Beanspruchungsmessung am Arbeitsplatz - Erfassung physiologischer Korrelate mentaler Beanspruchung On-The-Job
Die zunehmende Digitalisierung und Technologisierung sorgt branchenübergreifend für eine Verlagerung der subjektiv erlebten Beanspruchung von physischer hin zu mentaler Beanspruchung. Um Arbeitsabläufe hinsichtlich auftretender Schwankungen mentaler Beanspruchung optimierbar zu machen, muss diese in Echtzeit am Arbeitsplatz erfassbar sein. Die Verwendung physiologischer Messinstrumente wie Elektrokardiogramm, Eye Tracking und Hautleitfähigkeit bieten dabei eine Möglichkeit der objektiven Quantifizierung der auftretenden Schwankungen. Im Rahmen verschiedener Feld- und Laborstudien konnte gezeigt werden, dass, in Abhängigkeit der Analyseeinheit (gesamter Prozess oder einzelne beanspruchungsinduzierende Events), unterschiedliche physiologische Parameter in der Lage sind Veränderungen der mentalen Beanspruchung nachzuweisen. Insgesamt erwiesen sich dabei die Herzfrequenz sowie die Ausdehnung der Pupille als sensitivste Indikatoren. Für eine live Erfassung im Arbeitsprozess bedarf es zukünftig, neben der Weiterentwicklung von tragbarer Messmethodik (Wearables), eine Entwicklung neuer Algorithmen zur Kombination verschiedener Parameter zu einem allgemeinen Indikator für mentale Beanspruchung, sowie der Bearbeitung einiger theoretischer Probleme, wie unter anderem der Definition von Grenzwerten der mentalen Beanspruchung. Um abseits bestehender Probleme Veränderungen der Beanspruchung am konkreten Arbeitsplatz einschätzbar zu machen, wurde eine alternative Auswertungsstrategie basierend auf kurzfristigen Peaks und längerfristigen Plateaus vorgeschlagen
The medieval roots of antisemitism in Sweden: Old Swedish and Latin manuscript traditions
The lack of a local Jewish community did not prevent medieval Swedish clerics and lay people from being interested in Jews and Jewish questions. They bought, translated, read and preached from most of the available textual sources and thus spread the widely available views of the hermeneutical Jew: a cruel, stubborn and ugly person and at the same time a cipher for the entire Jewish people both in biblical times and today. This article gives an overview of the Latin and vernacular manuscripts with anti-Jewish motifs and texts and shows that the main and most common textual models and motifs were available in Swedish libraries and collections, from legends via apocryphal texts to fake disputations – adding up to a relatively complete ‘hermeneutical Jew’. A focus was, as in the rest of Europe, on Passion-related piety, which was the most common form of piety in the late Middle Ages – and usually connected with distinct anti-Jewish features. The fact that we can establish direct and indirect textual and narrative lines of tradition between the medieval codices and modern printed booklets of the nineteenth century proves the long-lasting intelligibility of anti-Jewish stereotypes in Sweden – developed and spread completely independently from the Jewish minority. The medieval perspective thus adds a much-needed nuance to the debate about antisemitism in the North: it did not need any actual Jews; it simply made up its own, based on the general Christian tradition
Effects of body mass index on the immune response within the first days after major stroke in humans
Introduction
Immunological alterations associated with increased susceptibility to infection are an essential aspect of stroke pathophysiology. Several immunological functions of adipose tissue are altered by obesity and are accompanied by chronic immune activation. The purpose of this study was to examine immune function (monocytes, granulocytes, cytokines) as a function of body mass index (BMI: 1st group: 25; 2nd group: 25 BMI 30; 3rd group: 30) and changes in body weight post stroke.
Method
Fat status was assessed using standardized weight measurements on days 1, 2, 3, 4, 5, and 7 after ischemic stroke in a cohort of 40 stroke patients and 16 control patients. Liver fat and visceral fat were assessed by MRI on day 1 or 2 [I] and on day 5 or 7 [II]. Leukocyte subpopulations in peripheral blood, cytokines, chemokines, and adipokine concentrations in sera were quantified. In a second cohort (stroke and control group, n = 17), multiple regression analysis was used to identify correlations between BMI and monocyte and granulocyte subpopulations.
Results
Weight and fat loss occurred from the day of admission to day 1 after stroke without further reduction in the postischemic course. No significant changes in liver or visceral fat were observed between MRI I and MRI II. BMI was inversely associated with IL-6 levels, while proinflammatory cytokines such as eotaxin, IFN-β, IFN -γ and TNF-α were upregulated when BMI increased. The numbers of anti-inflammatory CD14+CD16+ monocytes and CD16+CD62L− granulocytes were reduced in patients with higher BMI values, while that of proinflammatory CD16dimCD62L+ granulocytes was increased.
Conclusion
A small weight loss in stroke patients was detectable. The data demonstrate a positive correlation between BMI and a proinflammatory poststroke immune response. This provides a potential link to how obesity may affect the clinical outcome of stroke patients
Welchen Beitrag leisten neue ambulante Versorgungsmodelle im Rahmen der Covid-19-Pandemie in Vorpommern-Greifswald - eine Mixed Methods Studie -
Hintergrund
Zu Beginn des Covid-19-Pandemiegeschehens wurde die ambulante Patientenversorgung deutschlandweit vor große Herausforderungen gestellt. Insbesondere die mangelnde Verfügbarkeit von Schutzausrüstung und eine vermutete Überforderung des ambulanten und stationären Sektors machten die Entwicklung alternativer Versorgungsmodelle notwendig.
Auf Aufforderung der Landesregierung wurde von der Universitätsmedizin Greifswald eine sogenannte Fieberambulanz für Patienten mit möglichen Symptomen einer Covid-19-Infektion eingerichtet, die nicht telefonisch abgeklärt werden konnten.
Methoden
Es handelt sich um eine Mixed-Methods-Studie.
Im quantitativen Teil der Studie erfolgten die Auswertung des Konsultationsanlasses, der Symptome sowie Beratungsergebnisse mit einem selbst entwickelten Fragebogen. Einnahmen und Ausgaben der Fieberambulanz wurden erfasst.
Retrospektiv wurden leitfadengestützte Experteninterviews mit den hauptverantwortlichen Mitarbeitern der Fieberambulanz Greifswald, sowie niedergelassenen Ärzten und Vertretern aus den Bereichen Hygiene, sowie der Kassenärztlichen Vereinigung MV und dem Landkreis Vorpommern-Greifswald geführt.
Ergebnisse
Es gelang innerhalb von zehn Tagen, die Fieberambulanz in den Räumlichkeiten einer Berufsschule aufzubauen. Besondere Herausforderungen, die in den Interviews beschrieben wurden, waren die Organisation von Logistik und Personal, Klärung der Verantwortlichkeiten, Kostenträgerschaft und Abrechnung. Es wurden Grenzen und Schwächen der Fieberambulanz aufgezeigt, sowie alternative Versorgungsmodelle in den Interviews diskutiert.
Im Zeitraum vom 08.04.-11.05.2020 wurden insgesamt 107 Konsultationen durchgeführt (53 % weiblich, Ø42 Jahre), bei deutlich mehr telefonischen Anfragen. Die häufigsten Diagnosen waren akute Bronchitis (43%), obere Atemwegsinfekte (28%) und Fieber (8,4%).
In 42 Fällen wurde ein Nasen-Rachen-Abstrich durchgeführt. Schwerere Erkrankungen wie eitrige Tonsillitiden oder Asthma Exazerbationen wurden in <10% der Fälle diagnostiziert. Covid-19-Neuinfektionen wurden nicht nachgewiesen. Eine kosteneffiziente Auslastung konnte nicht erreicht werden.
Schlussfolgerung
Die Fieberambulanz wurde zu Pandemiebeginn als Anlaufstelle für Unsicherheiten im Umgang mit dem Infektionsgeschehen genutzt, als viele Versorgungssituationen noch ungeregelt waren. Sie erwies sich aber rückblickend bei niedriger Inzidenz als unwirtschaftlich und nicht notwendig. Der Großteil der Patienten konnte weiterhin hausärztlich betreut werden
Mass spectrometry-based Proteome analysis of porcine cells infected with African swine fever virus
ASFV, a highly contagious, pathogenic and lethal pathogen of swine, poses a major threat to domestic and wild suids worldwide as neither vaccines nor treatments are available. Compared to other well-characterized similarly complex viruses like herpesviruses or adenoviruses, the understanding of ASFV biology is poor.
To improve the understanding of ASFV biology, following the establishment of a robust protocol for the isolation of primary monocyte-derived porcine macrophages (moMΦ) and their infection with ASFV for mass spectrometry (MS)-based proteome analysis was performed.
Under both conditions, naïve and infected, the isolated cells showed cell type-specific characteristics like phagocytosis and antigen presentation and protein expression patterns, including the expression of swine leucocyte antigens and CD markers. Furthermore, moMΦ could be reproducibly infected with ASFV isolates of different genotypes and pathogenicity.
The ASFV protein expression patterns in moMΦ correlate well with those observed in established cell lines at transcript and protein level. The expression of 27 ASFV proteins was confirmed at the protein level. Among them, 9 members of multi-gene families (MGF) and 12 novel open reading frames (nORFs) were recently predicted based on transcription start site mapping.
The direct comparison of closely related ASFV genotype II isolates revealed no virulence-associated protein expression patterns beyond those expected based on the genome sequences of the isolates.
Using different MS quantification strategies, it was shown that ASFV affects both static protein expression levels and protein synthesis. These changes in protein expression impact proteins and pathways known to be targeted by ASFV, including CD-markers, ER-stress and cell death pathways, and cellular antiviral responses. Beyond these observations that further validated the moMΦ infection model, novel effects of the ASFV infection on the cellular proteome were noticed.
These effects include the decreased expression levels of cathepsins, especially cathepsins D (CTSD), H (CTSH) and L (CTSL) as well as the transient activation of MAPK14/p38 prior to its strong downregulation. In addition to MAPK14/p38 further members of the MAPK14/p38 signaling pathway, like MAPKAPK2, were affected by ASFV infection.
As these modulations of the cellular proteome would in general result in decreased pro-inflammatory responses, it did stand out that the synthesis of interferon-response related genes including MX1 and ISG15 evaded the ASFV-induced global reduction of protein synthesis. In contrast, the synthesis of genes involved in RNA processing and splicing was significantly impaired. In total, the regulations of individual host proteins assessed in the context of the whole cellular proteome integrate well with each other and other cellular responses to ASFV infection and may help to improve the understanding of host-virus interactions.
Overall, this thesis provides novel insights into the expression of ASFV-encoded ORFs of different isolates and the host response to ASFV infection. It points out that the current knowledge of the ASFV coding capacity, temporal protein expression patterns, protein functionality, post-translational modifications and host interactions is still sketchy as many aspects of ASFV replication have yet to be understood. The established moMΦ-model to study ASFV infections in vitro provides a powerful tool for future applications to increase the understanding of ASFV biology
Komplexe Zellkulturmodelle und CRISPR/Cas9-basierte Nanopore-Sequenzierung in der Forschung zu zerebralen kavernösen Malformationen
Bei der familiären Form zerebraler kavernöser Malformationen (CCMs) handelt es sich um eine autosomal-dominant erbliche Erkrankung, die durch pathogene Keimbahnvarianten in den Genen CCM1/KRIT1, CCM2 bzw. CCM3/PDCD10 ausgelöst wird und mit einer Prävalenz von 1:3300 bis 1:3800 in der Allgemeinbevölkerung vorkommt. Neben Punktmutationen und kleinen Indels werden auch Strukturvarianten (SVs) in den Krankheitsgenen beobachtet. Diese sind jedoch mithilfe von Short-Read-basierten Genpanel- oder Exomsequenzierungen in der Routinediagnostik zum Teil nur schwer zu identifizieren.
Im Rahmen der vorliegenden Arbeit wurde daher eine neue CRISPR/Cas9-basierte Nanopore-Sequenzierung der Gene CCM1, CCM2 und CCM3 sowie der flankierenden genomischen Regionen etabliert. Die Abdeckung der Zielregionen mit Reads länger als 20 kb erlaubte die zuverlässige Detektion von Kopienzahlveränderungen. Darüber hinaus konnte anhand einer interchromosomalen Insertion und einer 24 kb großen Inversion gezeigt werden, dass die Methode auch komplexere SVs nachweisen kann. Das Protokoll einer Cas9-basierten Sequenzierung ausgewählter genomischer Zielregionen auf der MinION-Plattform kann gut auf andere Fragestellungen übertragen werden und lässt sich mit vergleichsweise geringem Aufwand in vielen Laboren etablieren.
Ein zweiter Schwerpunkt der Promotionsarbeit lag auf der Entwicklung neuer, vielseitig einsetzbarer Zellkulturmodelle der CCM-Erkrankung. Mithilfe CRISPR/Cas9-vermittelter Genomeditierungen in fluoreszenzmarkierten humanen induzierten pluripotenten Stammzellen (hiPSCs) konnten komplexe Kokulturmodelle sowie dreidimensionale vaskuläre Organoidkulturen etabliert werden. Anhand von aus CCM3-defizienten hiPSCs differenzierten humanen Endothelzellen (ECs) und vaskulären Mosaikorganoiden ließ sich ein klonaler Überlebensvorteil von CCM3-/--Zellen in Kokultur mit Wildtyp-Zellen nachweisen, welcher kürzlich auch in CCM-Mausmodellen als Schlüsselelement der CCM-Pathogenese beschrieben wurde. In den neuen Zellkulturmodellen ließ sich auch der positive Effekt des in der Greifswalder Arbeitsgruppe identifizierten Kandidatenwirkstoffs NSC59984 auf die tumorähnliche Proliferation von CCM3-/--Zellen in Kokultur validieren. Durch systematische RNA-Sequenzierungen konnte zudem nachgewiesen werden, dass CCM1 für die Differenzierung von ECs aus hiPSCs nicht erforderlich ist, jedoch die physiologische Funktion von ECs aufrechterhält.
Die Ergebnisse der vorliegenden Arbeiten belegen nicht nur die Praktikabilität und Sicherheit der Identifizierung komplexer genomischer Strukturvarianten mittels CRISPR/Cas9-vermittelter Nanopore-Sequenzierungen, sondern sie ebnen zudem den Weg zur effektiven Testung neuer CCM-Therapieansätze im Hochdurchsatzverfahren und erlauben einen neuen Einblick in die noch immer unvollständig verstandene CCM-Pathogenese
Untersuchung der Rolle regulatorischer myeloider Zellen im Rahmen einer Immuntherapie und der Kombination mit dem chimären Anti-GD2-Antikörper ch14.18/CHO beim Neuroblastom
Das Neuroblastom (NB) stellt als Tumor des sympathischen Nervenstranges den häufigsten extrakraniellen soliden Tumor des Kindesalters dar. Ein Bedarf innovativer Therapien, welche eine effektive Anti-Tumorantwort induzieren, ergibt sich aus dessen heterogener Charakteristik, höchst aggressivem Wachstum und trotz Therapien hohen Rezidivraten der high-risk-Patient*innen. In den letzten Jahren konnten durch zunehmende Erkenntnisse im Bereich der Tumor- immunologie verschiedene immuntherapeutische Verfahren etabliert werden. So stellt die Anti-GD2-Therapie (ch14.18/CHO) einen zentralen klinischen Pfeiler der multimodalen Therapie des high-risk-NB dar. Hierbei werden Anti-GD2-AK verab- reicht, die das NB-spezifische Antigen GD2 erkennen, was zur gezielten Aktivierung des körpereigenen Immunsystems hauptsächlich durch NK-Zellen mit anschlie- ßender Tumorzelllyse führt. Herausforderungen ergeben sich aus der Überwindung der immuninhibitorischen Tumorumgebung sowie der Reduzierung natürlicher Regulationsmechanismen.
Vorherige Arbeiten der Arbeitsgruppe haben eine Anti-GD2-Immuntherapie abhängige Induktion des Immuncheckpoints PD-1/PD-L1 sowie der regulatorischen myeloiden CD11b+-Zellen (MRC) gezeigt. Für die vorliegende Arbeit war nun die Rolle der CD11b+ MRC von besonderem Interesse. Hierfür wurden CD11b+ Zellen in einem syngenen NB-Mausmodell mittels Anti-CD11b-AK blockiert oder durch 5-FU selektiv depletiert, was zu einem deutlich reduzierten Tumorwachstum und verbessertem Überleben führte. Zusätzlich wurde mittels RT-PCR die relative Gen- expression MRC-modulierender Gene (Arg1, CCL2, GM-CSF, IDO, IFNγ, IL-1β, IL-4, IL-6, IL-6R, IL-8, IL-10, M-CSF, M-CSFR, iNOS, TGF-β1 und VEGF-A) in der Tumormikroumgebung sowie der Milz evaluiert. Hier wurde eine Reduktion der modulierenden Gene nicht nur in der Tumormikroumgebung, sondern sogar peri- pher durch die Depletion von MRC nachgewiesen. Somit konnte eine negative Rolle der MRC beim NB bestätigt werden.
Diese Ergebnisse stellten eine Grundlage für die kombinierte Immuntherapie aus Anti-GD2-AK und MRC-Depletion dar. Eine Steigerung der antitumoralen Wirksam- keit im Vergleich zu den entsprechenden Monotherapien wurde gezeigt. Dabei wurden eine verbesserte Überlebenswahrscheinlichkeit und ein reduziertes Tumor- wachstum festgestellt. Die vorliegenden Ergebnisse liefern daher die Grundlage für einen neuartigen kombinierten Therapieansatz, um die immer noch schlechten Überlebenschancen der high-risk-Patient*innen zu verbessern.Neuroblastoma (NB) is a tumor derived of the sympathetic nerve cord and the most common extracranial solid tumor of childhood. A need for new, innovative therapies that induce an effective anti-tumor response arises from its heterogeneous characteristics, highly aggressive growth, and high recurrence rates in high-risk patients despite multimodal therapies. In recent years, increasing knowledge in the field of tumor immunology has led to the establishment of various immuno- therapeutic methods. Challenges arise from the identification of a tumor-specific antigen, overcoming the immunoinhibitory tumor environment, and reducing natural regulatory mechanisms. The anti-GD2 therapy (ch14.18/CHO) represents a central clinical pillar of multimodal therapy for high-risk NB. Here, tumor cells highly expressing GD2 are recognized and eliminated by anti-GD2-dependent activation of the immune system.
The previous findings of our group showed that anti-GD2 immunotherapy led to the induction of the immune checkpoint PD-1/PD-L1 as well as regulatory CD11b+ cells, we investigated whether blockade of these regulatory mechanisms leads to an improvement of the antitumor immune response mediated by anti-GD2 antibodies. This hypothesis was previously confirmed for anti-PD-1 immunotherapy. In this project, we investigated the role of CD11b+ myeloid regulatory cells (MRC). First, CD11b+ cells were blocked in a syngeneic NB mouse model by an anti-CD11b antibody or selectively depleted by 5-FU. Significantly reduced tumor growth and improved survival was observed for both therapy groups. In addition, RT-PCR analysis was performed to evaluate the expression of MRC-modulating genes (Arg1, CCL2, GM-CSF, IDO, IFNγ, IL-1β, IL-4, IL-6, IL-6R, IL-8, IL-10, M-CSF, M-CSFR, iNOS, TGF-β1 und VEGF-A) in the tumor microenvironment and spleen. Here, a reduction of these modulating genes was not only demonstrated in the tumor microenvironment but also peripherally through the depletion of myeloid regulatory cells. These results provided a rationale to combine the anti-GD2 immunotherapy with an MRC blockade to further improve the antitumor response against NB.
As hypothesized, the combination of immunotherapy with ch14.18/CHO and 5-FU used for MRC depletion improved survival and reduced tumor growth compared to the respective single-agent treatment even further. Thus, these results may provide a novel therapeutic approach to effectively reduce NB growth and to improve survival of high-risk NB patients
Epileptic seizures at multiple sclerosis onset and their role in disease progression
Background:
Epileptic seizures can occur throughout the course of multiple sclerosis (MS) and are associated with increasing disability progression over time. However, there are no data on whether epileptic seizures at the onset of MS also lead to increasing disability.
Objective:
To examine disease progression over time for MS patients with epileptic seizures at onset.
Methods:
We analyzed the data of 30,713 patients on the German Multiple Sclerosis Register in a case–control study for more than 15 years. MS patients with seizures at onset were further divided into subgroups with polysymptomatic and monosymptomatic onset to assess the impact of additional symptoms on disease progression.
Results:
A total of 46 patients had seizures as onset symptoms. Expanded Disability Status Scale (EDSS) within the first year was lower in the group with seizures at onset compared to controls (0.75 versus 1.6, p < 0.05), which changed until the last reported visit (3.11 versus 3.0). Both subgroups revealed increased EDSS progression over time compared to controls.
Conclusion:
Epileptic seizures at MS onset are associated with a higher amount of disability progression over time. Additional longitudinal data are needed to further clarify the impact of seizures on the pathophysiology of MS disease progression
The effects of causal and self-efficacy beliefs on help-seeking for people with depressive complaints: a quasi-experimental online study
Background: Only approximately a third of people with depressive symptoms seek professional health care. Furthermore, people labelled as mentally ill may experience stigmatisation, which can impede help-seeking behaviour.
Aim: To examine the effects of three vignette-based interventions endorsing biopsychosocial causal beliefs and strengthening self-efficacy on help-seeking intention and behaviour, as well as the predictive values of these variables and previous treatment experience.
Method: A quasi-experimental online study utilising a fractioned factorial design was carried out. People were screened for depressive symptoms and their current treatment status. After baseline assessment, they were randomly allocated into one of 24 groups receiving a combination of interventional messages. Actual help-seeking behaviour was measured at follow-ups 3 and 6 months after baseline.
Results: Altogether, N = 1,368 participants were included in the final analyses and N = 983 provided data on their help-seeking behaviour within 3 to 6 months after the baseline assessment. The intention to seek help from a general practitioner or a mental health professional was significantly influenced by the interventions. However, help-seeking behaviour was not influenced by the interventions. On a conceptual level, biopsychosocial causal beliefs (β = 0.09–0.23) and self-efficacy to seek help (β = 0.16–0.25) predicted help-seeking intention. There was a negative interaction effect of both self-efficacy beliefs on intention and behaviour, which changed depending on depression severity. In all models, the intention was the main predictor of actual behaviour. Treatment experience predicted both help-seeking intention and behaviour.
Conclusion: Biopsychosocial causal beliefs and self-efficacy have a direct effect on help-seeking intention. Interventions should include information on how to actually seek help as a means to strengthen self-efficacy beliefs and simulate previous treatment experience. Further research is needed to investigate the respective interaction effects on intention and behaviour.

Determination of Gastric Water Emptying in Fasted and Fed State Conditions Using a Compression-Coated Tablet and Salivary Caffeine Kinetics
Because of the importance of gastric emptying for pharmacokinetics, numerous methods have been developed for its determination. One of the methods is the salivary tracer technique, which utilizes an ice capsule containing caffeine as a salivary tracer. Despite the ice capsule’s advantage in labeling ingested fluids with caffeine for subsequent salivary detection, its risk of premature melting before swallowing, and its complicated storage and preparation, limit its application, particularly in special populations (e.g., older people). For this reason, here, a compression-coated tablet was developed and validated against the ice capsule in a cross-over clinical trial. The two dosage forms were administered simultaneously to 12 volunteers in an upright position under fasted and fed state conditions. To distinguish the caffeine concentrations in saliva from each dosage form, regular type of caffeine (12C) was added to the tablet, while for the ice capsule 13C3 labelled caffeine was used. The salivary caffeine concentrations showed no statistically significant differences for the pharmacokinetic parameters tmax and AUC0→60 (p > 0.05). Thus, the new formulation is a useful tool for determining gastric emptying that can also be used in special populations
