    ATTITUDE - Addressing attrition in longitudinal cancer cohorts: an in-depth qualitative analysis of experiences and perspectives on participation in longitudinal studies among breast cancer survivors

    International audienceAbstract Purpose Longitudinal studies are essential for investigating outcome evolution among cancer survivors. However, longitudinal designs pose specific challenges, such as attrition and premature dropout. We aimed to assess experiences and perspectives on participation in longitudinal studies among survivors of early-stage breast cancer, and inform interventions aiming to reduce attrition. Methods Motivation, facilitators, challenges, and perspectives regarding participation in a longitudinal study were qualitatively assessed via interviews and focus groups. A thematic content analysis was performed. Results Between May and August 2023, 30 patients previously enrolled in the longitudinal CANTO cohort study (NCT01993498) were included: 17 participated in individual semi-structured interviews and 13 in two separate focus groups involving distinct participants. We identified four key themes: (1) joining the study as an expression of purpose and personal security, (2) ongoing engagement, as a response to flexible processes, a reassuring study environment, and personal commitment, (3) ongoing engagement, challenged by accumulating (practical and emotional) burdens and a reduced sense of connection to the study, and (4) participant-identified needs for a more supportive study experience. Findings emphasized the importance of enhancing communication with study participants, flexibility enabling decentralized participation, reducing burden of patient-generated data via repetitive questionnaires, supporting navigation of study procedures and minimizing the risk of a digital divide, and routinely disseminating study findings. Conclusion These findings will inform strategies to reduce attrition and enhance the overall participant experience in longitudinal studies

    Efficacy, safety, pharmacokinetics, immunogenicity, and serum neutralizing activity of AZD7442 (tixagevimab-cilgavimab) in patients hospitalized with COVID-19: long-term results from the DisCoVeRy trial

    International audienceObjectives: To report long-term clinical efficacy, safety, pharmacokinetics, immunogenicity and seroneutralization results of AZD7442 (monoclonal antibodies tixagevimab-cilgavimab) in patients hospitalized with COVID-19.Methods: In this phase 3, double-blind, randomized, multicentre trial, hospitalized adults with PCR-confirmed SARS-CoV-2 infection were randomly assigned 1:1 to receive AZD7442 or placebo, and followed-up until day 456, with repeated blood sample collections until day 365. Clinical endpoints included clinical status, mortality, rehospitalization, SARS-CoV-2 reinfection, and adverse events. Antidrug antibodies and serum drug concentrations were measured. Analyses were performed on the modified intention-to-treat (mITT) populations, defined as participants who actually received the intervention.Results: Between April 28, 2021, and June 23, 2022, 237 participants were randomly assigned to AZD7442 (n = 127) or placebo (n = 110), and 123 participants actually received AZD7442. Participants were infected with pre-Omicron variants in 58.8% (133/226) of cases, versus 33.2% (75/226) of Omicron BA1, BA2, or BA5, and 8% (18/226) missing data. There was no significant difference in the distribution of the 7-point ordinal scale between the AZD7442 and placebo groups, either on day 15 (primary endpoint) (OR = 0.93 [0.54-1.61], p 0.81), or any other time point. Significantly more rehospitalizations occurred between discharge and day 456 among participants who received AZD7442 in the global mITT population (OR = 2.04 [1.03-4.05], p 0.04), but not in the antigen-positive mITT population (OR = 1.78 [0.80-3.94], p 0.15). No significant differences were observed in mortality, SARS-CoV-2 reinfection, or adverse events. In the AZD7442 group, 12 of 87 participants (13.8%) had treatment-emergent antidrug antibodies versus 5 of 69 (7.2%) in the placebo group (OR = 2.02 [0.66-6.14], p 0.21). Serum drug concentrations were detectable up to day 365 for all sampled participants (35/35). Neutralizing antibody titres were significantly higher in the AZD7442 group up to day 180.Conclusions: AZD7442 did not demonstrate any clinical benefit and was safe up to 15 months. This study also provides valuable data on the pharmacokinetics, immunogenicity, and neutralizing activity of AZD7442 in patients hospitalized with COVID-19

The elder, Eugène, became the accountant and then the manager of the Bazar Bonne-Nouvelle in Paris. The younger, Adolphe, was a former career soldier who ended his professional life as the general manager of the Suez Canal, where he was killed by local inhabitants. The editor of this work is Luc Marco, honorary professor of management science at Sorbonne Paris Nord University, a member of the CEPN-CNRS laboratory, and president of the Institute for the History and Foresight of Management, based in Castres (Tarn). The afterword is written by Anis Gnichi, lecturer at the University of Sousse in Tunisia.Les auteurs de ce livre, précurseurs de la finance moderne, étaient les frères Sala. L’aîné, Eugène, deviendra le comptable puis le gérant du Bazar Bonne-Nouvelle à Paris. Le cadet, Adolphe, était un ancien militaire de carrière qui finira son activité professionnelle comme intendant général du Canal de Suez où il mourra tué par les indigènes.     Does urban environment at birth and adolescence affect cardiometabolic morbidity in adolescents? Results from the PARIS birth cohort study

    International audienceThis study aims to examine the association between urban environment characteristics at birth and at adolescence and cardiometabolic health in adolescents from the PARIS birth cohort.Body mass index z-scores (BMIz) trajectories from birth to adolescence, weight status and two cardiometabolic profiles at adolescence were determined. Traffic-related air pollution (TRAP) exposure was estimated by a nitrogen oxides air dispersion model. The French Deprivation Index, walkability percentage, and the use of green spaces were considered. Built environment typologies were identified by a cluster analysis. Associations were assessed using multivariable (multinomial) logistic regression models and potential modifier effects were examined.Among 617 adolescents who participated in the health checkup, around one in ten were living with overweight/obesity. Five BMIz trajectories were identified. After adjustment, walkability and the use of green spaces at adolescence were negatively associated with overweight/obesity. Early TRAP exposure showed a positive association with overweight/obesity and with each BMIz trajectory compared to the low stable trajectory, with a higher level of association observed for the two ascending trajectories. TRAP exposure, parental overweight, use of green spaces, and sedentary behavior modified the associations between the urban environment and overweight.These findings show the beneficial effects of neighborhood walkability and using green spaces on overweight/obesity in adolescents. They also demonstrate that early TRAP exposure increases the risk of overweight. These results highlight the necessity of effective urban planning to contribute to a healthy environment

    Cluster of Severe Arginase 1 Deficiency in the Comoros: Clinical, Neuroimaging, and Molecular Features in 17 Patients From Mayotte Compared With 10 From Paris

    International audienceABSTRACT Arginase 1 deficiency (ARG1D) is the least common urea cycle disorder. Neonatal onset is rarely described, and hyperammonemic coma is less common in patients with ARG1D compared to other urea cycle disorders. In recent years, we diagnosed a high number of ARG1D patients in Mayotte, an insular (Comoro Islands) department of France. This study aimed at retrospectively analyze and compare our patients' data with those diagnosed and followed in Paris. We present a series of 27 subjects included either in the Center Hospitalier de Mayotte (CHM), Mamoudzou, France ( n = 17), or in Hôpital Necker‐Enfants Malades (NEM), Paris, France ( n = 10). Median age at diagnosis ( n = 27) was 2 years (ranging from birth to 15 years). Patients diagnosed at CHM were generally older than those diagnosed at NEM, which may reflect a longer diagnostic delay in CHM. The main clinical features at diagnosis included hyperammonemic coma (with or without liver failure) and neurodevelopmental delay with spastic diplegia. Brain MRI could be either normal or showed severe lesions in patients with overt hyperammonemia at diagnosis. Eight different ARG1 variants were identified, including two variants (c.466‐2 A>G and c.766G>A) that were found exclusively in subjects originating from the Comoro Islands. Despite no significant differences in laboratory parameters, clinical outcomes remained better in NEM versus CHM possibly ascribable to a longer diagnostic delay in CHM. These findings revealed a new cluster of ARG1D associated with two severe variants in Mayotte, suggesting that this region could possibly benefit from targeted newborn screening for ARG1D due to its overrepresentation
