    National Health Insurance Scheme: Internal and External Barriers in the Use of Reproductive Health Services among Women

    Lack of familiarity among the community, medical workers, and administrative staff regarding reproductive health services covered by Badan Penyelenggara Jaminan Sosial (BPJS) or the National Health Insurance (NHI) in Indonesia remained a problem. Therefore, this resulted in sub-optimal use of the medical services, as shown by surveys from the Women's Health Foundation for three consecutive years (2015-2017). This qualitative study was conducted with a Rapid Assessment Procedure design in three cities within Indonesia: Padang Pariaman, Manado, and Kupang. Data were collected through IDIs (n = 47informants) and 6 FGDs (7 persons/group). Participants also consisted of NHI RHS users (mothers and young women), administrative officers at health facilities, medical services providers, and officials related to the NHI assistance. Data were managed using NVivo version 2.0 software, accompanied by thematic analysis. The internal barriers in NHI use included inadequate knowledge of RHS covered by NHI, and a culture of shame in informants. External barriers included additional costs for medicines not covered by NHI, the dissatisfaction of health services provided by medical workers, busy work and household activities, and lack of women's role in decision-making within families, which related to reproductive wellness

    A Path Analysis Model for Explaining the Factors Influencing Wearing a Mask among Commuting Workers Using Commuter Line Bogor-Jakarta

    Jakarta is one of the most air-polluted cities in the world, which can increase the health risk of commuting workers exposed to pollutants. This study aimed to determine the factors that directly and indirectly affect healthy behavior (wearing a mask) for commuters using the Bogor-Jakarta Commuter Line. Furthermore, a total of 155 respondents participated and the data collected were analyzed using descriptive and path analysis. The commuters aware of the hazard potential related to air pollution during commuting were 137 people (88.4%). While 104 people (67.1%) were aware of the good perception of pollution and 125 uses masks during commuting (80.6%). The healthy behavior in commuting workers using Commuter Line was affected by some factors, both directly and indirectly. The knowledge and commuting experience toward healthy behavior were two variables that significantly influenced on healthy behavior. The knowledge was also the only mediated variable with a significant indirect effect of education on healthy behavior

    Fulfilment of Minimum Acceptable Diet as Dominant Factor in Wasting in Children Aged 6–23 Months in Central Jakarta, Indonesia, 2019

    AbstractWasting is a form of acute malnutrition characterized by significant and rapid weight loss resulting from inadequate food intake and infectious diseases. This study was to investigate the factors associated with wasting in children aged 6–23 months and the dominant factor among these. The study followed a cross-sectional study design. A total sample of 261 children was selected using multistage random sampling from 13 integrated healthcare centres in six administrative villages located in three sub-districts of Central Jakarta. Data was collected by measuring body weight and height and questionnaires completed through interviews carried out by trained data collectors. This study shows that the prevalence of wasting in children aged 6–23 months in Central Jakarta is 6.9%. Based on logistic regression analysis, fulfillment Minimum Acceptable Diet (MAD) (OR=3.2 90% CI 1.1-9.5) was found to be the dominant factor in wasting after controlling by mother's level of education (OR=1.7 90% CI 0.7–4.2) and the level of family income (OR=2.9; 90% CI 0.9-8.9). This research shows that there is a need for improvement of maternal behaviour in providing appropriate care for children. Based on this research, we recommend that intervention on appropriate breastfeeding and complementary feeding practices be strengthened, especially for mothers with low levels of education and family income.AbstrakWasting adalah salah satu bentuk malnutrisi akut yang ditandai dengan penurunan berat badan yang signifikan dan cepat akibat asupan makanan yang tidak memadai dan penyakit infeksi. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan wasting pada anak usia 6-23 bulan dan untuk mengetahui faktor dominan di antaranya. Penelitian menggunakan desain studi cross-sectional. Sampel berjumlah 261 anak dipilih dengan menggunakan multistage random sampling dari 13 puskesmas di enam kelurahan yang berada di tiga kecamatan di Jakarta Pusat. Pengumpulan data dilakukan dengan mengukur berat badan dan tinggi badan serta pengisian kuesioner melalui wawancara yang dilakukan oleh pengumpul data terlatih. Hasil penelitian menunjukkan bahwa prevalensi wasting pada anak umur 6–23 bulan di Jakarta Pusat sebesar 6,9%. Berdasarkan analisis regresi logistik, pemenuhan Minimum Acceptable Diet (MAD) (OR = 3,2 90% CI 1,1-9,5) ditemukan menjadi faktor dominan kejadian wasting selah dikontrol oleh tingkat pendidikan ibu (OR = 1,7 90% CI 0,7– 4.2) dan tingkat pendapatan keluarga (OR = 2.9; 90% CI 0.9-8.9). Penelitian ini menunjukkan perlunya peningkatan perilaku ibu dalam memberikan pengasuhan yang tepat kepada anak. Berdasarkan penelitian ini, kami merekomendasikan agar intervensi praktik pemberian ASI dan MP-ASI yang tepat diperkuat, terutama bagi ibu dengan tingkat pendidikan dan pendapatan keluarga yang rendah

    Peran Organisasi Civil Society dan Belanja Program Penanggulangan TB (Tuberkulosis) di Indonesia: Studi Kasus PW (Persatuan Wilayah) Aisyiyah Aceh

    AbstrakJumlah kasus TB di Aceh tahun 2018 mencapai 8471 kasus; 240 (2.8%) diantaranya terjadi dikalangan anak-anak. Jumlah kasus real di masyarakat diperkirakan lebih tinggi dari angka tersebut. Keberhasilan penanggulangan TB dipengaruhi oleh banyak faktor, termasuk peran dari organisasi kemasyarakatan (civil society). Penelitian ini dilakukan untuk menganalisis peran organisasi kemasyarakatan dalam  penanggulangan kasus TB di masyarakat, dengan fokus pada belanja program.  Studi kasus dilakukan di PW (Persatuan Wilayah) Aisyiyah Aceh menggunakan metodologi evaluasi ekonomi, penelusuran dana health account kegiatan penanggulangan TB (tahun 2011-2013) dibandingkan dengan pencapaian target. Hasil studi menunjukkan PW Aisyiyah telah berperan positif dalam penanggulangan program TB di Aceh, terutama dalam community engagement melalui kelompok Peduli TB; Pengelola Kegiatan TB, Penjangkauan Kelompok Berisiko; Pemantauan Langsung Pasien TB dan TB MDR hingga pengembangan jaringan dan informasi pelayanan TB. PW Aisyiyah Aceh menggunakan anggaran Rp 844,- Juta per tahun dengan penggunaan untuk kegiatan promotif/preventif sebesar 22,3%. Penggunaan untuk kegiatan tidak langsung mencapai 77,5%. PW Aisyiyah Aceh memiliki peluang untuk mengembangkan skema pendanaan Public-Private mix, agar dapat berperan lebih maksimal lagi dalam program penanggulangan TB di masa yang akan datang. PW Aisyiyah juga dapat meningkatkan penggunaan anggaran untuk kegiatan upaya kesehatan masyarakat dengan pendekatan promotif dan preventif disamping kegiatan langsung yang selama ini menjadi misi utama organisasi kemasyarakatan PW Aisyiyah di Indonesia.AbstractThe number of TB cases in Aceh has reached 8471 cases in 2018; 240 (2.8%) cases occurred among children. The real cases in the community were estimated higher than this figure. The success of TB control was influenced by many factors, including the roles of civil society organizations. This research was conducted to analyze the roles of civil society organizations in controlling TB cases in the community. A case study was conducted at PW Aisyiyah Aceh using an economic evaluation methodology for tracking expenditures for TB program activities (2011-2013) compared to the targets. The study showed that PW Aisyiyah has played significant roles in controlling cases of TB in Aceh through many community engagement activities included the TB Care Group; the TB Community activities, High-Risk Group Outreach activities; direct TB and MDR-TB patients monitoring; assisted and providing TB care information and networking to the community. PW Aisyiyah Aceh has spent budgets Rp. 844 million per year for TB program activities with 22.3% spent on promotive/preventive activities, 77.5% spent on indirect activities. To maximize and sustain the roles of PW Aisyiyah Aceh as a civil society organization in controlling the TB program, it is strongly suggested to explore other sources of budgets, including exploring the potential of collaborating Public-Private mix funding mechanism. PW Aisyiyah is also suggested to increase budget spending on promotive and preventive public health activities in addition to curative activities which have been the main mission of Aisyiyah community organizations in Indonesia

    Strengthening the Strategic and Operational Response for Reducing COVID-19 Transmission in Indonesia

    Indonesia reported the first two cases of COVID-19 from Depok City, West Java, on March 2, 2020. This study aimed to evaluate the strategic response for reducing the COVID-19 transmission which mainly comprised case management, large-scale social restrictions, including micro-scale social restrictions, and the development of drugs and vaccines. The data were collected from the Indonesian Government’s official websites and the latest information from March 2020 to May 2021. Furthermore, a logical framework approach and a theory of change were used to describe, evaluate, and strengthen the strategic response. The current strategic response has not reduce the COVID-19 transmission. As of May 30, 2021, 1,879,730 confirmed cases with 101,639 active cases,1,663,998 recovered, and 50,404 deaths have been reported from 34 provinces. The case management faced a high positive rate and case fatality. Furthermore, the large-scale social restrictions have not increased public awareness and behavior practice on the prevention and control. Currently, there is no cure, and the vaccination needs more time to complete. Therefore, strengthening the current strategic response needs more testing, contact tracing, better quality treatment, community education for behavior change, and effective vaccination

    Strategy for Diagnosing Breast Cancer in Indonesia during the COVID-19 Pandemic: Switching to Ultrasound-Guided Percutaneous Core Needle Biopsy

    In this era of COVID-19, suspected breast cancer patients experience delay in diagnosis due to the fear of contracting the virus and reduction of non-COVID-19 health services. Furthermore, it may lead to potential increase in the incidence of advanced cancers in the future. Ultrasound-guided (US-guided) percutaneous core needle biopsy (CNB) is a great option for the diagnosis of cancer but it is poorly utilized. This study aimed to prove that the US-guided CNBis accurate when performed in a local setting and a potential solution for diagnosing breast cancer patients in this pandemic. In addition, it was a single health center cross-sectional study, and the participants were all breast cancer patients that had US-guided CNB from 2013-2019. The pathology results from US-guided CNB were compared to specimens from post-CNB surgeries. The data were collected from medical records and the immunohistochemistry (IHC) examinations were carried out for malignancy. There were 163 patients who were included in this study, 86 had malignancies and 77 had benign tumor reported in their CNB results. The US-guided CNB had 100% sensitivity and specificity compared to surgery. With its lower cost, time usage, and patient exposure to the hospital environment, US-guided CNB should replace open surgery biopsy for diagnosing suspicious breast cancers during the pandemic in Indonesia

    Proses Perubahan Perilaku Berhenti Merokok: Studi Kualitatif Mengenai Motif, Dukungan Sosial dan Mekanisme Coping

    Latar Belakang. Tingginya jumlah perokok sebenarnya juga diiringi dengan tingginya keinginan untuk berhenti merokok, namun tidak semua berhasil berhenti merokok.Tujuan. untuk menggali peran motivasi, dukungan sosial, mekanisme coping dalam upaya berhenti merokok.Metode. Penelitian kualitatif dengan rancangan penelitian fenomonologi. Teknik snowball sampling dan rekrutmen via whatsapp digunakan untuk mendapatkan informan, dan dipilih menggunakan purposive sampling. Data dikumpulkan melalui wawancara mendalam kapada 18 orang yang terdiri dari 5 orang (1 perempuan 4 laki-laki) yang sudah berhenti merokok 6 bulan sampai 2 tahun, 4 orang yang sedang berhenti merokok (<6 bulan) dan 9 orang sebagai significant others. Keabsahan data melalui triangulasi, member checking dan peer debrieving.   Hasil. Faktor sosial merupakan penyebab yang mendominasi untuk merokok. Motif kesehatan adalah motif utama untuk berhenti merokok. Dukungan untuk berhenti merokok diantaranya dukungan secara emosional dan instrumental.Kesimpulan. Perokok berhenti merokok karena motif kesehatan seperti ingin lebih sehat.  Motif non kesehatan diantaranya haram dan pengeluaran membeli rokok lebih banyak dari kebutuhan untuk makan. Coping kognitif seperti mensugesti diri melalui perubahan mindset sebagai salah satu strategi yang dapat dilakukan untuk mengendalikan perilaku merokok. Dukungan sosial hanya sebagai penguat atau moderator. Coping merupakan pengendali utama dalam berhenti merokok. Dukungan sosial sebagai moderator dalam proses berhenti merokok. ABSTRACTIntroduction. A high number of smokers aligned with smoking cessation eagerness, but not all succeed.Objective. to explore the motive, social support and coping mechanism for smoking cessationMethods. Qualitative research with phenomenology research design. We did the snowball sampling technique and participants’ recruitment via WhatsApp and Purposive sampling. 18 in-depth interviews consisted of 5 participants (1 woman and 4 men) who quit smoking within the past 6 months to 2 years, 4 participants who are quitting smoking (<6 months) and 9 people as significant others. Data validation was through triangulation, member checking and peer debriefing.Results. Social factors are the dominant cause of smoking. Health motives are the main motives for quitting smoking. Support for quitting smoking includes emotional and instrumental support Conclusion. Smokers’ motivations to quit due to health reasons such as a better level of health. Non-health reasons are religious prohibition (haram) and cigarette expenses higher than primary (food) expenditure. Research also found cognitive coping such as personal suggestion through mindset change, is one of the strategies to control smoking behavior. Social support as a booster or moderator. Coping is the primary controller in smoking cessation. It’s strengthened by personal willingness. Social support acted a moderator.

    Corporate Governance Policy Implementation Analysis using D. Wayne Taylor Theory. Case Study: Dental and Oral Hospital by Faculty of Dentistry Universitas Indonesia (RSKGM FKG UI) in 2019

    Corporate Governance is a term that is related to the direction and control of an organization, institution, or function, to fit the goals and expectations of decision makers. This research aims to conduct an analysis of corporate governance using D. Wayne Taylor’s theory by conducting a case study at the Dental and Oral Hospital by Faculty of Dentistry Universitas Indonesia (RSKGM FKG UI). The design of this research is qulitative analysis with methods of collection data through document review and in-depth interview. The results of the research are reflected in nine principles that are recognized by six incompatible principles, two principles that are appropriate, and one principle that cannot be seen in accordance with the requirements in RSKGM FKG UI that have not supported yet.     Hubungan Faktor Individu, Lingkungan dan Komputer dengan Kejadian Computer Vision Syndrome (CVS) pada Staf Polres Metro Jakarta Pusat Tahun 2020

    Latar Belakang. Computer Vision Syndrome (CVS) merupakan penyakit yang muncul sejak perkembangan teknologi diabad ke-21 dengan tingkat prevalensi kejadian secara global sebesar 60 juta dan kerugian Rp192 trilliun setiap tahunnya. Mengetahui hubungan faktor individu, lingkungan dan komputer serta faktor risiko dominan dengan kejadian CVS pada staf POLRES Metro Jakarta Pusat tahun 2020. Metode. Penelitian ini menggunakan pendekatan studi potong lintang dengan jumlah sampel 92 staf kepolisian yang bertugas di markas besar POLRES Metro Jakarta Pusat dan waktu penelitian pada bulan Juni 2020. Pengumpulan data dilakukan dengan instrumen kuesioner dan pengukuran lingkungan langsung menggunakan lux meter dan RH index. Analisis deskriptif dengan melihat frekuensi serta proporsi, uji kai kuadrat memunculkan nilai odd ratio dan uji regresi logistik ganda. Hasil. Hasil penelitian menunjukan tidak ada hubungan yang signifikan pada semua variabel dari faktor individu, lingkungan dan komputer dengan kejadian CVS. Walaupun begitu, terdapat empat variabel yang menjadi faktor risiko dengan kejadian CVS diantaranya kelainan refraksi (OR=1,65), perilaku merokok (OR=1,89), kelembaban (OR=2,50) dan jenis monitor (OR=1,11). Analisis multivariat menunjukan kelembaban ruang kerja memiliki hubungan yang signifikan dengan kejadian CVS (p=0,04) dan merupakan faktor risiko dominan (OR=2,50). Kesimpulan. Terdapat empat faktor risiko yang dapat menyebabkan kejadian CVS pada staf POLRES Metro Jakarta Pusat.Kata kunci: Computer Vision Syndrome, Kesehatan Lingkungan Perkantoran, Kompute

    Latar Belakang. Sick Building Syndrome (SBS) merupakan salah satu masalah yang sering dialami oleh penghuni di gedung perkantoran. SBS dapat disebabkan karena kualitas udara dalam ruang dan karakteristik individu. Penelitian ini bertujuan untuk mengetahui hubungan antara jumlah total koloni bakteri di udara dalam ruang dengan kejadian SBS di Arsip Nasional Republik Indonesia. Metode. Digunakan desain studi cross sectional, variabel independen yaitu jumlah total koloni, variabel confounding yaitu suhu, kelembaban relatif, pencahayaan, usia, jenis kelamin, masa kerja, riwayat alergi dan kebiasaan merokok. Hasil. Analisis statistik memberikan hasil proporsi kejadian SBS pada pegawai di Arsip Nasional Republik Indonesia Tahun 2019 sebesar 60%. Simpulan. Dari 9 variabel yang diuji, hanya variabel usia (OR= 0,43; 95%CI= 0,189-0,969) yang berhubungan signifikan secara statistik.Kata Kunci:Sick Building Syndrom, Bakteri, Kualitas Udara dalam Ruan
