Situations d'exposition aux pesticides et machinisme agricole : quelles responsabilités ?

Introduction: (Pre-)clinical studies have not ruled out a potential risk of second primary cancer (SCP) under the effect of some new androgen receptor pathway inhibitors (ARPIs), especially enzalutamide (ENZ).Methods: Using the French health reimbursement claims database (Système National des Données de Santé), we designed a case-control study nested in a 2013-2020 cohort of new users of androgen-deprivation therapy. The cases were patients with a first diagnosis of SPC, identified beyond 12 months following cohort entry and up to December 31st, 2021; up to 10 controls were matched per case, based on age and cohort entry date. The main analysis focused on patients who had not switched to a different ARPI. Applying a one-year lag time, we determined the most frequent and longest cumulative exposure patterns to abiraterone (ABI) or ENZ and estimated the odds ratios.Results: The cohort comprised 147 092 patients, including 7928 cases and 78 554 controls eligible for analysis. The SPCs mainly involve the digestive organs, the urinary tract, or the lungs. Recent and short exposure to ENZ was associated with SPC: OR 1.7, 95% CI [1.2-2.4]. Recent one full year of exposure to ABI, as well as full-year plus part of the second year, was associated with SPC: OR 1.8 [1.2-2.7] and 2.3 [1.3-4.0], respectively.Discussion/conclusion: SPC cases were mainly observed among recently exposed patients, which could be linked to a detection bias. The insufficient number of patients exposed over many years means that no definitive conclusions can be drawn

BACKGROUND: After an anterior cruciate ligament reconstruction (ACLR), mounting evidence suggests that open kinetic chain (OKC) strengthening is safe, reduces the risk of anterior knee pain, and significantly improves the quadriceps strength. However, clinicians are reluctant to use OKC knee strengthening exercises mainly due to the strong beliefs that they might increase graft laxity. The objective of this scoping review is to identify the key criteria employed in the scientific literature for the safe introduction of OKC quadriceps strengthening following ACLR. METHODS: A scoping review of the literature was conducted on the online databases MEDLINE (PubMed), ScienceDirect, Embase and CINAHL Library online. Data regarding time-based criteria and/or clinical based criteria allowing OKC exercises introduction following ACLR were searched for. Only studies involving patients who performed quadriceps strengthening using any type of OKC exercises were included, regardless of the type, resistance location, load magnitude, type of muscle contraction, knee range of motion, or duration of the strengthening protocol. RESULTS: Twenty-six studies met the inclusion criteria. Twenty-one employed time-based criteria for the introduction of OKC exercise. The median time from when OKC was permitted was 15 postoperative days (range 1-270 days), while the mean time was 31.6 ± 56.7 postoperative days. In 30.7% of the studies additional clinical examination components were used. These components included range of motion (0-100°), numeric pain scale score < 2 or 3, absence of joint effusion (assess by the stroke test), full knee active extension (assess by the straight leg raise), and walking without crutches for the decision-making regarding OKC exercise introduction. CONCLUSION: Less than one study in 3 reported clinical criteria for the introduction of OKC exercise. This highlights the absence of consensus among surgeons and physiotherapists, thereby hindering their ability to make informed decisions based on scientific evidence. Although the use of OKC exercise appears to be safe, precautions to maintain the integrity of the surgical repair need to be implemented. The establishment of valid criteria is crucial to support evidence-based decision-making

Background: Adverse childhood experiences (ACEs) may negatively affect prenatal mental health. However, the use of a cumulative ACEs score may obscure the identification of which specific types of adversity are most strongly associated with unfavorable mental health outcomes. Aim: This study aims to evaluate the association between ACEs and prenatal symptoms of depression and anxiety using a cumulative score, a person-centered approach, and the dimensional model of adversity and psychopathology (DMAP). Methods: Data were collected from 1887 pregnant women in the French Etude des D & eacute;terminants du d & eacute;veloppement et de la sant & eacute; de l'ENfant (EDEN) cohort. To operationalize our exposure, we calculated a cumulative ACE score, threat and deprivation scores, and conducted latent class analysis (LCA). Depressive and anxious symptoms were assessed with the Center for Epidemiologic Studies-Depression Scale (CES-D) and the State-Trait Anxiety Inventory state subscale (STAI-S) questionnaires, using cutoffs of 16 and 38 indicating high symptoms. Participants were categorized into four outcome groups: (1) no symptoms, (2) high depressive symptoms only, (3) high anxious symptoms only, and (4) comorbid high symptoms. Multinomial regressions were performed. Results: LCA identified three ACE classes: low-risk, family discordance, and multidimensional adversity. Women reporting two or more ACEs had higher odds of depressive and comorbid symptoms, compared to those with zero ACEs. Compared to the low-risk class, women in the family discordance class had increased odds of high depressive symptoms (adjusted odds ratios [aOR] 95% confidence interval [CI] = 1.80 [1.33, 2.56]) and comorbid high symptoms (aOR [95% CI] = 2.04 [1.43, 2.89]). Threat experiences were significantly linked to high depressive symptoms (aOR [95% CI] = 1.48 [1.22, 1.79]) and comorbid high symptoms (aOR [95% CI] = 1.53 [1.25, 1.87]). Conclusion: Using the DMAP and LCA approaches, we found that ACEs related to the familial environment and relationships during childhood were most strongly associated with prenatal high depressive and comorbid symptoms. This highlights the importance of operationalizing ACEs beyond a cumulative score to better capture their role in the development of prenatal mental health difficulties
Generation of metabolomic-informed models of metabolism for microbial communities
La génération de réseaux métaboliques à l'échelle du génome est devenue une analyse de routine pour des organismes individuels ou des communautés. Cependant, ces réseaux métaboliques générés automatiquement sont incomplets car ils sont construits sur la base de la combinaison de l'annotation des gènes et des réactions disponibles dans des bases de données génériques (Metacyc, BIGG, ModelSEED...). Ces bases de données sont orientées vers des organismes bien connus ou des organismes modèles et passent à côté de fonctions importantes du métabolisme secondaire. Nous proposons de combiner l'analyse de données métabolomiques, la modélisation métabolique et l'annotation métabolique et la modélisation métabolique et l'annotation minière pour construire des modèles de haute qualité du métabolisme microbien avec l'objectif à long terme d'une meilleure compréhension des communautés microbiennes. En termes d'application des méthodes aux communautés microbiennes végétales, nous espérons que les modèles nouvellement développés permettront de mieux comprendre le processus de recrutement microbien par la plante : fonctions métaboliques impliquées, micro-organismes associés à ces fonctions.The generation of genome-wide metabolic networks has become a routine analysis for individual organisms or communities communities. However, these automatically generated metabolic networks are incomplete because they are constructed by based on the combination of gene annotation and reactions available in generic available in generic databases (Metacyc, BIGG, ModelSEED...). These are oriented towards well-known organisms or organisms or model organisms and miss out on important functions secondary metabolism. We propose to combine metabolomic data analysis, metabolic modelling and annotation metabolic modelling and annotation mining to build high-quality models of high quality models of microbial metabolism with the long-term aim of better understanding of microbial communities. In terms of application of the methods to plant microbial communities, we hope that the plant microbial communities, we hope that the newly developed models will provide a better understanding of the process of microbial recruitment by the plant: metabolic functions involved, micro-organisms associated with these functions.Computationel models of crop plant microbial biodiversit
Children’s participation in decision-making in the context of child protection
Cet article interroge les expériences citoyennes des enfants sous le prisme de leur participation aux prises de décisions selon une approche psycho-sociale et développementale, dans le contexte de la Protection de l’Enfance et de la recherche en psychologie. La participation est abordée selon un triple versant théorique, éthique et méthodologique, à partir d’une recherche encours en psychologie du développement, conduite auprès d’enfants placés en famille d’accueil et âgés de 6 à 12 ans, ainsi que des professionnels en charge de leur placement. Le premier objectif de cet article est d’examiner les modèles théoriques de la participation afin d’en proposer une conception psycho-sociale et développementale. Le second objectif vise à présenter les enjeux posés par la participation des enfants, dans les prises de décisions, aux professionnels de la Protection de l’Enfance. Enfin, le troisième objectif consiste à aborder les défis éthiques et méthodologiques qui se posent aux chercheurs, dans le cadre d’une recherche qualitative en psychologie visant à favoriser la participation des enfants tout au long du processus de recueil des données. En conclusion, sont proposées des pistes de réflexions sur les différentes manières de prendre en compte la perspective des enfants et de les impliquer dans les processus décisionnels dans les pratiques de terrain et dans la recherche.This article examines children’s experiences of citizenship through the prism of their participation in decision-making, using a psycho-social and developmental approach in the context of child protection and psychological research. Participation is approached from a threefold theoretical, ethical and methodological perspective, based on developmental psychology research conducted with foster children aged between 6 and 12 and the professionals in charge of their placement. The first aim of this article is to examine theoretical models of participation in order to propose a psycho-social and developmental conception of participation. The second aim of this article is to present the issues raised by children’s participation in decision-making for child welfare professionals. Finally, the third aim of this article is to discuss the ethical and methodological challenges facing researchers in the context of qualitative psychological research aimed at encouraging children’s participation throughout the data collection process. In conclusion, we suggest some perspectives about the ways of taking children’s perspectives into account and involving them in decision-making processes in field practice and in research
