direct and indirect effects of thyroxin on the release of ANP
