BROWN TUMOR OF BONE AS INITIAL MANIFESTATION OF PRIMARY HYPERPARATHYROIDISM
Primary hyperparathyroidism (PHPT) is characterized with primary hyperproduction of parathyroid hormone (PTH) by affected parathyroid glands and with a corresponding increase in concentration of extracellular calcium. Long-term development of PHPT through decades leads to calcium-related symptoms and distinct changes in the skeleton, kidneys, digestive tract and other organs. In the clinical picture of PHPT, granulomatous pseudo-tumors in bones are observed as a rare manifestation of the disease – these lesions, which could imitate malignancy, are called «brown tumors». The present publication that goes into details about a clinical diagnosis, in which a brown tumor became the primary manifestation of PHPT, may be useful for physicians across various fields
ELABORATION OF ORGANIZATIONAL ARRANGEMENTS ON THE IMPROVEMENT OF FUNCTIONING OF THE SYSTEM OF PHARMACEUTICAL SUPPORT OF SUDAN POPULATION
In the article is considered the problem of support of Sudan population with available pharmaceutical production. The aim of research is an analysis of the price and assortment policy realized by the different pharmaceutical structures, determination of the level of their functions on guaranteeing the availability of medicaments, elaboration on the base of received results the arrangements on optimization of the pharmaceutical support system. It was determined that not all subjects of Sudan pharmaceutical market responsible for the pharmaceutical support cope with such problems at the proper level. Thus in the private drugstores, in the net drugstores of the Ministry of defense «Alia» and in drugstores of the health insurance system the prices of medicament exceed the recommended ones in more than 2 times, an assortment also does not corresponds to both internal and international requirements. The received results indicate the unreasonable use of budget means by the separate state structures, the negative tendencies in the system of guaranteeing the availability of pharmaceutical help.It was established that the biggest suppliers of the medicaments in Sudan, excluding the state Centre of medicament support CMS cannot ensure the system of health protection with medicaments of the proper quality, assortment and the price rate.There were elaborated the organizational-economic approaches to optimization of the structure of medical support of Sudan that include the change of the flows of medicaments distribution. There was offered the structure that unites all inflows in the aforesaid CMS and then distributes resources according to the needs of organizations and regions.Introduction of the offered structures will allow improve the system of purchase and distribution of medicaments, make it transparent. The result of change will be the rise of physical and economic availability of medicaments, widening of assortment, decrease of the price of preparations, significant economy of budget resources
ALCOHOLIZATION IN THE STRUCTURE OF POSTSTRESS MALADJUSTMENT
Analysis of the literature data on alcoholic addiction, poststress maladjustment and its combination revealed the significant number of common node moments in pathogenic mechanisms of its appearing and realization. It concerns biological mechanisms that are common for both processes and its activation at one of them provokes the development of the other. First of all it is stem structures of brain and neuroendocrine mechanisms of central and peripheral regulation and its activation is cross for stressor reaction and alcoholization. There was revealed the leading role of stressor reaction mechanisms and its association with poststress maladjustment in formation of psychological alcoholic addiction. There was constructed hypothetical model of development and functioning of association pathogenetic mechanisms with formation of comorbidity of analyzed pathological states
THE STATE OF LIPID METABOLISM IN PATIENTS WITH ASPIRIN ASSOCIATED POLYPOUS RHINOSINUSITIS
Nowadays it is observed the wide prevalence of polypous rhinosinusitis, associated with intolerance to aspirin. From today positions the main principle of pathogenesis is in presence of “genetic block” of constitutive cyclooxigenase – the node enzyme of metabolism of arachidonic acid that leads to its active accumulation. It leads to the disorder of lipid metabolism and conditions the necessity to study its biochemical features.
Aim of research. To study biochemical parameters of the lipid exchange in patients with chronic polypous rhinosinusitis, associated with intolerance to acetylsalicylic acid by determination of the main metabolites.
Materials and methods. The results of laboratory examination of 20 patients with aspirin associated polypous rhinosinusitis are presented in the article. The control group included 7 practically healthy donors. There were studied the level of arachidonic acid in condensate of exhaled air, arachidonic, linoleic, eicosapentaenoic acids, phospholipids, malonic dialdehyde and oxide modification of protein in the blood serum.
Results. There were determined the changes of lipid exchange, which essence is in increase of the level of arachidonic acid in condensate of exhaled air and blood serum that leads to the decrease of the content of its main predecessors – linoleic and eicosapentaenoic acids and also phospholipids. The increase of arachidonic acid content stimulates the processes of peroxidation and leads to the increase of malonic dialdehyde content and oxide modifications of protein. The features of biochemical disorders testify to the proinflammatory direction of lipids metabolism.
Conclusions. The received data confirm the hypothesis of “genetic block” of arachidonic acid metabolism as the main pathogenetic link of polypous rhinosinusitis, associated with intolerance to aspirin, and allow clearly interpret the clinical image and the features of clinical course
THREE-DIMENSIONAL COMPUTER MODELLING AND STRESS-STRAIN ANALYSIS OF TEMPOROMANDIBULAR JOINT
The main purpose of the study is to build 3D model of temporomandibular joint and mandible using programs for modelling and design, research stress and strain in cartilage and bone of model depending on different contraction of chewing muscles (especially lateral pterygoid muscle). There were used such methods as computer tomography data, finite element method and analysis topography of lateral pterygoid muscle on cadavers. In results of study was obtained maximal stress in anterior and distal parts of articular disc between 1,4–3 MPa and 0,4–1 MPa. Thus, static loads of articular disc were different, depends on force of contraction lateral pterygoid muscle. Muscle disturbances (hyperfunctioning, atrophy etc.) should be considered in treatment of temporomandibular dysfunction and examined on magnetic resonance investigation if it is present in the same patient
THE METHOD OF TREATMENT OF PATIENTS WITH LOCALLY-ADVANCED BREAST CANCER
A method of care of patients with locally-advanced breast cancer consisting of using regional selective intraarterial chemotherapy in the schedule of the complex (palliative) treatment is presented. Results of treatment showed an advantage in comparison with the application of traditional methods of the breast cancer treatment. The methodology on intra-arterial introduction of chemotherapeutic agents developed at Donetsk Regional Antitumoral Center and University clinic of Odessa showed its undisputable contribution into development of modern oncology through decrease of primary tumoral locus, transition from inoperable state into the state at which it is possible to perform the radical volume of surgical interference to patient. The special attention is paid to development of new methods of treating BC patients (regional forms of disease) with unfavorable factors for forecast of tumor growth
THE USE OF RADIO-FREQUENCY ABLATION IN THE TREATMENT OF MALIGNANT TUMORS OF LUNGS AND PLEURA
Aim of the work. Analysis of the possibilities of the different methods of RFA of lung and pleura tumors.Material and methods. There were considered the different methods of radio-frequency ablations using FOTEK-150 apparatus in 186 patients with the malignant tumors of lungs and pleura.Results. In the result of use of the different methods of radio-frequency ablation the direct positive effect was attained in 91 % of observations. The complications were observed in 9 % of patients. This method must be used for patients with the malignant tumors of lungs and pleura when the radical surgical treatment is impossible.Conclusions. The radio-frequency ablation can be used as a palliative method of treatment of the malignant tumors of lungs and pleura
