Aspects of sexual life that are associated with the occurrence of depressive disorders and autonomic dysfunction among medical students of National Pirogov Memorial Medical University, Vinnytsya
Depressive disorders are widespread nowadays. First of all, this was affected by social isolation related to covid-19, and starting from 2022 – the war in our country. Depression and sexual life are connected and important, because a person with a depressive disorder cannot fully live a social and sexual life. As a result, the birth rate in the country and the population in general decreases. Therefore, the issue of studying the occurrence of depressive disorders in young sexually active and inactive people is extremely relevant.
The aim. The main objective of this work was to explore the different areas of sexual life of residents and its association with mental health. Additionally, differences in sexual life and mental health (depression) based on gender were also analyzed.
Materials and methods. An anonymous survey of 506 medical students of NPMMU, V has been conducted using a questionnaire that included 25 questions on various aspects of the sexual life of the respondents. The analysis of the obtained results of the study was carried out on the basis of the computer programme “Statistica 6.1” using nonparametric methods to evaluate obtained results.
The results. We found a statistically significant association between sexual activity and condition of mental health. Results regarding young adults, presented lower levels of sexual satisfaction and greater depression, whereas young women also exhibited higher levels of depression.
Conclusions. Sexual life has a positive effect on the mental and psychological health of respondents. This information should be conveyed to young people
The impact of the COVID-19 pandemic, online-learning on the state of visual system and psychological state of students
The aim of this study: to identify and analyze changes in the lifestyle and psychological sphere of students who are undergoing online training in quarantine isolation due to the COVID-19 pandemic.
Material and methods: 160 students were examined for the myopic refraction using the Duochrome Test, as well as shifts in the quality of vision as a result of online learning. All participants were also interviewed to ascertain living conditions and psychological status during domestic learning related to COVID-19 isolation.
Result: it was found that 42.5 % participants have myopia, most of them students of 1 and 4 courses (26.5 % and 30.9 %, respectively). 85.6 % respondents noted deterioration in health after long-term visual work with personal computer and/or smartphone. The leading symptoms associated with work on digital displays were: decreased visual acuity and binocular vision disorders (81.9 %), headache (75.6 %), back muscle pain (71.9 %), eyes redness (55.6 %), dry and itchy eyes (45 % of respondents). It has been established what 75.6 % respondents indicated deterioration in their psychological condition as a result of quarantine restrictions: 68.1 % – stress, 48.8 % – emotional instability (irritability), 51.9 % – sleep disorders (insomnia), 16.3 % – depression. It is determined that daily average working time in the conditions of domestic online learning was 6.4±2.6 hours (compared to 4.1±1.7 hours in the pre-quarantine period).
Conclusion: revealed deterioration in the quality of vision, general well-being and the psychological sphere of students who are undergoing online quarantine training due to the COVID-19 pandemic
Role of macrophages in the immunopathogenesis of adenomyosis
The aim of the research: to study pathophysiological mechanisms of adenomyosis development by determining the role of macrophages in the uterine microenvironment.
Materials and methods: a prospective study has been conducted on 35 women. There were 20 (57.1 %) who had adenomyosis of I degree. The control group consisted of 15 (34.3 %) gynecologically healthy women. The patients underwent general clinical, instrumental (ultrasound, hysteroscopy) examinations. Fragments of the uterine wall obtained by hysterorectoscopy were used for morphological study. The method of immunohistochemical determination of CD68+ and CD163+ macrophages was used to analyze the characteristics of phenotypic equivalents of M1 and M2 macrophages in uterine tissue samples.
Results: The increase in the number of macrophages in the myometrium of patients with adenomyosis revealed in this study, which is found in large numbers in the areas of infiltration of the stroma of myometrial cells in close association with the perivascular region, can be regarded as the basis of the mechanism for the formation of endometrioid heterotopia. Furthermore, distortion of the CD68/CD163 ratio of macrophages is characterized by proinflammatory shift.
Conclusions: The study's main result is an increase in the quantitative indicators of CD68+ macrophages associated with adenomyosis, which indicates an immunopathological process in adenomyosis
Prediction of one-year adverse clinical outcomes by macrophage migration inhibitory factor in stemi patients
Biomarkers have taken one of the first places as diagnostic and prognostic tools in ST-segment elevation myocardial infarction (STEMI) and are consequently widely used as predictors of short-term and long-term prognosis. One of the promising biomarkers for early cardiovascular outcomes prediction is the pro-inflammatory cytokine macrophage migration inhibitory factor (MIF).
The aim of the study was to elucidate a plausible predictive value of the MIF levels for one-year clinical outcomes in STEMI patients who underwent primary percutaneous coronary intervention (PCI).
Materials and methods. 134 STEMI patients were enrolled in the study after receiving voluntary informed consent. All patients underwent conventional investigations, and additionally, the MIF levels were determined at baseline, directly before and after PCI. During 1-year follow-up, 37 % of patients reached the endpoint, which was composite and included all-cause mortality, non-fatal myocardial infarction, non-fatal stroke, hospitalization for unstable angina, heart failure decompensation, and urgent revascularization.
Results. We have found that pre-PCI MIF levels > 3934 pg/mL (AUC=0.7; 95 % CI 0.578 to 0.753; Youden index=0.31; p=0.008) might be an independent predictor of composite endpoints with sensitivity 54 % and specificity 82 %. A positive correlation between MIF and inflammatory biomarkers was revealed (WBC count r=0.33, p=0.0001; CRP r=0.19, p=0.032). Adverse outcomes associated with higher pre- and post-PCI MIF levels (OR 1.0, 95 % CI 1.0001–1.0008; p=0.013 and OR 1.0, 95 % CI 1.0001–1.0009; p=0.019) and CRP that determined during the first week after the event (OR 1.0, 95 % CI 1.005–1.2, p=0.03). Kaplan-Meier analysis has shown a substantially lower long-term survival rate in patients with a MIF level > 3493 pg/ml compared to a MIF level ≤ 3493 pg/ml (Log rank=0.00025).
Conclusions. The MIF levels exceeding 3934 ng/ml were associated with a higher risk of one-year adverse clinical outcomes in STEMI patients who underwent primary PCI
Factors associated with smoking quit attempts among tobacco smokers
Despite more than one-third of tobacco users attempting to quit tobacco each year, only 4–6 % of them succeed to remains abstinent after one year.
The aim: This study aims to estimate the proportion of smokers who attempt to quit tobacco smoking and determine factors associated with quitting behaviour.
Methods: This community-based, cross-sectional study was conducted among 720 smokers aged 18 years and above residing in a rural area of Haryana state of India. A chi-square test determined the association between various variables under study and quitting attempts among study subjects. Factors found to be statistically significant using a chi-square test were entered into a binary logistic regression analysis to obtain determinants of smoking quitting attempts among the respondents.
Results: 28.5 % of the respondents had attempted to quit smoking during the past 1 year. The majority of them, i.e., 73.7 %, had attempted once, whereas 21.0 % and 5.4 % of respondents had made 2 and 3 attempts, respectively, to quit smoking during the last 1 year. Factors independently associated with quitting behaviour were educational level (AOR=1.851, p=0.028), age of initiation (AOR=0.620, p=0.026), reasons for smoking (AOR=1.681, p=0.016), money spent on smoking (AOR=1.700, p=0.003), family pressure to quit smoking (AOR=1.725, p=0.006) and advice from health professionals to quit smoking (AOR=2.104, p<0.001).
Conclusion: Around one-third of smokers have attempted to quit tobacco smoking in the past year. Hence there is a need to formulate policies towards a targeted intervention for smokers who have not attempted to quit smoking in the past 1 year for effective tobacco contro
Monitoring and personalization in treatment of breast cancer patients with metastatic bone lesions
The aim. To increase the efficiency of treatment of BC patients with metastatic lesions of long tubular bones by using, Multidetector computed tomography (MDCT) and bone marrow markers for diagnostics and monitoring the clinical course of the oncologic process, accompanied by surgical intervention with endoprosthetics along with the treatment of polymorbid pathology in a specific patient.
Materials and methods. Authors provide systemic personification including visualization of the tumor site and its vascularization; printing out the 3D model; surgical planning, including optimal surgical access to the tumor site considering the volume and topographic and anatomical location and dissemination of the tumor, the convenience of intraoperative tasks (removal of the tumor, bone grafting or endoprosthetics), preoperative planning of bone resection lines with maximum preservation of intact bone tissue.
Results. Personalization of the treatment of breast cancer patients with metastatic bone lesions contributes to a significant reduction in postoperative complications of endoprosthetic replacement of large joints (up to 15.2 %) and increases the overall three-year survival rate (up to 40.6 %), as well as significantly improves their quality of life.
Conclusions. The personalization of treatment of patients with tumor lesions of the skeletons contributes to a significant decrease in the indicator of postoperative complications of endoprosthetics of great joints and to an increase in the total three-year survival rate, as well as to the improvement of the quality of life after the conducted treatment
Morphometric measurements of calcaneum in North Indian population
Calcaneum is the largest tarsal bone in the foot, which participate in the formation of subtalar joint.
Aims and objectives The difference in the anatomical structure of calcaneum plays an important role in dynamic, kinetic, and static of the foot. Hence this study was planned.
Material and methods. Present study was conducted in Pt. B. D. Sharma University of health sciences on 54 pair of dry human calcanei in which linear measurements were taken using digital vernier caliper with accuracy of 0.01mm and Bohler’s angle (BA) was measured with goniometer. Correlation of Bohler’s angle and maximum antero-posterior length (MXL) with another parameter were seen.
Results. In present study no significant difference was seen in the measurements of right and the left calcanei. Correlation of MXL of calcaneum with other parameters was done which came out to be significant except for CFH (right side) and DAFW (right side). Presented findings suggest that the MXL of the calcaneum bone can sometimes be calculated from its other parameters if well preserved, which could be helpful for forensic experts.
Conclusion. Present study on morphometric parameters of calcaneum would provide useful information for orthopaedic surgeries. Also, the knowledge of morphometry of this bone would be useful for forensic experts, foot rehabilitation procedure, and this study would contribute to the anatomic literature and clinical fields
Study of quality of sleep in patients with psychiatric illness under remission in relation to quality of life and functional disability
Sleep is one of the essential biological rhythms of a human being. Sleep disturbances could precipitate psychiatric illnesses like schizophrenia, bipolar affective disorder, depression, anxiety disorders, substance use disorders, etc. Even when psychiatric disorders are successfully treated, sleep disturbances often fail to remit. The quality of sleep in such patients is badly affected and influences the daytime functioning and impairs the overall quality of life.
The aim of the study. To study quality of sleep of patients with psychiatric illness under remission in relation to quality of life and functional disability.
Material and methods. Patients attending the review outpatient department of Tertiary hospital are selected for the study. Patients diagnosed with bipolar affective disorder and schizophrenia according to ICD-10 criteria were taken. Patients meeting remission criteria for bipolar affective disorder i.e., YMRS score </=8, MADRS score </=10 and for schizophrenia –eight PANSS signs and symptoms score <3 are selected from them. Quality of sleep is compared with quality of life and functional disability.
Results: Mean age was found to be 36.59. Mean PSQI score is 8.35. The mean score of physical quality of life domain is 58.28, mean score of psychological quality of life domain is 56.63, mean score of social quality of life domain is 55.39, and mean score of environmental quality of life domain is 55.41. Mean score of functional disability score is 30.14.
Conclusion: Poor quality of sleep in Schizophrenia and BPAD needs to be addressed
The importance of neuroimaging methods in the diagnosis of epilepsy in young men: literature review and own clinical case
Lifetime visualization of structural morphological changes of the brain in patients suffering from epilepsy and epileptic syndromes has become possible due to the introduction of neuroimaging methods into everyday medical practice. At the same time, neuroimaging is the main method of determining the etiology of epilepsy and its taxonomic position. One of these methods is magnetic resonance imaging (MRI). MRI is the method of choice in the examination of patients with diseases associated with brain development abnormalities.
The aim of this study is to consider the importance of using neuroimaging methods in this clinical case of epilepsy in a young person, taking into account complaints, anamnesis of the disease and life, neurological status, laboratory results for the correct diagnosis.
Conclusion. The presented clinical case of symptomatic epilepsy with focal vegetative-sensory seizures of medium frequency due to mass formation of the medial part of the right temporal lobe demonstrates the need and importance of a comprehensive examination using neuroimaging methods to clarify the etiology of the disease. Thus, magnetic resonance imaging has a high diagnostic value.

Two-year outcomes of pulmonary vein isolation and cava-tricuspid isthmus radiofrequency ablation vs pharmacological only antiarrhythmic therapy: a single center experience
The benefit of radiofrequency ablation (RFA) in rhythm control in atrial fibrillation (AF) and flutter patients is uncertain, but risk of death, arrhythmia recurrence and other post ablation complications remains high. Existing data on the impact of pulmonary vein isolation and cava-tricuspid isthmus RFA on long-term prognosis of patients with AF and flutter and its advantage over pharmacological antiarrhythmic therapy (AAT) are insufficient and contradictory.
The aim: we sought to evaluate two-year outcomes of pulmonary vein isolation and cava-tricuspid isthmus RFA vs pharmacological only AAT according to a single center experience.
Material and methods: we enrolled 174 patients after pulmonary vein isolation RFA, cava-tricuspid isthmus RFA and their combination and 122 patient who did not undergo RFA and got pharmacological AAT only.
Results: there was no significant difference in mortality between the RFA and AAT only groups (5.8 % and 9.0 % respectively) with the same structure of causes of death. The Caplan-Meyer curve analysis demonstrated better survivance (p=0.031) after RFA just during first year of observation. RFA effectiveness in arrhythmia relapse prevention was the highest for cava-tricuspid isthmus RFA procedure and worst – in group of combined pulmonary vein isolation and cava-tricuspid isthmus procedures. RFA showed an advantage over AAT in smaller quantities of non-fatal cardiovascular events (p<0.001) and cardiovascular hospitalizations (p=0.0026).
Conclusions: RFA of pulmonary vein isolation and cava-tricuspid isthmus RFA decrease arrhythmia episodes frequency, risk of non-fatal cardiovascular events and cardiovascular hospitalizations. Timely combined PVI and CTI procedure is associated with worsening of all outcomes
