    Étude des représentations sociales du paysage : caractériser la place du paysage dans les processus d’aménagement à la Réunion

    In a context where social demand increasingly makes the environmental and aesthetic quality of the living conditions an essential element in Western societies, French and European public policies have shown a growing interest towards the concept of the landscape for some twenty years. The scope of the political action on landscape has been extended from the so-called remarkable spaces to the everyday environment. Moreover, the landscape appears in these policies from the angle of a project, which must be socially built with all the actors of the society, in order to organise collectively the land use planning and management. A large diversity of stakeholders, including planning professionals but also lay people, is called upon to act together in a political landscape action that reflects the aspiration of people. However, it implies that the concept is discussed in the public sphere and that the actors agree or at least discuss their representations of the landscape, that is to say beliefs and values assigned to the landscape, which determine behaviour of the stakeholders, and thus the political action on landscape. Research has shown the difficulties involved in this proliferation of actors and the gap between the regulatory objectives and operational reality. Thus, this research mobilises the theory of social representations as it is developed in the field of research in social psychology to question the social context through which the landscape can be the support of a shared, discussed and negotiated action.The thesis focuses on the context of the territory of la Réunion, characterised by a late political action on landscape and the recent emergence of urban policies and projects where the landscape seems to play a significant role in the design of a more sustainable urban planning. Some research works have shown that landscape policies or urban policies are not always in line with the vernacular landscape, imbued with the social and cultural history of Reunionese and the individual experiences of the inhabitants. Thus, the objective of our work is to understand the possible link between the “political landscape”, thought, shaped and managed by the professionals of the planning, and the landscape inhabited by the populations. It is a question of understanding whether and how the social representations of the landscape are shared, negotiated and collectively built by these actors. The methodological protocol is mainly qualitative and structured around semi-structured interviews and go-along methods. During the interviews, an exercise of free association task was given, inspired by the work in social psychology. This exercise highlights the organization of social representations, showing what is at the heart of the representations and the more peripheral and fluctuating elements of them-here. Based on these three methodological materials, we identify the cognitive (beliefs associated with the landscape), normative (values projected on the landscape) and sensitive (emotions and sensations felt by the landscape) aspects of the social representations of the landscape. We discuss from these results the place of the landscape in the public debate and the planning process, and the potential of a participatory landscape action on la Réunion, as it is driven in the public policies of the landscape.Dans un contexte où la demande sociale pointe de plus en plus la qualité environnementale et esthétique du cadre de vie comme un élément essentiel dans les sociétés occidentales, le concept de paysage connaît depuis une vingtaine d’années un intérêt grandissant dans les politiques publiques françaises et européennes. Des espaces dits remarquables au cadre de vie quotidien, les périmètres de l’action paysagère ont été étendus et le paysage apparaît davantage sous l’angle d’un projet, devant être construit socialement dans le débat et l’interaction de l’ensemble des acteurs de la société, pour en organiser collectivement la gestion et l’aménagement. Une multiplicité d’acteurs est ainsi convoquée à agir ensemble en faveur d’une action paysagère qui traduise les aspirations des populations. Or, cela suppose que le concept soit approprié dans la sphère publique et que les acteurs s’accordent ou du moins discutent de leurs représentations du paysage, c’est-à-dire de leurs opinions et des valeurs qu’ils attribuent au paysage, qui conditionnent leurs attitudes et, par-là, l’action paysagère. La littérature tend précisément à souligner les difficultés associées à ce foisonnement d’acteurs et le décalage entre les objectifs promulgués par la réglementation et la réalité opérationnelle. Cette recherche mobilise la théorie des représentations sociales telle qu’elle est développée dans le champ de la recherche en psychologie sociale, pour questionner le contexte social à travers lequel le paysage peut être le support d’une action partagée, discutée et négociée. La thèse s’intéresse au territoire réunionnais, marqué par une action paysagère (affichée) tardive, et l’émergence de projets urbains récents où le paysage semble jouer un rôle non négligeable dans la conception d’un urbanisme plus durable. Certains travaux de recherches ayant montré que des politiques paysagères ou urbaines n’étaient pas toujours en accord avec le paysage vernaculaire, imprégné de l’histoire sociale et culturelle des Réunionnais et du vécu individuel des habitants, la thèse interroge les médiations possibles entre le paysage pensé, façonné et géré par les professionnels de l’aménagement, et le paysage habité et vécu par les populations. Il s’agit de comprendre si et comment les représentations sociales du paysage sont partagées, négociées et collectivement construites par ces acteurs. Le protocole méthodologique utilisé est principalement qualitatif, et se structure autour d’entretiens semi-directifs et de parcours commentés. Dans le cadre des entretiens, un exercice d’association libre de mots, inspiré des travaux en psychologie sociale met en lumière l’organisation des représentations sociales, en montrant ce qui est au cœur des représentations et les éléments plus périphériques et fluctuants de celles-ci. Nous identifions à partir de ces trois outils méthodologiques les aspects cognitifs (les attributs associés au paysage), normatifs (les valeurs projetées sur le paysage) et sensibles (les sensations et émotions ressenties par le paysage) qui constituent les représentations sociales du paysage, et discutons à partir de ces résultats de la place que tient le paysage dans le débat public et dans les processus d’aménagement ainsi que des potentialités d’une action paysagère participative sur le territoire réunionnais telle qu’elle est impulsée dans les politiques publiques du paysage

    Review of the Situation of Human African Trypanosomiasis in the Republic of Congo From the 1950s to 2020

    International audienceAbstract Human African trypanosomiasis (HAT), despite considerable progress in the control, is still occurring in many countries in both west and central African regions. The HAT situation in the Republic of Congo has always been overshadowed by its neighbor the Democratic Republic of Congo where over 60% of all HAT cases occur. In the Republic of Congo, HAT cases have been significantly reduced to about 20 reported cases yearly and the disease is still prevalent in few foci across the country. Although continuous assessment of HAT situation in Congo is been led by the National Control Program for HAT, research on the vector, parasite, and vector control has received little attention. Because there have not been enough reviews summarizing key findings from studies conducted so far, there is still a poor understanding of the global situation of HAT in Congo. In order to achieve sustainable elimination of HAT in Congo a deep appraisal of HAT situation is required. The present study provides a review of studies conducted on HAT in the republic of Congo since the 1950s to date in order to identify gaps in knowledge and help consolidate the gains and progress towards the elimination of sleeping sickness

    Vécu de la maladie Covid-19 chez les patients infectés et hospitalisés obligatoirement à La Réunion lors de la première vague de l'épidémie en 2020

    Introduction: Covid-19 has caused the death of millions of people around the world. Unprecedented measures were decided in France in 2020, such as the establishment of a state of health emergency. In Reunion Island, during the first month of the first wave, any positive patient with at least one symptom was compulsorily hospitalized at the CHU-Nord. The main objective of the study was to explore the experience of patients infected with Covid-19 and hospitalized in Reunion in March 2020.Material and method: Qualitative study by semi-directed individual interviews in patients who have been infected with Covid-19 and hospitalized. A phenomenal-pragmatic approach was chosen.Results: Ten patients were interviewed. The hospitalization was at the origin of multiple emotions, fear is at the center of each one. Around her gravitate : shame, anger, fear of contaminating loved ones, or fear in the eyes of others, directly linked to stigmatization, and exclusion. The follow-up of our subjects over time has revealed many signs of PTSD at the origin of avoidance behavior and social isolation, well after the regulatory periods of isolation.Conclusion: Mental and physical suffering could be found in hospitalized patients, as well as signs of PTSD. They should be systematically sought out and taken care of.En 2020, l'épidémie Covid-19 mobilisait l'ensemble de la société et en particulier les soignants. Elle suscitait beaucoup d'interrogations. Les premières études étaient publiées et d'autres étaient en cours. En effet, l'épidémie, le confinement, les mesures barrières, l'organisation des soins, ainsi que le vécu et les conséquences de cette épidémie étaient importantes à explorer et à étudier. C'est pourquoi, à cette époque, le département universitaire de médecine générale (DUMG) a proposé différents sujets de thèse relatifs à la Covid-19 dont le vécu de la maladie chez les personnes positives à la Covid-19 à La Réunion. Ce sujet de thèse a donc été proposé par le DUMG de l'Océan Indien, dont fait partie le directeur de thèse, le Pr Line RIQUEL, et il a été choisi par l'auteur principal de la thèse, Alexis Cappelle, étudiant en médecine générale. Un second étudiant en médecine générale, Julia Fernandes Da Rocha, avait aussi choisi d'étudier le vécu de la maladie Covid-19 chez les patients infectés. Afin de différencier nos travaux de recherche et de les enrichir mutuellement, les deux chercheurs, avec l'aide de leur directeur de thèse, ont décidé d'explorer deux populations différentes. Alexis Cappelle avait choisi d'étudier le vécu de la maladie chez les patients hospitalisés, et Julia Fernandes Da Rocha, chez les patients pris en charge à domicile

    Background : Abortion rates are increasing each year in France, particularly in Reunion Island. The main factors are misuse, or lack of contraceptive use due to ignorance or fear of side effects. CeGIDDS centers aim to provide overall patient’s sexual healthcare Method : We did evaluate women’s knowledge on contraception at the North-East CeGIDD center and its university office and did propose contraception with follow-up to those who didn’t have any. A univariate analysis was performed on characteristics of women already on contraceptive use and those beginning use. A second analysis was performed in the « Contraception parcourse » group to evaluate the patient’s compliance to the follow-up and the contraception use according to the chosen contraceptive method.Results : Women already aware of birth control methods had usually been advised through their relatives, doctors (GP and specialist) or friends. No statistical difference was found regarding social economic status. Patient using oral contraception have a lower compliance and a lower follow-up rate. On the other hand, patients with good compliance have a higher rate of emergency contraceptive use. Conclusion : Medical counseling on contraception is essential in the overall women’s gynecologic care and must be structured. Given the increasing need of sexual healthcare follow-up in women’s population, healthcare providers must be trained to provide appropriate help. CeGIDD Nord-Est center aims to reinforce its gynecologic healthcare to deal with this public health problem.Introduction : L’IVG est en augmentation chaque année en France et notamment à la Réunion. L’absence de contraception liée à une mauvaise connaissance et la crainte d’effets secondaires ou son mauvais usage en est la principale raison. Les CeGIDDS ont pour mission la prise en charge globale de la santé sexuelle des patients. Méthode : Nous avons évalué les connaissances en contraception des usagères du CeGIDD Nord-Est et de son antenne universitaire, puis proposer une contraception associée à un suivi pour celles n’en ayant pas. Une analyse univariée a été réalisée sur les caractéristiques des patientes ayant une contraception initiale et les autres. Une seconde analyse a été réalisée dans le groupe “Parcours contraception” pour évaluer les différences entre patientes en fonction du taux d’observance des consultations de suivi et de la contraception prescrite.Résultats : L’information est davantage obtenue via les parents, les médecins spécialistes et traitant et le cercle amical pour les patientes ayant déjà recours à une contraception. Aucune différence n’a été mise en évidence concernant le statut professionnel et socio-économique des patientes. Pour les patientes suivies, les utilisatrices de la contraception œstro-progestative montrent une moins bonne observance du suivi et de leur contraception. Les patientes qui ont été observantes ont davantage recours à la contraception d’urgence. Conclusion : La consultation de contraception est une étape majeure du suivi gynécologique et doit être structurée. Les professionnels de santé doivent être aidés pour un entretien spécifique face à la demande croissante des patientes d’un suivi de leur santé sexuelle

    Incidence et caractéristiques de la néphropathie lupique sur l’île de La Réunion (2009-2018)

    Introduction: Lupus nephritis (LN) is a common and severe damage in the course of systemic lupus. The multi-ethnic population and the intensity of the UV radiation raise questions about the frequency and severityof this pathology in Reunion. Our main objective is to determine the average annual incidence of NL in Reunion Island over ten years. The clinical characteristics and the evolution were also studied.Methods: Our observational study included any patient, regardless of age and sex, with NL diagnosed on a native kidney biopsy performed between January 1, 2009 and December 31, 2018. The SLICC criteria also had to be fulfilled. Results: 76 new cases of NL were identified. The standardized incidence of LN in Reunion was 0.97/100,000 inhabitants per year (CI: 0.30-1.64/100,000 inhabitants). In Reunion, 53.9% of LN cases occur in the first year after lupus diagnosis. In the majority of cases, it is initially manifested by an active proliferative form. Compared to membranous LN, proliferative LN is significantly associated with higher complementconsumption and higher anti-native DNA antibody positivity. The average follow-up time of our cohort was 5.8 years. One-third of patients experienced a relapse during follow-up. Overall and renal survival at 5 years were97.4% and 97.2%, respectively. Conclusion: The incidence of NL in Reunion appears to be higher than in Caucasian cohorts without being able to affirm this statistically. It remains significantly lower than that found in the Afro-American population.Introduction : La néphropathie lupique (NL) est une atteinte fréquente et grave au cours du lupus systémique. La population multi-ethnique et l'intensité d'ensoleillement questionnent sur la fréquence et lagravité de cette atteinte à la Réunion. Notre objectif principal est de déterminer l'incidence annuelle moyenne de la NL réunionnaise sur dix ans. Les caractéristiques cliniques et évolutives ont également été étudiées.Méthode : Notre étude observationnelle incluait tout patient quel que soit l'âge et le sexe, atteint de NL diagnostiquée sur une biopsie de rein natif réalisée entre le 1er janvier 2009 et le 31 décembre 2018. Les critères SLICC de 2012 devaient également être remplis.Résultats : 76 cas incidents de NL ont été identifiés. L'incidence de la NL réunionnaise standardisée est de 0,97/100 000 habitants par an (IC : 0,30-1,64 /100 000 habitants). A la Réunion, 53,9% des cas de NL surviennent la première année après le diagnostic de lupus. Dans la majorité des cas, elle se manifeste initialement par une forme proliférative active. En comparaison avec les formes extra-membraneuses, les formes prolifératives sont associées significativement à une plus grande consommation du complément et à une positivité plus importante des anticorps anti-ADN natifs. Un tiers des patients ont présenté une rechute au cours du suivi. Les survies globale et rénale à 5 ans sont respectivement de 97,4% et 97,2%.Conclusion : L'incidence de la NL réunionnaise semble ssemble supérieure à celle dans les cohortes caucasiennes sans pouvoir l'affirmer statistiquement. Elle reste nettement inférieure à celle retrouvée en population afro-américaine

    Dimensioning of Fixed Frequency Patch Antennas Based on Neural Networks

    International audienceDuring the last decades the race for innovation in communication systems has not ceased to evolve, which has led to important studies in the field of antennas, which today have very different forms depending on the applications such as: mobile telecommunications, television, radio, satellites, communicating systems, radar, remote sensing, radio astronomy [1]. Among the most used antenna families are the microstrip antennas (also called printed antennas or patch antennas) [2]. These antennas are characterized by: low manufacturing cost, mass production possible, linear and circular polarization, feed and matching networks manufactured simultaneously with the antenna. The limitations of conventional neural modeling have been overcome by the introduction of neural networks based on electromagnetic knowledge. A neural network using effective parameters in conjunction with the GALERKIN function has been developed for modeling the resonant frequency of a rectangular antenna printed on a substrate [3]. Thus, we set up a rectangular PATCH antenna model on HFSS for a resonant frequency close to 6GHZ and extract the parameters S 11 , Z 11 , and VSWR, and also check the training errors on MATLAB by generating the error types on a 4-input model (Thickness, Length, Width and substrate type) and one (1) output (Resonant Frequency) that will allow to extract the final dimensions of the antennas thanks to this neural model
