    THE MODERN APPROACH TO THE COMPLEX THERAPY OF DEMODICOSIS

    Demodicosis is one of the most common diseases of the skin. Despite the large number of scientific publications on this issue the question of the etiology and pathogenesis of this disease still remains unresolved and the development of more effective methods of treatment and prevention has not only medical but also social significance as patients preserving the working ability have actually long enough to be on outpatient and even inpatient treatment. In addition the long existence of the rash on the face that is the "business card" leads to the secondary sometimes severe neurotic disorders that results in reducing abilities, isolation, unwillingness to be in a team, family, etc. We investigated the effect of complex therapy which had been conducted by means of antiparasitic, immunomodulatory, anti-inflammatory and desensitizing drugs in patients with demodicosis, as well as mandatory adjustment of changes in the hepato- panсreato-duodenal system. We examined 109 patients with demodicosis from 18 to 39 years old and 10 healthy individuals of the corresponding age and sex. It was established that the effectiveness of treatment of demodicosis based on the application of albendazole in the complex treatment along with immune-modulating therapy and hepatoprotection increased significantly. There has been a reliable rapid regression of clinical symptoms in most patients with demodicosis

    SOMATO-PSYCHIC DISORDER CLINICAL CASE IN A FEMALE PATIENT WITH MULTINODULAR EUTHYROID GOITER

    oai:ojs.localhost:article/13The article presents a clinical case of somato-psychic disorder in a patient with multinodular euthyroid goiter. The patient was diagnosed with thyroid cancer on outpatient basis and was suggested urgent surgical treatment. After receiving information about cancer pathology the patient developed symptoms of hypernosognostic neurotic syndrome with manifestations of obsessions and hystero-hypohondriasis. The patient was focused on the rigid hypochondria with detailed statement of the slightest signs of bodily distress. Therefore, the above example demonstrates difficulty in selecting the tactics of management for such patients due to development of symptoms primarily determined by psycho-emotional tension. This category of patients should be followed not only by endocrinologists, surgeons, but the multidisciplinary team, including psychiatrists, and psychotherapists, since in case of combined impact of a number of adverse factors, the reaction to the disease can become so extreme that its management in the early stages of therapy seems no less important than the direct treatment of somatic condition

    The aim of research: to study the causes and mechanisms of postoperative intestinal paresis on the base of assessment of genetic determinism of neurohormonal processes which regulate the contractile ability of the intestines, to determine their role for the choice of treatment tactics. The study presents data about connection between variants of SERT gene, which regulates the reuptake of serotonin, and its concentration in blood plasma and the probability of occurrence of postoperative intestinal paresis. This made it possible not only to predict the occurrence of postoperative disorders of motor-evacuation function of the intestines, but also to improve the algorithms of prevention and pathogenetic based treatment

    Treatment of class III is a current problem in orthodontics that requires constant improvement of its methods, development of new or modifications of known methods and techniques. We have developed and studied the modification of removable functionally-directing orthodontic appliances for treatment of Class III, which consists of a plastic base, vestibular arc, retaining clasps, ramp, which is connected with the base by means of two torsion springs. Its usage ensures a prolonged contact of ramp with the teeth.We studied two types of club-shaped springs (torsion springs): one spring, which create an amortization effect during the action of the ramp, but do not change its inclination angle and second one – spring that seek to increase the angle of the ramp inclination due to the disclosure of its curl

    ADHESIVE PROPERTIES AND SENSITIVITY TO ANTIFUNGAL DRUGS OF CANDIDA ALBICANS, WHICH ARE RELEASED FROM PATIENTS OF GASTROENTEROLOGICAL PROFILE WITH CANDIDIASIS OF THE MUCOSA OF THE UPPER PART OF THE DIGESTIVE TRACT

    Candida albicans were released from 146 loci from 89 patients in order to perform the investigation. The fourth stage of contamination by Candida albicans in the scrape from the tongue and candidiasis of the esophagus or /and the stomach during microbiologic investigations of the biopsy materials were the main criterion for this research. HIV-infected patients and patients in whom antibacterial therapy was done less than in four weeks were excluded from the investigation. Microorganisms were taken from the next biopsy materials: oropharyngeal area included 81, esophagus contained 30, stomach included 24 in general (the body of the stomach contained 14 and antrum included 10, gastric juice included 11. Adhesion evaluation was done by microtechnique in U-shaped microplates with formalized erythrocytes 0(І), Rh +. According to the results of the investigation, the released fungi in 51,37 % of cases were characterized as nonadherent ones, in 28,77 % of cases it was defined as low level of adhesiveness and only in 17,12 % of cases it was determined as average and high level of the adhesiveness. Adhesiveness increase was not associated with the increase of the contamination of the oral cavity and with the severity of the damage of the mucosa of the esophagus or /and the stomach. In 97,8 % of cases, strains of Candida albicans were sensitive to fluconazole, in 75,6 % of cases, strains were sensitive to itraconasole and in 24,4 % of cases they were determined as moderately sensitive and resistant that initiates fluconazole as the drug of the choice for the treatment of the candidiasis of the mucosa of the upper part of the digestive tract. So, received data determines the necessity to detect factors of dysfunction of antifungal resistance of the mucosa that detects the development of its candidal damage

    HEMORHEOLOGICAL FAILURE IN THE PATHOLOGY OF CARDIO-VASCULAR COMPLICATIONS IN PATIENTS WITH DIABETIC FOOT SYNDROME

    The literature that includes the study of cardiovascular complications in patients with diabetic foot syndrome was analyzed. The topicality of this problem is caused by the steady growth of diabetes mellitus morbidity among people. For today there are more than 170 mln people throughout the world with diabetes mellitus, among them 65–80 % have cardiovascular complications (myocardium infarction, acute disorder of brain blood circulation and so on). It is established for today, that pathogenesis of diabetic foot syndrome is multi-factor one and the development of purulent-necrotic stages of DFS is connected with the combination of different factors, especially microcirculation disorder. In this article we’ll consider the mechanisms of diabetic angiopathy development, the state of platelet-vascular hemostasis link in this category of patients

    ATHEROGENIC PARAMETERS AND CARDIAC INDICES IN PROGNOSTICATION OF DEVELOPMENT OF PRE-ECLAMPSIA AND ITS CARDIOMETABOLIC CONSEQUENCES

    Aim of the work. The study of interconnection between atherogenic parameters of the blood serum and cardiac indices during pregnancy with the risk of pre-eclampsia development and prognostication of remote cardio-vascular and/or metabolic pathology among pregnant. Materials and methods. The indices of lipid metabolism in the blood serum of 267 pregnant women were studied using diagnostic sets of reagents. The blood sampling was carried out in II (16-24 gestational weeks) trimester. The general cholesterol (GC) and triglycerides (TG) were determined by colorimetric and enzymatic method; high-density lipoproteins – HDL – by homogenous method of direct determination of HDL-cholesterol concentration in the blood serum without preliminary processing and centrifugation, low-density lipoproteins – LDL – by the direct method. VLDL concentration was calculated by Friedewald formula: LDL = TG/2,2.   For evaluation of GC-VLDL concentration TG/2,2 parameter is used, if TG concentration is expressed in mmol/l (or TG/5, if concentration is expressed in mg/100 ml). Cardiac indices were calculated taking into account the lipid parameters according to the correspondent mathematical formulas. Lactate and LDH concentrations were determined by optimized method using diagnostic reagents.  Lactate concentration was determined at the use of diagnostic set for determination of lactate concentration, and DLH was also determined by diagnostic set for determination of LDH (DL) activity by optimized method, elaborated taking into account the recommendations of German Association of clinical chemistry (DGKC) after correspondent calibration of automatic analyzer ACCENT-200 II GEN. Depending on the development of clinical presentation of PE 43 women were divided in groups with light and middle severity of course of this pathology. Results and discussion. Among the women with PE development was observed the essential changes in II trimester in the aspect of atherogenic parameters and cardiac indices mainly at the expense of progressing increase of TG in the blood serum (mmol/l: I group - 1,73 ± 0,14, II group - 1,86 ± 0,18 comparing with III control group - 1,51 ± 0,06. The gradual growth of cardiac (atherogenic) indices - AIP, CR and CА and connection between AIP and the levels of maternal serumal TG and arterial pressure were observed in women with PE development at the absence of such connection among the women of control group (p <0,05) . LDH level grew with the progressing of PE severity: for the women of I group LDH level was - 389 ± 1,6 U/l at 778 ± 0,25 in women of II group and 265 ± 0,19 in the control group (p <0,05). Conclusions. It was demonstrated, that hypertriglyceridemia at pregnancy is a predicator relative to PE development and together with progressive growth of atherogenic indices can be considered as prognostic factor for cardiometabolic pathology, especially in women with undergone severe forms of this pathology. It was established, that the increase of LDH level since the second trimester can also testify to the development of PE and remote cardiac pathology in this category of women

    PATHOMORPHOSIS OF EXTRAPULMONARY TUBERCULOSIS IN CHILDREN

    Aim. To study clinical and epidemiological aspects of pathomorphosis of extrapulmonary tuberculosis clinical forms. Materials and Methods. Retrospective analysis of 138 case histories in children aged to 15 years, obtaining extrapulmonary tuberculosis treatment in a specialized children's hospital during 1988-2015 was carried out. Among them, in 103 children, clinical forms of extrapulmonary tuberculosis combined with respiratory tuberculosis and in 35 children independent forms of extrapulmonary tuberculosis were found. Studied stage (1988-2015) was divided into 3 periods: I – 1988-1997, II – 1998-2007, III – 2008-2015. During 2008-2015, incomparison to previous periods, the number of cases of peripheral lymph node tuberculosis (PLN), meninges and central nervous system tuberculosis, bones and joints tuberculosis, and ocular tuberculosis decreased. In 2008-2015, the share of combined forms of meninges and central nervous system tuberculosis and ocular tuberculosis remained at 1998-2007 level. During 2008-2015, skin tuberculosis was not diagnosed. Nevertheless, in 2008-2015, in6.8±10.2 % of cases extrapulmonary tuberculosis combined with miliary tuberculosis, and in 5.8±10.4 % of cases the process characterized by lethal outcome, moreover 4.9±10.7% from them – in recent years. It is important that during period I, in 19.6±13.2 % of cases extrapulmonary tuberculosis combined with respiratory tuberculosis in reverse development phases, during period II – in 12.9±19.3 % of cases, during period III – in 23.8±21.2 % of cases. Conclusion. Despite the reduction, extrapulmonary tuberculosis combined with severe forms of respiratory tuberculosis, which led to lethal outcome in children in 5.8±10.4% of cases. Tuberculosis epidemiological situation worsening, reversion of severe forms of tuberculosis with the development of extrapulmonary one and several organs injuries in children shows the necessity of social, preventive and therapeutic measures intensification among children

    THE COMPLICATIONS OF DIFFERENTIAL DIAGNOSTICS OF HODGKIN LYMPHOMA

    In the article are presented the questions of classification, pathogenesis and clinical diagnostics of Hodgkin Lymphoma with soft-tissue paravetebral component. The difficulties of early diagnostics, expressed at the expanse by oncological process localization, not typical for this disease, are shown on clinical case. The symptoms, conditioned by the compression of organs by tumor masses depend on pathological process localization: intrahepatic and extrahepatic obstruction of bile duct is manifested by jaundice; obstruction of lymphatic ducts in groin zone or in zone of small pelvic – by legs edema; at tracheobronchial compression is observed the pant and hoarse breathing; pulmonary abscesses or caverns can take place as a result of infiltration of pulmonary parenchyma that can stimulate lobar consolidation or bronchopneumonia; paraplegia can be observed because of epidural invasion with spine cord compression; compression of sympathetic cervical recurrent guttural nerve by increased lymph nodes can cause Horner’s syndrome and laryngoparalysis; neuralgias can be the result of nerve-root incarceration. Differential diagnostics can take a long time that lowers the effectiveness of therapy and decreases chances for recovery because of special treatment delay. The setting of correct diagnosis and choice of adequate therapy raises the chances for recovery, even at spead stage of tumor process that is attained at Hodgkin lymphoma in most cases.&nbsp

    THE CLINICAL EFFICACY OF SUBLINGUAL ALLERGEN-SPECIFIC IMMUNOTHERAPY IN CHILDREN AGED 3-5 YEARS

    The aim of this study was to determine the efficacy of sublingual allergen–specific immunotherapy (SLIT) in Ukrainian children younger than 5 years old with allergic rhinitis and bronchial asthma sensitized to house dust mite allergens. Material and methods: Four hundred and fifty children aged 28 months up to 5 years with rhinitis or asthma were examined. One hundred and twenty five children sensitized to house dust mites Dermatophagoides pteronyssinus and/or Dermatophagoides farina were included. In vivo and in vitro tests were made with a standard inhalant allergens panel. Results: The high information value of molecular diagnostics methods applied prior to prescription of the given therapy in children is analyzed. It has been found that in children under 5 sensitized to allergens of house dust mites Dermatophagoides pteronyssinus and/or Dermatophagoides farinae the application of sublingual allergen–specific immunotherapy therapy allows gaining control over the symptoms of the disease during the first 6 months. Conclusion: The high safety of SLIT in children has been proven. Comparative analysis in the group of patients not receiving SLIT shows a high frequency of symptoms of the disease after “free-of-symptoms interval” against full or partial baseline therapy denial
