    INTRODUCTION: Overcrowding in emergency departments (ED) is a major public health issue, leading to increased workload and exhaustion for the teams, resulting poor outcomes. It seems interesting to be able to predict the admissions of patients in the ED. AIM: The main objective of this study was to build and test a prediction tool for ED admissions using artificial intelligence. METHODS: We performed a retrospective multicenter study in two French ED from January 1st, 2010 to December 31st, 2019.We tested several machine learning algorithms and compared the results. RESULTS: The arrival and departure times from the ED of 2 hospitals were collected from all consultations during the study period, then grouped into 87 600 one-hour slots. Through the development of two models (one for each location), we found that the XGBoost method with hyperparameter adaptations was the best, suggesting that the studied data could be predicted (mean absolute error) at 2.63 for Hospital 1 and 2.64 for Hospital 2). CONCLUSIONS: This study ran the construction and validation of a powerful tool for predicting ED admissions in 2 French ED. This type of tool should be integrated into the overall organization of an ED, to optimize the resources of healthcare professionals

    Mettre en évidence les effets d’une thérapie centrée sur l’élaboration métacognitive chez despersonnes souffrant de troubles psychiques.Méthode. – Une revue systématique a été effectuée sur PubMed, PsychInfo, Psycharticles et Psychologicaland Behavioral Science Collection sur la période 1980 à 2024.Résultats. – Cette revue a rassemblé cinq études contrôlées randomisées, quatre études groupales obser-vationnelles et 22 études de cas avec mesures quantitatives. Les analyses indiquent que MERIT augmentesignificativement les capacités métacognitives de réflexion sur soi, sur les autres, ainsi que le niveau demaîtrise des stratégies. De plus, certaines études retrouvent une amélioration de l’insight ainsi que de lasymptomatologie. La majorité des études incluaient des participants souffrant de troubles psychotiques.Conclusion. – MERIT est une thérapie efficace, visant à favoriser le rétablissement subjectif vial’amélioration des capacités métacognitives chez les personnes souffrant de troubles psychotiques.D’autres études seraient cependant nécessaires pour généraliser ce résultat aux autres troubles psy-chiques.Objectives: To demonstrate the effects of a metacognitive reflection and insight therapy (MERIT) in people suffering from psychiatric disorders. Method: A systematic review was carried out on PubMed, PsychInfo, Psycharticles and Psychological and Behavioral Science Collection from 1980 to 2024. Results: The review included five randomized controlled studies, four observational group studies and 22 case studies with quantitative measures. Analyses indicated that MERIT significantly increases metacognitive abilities to reflect on oneself and others, as well as mastery of one's strategies. In addition, some studies found an improvement in insight and symptomatology. Most studies included participants with psychotic disorders. Conclusion: MERIT is an effective therapy for promoting subjective recovery by improving metacognitive abilities in people with psychotic disorders.     Reshape the relationship with the Living. : trees in agriculture

    L’arbre a perdu sa place historique dans l’agriculture française avec les modernisations agricoles. Depuis quelques décennies, les agriculteur.ices et les autres acteurs de la transition agroécologique reconsidèrent son rôle dans l’équilibre des écosystèmes. La thèse examine comment cette pratique agroforestière se déploie dans le secteur agricole en France. L’objectif est d’analyser comment ce retour de l’arbre bouleverse les rapports des agriculteur.ices avec le vivant humain.e et non-humain. L’enquête s’appuie sur un ensemble d’entretiens semi-directifs auprès de trois groupes d’acteurs : les agriculteur.ices, les acteurs intermédiaires : conseillers techniques et animateurs de réseaux de l’agroécologie, et les agents des politiques publiques de l’agroforesterie. Les entretiens s’accompagnent de journées d’observations sur les fermes et de prises de vues. L’enquête est menée dans une démarche interdisciplinaire entre sciences sociales et agronomie, et avec une importance accordée à la dimension visuelle (photographies, cartes et illustrations). Il en ressort 10 chapitres répartis en 3 parties. La première partie montre en quoi l’agroforesterie constitue une alternative au mode de production conventionnel, en tant qu’adaptation aux effets du dérèglement climatique. Le premier chapitre montre les différentes représentations de l’agroforesterie entretenues par les différents acteurs de la transition et fait apparaître une première caractérisation des fermes agroforestières. Le deuxième chapitre se penche sur l’identité professionnelle agroforestière, une posture qui tend au renouvellement des relations avec le vivant. Le troisième chapitre détaille comment cette recherche d’une synergie avec le vivant prend forme, en montrant les multiples dimensions de l’arbre sur la ferme, et leurs aspects complémentaires. La deuxième partie est centrée sur les mécanismes de la décision à s’engager dans une transition de pratiques. Pour cela, l’analyse est menée sur différentes échelles (macro, méso et micro sociologiques) dans lesquelles intervient la réflexion au changement. Je détaille les processus politiques, économiques (chapitre 4) et sociotechniques (chapitre 5) et les processus psychologiques et émotionnels (chapitre 6) qui entrent en jeu dans la prise de décision de l’engagement. Je défends l’ouverture à une échelle du sensible (chapitre 7) et à l’analyse des ressentis pour mieux saisir les processus de décision. La troisième partie est centrée sur les conditions de développement et de maintien de l’agroforesterie, tant auprès des agriculteurs (chapitre 8) que des acteurs intermédiaires (chapitre 9) et agents des politiques publiques (chapitre 10). Il en ressort que l’agriculture alternative ne fonctionne pas sur le même rapport au monde et au vivant que l’agriculture conventionnelle dominante. Cela fait apparaître un caractère naturaliste des politiques d’encadrement de l’agriculture, qui restreint les possibilités de développement de l’agriculture alternative.Trees have been removed from agricultural field and have lost their ancestral role in agriculture with agricultural modernisations. However, since a few decades, farmers and other actors in the agroecological transition have reconsidered trees’s ecological functions and their role for the balance of ecosystems. This PhD identifies how the practice of agroforestry spreads into the french agricultural field. It discusses how relationships between the farmers and the living, human and non-human, are affected by the return of trees in agriculture. The survey is based on visits to farms, observations and interviews with three groups of actors : farmers, intermediary actors (technical advisories and associations), and agents of agroforestry policies. The survey is carried out in an interdisciplinary approach between social sciences and agronomy, with a strong visual dimension (photographs, maps and illustrations). The PhD is organised in 3 parts and 10 chapters. The first part shows what makes agroforestry an alternative to the conventional model of production, as a way to adapt to climate change issue. Actors from the different groups have different perceptions of agroforestry. It brings out a first caracterisation of agroforestry farms in chapter one. Chapter two is focused on the professional identity of agroforestry farmers : their posture leads them to a new way to interact with the living. This synergia with the living is shaped by multiple and complementary aspects of trees into the farm, wich are the object of chapter three. The second part is focused on the decision process of engagement into the agroecological transition. These thinking take place in different scales : macro, meso and micro-sociological scales. The analysis of the decision mecanisms to the transition is conducted on each of these scales : political and economical (chapter 4), sociotechnical (chapter 5) and psychological and emotional (chapter 6). In chapter 7, I stand up for the analysis on a new scale, the scale of sensitivity. The third part discusses the conditions for agroforestry to be developed and sustained, by holding close to the farmers’s words (chapter 8), the intermediary actors’s words (chapter 9), and the policy agents’s words (chapter 10). It appears that the alternative agriculture and conventional agriculture are based on two different views on what relationship with the living shoud be. European agricultural policies are based on a naturalist view of relationship with the living, which hold back the development of the alternative agriculture

    Headache is a common neurological symptom, often leading to the investigation of secondary causes, including cerebrovascular conditions such as cranial and cervical artery dissection (CCAD). CCAD, a significant cause of stroke in younger adults, commonly presents with headache or neck pain, isolated or accompanied by neurological deficits, and may mimic primary headache disorders, complicating timely diagnosis. This review explores the role of headache in CCAD, specifically addressing headache as an initial presentation, its evolution post-dissection, and as a potential risk factor of CCAD. By synthesizing current evidence, the review aims to improve early detection and clinical management of CCAD in headache patients

    La caractérisation de l'infraction de harcèlement moral, prévu à l'article 222-33-2 du code pénal, n'exige pas, lorsque les agissements reprochés ont pour objet la dégradation des conditions de travail, qu'ils concernent un ou plusieurs salariés en relation directe avec leur auteur ni que les salariés victimes soient individuellement désignés. En revanche, lorsque de tels agissements ont pour effet une dégradation des conditions de travail, la caractérisation de l'infraction de harcèlement moral suppose que soient précisément identifiées les victimes de tels agissements. Indépendamment de toute considération sur les choix stratégiques qui relèvent des seuls organes décisionnels de la société, constituent des agissements entrant dans les prévisions de l'article 222-33-2 du code pénal, dans sa version résultant de la loi n° 2002-73 du 17 janvier 2002, et pouvant caractériser une situation de harcèlement moral institutionnel, les agissements visant à arrêter et à mettre en oeuvre, en connaissance de cause, une politique d'entreprise qui a pour objet de dégrader les conditions de travail de tout ou partie des salariés aux fins de parvenir à une réduction des effectifs ou d'atteindre tout autre objectif, qu'il soit managérial, économique ou financier, ou qui a pour effet une telle dégradation, susceptible de porter atteinte aux droits et à la dignité de ces salariés, d'altérer leur santé physique ou mentale ou de compromettre leur avenir professionnel

    OBJECTIVE: People with HIV-HCV co-infection need antiretroviral treatment (ART) to suppress HIV and direct-acting antivirals (DAAs) to cure HCV. ART is typically prioritized, but delays in DAA initiation may increase the risk of liver-related events and HCV transmission to others. DESIGN: Target trial emulation with observational data collected in routine clinical practice from a collaboration of cohorts from Europe and North America. METHODS: We included DAA-naïve adults with HIV-HCV co-infection who achieved HIV virologic suppression (HIV RNA6 months) DAA initiation and the 36-month risk of liver-related events (liver decompensation or hepatocellular carcinoma). RESULTS: Of 862 eligible individuals (median age 46 years; interquartile range 36 to 56), 14% were women, and 52% had a history of injection drug use. The 6 and 36-month probabilities of not initiating DAA were 58% (95% CI: 55, 61) and 24% (21, 27), respectively. The 36-month risk of liver-related events was 1.1% (0.4, 2.0) for early initiation and 1.7% (0.7, 2.5) for delayed initiation; risk difference -0.5% (-1.2, 0.4). CONCLUSIONS: Almost one-quarter of people with HIV-HCV co-infection on ART had not initiated DAA 3 years after HIV virologic suppression. Because the 3-year risk of liver-related events was low, estimates of the impact of delayed DAA initiation were imprecise

    Exposure to COVID-19 Pandemic-Related Stressors and Their Association With Distress, Psychological Growth and Drug Use in People With HIV in Nouvelle Aquitaine, France (ANRS CO3 AQUIVIH-NA Cohort-QuAliV-QuAliCOV Study)

    We investigated people living with HIV (PLWH)'s exposure to COVID-19 pandemic stressors and their association with distress, psychological growth, and substance use. PLWH in the ANRS CO3 AQUIVIH-NA cohort's QuAliV study (Nouvelle Aquitaine, France) completed an adapted CAIR Lab Pandemic Impact Questionnaire (C-PIQ) and reported substance use between 9/2021 to 3/2022. We described cumulative stressor exposure (score 0-16) and explored variation by PLWH characteristics (demographic, HIV-related, risk factors, psychosocial). Associations with distress (score 0-23), psychological growth (score 0-20), and substance use were assessed using regression models. Participants reported exposure to a median of 2 (IQR: 1-4) stressors. Stressor exposure was higher in working-age (<60) and psychosocially vulnerable PLWH. Exposure to an additional stressor correlated with a 0.7-point increase in distress scores (95% C.I. 0.5-1.0, p<0.001), a 0.04-point increase (95% C.I. 0.01-0.07, p=0.002) in psychological growth scores in working-age PLWH. In older PLWH, additional stressor correlated with a 0.8-point (95% C.I. 0.4-1.2, p<0.001) increase in distress and a 0.1-point increase (95% C.I. 0.06-0.2, p=0.001) in growth scores. Each additional stressor was associated with 1.2 (95% C.I. 1.0-1.4, p=0.02) higher adjusted odds of cannabis use in working-age PLWH, and 1.2 (95% C.I. 1.0-1.4, p=0.004) higher adjusted odds of drug use. Exposure to stressors was linked to increased distress, cannabis and drug use but also growth. Providers should not only be aware of risk (of severe COVID-19) but also be mindful of the social and psychological challenges PLWH face as these may affect their retention in care, especially during challenging times

    From sugar-coating to film-coating: adapting the coating process for a micro-dosed tablet transferred between two manufacturing sites

    Dans le cadre du transfert industriel d’un comprimé microdosé (produit X) d’un site donneur vers un site receveur, l’adaptation du procédé d’enrobage s’est avérée primordiale. Ce projet présentait deux défis majeurs. D’une part, les équipements utilisés pour l’enrobage du produit étaient différents entre le site donneur et le site receveur : le site donneur utilisait des turbines de dragéification automatisées tandis que le site receveur disposait de turbines de dragéification conventionnelles ainsi que de turbines de pelliculage. D’autre part, l'uniformité de masse des dragées était un paramètre crucial à respecter puisqu’enregistré dans le dossier d’autorisation de mise sur le marché du produit. Cependant, garantir une uniformité de masse s’avère complexe avec des turbines de dragéification conventionnelles, car ce procédé, manuel, est tributaire du savoir-faire et de l’expertise de l’opérateur. Or, pour les médicaments microdosés, la maîtrise de l’uniformité de masse est primordiale puisque des variations mineures peuvent altérer leur dosage et compromettre leur efficacité thérapeutique. L’objectif de cette thèse est d’étudier la méthodologie d’adaptation du procédé d’enrobage aux contraintes industrielles du site receveur, tout en veillant à respecter les exigences réglementaires du produit X. La réalisation des lots d’essai réalisés y est détaillée, les problématiques rencontrées sont exposées, et les solutions apportées lors de la validation sont présentées avec leurs résultats.As part of the industrial transfer of a micro-dosed tablet (product X) from a donor site to a receiving site, it was essential to adapt the coating process. This project presented two major challenges. Firstly, the equipment used to coat the product differed between the donor site and the receiver site: the donor site used automated sugar- coating pans, while the receiver site had conventional sugar-coating pans as well as film- coating pans. Furthermore, the uniformity of the sugar-coated tablets weight was a crucial parameter to be respected, as it was recorded in the product's marketing authorization file. However, guaranteeing weight uniformity proves complex with conventional sugar-coating pans, as this manual process depends on the know-how and expertise of the operator. However, for micro-dosed drugs, control of weight uniformity is essential, since minor variaDons can alter their dosage and compromise their therapeutic efficacy. The aim of this thesis is to study the methodology for adapting the coating process to the industrial constraints of the receiving site, while ensuring compliance with the regulatory requirements for product X. Details of the trial batches carried out are given, the problems encountered are described, and the solutions applied during validation are presented, along with the results

    Effects of climate on leaf phenolics, insect herbivory, and their relationship in pedunculate oak (Quercus robur) across its geographic range in Europe

    An increase in biotic interactions towards lower latitudes is one of the most consistent patterns in ecology. Higher temperatures and more stable climatic conditions at low latitudes are thought to enhance biotic interactions, accelerating biological evolution and leading to stronger anti-herbivore defences in plants. However, some studies report contradictory findings, highlighting the need for further investigation into the underlying mechanisms. We used a combination of field observations and feeding trials in controlled environments to investigate the effect of climate on chemical defences and insect herbivory in pedunculate oak ( Quercus robur L.) throughout most of its geographic range in Europe, while controlling for physical defences. The concentration of lignin, flavonoids, and total phenolics increased significantly with temperature, whereas both field herbivory and weight of spongy moth ( Lymantria dispar L.) larvae were negatively influenced by temperature. Lignin concentration positively influenced the weight of spongy moth larvae whereas it had no effect on field herbivory. We found no evidence of strong positive relationships between insect herbivory and larvae growth with leaf defences. Our study underscores the complexity of plant–herbivore interactions along climatic gradients and highlights the need for further research to disentangle these intricate relationships.COntinental To coastal Ecosystems: evolution, adaptability and governanc
