    Get PDF
Pilonidal sinus (PNS) is a chronic disorder that occurs in the space between the buttocks (natal cleft). Men are more susceptible (because they have more hair) to pilonidal sinus than women. Excision, primary closure and reconstructive flap surgery are the most commonly used surgical techniques for this disease. However, there is a high risk of recurrence or wound infection after surgery. In addition, the patient needs a longer hospital stay, and the procedure is expensive. Acharya Susrutha (800 BC) considered this disease under Nadivrana (Shalyaja) and advocated a minimally invasive para surgical treatment namely the Kshara Sutra procedure. Susrutha mentioned Kshara dravyas like Apamarga, Palasha, Vasa, Vibhithaki, etc used in the preparation of the Ksharasutra. Here we selected Vibhitaki to prepare Kshara and Ksharasutra. Vibhithiki is cost-effective as it is easily available and the properties of this plant are favourable to Kshara qualities. The present case study deals with the efficacy of Vibhitaki Ksharasutra in the management of Nadivrana (pilonidal sinus)

Bhagandara (Fistula-In-Ano) is one of the Astamahagadha disease mentioned in Susrutha Samhitha, which is difficult to cure. Fistula-In-Ano is an abnormal communication between two epithelial-lined surfaces in the anorectal region. Various operative procedures (fistulectomy, etc.) often lead to complication ns like recurrence, incontinence of the anal sphincter, and other infections resulting from surgery. The Kshara Sutra is a parasurgical procedure that is found to be more effective in the management of fistulas in ano. The Apamarga Ksharasutra is widely used and is made up of Apamarga Kshara, Snuhi Kshira and Haridra. The preparation process for Apamarga Kshara can be difficult and time-consuming. If we turn to the classics, we find the reference to Vibhithaki, mentioned under Kshara Dravys, which are used to prepare Ksharasutra. Vibhitaki is cost-effective and easily available as it is a big tree. The properties of Vibhitaki are favourable to Kshara qualities The present clinical study deals with the efficacy of Vibhitaki and Apamarga Kshara Sutras in the management of Bhagandara

Hearing is an important medium, which keeps one connected to other and with the surroundings. Impairment of hearing and its severity may vary from mild to moderate or profound. It can be unilateral or bilateral and conductive or sensorineural or mixed type. Acharya Sushruta has explained 28 types of Karna-roga in Uttar Tantra an Badhriya is one of them. It is caused by Vata dosha or Kaphaavritta vata dosha by Srotorodha (obstructing of channels) of Shabdavaha srotas (pathway of hearing) and Shabdavaha sira (auditory nerve) and further ignorance of this condition leads to Badhriya. This case report presents 18 years old girl with hearing impairment in the right ear from 3 years. Audiometry reports shows moderate to severe mixed HL in right ear and left ear is normal. Treatment was done with Marsha nasyam with Shadbindu oil and Karanpooran with Bilwadi oil (right ear) for 7days. Along with Sarivadi vati, Chandraprabha vati as internal medicine for 1 month

Standardization of Shodhit Shilajit is a major challenge. Objective: Objective of the present study was to assessing the effect of Shodhana purification process on Shilajit as well as assessing the difference in fingerprint of raw Shilajit and Shodhit Shilajit by HPTLC and physicochemical tests. Methods: The standardization parameters such as appearance, identification of active marker compound Gallic acid and Fingerprint comparison by HPTLC, Physicochemical test parameters like loss on drying, pH value, ash value, water soluble extractive, ethanol soluble extractive and assay of humic acid and fulvic acid were evaluated during the study. Chromatographic separation was achieved by using Toluene: Ethyl acetate: Formic acid (5.0:4.0:1.0 v/v/v) as mobile phase followed by comparing the Rf values. Results: The presence of active marker compound gallic acid in Shodhit Shilajit was confirmed by comparing the Rf value with standard. HPTLC fingerprint also confirmed that constituents from raw Shilajit transferring to the Shodhit Shilajit after the process of Shodhana. Total Ash value and Acid insoluble ash value reduced from 52.95% to 21.67% and 2.35% to 0.39% respectively while Water soluble extractive value increased from 59.33% to 72.13% after Shodhana process. There is also an increase in percentages of humic acid and fulvic acid from 7.10% to 8.23% and 36.91% to 50.12 % in Shodhit Shilajit. Conclusion: All these results showed the significance and effectiveness of Shodhana process in reducing the impurities and ultimately enhancing the therapeutic activity of Shodhit Shilajit. These evaluation parameters can be used for the standardization of Shodhit Shilajit and also can be routinely employed for analysis in Quality Control Lab
