    INTRODUCTION: Older adults with cancer are exposed to greater difficulties in carrying out their daily activities due to cancer itself, its treatment, or both. The aim of this study was to describe functional decline after cancer diagnosis and to investigate the determinants of this decline among older individuals with cancer. MATERIALS AND METHODS: Using the Gironde cancer registries, older subjects (≥65 years) with a diagnosis of cancer between 2005 and 2018 were identified in three prospective cohorts on aging. Functional decline was defined as an increase of 1 point for Activities of Daily Living (ADL), 2 points for Instrumental Activities of Daily Living (IADL), and 3 points for the overall score (ADL + IADL) between cancer pre- and post-diagnosis visits. Logistic regression models were used to identify determinants of functional decline among older subjects who underwent a post-diagnostic assessment. Additionally, multinomial logistic regression models were performed to account for individuals who had died prior to the post-diagnostic cancer visit. RESULTS: A total of 306 individuals followed-up after the cancer diagnosis were included (median age at cancer diagnosis: 83; 44 % female). Older age at cancer, low educational level, impaired initial functional status, and poor five-year cancer-related prognosis were significantly associated with functional decline across all three scores. Multinomial logistic regression analyses (n = 489) yielded similar results, but only cancer-related factors, specifically unfavorable vital prognosis, were associated with higher risk of death. DISCUSSION: Functional decline in older individuals with cancer is both multifactorial and multidimensional. Further studies are needed to disentangle the effects of cancer and aging

    Introduction Empirical data on the barriers limiting artificial intelligence (AI)'s impact on healthcare are scarce, particularly within the Canadian context. This study aims to address this gap by conducting a scoping review to identify and evaluate AI algorithms developed by researchers affiliated with Canadian institutions for patient triage, diagnosis and care management. The goal is to identify characteristics in the developed AI algorithms that can be leveraged for a better impact.Methods and analysis A scoping review will be conducted following the JBI Methodology for Scoping Reviews and reported following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews guidelines. Relevant literature will be identified through comprehensive searches of MEDLINE (PubMed), CINAHL (EBSCO) and Web of Science (Clarivate) databases, combining keywords related to AI, clinical management and the Canadian context. Studies published after 2014, in English or French, that discuss AI algorithms developed for patient triage, diagnosis or care management by researchers affiliated with Canadian institutions will be included. Data from the selected articles will be extracted and analysed descriptively, and findings will be presented in tabular form accompanied by a narrative summary.Ethics and dissemination Ethical approval is not required for this study as it involves the review of publicly available literature. The scoping review is expected to be completed by November 2025. The findings will be disseminated through publications in peer-reviewed journals and presentations at conferences focused on AI and healthcare practice

    The diagnostic and treatment approaches for schistosomiasis in individual patients, outside endemic areas, are not standardised. This study aimed to appraise the reference documents that the experts from the TropNet and GeoSentinel networks use in practice as guidance for the clinical management of their patients with (suspect) schistosomiasis. We systematically appraised the following data from the referenced guidance documents: i) document type, ii) case definitions, iii) diagnostic techniques envisaged; iv) treatment recommendations; v) follow-up recommendations; vi) screening recommendations, and vii) symptom-based diagnostic suspicion. Twenty-two of the 30 responders (73.3 %) indicated 19 reference documents, three of which were WHO material not intended for individual clinical management. Only 4/19 (21.1 %) documents were national recommendations; no international guideline was indicated. Case definitions were explicitly presented in only one document (1/19; 5.3 %). Diagnostic tools were detailed in 11/16 (68.8 %) and follow-up guidance in 8/16 (50 %) documents. Treatment guidance was provided in 14/16 (87.5 %) documents. Heterogeneity in clinical guidance was evident, although with noticeable overlap at least for chronic schistosomiasis. This confirms the need to formalise case definitions, which should be used to design trials to rigorously assess diagnostic tools and treatment schemes, and eventually come to harmonization of clinical management guidance

    Speaking is affected under dual-task conditions and studies have shown an impact on lexical and on phonological processes even in single word production. In the present study, we aimed at investigating if dual-task interference on lexical and phonological encoding is modulated by the linguistic nature of the concurrent task and in particular to determine whether the increase of phonological errors observed in previous studies using auditory syllables as concurrent stimuli is due to the phonological overlap between the tasks. Patients with aphasia (PWA, Experiment 1) and neurotypical individuals (Experiment 2) underwent a picture naming task and an auditory detection task of non-verbal sounds of daily life under single and dual-task conditions with different stimulus onset asynchronies (SOAs), namely +150 and + 300 ms, likely targeting lexical processes and phonological encoding. In both groups, naming latencies were slower under dual-task conditions at each SOA and in PWA, phonological errors increased with sounds of daily life presented at SOA +300. The results on errors replicate what was observed previously in studies using syllables as concurrent auditory stimuli in PWA. To investigate whether the increase of errors is larger with syllables relative to non-verbal sounds independent of the clinical condition, another neurotypical group performed a dual-task paradigm with syllables as concurrent auditory stimuli (Experiment 3). RESULTS: showed an increase of phonological errors at late SOA also in neurotypical participants, thus confirming the impact of concurrent syllables on phonological errors independently of the population. The results indicate that non-verbal auditory stimuli in the concurrent task impact phonological errors, although to a lesser extent than syllables, suggesting that the increase of errors in previous studies cannot be entirely attributed to the phonological overlap between the tasks

    Genome-wide association study dissects the genetic architecture of progesterone content in Persian walnut leaves (Juglans regia L.)

    Progesterone (P4) is an endogenous sex steroid hormone involved in the ovulatory cycle and pregnancy of animal species. In sheep and goats, P4 analogues are used to induce synchronized ovulations and oestrus behavior of the females. In humans, P4 from chemical synthesis is used to treat peri-menopausal disorders. However, such molecules are released into aquatic environment and can be a source of pollution, are prohibited in organic farms and go against the trend of "naturality" in animal production as well as in human health. A natural alternative may consist in the extraction and use of P4 in plants. Mammalian hormones were discovered in an increasing number of plant species, including walnut leaves that contain high levels of P4. We compared the content of P4 in leaves of 170 accessions of Juglans regia from the walnut germplasm collection of INRAE Prunus-Juglans Biological Resources Center previously genotyped using the Axiom ™ J. regia 700 K SNP array. We conducted a genome-wide association study (GWAS) using multi-locus models. When collected in October, P4 content goes from 34,1 to 287,5 mg/kg dry weight of leaves. The two laciniate accessions have the largest P4 content. We identified seven significant marker-trait associations on chromosomes 1, 2, 3, 6, 7, 15 and 16, and a candidate gene involved in the metabolism of sterols, precursors of plant steroid hormones. Our results raise the huge variability of P4 content within J. regia and propose a candidate gene which may have a role in the control of this variability, opening the way to a potential use of walnut P4 by the pharmaceutical industry towards more natural source of chemical compounds.</div

    A full Monte-Carlo resolution scheme of a photon-phonon model for conducto-radiative heat transfer in heterogeneous media

    In this paper, we tackle the problem of solving the system of radiative heat transfer coupled with thediffusion equation for material temperature with a full Monte-Carlo (MC) scheme in heterogeneousmedia, i.e. possibly containing discontinuities of thermophysical material parameters. MC schemesbased on Brownian paths for the diffusion equation in layered media are known to present someissues: corrections at the interface between media are usually mandatory or must be replaced withcomplex samplings. In this paper, based on the observations that transport models are easily solvedby MC methods in layered media, and that state-of-the-art MC schemes capture the diffusion limit,we suggest to build a phonon like model and to solve it with an MC scheme. For capturing thediffusion limit, physical constraints are explicited and enforced within the MC resolution. Theseconstraints can even help revisiting the Brownian paths for efficient corrections. Numerical resultsare provided in different regimes in order to highlight the relevance of the strategies suggested inthis paper
