Thigh phlegmon as a first sign of a ruptured pelvic presacral abscess caused by ileal diverticulum fistula: a case report /
Introduction Ileal diverticula usually remain asymptomatic. If complicated, they may present as intra-abdominal or pelvic abscess. Abscess formation in the presacral space is very rare. The rupture of abscess and spread of pus to extra-pelvic sites through anatomical structures of the pelvis is extremely rare. It carries high mortality if not diagnosed on time. Case presentation We report a case of 52-year-old woman presented with thigh phlegmon and septic condition. The CT scan revealed free air in the left leg and pelvic presacral fluid collection descending to extra-pelvic direction through the greater sciatic notch. Moreover, ileal fistula to presacral abscess was suspected. Multiple incisions and fasciotomies were urgently performed to treat thigh phlegmon. Subsequently, laparotomy was carried out and ileal fistula was excised. Histological examination of the surgical specimen demonstrated that the fistula to presacral abscess has formed due to perforated ileal diverticulum. 3 years after the surgery the patient remains healthy without recurrence. Discussion Rupture of presacral abscess to extra-pelvic site due to ileal diverticulum fistula is an extremely rare case, to our best knowledge, never reported in literature. Due to a rare occurrence and early septic complications if diagnosed late, this condition carries a high mortality rate. Conclusion Intrapelvic pathology must be considered in patients with thigh phlegmon in order to prevent complications and associated mortality
Anti-corruption education of patients in health care institutions.
The topic of the thesis is anti-corruption education of patients in health care institutions. The relevance of the topic is based on the fact that Lithuanian health care institutions are the most corrupt. Corruption in healthcare is very difficult to avoid, as patients are afraid to risk their health and expect that a “bribe” to a healthcare professional will improve the quality of provided services and speed up access to them. Thus, the aim of the work is to comprehensively analyze the possibilities of anti-corruption education of patients in health care institutions, theoretical and practical problems of education and to create a qualitatively new patient education program that meets today's requirements. To achieve this goal, four tasks have been set to summarize the concept, forms and causes of corruption, to analyze development of patients' anti-corruption awareness as an effective anti-corruption tool in health care institutions, to investigate existing raising of anti-corruption awareness in health care institutions and to present possibilities of new anti-corruption awareness in health care facilities. The object of the work is raising of the anti-corruption awareness of patients in health care institutions. The research was carried out using a questionnaire, the data of which were analyzed, compared and grouped. Summarizing the results of the research, it was found that the most common forms of corruption in health care institutions are facilitation and bribery, and the main causes of corruption are desire to receive qualitative services and desire to speed up the process of provided services. The whole health system suffers from corruption, and more than a half of the respondents experienced pressure from health professionals to give a “bribe”. The impact of COVID-19 pandemic on tourism industry in Lithuania: the case of tour operators.
The relevance of this Master thesis is that Covid-19 pandemic, which has been announced by WHO (World Health Organisation) on the 30th of March, 2020, has caused blockages in all economic sectors, but tourism is the most extremely affected. It is difficult to estimate the real impact of the pandemic on international tourism. According to UNWTO (United Nations World Tourism Organization), prior to the pandemic, tourism sector generated 10% of global GDP (Gross Domestic Product) and accounted for 1 in 10 jobs worldwide. Quarantine measures, flight cancellations, bans and border closures are expected to significantly reduce tourism for several years ahead. Every country in the world has been affected by the pandemics, including Lithuania. This article presents the effects caused by the Covid-19 pandemic on the tourism sector in Lithuania, particulary on tour operators and travel agencies. Tourism is a fast changing industry which depends on many external and internal factors, which have been analysed in the first part of this thesis. These are economical, political and social factors, although highly competitive market and fast changes during the pandemics obliged tour operators and travel agencies to look for new ways and better solutions to create value for their customers and to stay competitive in the market. The aim of this work is to look at the perspectives of tour operators in an outgoing tourism market in Lithuania through the analysis of tour operators and travellers and to suggest the possibilities for its further successful development. The research methodology consists of scientific literature, documentary and statistical data, regarding the effects of the coronavirus pandemic on the tourism sector, analysis, also qualitative research of 6 largest tour operators in Lithuania, quantitative research of travellers in Lithuania and the entification of both results. The results underline that Covid-19 pandemic has affected both tourism industry segments for more then 80%. Further along, business development theory and its strategy, strategic possibilities and competitiveness are being analysed, as well as business model and tourism concepts are being suggested. The results of both surveys are presented accordingly. At the end of this thesis conclusions and suggestions of business development strategies for tour operators are introduced
Mental health services during the pandemic: the experiences of Lithuanian psychologists /
With the COVID-19 pandemic and its restrictions, many countries face an unprecedented mental health crisis, which is being addressed in various ways, including the use of remote mental health services. Lithuania faced two quarantines: in March-June of 2020 and starting November 2020 up to Spring of 2021. The aim of this study is to explore the experiences of Lithuanian psychologists providing mental health services during the pandemic. Using the qualitative content analysis method, the following categories were made: from shock to discovery of new opportunities (differences in two lockdowns, better accessibility of services, help-seeking during the pandemic, and the importance of self-care) and contextual challenges (confidentiality, computer literacy, and blurred home/work boundaries). Implications for addressing psychological service issues are discussed, with an emphasis on self-care, setting boundaries, and finding new ways to enhance mental health via mediated communication as well as to reach out to vulnerable groups
Social dimensions of projected climate change impacts on ecosystem services in the coastal-rural area of Nemunas River reaches and Curonian Lagoon (Lithuania) /
The capacity of ecosystems to provide ecosystem services (hereinafter referred to as ES) depends on the state of their structure, processes, and functions, which is determined by interactions with other systems. These interactions are complex and take place in different climatic areas, and have different impacts on ecosystems and the use of ES. As the most sensitive part of the Lithuanian shore zone to climate change is the seaside zone or the coastal zone, which includes the Curonian Lagoon and the sea coast, the said area was therefore chosen for research. The case study examined those ES that were specific to the study area, without attempting to account for absolutely all ES. With an emphasis on the representation of different perspectives (in the process of assessing and preserving the potential of ES), the empirical study involved representatives of different (public and private) sectors. The public sector was represented by elders and eldership employees, the private sector by farmers and businesspeople. Respondents were selected purposefully to reflect the entire existing totality of the existing area. The evaluation of the obtained theoretical and practical results of the research identified the potential of the existing area ecosystem services and perspectives for the implementation of services by assessing their use according to respondents’ opinions, regional climate change, and national scale in EU environmental policy
Women's awareness of childbirth pain relief methods.
A woman who has acquired knowledge of ways to relieve labor pain has an established attitude about childbirth and accepts it asa physiological process. Training women can increase awareness and improve knowledge about labor pain and ways to alleviate it.The aim of the study–to reveal women's awareness of ways to alleviate labor pain.Research methods. Analysis of scientific literature; quantitative research; online survey.Results.The results of the study revealed women's awareness of ways to alleviate labor pain. The results of the study showed that the woman is interested in ways to relieve labor pain. Most subjects are aware of breathing exercises and epidural analgesia, but they also indicated that they would like to learn more about hydrotherapy, choosing the most appropriate posture, massage technique, opioid analgesia, spinal and inhalation analgesics, and the advantages, disadvantages, indications and contraindications of pain relief, efficiency, performance technique and duration of action.Conclusions.Many women use the Internet as their primary source of information on childbirth pain relief. A slightly smaller proportion of women are provided with information by a midwife and a general practice nurse. Such research results show that not all women receive the necessary information from health professionals, so they search for information on the Internet, and such information may not be medically correct and useful for a woman in all cases. Information obtained from websites can scare awoman and the pregnancy becomes tense, childbirth and future pain are taken to wait with fear and anxiety
The influence of applicant's and rater’s sex on decision making in hiring simulation.
Gender stereotypes have determined that the concepts of management and leadership are more associated with men than women. There are more men working in management positions than women in various countries, including Lithuania. The most widely discussed cause for that is discrimination against women in the labor market. The aim of the study was to examine evaluation differences between personnel specialists and comparison group, depending on their own and candidate‘s gender. Study was based on quasi-experimental strategy which included a hiring simulation. Participants had to evaluate potential candidates, a man and a woman, seeking for a job in management position. Data was collected from 128 people (age range - 19 to 56 years): 48 personnel specialists (M=29,38; SD=7,48), 49 women, who represented other specialties (M=26,29; SD=7,36) and 31 men, who represented other specialties (M=25,39; SD=5,05). Both personnel specialists and comparison group evaluated man and woman-candidate as similar. There were no significant differences between the two candidates on their hireability, reliability, competence, potential salary and promotability. However, results also indicate that personnel specialists and women, representing other specialties, suggested that man was more capable to work in teams than woman. The study gives an insight about the possibility of gender stereotype change
Health education of Klaipeda district rural community.
The aim of the study was to evaluate the health education of the X rural community. The methodology of the study. The research data were collected by in-depth interviews using the qualitative research method. During the interview, 15 respondent’s of Klaipėda district X rural community were interviewed. The study participants were selected according to the activity of the participation in health classes taking place in X rural communities. In total 15 respondent’s of Klaipėda district X rural community were interviewed. 6 out of them always attend all health classes. The other 6 respondent’s from the respondents did not attend all classes. The remaining 3 respondents did not attend any sessions at all. The results of the study. During the qualitative research it was found out that the health literacy of the residents of Klaipėda district X rural community is quite good. Residents have a concept, an attitude, but lack motivation for health education and practice it according to their understanding. This settlement needs a steady health education to provide residents with a deeper knowledge of how to improve their health and increase their motivation for a healthy lifestyle. The study revealed that the residents of rural community X are not sufficiently communal. This reduces the participation of the population in health education classes. Closer contact with the population, being a leader, presenting a wide range of health education topics and their continuity would activate the communication between X rural residents and more active participation in the offered activities
Educational activities performed by the Klaipėda Puppet Theatre.
Relevance of the topic: so many works have been written about Klaipeda Puppet Theater at Klaipeda University, but there is no research on performances and educational activities. That is why this topic is new and relevant. No one have written about performances and educational activities in quarantine. Work objective is to reveal the links between Klaipeda Puppet Theater and education. Research tasks: 1. To discuss the importance of puppet theater for the education of children (from the age of three months): a theoretical aspect. 2. To reveal the development and specifics of Klaipeda Puppet Theater, illustrating statements and performances. 3. To describe the trait and forms of Klaipeda Puppet Theater educational activities. 4. To investigate the activities of Klaipeda Puppet Theater during quarantine. Methods: the work uses methods of analysis and synthesis, observation (children‘s (in agreement with parents) videos and performances), comparative, interview and its analysis methods. Results and conclusions: research tasks completed. For performances, tree educations and two exibitions for children‘s are analyzed and according to the aspects of the theoretical part of the master‘s thesis. Klaipeda Puppet Theater professionally educates children, creatively and versatile develops children's intellect, physical abilities and spiritual sense of the world. Strengthens family ties, teaches and provides an opportunity to spend quality time together
