Body composition markers are associated with changes in inflammatory markers but not vice versa: A bi-directional longitudinal analysis in a population-based sample
Background and aim
Growing body of evidence consistently link obesity and inflammation, Although the direction of the association is still unclear. We aimed to investigate longitudinal associations of body anthropometric, composition and fat distribution parameters with inflammatory markers and vice versa.
Method and results
We used data from 2464 individuals of the SHIP-TREND cohort with a median follow-up of 7 years. Linear regression models adjusted for confounders were used to analyze associations of standardized body composition markers derived from classic anthropometry, bioelectrical impedance analysis (BIA) and magnetic resonance imaging (MRI) at baseline with changes in inflammatory markers (C-reactive protein (CRP), white blood cell (WBC), fibrinogen) and vice versa. Higher level of anthropometric markers at baseline were associated with an increase in the change of inflammatory markers. A 13.5 cm higher waist circumference (WC), 16.0 kg body weight and 7.76 % relative fat mass (FM) at baseline was associated with a change in CRP of 0.52 mg/L (95 % confidence interval [CI]: 0.29 to 0.74), 0.51 mg/L (95 % CI: 0.29; 0.74) and 0.58 mg/L (95 % CI: 0.34; 0.82) respectively. Absolute FM showed the strongest association with changes in serum fibrinogen levels (β for 8.69 kg higher FM: 0.07 g/L; 95 % CI: 0.05; 0.09). Baseline inflammatory markers were only associated with changes in hip circumference.
Conclusion
Our study indicates the importance of anthropometric, body composition and fat distribution markers as a risk factor for the development of inflammation. To prevent inflammatory-related complications, important is to take measures against the development of obesity
A case study to investigate the influence of extrusion temperature, 3D printing parameters and the use of antioxidants on the degradation of dexamethasone
The use of 3D printing techniques such as fused deposition modelling (FDM) for the manufacturing of dosage forms has become an interesting research field in pharmaceutics. When using FDM 3D printing, there are usually two temperature events that can have influence on temperature-sensitive drugs and excipients. The aim of this work was to investigate the influence of various parameters on the stability of the active ingredient dexamethasone. For this purpose, active ingredient-containing filaments were produced by hot melt extrusion and the influence of temperature and the presence of antioxidants on drug stability were investigated. Vitamin C, vitamin E and butylated hydroxytoluene were used as antioxidants. If suitable filaments were obtained, these filaments were used to 3D print test objects. In addition to the presence of antioxidants, the influence of printing parameters such as printing temperature, printing speed and print bed temperature on the active ingredient content was investigated. The variation of the temperature during the extrusion process showed an influence on the degradation of dexamethasone. Furthermore, during the investigation of the 3D printed objects, the presence of the antioxidants did not show any positive influence on the drug stability. The printing temperature was identified as a critical process parameter during 3D printing of dexamethasone, while other printing settings like the temperature of the print bed and the printing speed had no detectable influence
Charakterisierung der Epithelial-mesenchymalen Transition als Resistenzmechanismus und potenzielle Therapieoptionen beim EGFR-Tyrosinkinase-Inhibitor-resistenten nicht-kleinzelligen Bronchialkarzinom
Die Behandlung des NSCLCs mit aktivierenden Mutationen des EGFRs hat sich durch zahlreiche Therapieinnovationen der letzten Jahre erheblich verbessert. Doch auch vom aktuellen Therapiestandard Osimertinib profitieren Patienten aufgrund von Resistenzentwicklungen, deren genauer Mechanismus häufig nicht bekannt ist, nur für eine begrenzte Zeit. Die EMT als generationsübergreifender Resistenzmechanismus gewinnt mit der Entwicklung neuer EGFR-TKIs zusätzlich an Relevanz und hat zudem aufgrund ihres Einflusses auf die Metastasierung von Tumorzellen eine herausragende Bedeutung. Die Identifizierung zentraler Faktoren der Resistenz-induzierten EMT stellt eine Grundvoraussetzung für die Entwicklung neuer Diagnose- und Therapiemöglichkeiten dar.
Ziel dieser Arbeit war es daher, die Resistenz-induzierte EMT in mehreren HCC4006-Resistenzmodellen zu untersuchen, die Entwicklung der EMT unter Osimertinib zu charakterisieren und potenzielle Therapieoptionen zu prüfen.
Durch die Charakterisierung der resistenten Zelllinien konnten zunächst zentrale EMT-Marker, -TFs sowie -regulierende miRNAs identifiziert werden. Zudem wurde durch die Sequenzierung vesikulärer miRNA ZEB1 als primäres EMT-regulierendes miRNA-Target bestimmt. Die Entwicklung der Resistenz-induzierten EMT wurde durch die Analyse relevanter Marker, TFs sowie miRNAs bei stufenweiser Adaptation mit Osimertinib untersucht. Weiterhin konnte gezeigt werden, dass eine temporäre Adaptationspause zu einer Reversion der EMT, insbesondere auf der regulatorischen Ebene der EMT-assoziierten miRNAs, führte. Als potenzielle Therapieoptionen wurden Catenin-beta 1- sowie AXL-Inhibitoren als potenzielle EMT-Initiatoren untersucht. Des Weiteren wurde der Einfluss des CIP2A- und Proteasom-Inhibitors Bortezomib auf die Erlotinib-resistenten HCC-Zellen analysiert
Versorgungsrealität der kernspintomographischen Bildgebung und Befundung von Rotatorenmanschettenläsionen im kleinstädtischen Raum Mecklenburg-Vorpommerns
Die Magnetresonanztomographie spielt neben der klinischen Untersuchung eine wesentliche Rolle bei der Diagnostik von Rotatorenmanschettenläsionen. Im Gegensatz zur üblichen standardisierten Bildgebung und Befundung in Studien wird im klinischen Alltag die Diagnostik und Befundung durch eine Vielzahl von radiologischen Praxen erhoben. In der retrospektiven Studie wurden die externen MRT-Befunde von 146 Patienten, die sich eine primäre Rotatorenmanschettenrekonstruktion unterzogen, auf Qualität und Vollständigkeit untersucht und mit den präoperativen standardisierten Befunden, erhoben durch einen orthopädischen Schulterspezialisten, verglichen.
Die Auswertung der Daten ergab, dass die radiologische Befundung im kleinstädtischen Raum in Aspekten wie Erkennung von Supra- und Infraspinatussehnenläsionen gute Ergebnisse erzielen (Sensitivität 94 % und 70 %; Spezifität von Infraspinatusläsionen 91 %). Im Gegensatz dazu ist die Diagnostik der Subscapularissehnenläsion im Vergleich zur Literatur unzureichend (Sensitivität 52 %, Spezifität 88%). Eine Spezialisierung im Bereich muskuloskelettaler Bildgebung verbessert die allgemeine Befunderhebung, vor allem bei Subscapularisläsionen (Sensitivität 68 % im Gegensatz zu 52 %).
Weitere deutliche Defizite zeigen sich in der Befundung von präoperativen Prognosefaktoren wie Retraktion, Atrophie und fettige Degeneration. Obwohl die radiologische wie auch orthopädische Literatur eine Mitbeurteilung und Klassifikationseinteilung empfiehlt, wird dies bisher selten umgesetzt. Die Retraktion wurden in 63 %, die Atrophie in 42 % und die fettige Degeneration in 34 % der Fälle beurteilt.
Die Arbeit hebt hervor, dass eine Optimierung der aktuellen Diagnostikqualität notwenig ist, um eine zeitnahe und korrekte individuelle Behandlung zu gewährleisten. Es ist zu diskutieren, ob Verbesserungsansätze wie standardisierte Befunderhebungsschemata oder Spezialisierungen in größeren radiologischen Zentren im Bereich der muskuloskelettalen Bildgebung und Befundung umsetzbar sind
Beurteilung spirooxymetrischer und lungenfunktioneller Parameter hinsichtlich der Bedeutung zur Prognoseabschätzung bei Bronchialkarzinomen
BC ist in Deutschland eine führende krebsbedingte Todesursache und die
Erkrankungszahlen stiegen im Jahr 2020 weiter an, wobei Männer häufiger betroffen
sind. Die Prognose für BC-Patienten bleibt ungünstig, und die Erfassung von
Risikofaktoren, Symptomen und Metastasen ist entscheidend für die
Therapieentscheidungen. Unsere Studie untersuchte etablierte und potenzielle
Prognosefaktoren für BC. Wir haben die Wertigkeit von Lungenfunktionstestung und
CPET auf die Prognoseabschätzung in einem breiten Patientenkollektiv von 674 (501
Männer) neu diagnostizierten BC-Patienten zwischen 2005 und 2018 am
Universitätsklinikum Greifswald untersucht.
Das Durchschnittsalter betrug 66 Jahre, die 2-Jahres-Überlebensrate 35%, mit einer
mittleren Überlebenszeit von 14 Monaten. In 13,21% der Fälle wurde SCLC
histopathologisch nachgewiesen. Im untersuchten Kollektiv befanden sich 93
Patienten in Stadium IA, 50 in IB, 29 in IIA, 43 in IIB, 147 in IIIA, 99 in IIIB und 213 in
IV. Insgesamt hatten 68,2 % der Patienten zum Zeitpunkt der Diagnosestellung ein
Karzinom im Stadium III und 31,6% der Patienten waren aufgrund der
Fernmetastasierung bereits in Stadium IV.
Die Überlebenszeitanalyse ergab einen signifikanten Einfluss des Tumorstadiums.
Weiterhin konnte für die Spitzensauerstoffaufnahme, das Atemäquivalent an der
anaeroben Schwelle, die TLC und die FEV1 als objektiv ermittelbare Faktoren ein
signifikanter Einfluss unabhängig von etablierten Prognoseparametern nachgewiesen
werden. Die Berücksichtigung dieser Parameter in Verbindung mit dem Tumorstadium
in einem Modell zur Prognoseabschätzung ergab eine gute prädiktive Güte hinsichtlich
des 2-Jahres-Überleben (AUC von 0,807). Unsere Arbeit hebt hervor, dass ein
umfassendes Modell, das verschiedene Faktoren der Lungenfunktion aber
insbesondere auch der CPET berücksichtigt, eine genauere Prognoseabschätzung
erlaubt. Trotz einiger Limitationen bieten unsere Ergebnisse wichtige Einblicke und
ermutigen zur weiteren Forschung in diesem Bereich.
Es ist jedoch wichtig anzumerken, dass unsere Arbeit auf einer beschränkten
Stichprobe beruht und bestimmte Variablen nicht erfasst wurden. Zudem macht die
Vielfalt der verwendeten Klassifikationssysteme und Therapieoptionen den Vergleich
mit anderen Forschungsarbeiten komplexer. vor der breiten Anwendung in der
klinischen Praxis in Erwägung zu ziehen. Vor der breiten Anwendung in der klinischen
Praxis ist es angebracht, unsere Modelle in unabhängigen Kohorten nachzuvollziehen.
Zukünftige Forschung sollte eine umfassendere Datenerfassung und -analyse sowie
eine gründliche Überprüfung der Ergebnisse in Bezug auf Validität und
Vergleichbarkeit einschließen.
Unsere Studie zeigt den positiven Einfluss der kardiopulmonalen Leistungsfähigkeit
auf das 2-Jahres-Überleben von BC-Patienten in verschiedenen Krankheitsstadien
und unter verschiedenen Therapien. Sie untermauert die Bedeutung
belastungsabhängiger Faktoren für die Prognoseabschätzung und unterstützt die
Hypothese, dass eine gesteigerte kardiopulmonale Leistungsfähigkeit, die durch
Training und Rehabilitation gefördert werden kann, das Überleben verbessert
Esophageal transit of solid oral dosage forms – impact of different surface materials characterized in vitro and in vivo by MRI in healthy volunteers
Acceptable swallowability and complete esophageal transit are decisive for the safe and effective administration of solid oral dosage forms. This applies in particular to the main user group of medicines, older adults, who often suffer from swallowing difficulties. It is well known that surface properties play an important role in this respect. In the past, this has led to the development of numerous coating formulations for tablets with improved swallowability. However, in vitro and especially in vivo data investigating a positive effect of different coating materials is limited. Therefore, we investigated coating materials being based on polyvinyl alcohol, hydroxypropyl methylcellulose, and a copolymer of methacrylate in respect to their influence on swallowability and esophageal transit of a tablet formulation. They were compared to uncoated tablets as well as to hard gelatin capsules. Three in vitro assays suitable for routine use in pharmaceutical development were performed: i.) Wettability test in artificial saliva; ii.) Swelling measurement in artificial saliva; iii.) Measurement of the adhesion between surface materials and a simulated mucosa surface. All three assays resulted in a differentiation of the surface materials. The coated tablets showed favorable behavior compared to uncoated tablets and hard gelatin capsules. To test the effect of the different materials in vivo, an intervention study was conducted. 36 adults were included and the likeliness of prolonged esophageal transit of (un-)coated tablets as well as a hard gelatin capsule of the same weight was objectively evaluated by means of magnetic resonance imaging. While hard gelatin capsules showed highest rates for prolonged esophageal transit, the tendency for adhesion was reduced for uncoated tablets, and least for coated tablets, i.e., prolonged esophageal transit in 22.2 %, 11.1 %, and ≤5.6 % of the cases, respectively. Further differentiation of the coating materials was not possible. Subjective evaluations of each participant with respect to subjective swallowability and esophageal transit did not correlate well with the objective measurements by means of magnetic resonance imaging. The use of coatings in general has a positive influence on esophageal transit. However, the selection of coating type seems to be of greater importance in respect to patients' oral perception of the dosage forms compared to their influence on the probability for prolonged esophageal transit
Acetyl-CoA synthetase activity is enzymatically regulated by lysine acetylation using acetyl-CoA or acetyl-phosphate as donor molecule
The AMP-forming acetyl-CoA synthetase is regulated by lysine acetylation both in bacteria and eukaryotes. However, the underlying mechanism is poorly understood. The Bacillus subtilis acetyltransferase AcuA and the AMP-forming acetyl-CoA synthetase AcsA form an AcuA•AcsA complex, dissociating upon lysine acetylation of AcsA by AcuA. Crystal structures of AcsA from Chloroflexota bacterium in the apo form and in complex with acetyl-adenosine-5′-monophosphate (acetyl-AMP) support the flexible C-terminal domain adopting different conformations. AlphaFold2 predictions suggest binding of AcuA stabilizes AcsA in an undescribed conformation. We show the AcuA•AcsA complex dissociates upon acetyl-coenzyme A (acetyl-CoA) dependent acetylation of AcsA by AcuA. We discover an intrinsic phosphotransacetylase activity enabling AcuA•AcsA generating acetyl-CoA from acetyl-phosphate (AcP) and coenzyme A (CoA) used by AcuA to acetylate and inactivate AcsA. Here, we provide mechanistic insights into the regulation of AMP-forming acetyl-CoA synthetases by lysine acetylation and discover an intrinsic phosphotransacetylase allowing modulation of its activity based on AcP and CoA levels
Characterizing the flavodoxin landscape in Clostridioides difficile
Clostridioides difficile infections have become a major challenge in medical facilities. The bacterium is capable of spore formation allowing the survival of antibiotic treatment. Therefore, research on the physiology of C. difficile is important for the development of alternative treatment strategies. In this study, we investigated eight putative flavodoxins of C. difficile 630. Flavodoxins are small electron transfer proteins of specifically low potential. The unusually high number of flavodoxins in C. difficile suggests that they are expressed under different conditions. We determined high transcription levels for several flavodoxins during the exponential growth phase, especially for floX. Since flavodoxins are capable of replacing ferredoxins under iron deficiency conditions in other bacteria, we also examined their expression in C. difficile under low iron and no iron levels. In particular, the amount of fldX increased with decreasing iron concentration and thus could possibly replace ferredoxins. Moreover, we demonstrated that fldX is increasingly expressed under different oxidative stress conditions and thus may play an important role in the oxidative stress response. While increased fldX expression was detectable at both RNA and protein level, CD2825 showed increased expression only at mRNA level under H2O2 stress with sufficient iron availability and may indicate hydroxyl radical-dependent transcription. Although the exact function of the individual flavodoxins in C. difficile needs to be further investigated, the present study shows that flavodoxins could play an important role in several physiological processes and under infection-relevant conditions
Addressing mental illness stigma in German higher education: study protocol for a mixed-methods evaluation of a psychosocial setting-based intervention
Introduction
Mental illness stigma is associated with a range of negative consequences, such as reduced help-seeking for mental health problems. Since stigma affects individual, social, and structural aspects, multilevel interventions such as the Canadian programme The Working Mind have been proven to be the most effective. Given the solid evidence base for The Working Mind, it is our aim to implement and evaluate culturally adapted versions of the programme in German higher education, targeting students, employees and managers.
Methods and analysis
We will evaluate the programme with regard to its effect on mental illness stigma, openness to mental health problems, willingness to seek help, and positive mental health outcomes. Further, we will investigate the programme’s effectiveness dependent on gender and personal values, various mechanisms of change, and factors facilitating and hindering implementation. The study uses a sequential explanatory mixed-methods evaluation design (QUAN → qual) that consists of three steps: (1) quasi-experimental online survey with programme participants, (2) focus groups with programme participants, and (3) qualitative interviews with programme stakeholders. The quantitative data collected in step 1 will be analysed using 2×3 analysis of variances and a parallel multiple mediation analysis. The results will inform the qualitative data to be collected in steps 2 and 3, which will be analysed using qualitative content analysis.
Ethics and dissemination
The study was approved by the local Ethics Committee (Ethics Committee of University Medicine Greifswald; BB 098/23). Participants have to provide written consent before taking part in a focus group or interview. As for the online survey, participants have to give their consent by agreeing to an online data protection form before they can start completing the survey. We will publish central results and the anonymised data in an Open Access Journal. Further, the statistical code will be included as a supplement to the paper(s) documenting the results of the study.

Cold Physical Plasma Reduces Motility of Various Bone Sarcoma Cells While Remodeling the Cytoskeleton
Background/Aim: Cold physical plasma (CPP) has emerged as an effective therapy in oncology by inducing cytotoxic effects in various cancer cells, including chondrosarcoma (CS), Ewing’s sarcoma (ES), and osteosarcoma (OS). The current study investigated the impact of CPP on cell motility in CS (CAL-78), ES (A673), and OS (U2-OS) cell lines, focusing on the actin cytoskeleton. Materials and Methods: The CASY Cell Counter and Analyzer was used to study cell proliferation and determine the optimal concentrations of fetal calf serum to maintain viability without stimulation of cell proliferation. CellTiter-BlueCell viability assay was used to determine the effects of CPP on the viability of bone sarcoma cells. The Radius assay was used to determine cell migration. Staining for Deoxyribonuclease I, G-actin, and F-actin was used to assay for the effects on the cytoskeleton. Results: Reductions in cell viability and motility were observed across all cell lines following CPP treatment. CPP induced changes in the actin cytoskeleton, leading to decreased cell motility. Conclusion: CPP effectively reduces the motility of bone sarcoma cells by altering the actin cytoskeleton. These findings underscore CPP’s potential as a therapeutic tool for bone sarcomas and highlight the need for further research in this area
