    Integrative Approach to the Management of Diabetic Neuropathy Using Marma Chikitsa and Panchakarma Therapies

    The National Centre for Disease Control (NCDC) has reported that India has 6.51 crore diabetes cases, with projections reaching 10.9 crore by 2035. Diabetic neuropathy is a prevalent complication of long-standing Type 1 and Type 2 Diabetes mellitus, affecting approximately 10.5% to 44.9% of individuals. With Diabetes emerging as a global epidemic in both developed and developing nations, effective management of its complications is the need of the hour. Diabetic neuropathy manifests as nerve damage, leading to symptoms such as hyperesthesia, paresthesia, pain, and sensory loss. Despite advancements in glycemic control strategies, conventional treatment remains inadequate in addressing neuropathic symptoms comprehensively. To bridge this gap, a holistic approach integrating Marma Chikitsa with Panchakarma therapies was implemented at our center, for the management of Diabetic Neuropathy where-in 5 patients presented with symptoms of diabetic neuropathy. This integrative intervention yielded significant improvements, demonstrating the efficacy of Marma Chikitsa in conjunction with Panchakarma therapies. Notably, patients experienced faster and more effective relief, as validated by the Modified Toronto Clinical Neuropathy Score (TCNS) assessment. These findings highlight the potential of Ayurveda-based therapies in enhancing neuropathic symptom management and underscore the necessity of adopting a holistic framework for diabetic care

    Management of Recalcitrant Bhagandra (Fistula in Ano) Using IFTAK Technique (An Ayurvedic Kshara Sutra Approach)

    Fistula-in-Ano (Bhagandara) is a challenging anorectal disorder marked by chronicity, recurrence, and potential complications like incontinence. Ayurvedic texts classify it among the Ashta Mahagada (eight grave diseases). Ayurveda offers Kshara Sutra therapy as a minimally invasive alternative. Case Presentation: A 30-year-old male presented with a recurrent perianal fistula. He had a history of incision and drainage in 2017, and laser fistulectomy in sept 2023. Magnetic resonance imaging (MRI) fistulography identified a trans-sphincteric fistulous tract with the internal opening located at the 6 o\u27clock position. The patient was successfully managed at our hospital using the minimal invasive ayurvedic IFTAK technique (Interception of Fistulous Tract with Application of Kshara Sutra). Intervention and Outcome: The IFTAK technique involves identifying the fistulous tract and creating a controlled window at the intersphincteric plane. Through this window, the tract was intercepted, and a medicated Kshara Sutra (alkaline seton) is selectively applied to the remaining portion of the tract. Postoperative management included weekly replacement of the medicated thread, sitz baths, local application of Jatyadi Taila, and administration of oral Ayurvedic medications. Complete healing of the fistulous tract was achieved within five weeks. No recurrence or postoperative complications were observed during an 18-month follow-up period. Conclusion: The IFTAK technique, when integrated with Ayurvedic interventions, provides a safe and effective approach for the management of complex or recalcitrant fistula-in-ano. This sphincter-conserving method is associated with reduced postoperative pain, promoting controlled healing and drainage, lower recurrence rates, and shorter hospital stays. Further studies are recommended to establish its broader clinical utility

    Rasayana Therapy in Ayurveda: Potential Role in Cancer Prevention and Survivorship

    Rasayana therapy, a cornerstone of Ayurveda, the traditional Indian system of medicine, is a holistic regimen focused on rejuvenation, enhancing longevity, and improving overall health and resilience. Objective: This review aims to critically evaluate the scientific evidence supporting the potential of Rasayana therapy in the realms of cancer prevention and management of cancer survivorship issues, aligning its traditional principles with modern pharmacological and clinical findings. Methods: A narrative review of published literature was conducted using databases such as PubMed, Scopus, Google Scholar, and the AYUSH Research Portal. Search terms included "Rasayana," "Ayurveda cancer," "adaptogen," "immunomodulation," "antioxidant," "apoptosis," alongside specific herbs like Ashwagandha, Curcuma longa, Tinospora cordifolia, and Emblica officinalis. Results: Preclinical studies robustly demonstrate that key Rasayana herbs possess multifaceted anti-cancer properties, including potent antioxidant activity, induction of apoptosis, inhibition of angiogenesis and metastasis, modulation of detoxification enzymes, and enhancement of immune function. These effects are mediated through the regulation of key signalling pathways such as NF-κB, STAT3, and PI3K/AKT. Clinically, Rasayana therapy shows promise in improving the quality of life (QoL), reducing cancer-related fatigue, mitigating chemotherapy-induced toxicities (myelosuppression, neuropathy), and potentially enhancing chemoradiotherapy efficacy through radioprotective and chemosensitizing effects. Conclusion: Rasayana therapy presents a compelling, multi-targeted adjunctive approach in oncology. Its principles of enhancing host defense (Vyadhikshamatva) align with goals of preventing carcinogenesis and supporting survivors. While the existing evidence is promising, further high-quality, rigorous clinical trials are essential to standardize formulations, establish safety profiles, and integrate these ancient wisdom-based interventions into modern evidence-based oncologic care

    Establishment of Standard Pharmaceutical Parameters for Mahasetu Rasa: A Classical Herbo-Mineral Formulation

    Mahasetu Rasa, a classical Kupipakwa Rasayana preparation described in Rasa Kamdhenu, requires systematic purification (Shodhana) of Parada, Vanga, and Gandhaka, followed by Kajjali formation and heating in an Electric Muffle Furnace. The present study aimed to standardize its pharmaceutical preparation and evaluate the final product using modern analytical techniques. Pharmaceutical observations confirmed the successful detoxification of raw materials. Materials and Methods: Raw materials were purified by traditional methods: Parada with Churnodaka and garlic paste, Vanga via Samanya and Vishesh Shodhana in various media, and Gandhaka by repeated Dhalana with ghee and cow’s milk. Kajjali was prepared by amalgamation and trituration, and Mahasetu Rasa was heated upto 550°C in EMF. Results: Purified materials exhibited expected detoxification and physical changes. Kajjali showed classical properties. Pharmaceutical observations confirmed the successful detoxification of raw materials, yield was after Shodhana, Shuddha Vanga (Sn) - 90.8%, Shuddha Gandhaka (S) - 88.08%, Shuddha Parada (Hg) - 88.40%, Kajjali - 98.66%, Kupipakwa yielded MSR1 - 38.5%, MSR2 - 40.06, MSR3 - 39.1% final product after 26 hrs of heating and completion. Conclusion: The standardized method ensures reproducible preparation of Mahasetu Rasa through effective purification, controlled sublimation and fixation using EMF heating, thereby producing a safe and uniform product suitable for both clinical and research applications

    A Case Study on Ayurvedic Management of PCOS-Induced Infertility with Successful Conception

    Polycystic Ovarian Syndrome (PCOS) is a major cause of infertility in women of reproductive age, often linked to lifestyle and metabolic disturbances. Ovulatory dysfunction accounts for nearly 30–40% of infertility cases. Diagnosis is typically based on at least two of the following: clinical/biochemical hyperandrogenism, oligo/anovulation, and polycystic ovarian morphology on ultrasonography (USG). Objective: To assess the effectiveness of Ayurvedic management in a case of secondary infertility associated with PCOS, focusing on restoration of ovulatory function and achievement of natural conception. Case Description: A 27-year-old female presented to the Prasuti Tantra and Stri-Roga Outpatient Department, National Institute of Ayurveda, Jaipur, on 11 June 2024, with secondary infertility of 1.5 years. Routine clinical, physical, and laboratory findings were within normal limits, except for USG evidence of bilateral polycystic ovaries. Intervention: A personalized Ayurvedic regimen was initiated, aimed at correcting hormonal imbalance, promoting ovulation, and enhancing reproductive health. The treatment plan included dietary regulation, lifestyle modification, and classical Ayurvedic formulations tailored to the patient’s condition. Outcome: After ten months of therapy, the patient reported a missed menstrual cycle. A urine pregnancy test on 7 March 2025 confirmed conception. Conclusion: This case highlights the potential role of Ayurvedic interventions in managing PCOS-related infertility. The observed outcome suggests that integrative, individualized treatment approaches may effectively restore ovulatory function and support natural conception, offering a promising alternative for women with reproductive disorders linked to ovulatory dysfunction

    Neuro-Muscular Rehabilitation Through Basti and Mamsa Pinda Sweda in Hirayama Disease

    Hirayama disease is characterised by juvenile onset of unilateral muscular atrophy of distal upper extremity. The disability originates with impaired functioning of anterior horn cells of the cervical cord. Studies consistently note a loss of the cervical lordosis and compression of the cervical cord by the dural sac in forward flexion. Here is a case report of a 24-year old male with Hirayama disease who had been treated as inpatient. The case was considered as a Vatavyadhi and the treatment protocol applied accordingly with incorporation of treatment of Asthikshaya, Vishwachi and Bahushosha particularly. The treatment included Abhyanga, Seka, Mamsa Pinda Sweda, Mustadi Yapana Basti and Rasayana Oushadhi. Even though the patient condition was irreversible, he was able to regain the functional status after restarting the treatment. The case highlights the improvement in quality of life of the patient after Ayurvedic management

    Vacho Agni: An Analytical Orientation of Association of Vak (Speech) & Agni

    Vak or speech is the main feature in human, that makes him unique from other species. This is the Karma of Vakindriya in the body. Speech is exhibited in three different levels in the body, as it is mentioned in Sushruta Samhita, i.e., Adhyathmika level of expression of Speech is Swara, Adhiboutika level of expression is Vak/Vachana and Adidaivika level of expression of speech is Agni. In this context, Vacho-Agni is been said, that specifies Agni as the Deity of Speech. Similar expression of referring Agni to Vak is also available in the texts of Upanishads and Purana. In the current paper, the association of Agni with the speech was analytically assessed in all three different planes and its importance was framed. This comprehensive information of the body in three levels, is required to get the true knowledge and to attain final salvation

    Amlapitta is one of the disturbing illnesses of the developing world. It usually affects people of all age groups. Modern day lifestyle and urbanization strongly affected the dietary habits of upper and lower middle-class people. Due of the indulgence of faulty foods in diet, half cooked foods, exposure to Helicobacter pylori infection, not eating foods at regular intervals, eating during indigestion, not taking enough sleep, etc. are the few causes that reduce the digestive fire causing toxins or acid to accumulate and increase the level of gases in the stomach causing the disease Amlapitta. The signs and symptoms of Amlapitta are similar to gastritis in modern medicine. The conventional treatment for gastritis starts with the use of proton pump inhibitors and H2 receptor blockers. But due to the side effects like headache, constipation, dizziness etc. patients usually fail to continue medicines for adequate relief. In these situations, Ayurvedic medicines have an edge over western medicines as herbal medicines have good gastrointestinal tolerance with least side effects. In this single clinical study, 30 patients were taken having Amlapitta and treated with 20ml twice daily dose of Bhoonimadi kadha before meals with equal quantity of warm water for 60 days. At the end of the study, it is seen that Shamana Chikitsa with Bhoonimadi kadha has shown remarkable improvement with complete relief in burning sensation and pain abdomen

    Psychosocial and Health Impacts of Digital Sexual Violence: Bridging Modern Interventions with Ayurveda

    Digital Sexual Violence (DSV) has emerged as a pervasive global challenge with the advancement of technology and increased dependence on digital platforms. It includes cyber harassment, non-consensual image sharing, sextortion, online grooming, and deepfake exploitation. From an Ayurvedic standpoint, such violations disturb Manas (mind), Indriyas (senses), and Sattva (mental strength), affecting holistic health. Methods: This paper aims to critically review the prevalence, forms, psychosocial impact, and preventive strategies for DSV, while exploring Ayurvedic insights through Rasayana and Vajikarana principles for resilience and healing. Studies addressing DSV prevalence, psychological consequences, legal frameworks, preventive measures, and Ayurvedic conceptual parallels were included. Classical Ayurvedic texts were also reviewed for references to Sadvritta, Achara Rasayana, and Vajikarana principles relevant to sexual health. Results: Findings highlight a rising prevalence of DSV worldwide, particularly among adolescents and young adults. Consequences include depression, anxiety, post-traumatic stress disorder (PTSD), social isolation, and increased risk of self-harm. Current preventive strategies emphasize legal reforms, digital interventions, and education. Ayurvedic concepts such as Sadvritta (ethical conduct), Achara Rasayana (psychosocial rejuvenation), and Manonigraha (mental discipline) provide unique preventive and rehabilitative insights. Rasayana drugs like Brahmi (Bacopa monnieri), Ashwagandha (Withania somnifera), and Shankhapushpi (Convolvulus pluricaulis) may strengthen mental flexibility and reduce psychological distress. Conclusion: DSV is a complex threat needing comprehensive solutions. Merging modern strategies with Ayurvedic principles offers a holistic approach, emphasizing ethics, psychosocial balance, and mind-body wellness to support survivor well-being and resilience

    From Pain to Power: Efficacy of Panchakarma in Vataja Gridhrasi with Padabhramsha

    Gridhrasi is a painful neurological disorder characterized by radiating pain, stiffness, and impaired mobility, often correlated with lumbar radiculopathy in contemporary terms. Padabhramsha (foot drop) represents a serious complication arising from aggravated Vata dosha, often triggered by trauma or excessive exertion. This case report presents a 53-year-old female patient with low back pain radiating to the right lower limb, associated with foot drop, following trauma and physical strain. She was also a known case of Type 2 Diabetes Mellitus and hypothyroidism. A comprehensive Panchakarma protocol was designed, including Sarvanga Abhyanga, Parisheka, Shashtikashali Pinda Sweda, dry cupping, and Mustadi Yapana Basti administered in Kala Basti pattern. Treatment outcomes were assessed using clinical parameters such as muscle strength, gait improvement, and SLR test. Significant improvements were noted in pain relief, muscle power (Tibialis anterior and EHL from 1/5 to 2/5), and mobility (step count improved from 8 to 12 steps in 10 seconds). This case highlights the potential of Brimhana Chikitsa and classical Panchakarma therapies in managing Vataja Gridhrasi with Padabhramsha, even in the presence of metabolic comorbidities. Early Ayurvedic intervention, when tailored to the individual Samprapti, can play a pivotal role in functional recovery and quality of life enhancement
