Anticiper le pire : la dystopie pour appréhender le risque (France, 1930-1950)
International audienceLa dystopie, genre romanesque familier de la prospective mais distinct par sa dimension explicitement fictionnelle, constitue un observatoire privilégié du risque. En tant que récit conjectural représentant une communauté en danger ou sur le déclin, la dystopie se fonde sur la vraisemblance, sous l’angle de la « conjecture romanesque rationnelle » (Pierre Versins), et permet d’aborder le risque à la fois comme un faisceau collectif de représentations (imaginaire) et comme une réalité contextuelle (discours social) qui le caractérisent par son imminence et sa gravité plus ou moins grandes. Cette communication propose d’examiner le traitement spécifique du risque dans la dystopie d’expression française de la période allant de l’entre-deux-guerres à 1950, à travers un corpus comprenant notamment les œuvres de Léon Groc (Une invasion de Sélénites, 1930), Théo Varlet (La Grande Panne, 1930), Jean d’Agraives (Le Virus 34, 1930), Régis Messac (Quinzinzinzili, 1935), Jacques Spitz (La Guerre des mouches, 1937), Barjavel (Ravage, 1943), Christophe Paulin (S’il n’en reste qu’un, 1946), Claude Pearson (La Mort atomique, 1947). Ces récits d’anticipation anxiogènes, orientés vers le risque du pire – catastrophe, chaos, déliquescence, guerre, altérité menaçante –, relèvent d’une dynamique d’expérimentation fictionnelle qui s’intensifie à partir de l’entre-deux-guerres. Ils conjuguent les codes de la fiction populaire et ceux de la littérature générale, dans une hybridation qui témoigne de la modularité générique de la dystopie et de la variété des réactions programmées, entre distanciation critique et immersion de mise en garde. L’analyse visera à articuler poétique et pragmatique : la dystopie agit-elle pour anticiper les risques et contribuer à leur prévention, ou pour mettre en scène, de manière réflexive, les angoisses de son temps ? Il s’agira d’interroger les modalités fictionnelles de cette expérimentation – avant, pendant, après la catastrophe – et le rapport à l’avenir qu’elles construisent (irrévocable, modifiable, sous responsabilité humaine). En cela, cette littérature développe une double orientation : proactive et critique, elle cherche à infléchir les imaginaires du présent ; à rebours de la collapsologie, elle mise sur la possibilité d’éviter l’effondrement
Modélisation de l’interaction entre le membre résiduel et la prothèse chez la personne amputée d'un membre inférieur pour la conception numérique d’emboîtures
For lower limb prosthesis users, the prosthetic socket, which connects the prosthetic limb to the residual limb, is a critical interface. It must provide safe control of the prosthesis, while ensuring the user's comfort, as a poor socket fit can lead to pain, irritation, or even pressure ulcers. Traditionally, sockets are designed from a plaster cast of the residual limb, in a time-consuming, quasi-artisanal process requiring great expertise. Indeed, a slight rectification in the socket shape can have a major impact on the user’s comfort and mobility.To speed up this process and improve its repeatability, a digital alternative has been introduced. The socket can now be designed from a 3D scan of the residual limb, using Computer-Aided Design and Manufacturing (CAD/CAM) tools. This evolution paves the way for the integration of more quantitative tools into this workflow, to guide the prosthetist in the rectification process. This thesis is part of the ANR IMPRINT project, which proposes to integrate a biomechanical model of the residual limb/socket assembly, in order to estimate a number of mechanical indicators of socket fit before its manufacture.The first part of this thesis presents a state-of-the-art review of solutions for optimizing socket design. The interface pressure between the residual limb and the socket has been identified as a relevant mechanical indicator of socket fit. As a result, numerous models, based on the Finite Element (FE) method, have been developed to estimate this parameter. However, most of them have only been implemented on a single subject and have never been validated experimentally, which is a major barrier to their transfer to clinical practice.The aim of this thesis was therefore to develop and evaluate a subject-specific FE model for estimating interface pressure, with a view to its integration into CAD/CAM software.The lack of validation of FE models can be explained by the limitations of available pressure measurement systems in terms of accuracy and ease of use. The second chapter therefore focuses on the development of an accurate, robust and affordable solution, based on 3D printing and force sensors, for measuring interface pressure.The third chapter presents the development of a subject-specific FE model of the residual limb/socket assembly to estimate interface pressure, and its evaluation on 6 transtibial amputees using the aforementioned measurement system. To our knowledge, this is the first study in the literature to evaluate a FE model of the residual limb and socket on such a large cohort. The results highlight the impact of individual characteristics on model accuracy, and the importance of evaluating it on a larger number of subjects. A sensitivity study was carried out with the dual aim of improving the model's accuracy and simplifying it for clinical transfer. Two critical parameters were identified - the external geometry of the residual limb, and soft tissue compressibility - opening up two avenues for future research.The last chapter of the thesis addresses two additional barriers to clinical adoption. A first section explores the use of a statistical shape model to estimate the internal geometry of the residual limb without the need for clinically inaccessible medical imaging systems. The second presents a semi-automated socket rectification method based on FE modelling, intended to help interpret pressure data.Altogether, this work demonstrates the feasibility of prosthetic socket rectification guided by a subject-specific Finite Element model, and marks a first step towards the development of biomechanical tools to assist prosthetists in optimizing and accelerating socket design.Pour les utilisateurs de prothèse de membre inférieur, l’emboîture prothétique, qui fait le lien entre la prothèse et le membre résiduel, est une interface critique. Elle doit assurer un contrôle efficace de la prothèse, sans être inconfortable, car un mauvais ajustement peut entraîner des douleurs, des irritations, voire des escarres. Elle est conventionnellement conçue à partir d’un moulage en plâtre du membre résiduel, selon un processus chronophage et quasi-artisanal, qui nécessite une grande expertise. En effet, une rectification minime de la paroi peut avoir un impact majeur sur le ressenti et la mobilité de l’utilisateur.Pour accélérer ce processus et améliorer sa répétabilité, une alternative numérique se démocratise peu à peu. L’emboîture peut désormais être conçue à partir d’un scan 3D du membre résiduel, avec des outils de Conception et Fabrication Assistée par Ordinateur (CFAO). Cette évolution ouvre la voie à l’intégration d’outils quantitatifs dans le processus de rectification pour guider le prothésiste. Le projet ANR IMPRINT, dans lequel s’inscrit cette thèse, propose d’intégrer un modèle biomécanique du couple membre résiduel/emboîture, permettant d’estimer des indicateurs quantitatifs de l’ajustement de l’emboîture en amont de sa fabrication.La première partie de ce manuscrit présente un état de l’art des solutions pour optimiser la conception de l’emboîture. La distribution de pression à l’interface avec le membre résiduel a été identifiée comme un indicateur mécanique de l’ajustement de l’emboîture. De nombreux modèles, reposant sur la méthode des Éléments Finis (EF), ont donc été introduits pour estimer ce paramètre. Cependant, la majorité de ces modèles n’ont été implémentés que sur un seul sujet et n’ont pas été validés expérimentalement, ce qui constitue un frein majeur à leur transfert en pratique clinique.Ce travail de thèse visait donc à développer et évaluer un modèle EF personnalisé pour estimer la pression à l’interface, en vue de son intégration dans un logiciel de CFAO.L’absence de validation des modèles EF s’explique en partie par les limites des systèmes de mesure de pression disponibles en termes de précision et de praticité. Le second chapitre présente donc le développement d’une solution précise, robuste et abordable, reposant sur l’impression 3D et des capteurs de force, pour mesurer la pression à l’interface.La troisième partie présente le développement d’un modèle EF personnalisé de l’ensemble membre résiduel/emboîture pour estimer la pression à l’interface, et son évaluation sur 6 personnes amputées transtibiales à l’aide du système de mesure susmentionné. Il s’agit de la première étude de la littérature à évaluer un modèle EF de ce type sur une telle cohorte. Les résultats soulignent l’impact des caractéristiques individuelles sur la précision du modèle, et l’importance de l’évaluation sur plus de sujets. Une étude de sensibilité a été menée dans le double objectif d’améliorer la précision du modèle, et de le simplifier en vue d’un transfert clinique. Deux paramètres critiques ont été identifiés - la géométrie externe du membre résiduel, et la compressibilité des tissus mous - ouvrant deux axes de recherche.Enfin, la dernière partie aborde deux verrous au transfert clinique d’un tel outil. Une première section explore l’utilisation d’un modèle statistique pour estimer la géométrie interne du membre résiduel sans avoir recours à des systèmes d’imagerie médicale peu accessibles. La seconde propose une méthodologie semi-automatique de rectification de l’emboîture basée sur le modèle EF pour aider à l’interprétation des données de pression.Finalement, ces travaux démontrent la faisabilité d’une rectification de l’emboîture guidée par un modèle EF personnalisé, et constituent une première étape vers le développement d’outils biomécaniques destinés aux prothésistes pour optimiser et accélérer la conception des emboîtures
Higher levels of host-cell DNA methylation markers ZNF582 and ASCL1 on anal smears are predictive for progression to anal cancer in patients with previous high-grade lesions
International audienceBackground: French and International anal cancer screening recommendations for at-risk populations, published in 2024, are based on cytology and/or high-risk human papillomavirus (HPV) detection on anal smears. Biological markers to triage the patients most at-risk for anal cancer are crucial in prioritising patients needing high-resolution anoscopy consultations, which are frequently overwhelmed.Methods: The AIN3 cohort is a French national multicenter study including patients with a history of high-grade anal lesions (AIN3). Patients were followed-up for at least 3 years, with anal smears and clinical examinations performed yearly. Levels of ZNF582 and ASCL1 gene methylation were quantified using real-time PCR on anal smears collected at the time of inclusion.Findings: Overall, 514 anal smears were contributive for host-cell DNA methylation analysis. Patients' mean age was 50.8 years and 40% were women. Among the 41% who were living with HIV, 91% were men. Median follow-up duration was 48 months, and 22 patients (4%) developed anal cancer during follow-up. Higher methylation levels of ZNF582 and ASCL1 were significantly associated with high-grade squamous cell intraepithelial lesion (HSIL) cytology, p16-Ki67 dual-staining positivity, and high-risk HPV and HPV16 positivity on the same anal smear. Both methylation markers showed an AUC of 0.72 for discrimination between HSIL and non-HSIL cytology on the same anal smear. Higher methylation levels of both markers were significantly associated with evolution to anal cancer in univariate and multivariable analyses adjusted for age and HIV status (p < 0.001). When assessing the AUC over 1 and 3 years of follow-up, methylation markers demonstrated superior predictive value for anal cancer compared to other markers.Interpretation: We have demonstrated the predictive value of host-cell DNA methylation marker levels in anal smears with regard to evolution to anal cancer in a very high-risk population
Paroles d’enfants, attentes d’adultes : ce que « bien » parler de soi veut dire
International audienceCet article propose d’explorer ce que « bien » parler de soi veut dire pour un enfant, lorsque sa parole est sollicitée par un adulte, sur trois marchés linguistiques différents : un entretien avec une chercheuse au sein de l’espace domestique, une consultation médicale à l’hôpital, des activités expressives à l’école (principalement des débats et exposés). Le croisement de ces trois contextes permet de faire émerger les attentes normatives récurrentes entourant le parler de soi enfantin et d’identifier les effets des situations d’interaction sur les prises de parole enfantines. Trois caractéristiques des discours enfantins sont apparues, à travers les contextes, particulièrement valorisées : qu’ils soient volontaires, personnels et efficaces, ce à quoi les enfants sont inégalement disposés par leurs socialisations de classe et de genre
Investigating AI Approaches for Survival Prediction in Chronic Lymphocytic Leukemia
International audienceChronic lymphocytic leukemia (CLL) exhibits a heterogeneous clinical course. Prognostic markers that impact patient outcomes have been identified, including MYC gene abnormalities. This study investigates machine learning (ML) models for predicting survival in CLL, comparing the performance of Random Survival Forest (RSF), Decision Tree (DT), and Cox proportional hazards models across two cohorts: MYC-positive patients and a general CLL population. Three time-to-event outcomes were assessed: 10-year from diagnosis, 10-year from cytogenetic assessment, and time to first treatment. Model performance was evaluated using the C-index and AUC, revealing that RSF and DT models outperformed Cox models in predictive accuracy. Permutation importance highlighted key predictive variables; however, RSF and DT models pose interpretability challenges compared to Cox models, which offer clear hazard ratios. Additionally, an interactive application is available via Streamlit, and the source code is open access on GitHub. Despite limitations in dataset size and external validity, ML models show promise for personalized survival predictions in CLL, especially for MYC-positive cases, underscoring the potential for further model refinement to enhance clinical usability
OpenPVSignal Knowledge Graph: Pharmacovigilance Signal Reports in a Computationally Exploitable FAIR Representation
International audienceIntroduction: Pharmacovigilance signal report (PVSR) documents contain valuable condensed information published by drug monitoring organizations, typically in a free-text format. They provide initial insights into potential links between drugs and harmful effects. Still, their unstructured format prevents this valuable information from being integrated into data-processing pipelines (e.g., to support either the investigation of drug safety signals or decision-making in the clinical context).Objective: OpenPVSignal is a data model designed specifically to publish PVSRs via a computationally exploitable format, compliant with the FAIR (Findable, Accessible, Interoperable, Reusable) principles to promote ease of access and reusability of these valuable data.Methods: This paper outlines the procedure for converting pharmacovigilance signals published by the World Health Organization Uppsala Monitoring Centre (WHO-UMC) into the OpenPVSignal data model, resulting in a Knowledge Graph (KG). It details each step of the process, including the technical validation by KG engineers and the qualitative verification by medical and pharmacovigilance experts, leading to the finalized KG.Results: A total of 101 PVSRs from 2011 to 2019 were incorporated into the openly available KG.Conclusion: The presented KG could be useful in various data-processing pipelines, including systems that support drug safety activities

Better survival of older patients with stroke managed in a collaborative stroke pathway
International audienceAbstract Purpose To compare the survival of stroke patients over 70 years old managed in the collaborative Stroke Pathway dedicated to the OLD patients (SPOLD) with those referred to conventional rehabilitation wards. Methods This longitudinal observational retrospective cohort study involved over 70 years old patients referred from the same neurovascular emergency unit in a university hospital in France to a post-stroke geriatric unit within the SPOLD pathway, compared to patients referred to conventional rehabilitation ward during the same period. Initial stroke severity and comorbidities were assessed using the NIHSS score and the Charlson index, respectively. The primary endpoint was 2-year survival, analyzed using Cox models for both the entire cohort and a propensity score-matched cohort to control for referral bias. Results The study included 262 patients with a mean age of 84.5 ± 6.7 years, of whom 122 (46.6%) were male. Patients in the SPOLD group ( n = 101) had significantly higher age, NIHSS scores, and Charlson indices compared to those in the conventional rehabilitation ward ( n = 161). Adjusted mortality was significantly lower in SPOLD patients (OR: 0.525, 95% CI: 0.298 to 0.924, p = 0.025). In the propensity score-matched cohort, mortality was also significantly lower for SPOLD patients (OR: 0.426, 95% CI: 0.212 to 0.857, p = 0.017). Conclusion This study suggests that this collaborative organization between a neurovascular emergency unit and a rehabilitation geriatric unit may be associated with a better survival of older patients after stroke
