Les conceptions des patients atteints de DMLA en vue de la mise en place d’un programme d’éducation thérapeutique.
International audienceIntroduction: The introduction of an ETP programme is an excellent way of complementing medical care for people with age-related macular degeneration. With this in mind, we conducted qualitative descriptive research to identify patients’ conceptions of their disease, their care and their needs with regard to future ETP workshops. Methods: We used a qualitative methodology in a hospital setting with AMD patients. The results were compared with a sample of patients in two focus groups in order to establish the reliability of the results and compare them with the reality of patients’ experiences. Results - Discussion: Patients told us that AMD has a significant impact on their daily lives. What’s more, this disease generates anxiety which must be taken into account by the healthcare teams in the same way as the advice needed to improve patients’ quality of life. Conclusion: The results are likely to enlighten carers as to their day-to-day practices in the management of this disease, but also to be a source of suggestions for the design of contextualised and adapted ETP programmes.Introduction : La mise en place d’un programme d’ETP est tout à fait indiquée pour compléter la prise en charge médicale des personnes atteintes de dégénérescence maculaire liée à l’âge. C’est dans cet objectif que nous avons mené une recherche descriptive qualitative pour identifier les conceptions des patients sur leur maladie, leur prise en charge et leurs besoins quant à de futurs ateliers d’ETP. Méthodes : Nous avons utilisé une méthodologie qualitative en contexte hospitalier sur des patients atteints de DMLA. Les résultats ont été confrontés à un échantillon de patients dans le cadre de deux focus groupes afin d’asseoir la fiabilité des résultats et les confronter à la réalité du vécu des patients. Résultats – Discussion : Les patients nous indiquent que la DMLA à un impact significatif sur leur vie quotidienne. De plus, cette maladie génère une angoisse qui doit être prise en compte par les équipes soignantes au même titre que les conseils nécessaires à l’amélioration de la qualité de vie des patients. Conclusion : Les résultats sont susceptibles d’éclairer les soignants quant à leurs pratiques quotidiennes de prise en charge de cette maladie mais également d’être force de proposition pour concevoir des programmes d’ETP contextualisés et adaptés

Motor and Non-motor Complications Following Different Early Therapies in Parkinson’s Disease: Longitudinal Analysis of Real-Life Clinical and Therapeutic Data from the French NS-PARK Cohort
International audienceBackground: Levodopa, dopamine agonists (DA) and monoamine oxidase inhibitors (MAOI) are all approved first-line therapies for Parkinson's disease (PD), as monotherapy or in combination. Data on their use in the early management of patients with PD in real-life are lacking. Our objective was to assess the impact of early therapeutic strategies on the development of motor and neuropsychiatric complications using a nationwide PD cohort.Methods: NS-PARK is a cohort of patients with PD recruited between 2011 and 2021 from 26 expert centres for PD in France. We analysed the patients with less than 5-years disease duration and no motor complications at inclusion. We used interval censoring survival models to assess the associations between therapeutic strategies (levodopa monotherapy, levodopa alternative therapies or levodopa combinations) and motor fluctuations, dyskinesia, impulse control and related behaviours (ICRBs), apathy, psychosis/hallucination and daytime sleepiness. Analyses were adjusted for sex, age, disease duration, dopaminergic dose and disease severity.Results: We included 1722 patients (38.4% female, median age 67.7 years). At inclusion, 41% received levodopa monotherapy, 31% received levodopa alternative therapies and 28% received levodopa combinations. Compared with levodopa monotherapy, levodopa alternative therapies were associated with a lower dyskinesia risk (hazard ratio (HR) 0.48, 95% confidence interval (CI)[0.28-0.84]), but there was no significant difference in motor fluctuations. Both levodopa alternative and combinations therapies increased ICRBs risk (HR 4.06, 95% CI [2.48-6.67]; HR 5.16, 95% CI [3.00-8.86]) and decreased apathy risk (HR 0.36, 95% CI [0.26-0.49]; HR 0.52, 95% CI [0.39-0.69]). No association was found with psychosis/hallucination or daytime sleepiness.Conclusions: In this real-life cohort, our data supported an association between levodopa alternative therapies and a lower risk of dyskinesia and apathy, but a higher risk of ICRBs compared with levodopa monotherapy.Gov identifier: NCT04888364. Energetic approaches in wood fracture mechanics: A review of path-independent integrals
International audienceThis paper presents a review on the study of cracking in wood materials, focusing on analytical, numerical, and experimental modeling methods. A key emphasis is placed on the global energetic approach, using invariant integrals to calculate the energy release rate in cracked media of any dimensions. The mathematical formulations of these invariant integrals are briefly revisited, and a summary of experimental techniques and numerical methods is provided, highlighting work conducted to date, especially on wood material. This study includes a comprehensive analysis highlighting the trends observed in the conducted works, the challenges faced during the process, and proposed solutions to address these difficulties effectively. The main objective is to harmonize methods and approaches to develop advanced models for wood behavior, specifically regarding crack initiation andpropagation, critical factors tied to the failure and rupture of structural components. Thus, this paper simultaneously diagnose and address the challenges associated with modeling these complex behaviors, ultimately enhancing the strength and design criteria of wood materials
Transferability of European-derived Alzheimer’s Disease Polygenic Risk Scores across Multi-Ancestry Populations
International audienceA polygenic score (PGS) for Alzheimer’s disease (AD) was recently derived from data on genome-wide significant loci in European ancestry populations. We applied this PGS to populations in 17 European countries and observed a consistent association with AD risk, age at onset, and cerebrospinal fluid levels of AD biomarkers, independently of apolipoprotein E (APOE) . This PGS was also associated with the AD risk in many other populations of diverse ancestries. A cross-ancestry polygenic risk score (PRS) improved the association with AD risk in most of the multi-ancestry populations tested when the APOE region was included. Lastly, we found that the PGS/PRS, captured AD-specific information because the association weakened as the diagnosis was broadened. In conclusion, a simple PGS captures the AD-specific genetic information that is common to populations of different ancestries, but studies of more diverse populations are still needed for a better characterization of the AD genetics

ABO blood group and risk of pre-eclampsia. A case-control study
International audienceBackground It appears that some ABO blood groups may be a risk factor for venous thrombosis. The association between ABO group and risk of pre-eclampsia remains controversial. The purpose of this study was to evaluate the relationship between pre-eclampsia and ABO groups. Methods A retrospective case-control (1:2) study in a prospective monocentric registry including 20171 pregnancies was performed. Women with pre-eclampsia and severe pre-eclampsia were identified. Controls were matched according to age, parity, year of delivery and body mass index. Blood groups were considered. Results Two hundred fifty-three cases and 506 controls were included. No difference in distribution was found between cases and controls for non-O group (57.51% vs 62.56, p : 0.19), group A (39.92% vs 44.05, p : 0.29), group B (13.83% vs 15.42, p : 0.57) group AB (3.75 vs 3.08, p : 0.64). The analysis extended to severe pre-eclampsia showed no difference. Conclusion This study did not document any relationship between ABO groups and the risk of preeclampsia
