THE STUDY OF THE EXTRACTION DYNAMICS OF BIOLOGICALLY ACTIVE SUBSTANCES FROM THE BIDENS TRIPARTITA L. HERB AND ANTIOXIDANT ACTIVITY OF THE OBTAINED EXTRACTS
The interest to study the genus Bidens and in particular Bidens tripartita L. (bur-marigold herb) is unmitigated due to its antioxidant, anti-allergic, antimicrobial, antifungal, hepatoprotective, immunostimulating and hypotensive activity. This pharmacological activities are determined by the presence of flavonoids, polyphenolic compounds, polysaccharides, components of essential oils, polyacetylenes, etc.
The aim of the research was to study the process of extracting various biologically active substances (polysaccharides, flavonoids, polyphenols) from bur-marigold herb with water-alcohol mixtures of various concentrations, to study their component composition, as well as to evaluate their effect on the oxidative effect of free radicals.
Materials and methods. The object of the research was the herb of Bidens tripartita L. Raw materials were collected and procured on the territory of Kharkiv, Zhytomyr and Poltava regions of Ukraine. All used methods for the quantitative determination of biologically active substances were pharmacopeial and described in various monographs in the European Pharmacopoeia and State Pharmacopoeia of Ukraine.
Results. The extracts of the bur-marigold herb were analyzed for the content of the extractable matter, flavonoids, polysaccharides and it was found that they are maximally extracted (about 80 %) in the 1st percolate with a drug/solvent ratio of 1:10 for all the extractants. It was found that with an increase of ethanol concentration in the extraction mixture, the content of polyphenols and flavonoids, is expectedly increased, as well as the value of the antioxidant activity of the corresponding extracts, but the amount of extracted polysaccharides is significantly reduced.
Conclusions. With the selected evaluation criteria, it was found that 40 % ethanol is optimal from the point of view of balanced extraction of flavonoids, polysaccharides and polyphenols. A strong positive and statistically significant correlation was found between the content of polyphenols in the obtained extracts and their antioxidant activity (Pearson correlation coefficient, r=0.9998), slightly weaker for flavonoids (r=0.9886), and an inverse correlation between polysaccharides content and TEAC- value
CHANGES IN THE STRUCTURAL CHARACTERISTICS OF PLATELETS AND LEUKOCYTES IN PATIENTS WITH ARTERIAL HYPERTENSION UNDER THE INFLUENCE OF A HYDROGEN SULFIDE DONOR
The aim of the research. The influence of hydrogen sulphide donor on some structural characteristics of platelets and leukocytes in patients with arterial hypertension was studied.
Materials and methods. The survey involved patients (men) with hypertension aged 30–60 years, who were divided into 2 age groups: 30–40 years and 40–60 years. Control groups (healthy men without signs of hypertension) were randomized by age. Electron microscopic and morphometric research methods were used in the investigation.
Results. Electron microscopic and morphometric evaluation of the structure of these cells has shown that it undergoes significant changes depending on patient’s age. In the persons of the older age group, the cells contained a reduced number of dense δ-granules and increased – α-granules, which may indicate an impaired serotonin balance and increased susceptibility to thrombogenesis. The younger age group presented structural changes in platelets in a reversed order. Adding hydrogen sulphide donor at a dose of 400 mg per day (phytopreparation Full Spectrum Garlic – Swanson Health Products, USA) to the traditional therapy of arterial hypertension leads to positive changes in the ultrastructure of the studied cells aimed at restoring the balance of serotonin, reducing the tendency to thrombogenesis, increasing the energy capacity of the mitochondrial apparatus.
Conclusion. The obtained results indicate that detected effects significantly depend on the patient’s age (younger people respond more intensively) and the duration of the use of hydrogen sulphide donor (longer use is accompanied by more pronounced positive changes)
MARKETING ANALYSIS OF THE ASSORTMENT OF DRUGS FOR THE TREATMENT OF HERPES VIRAL DISEASES AT THE PHARMACEUTICAL MARKET OF UKRAINE
Nowadays there is a tendency towards the spread of viral diseases that cause significant complications to human health. Therefore, the question of creating new drugs that have a high antiviral effect is topical.
The aim. To study the range of drugs in Ukraine for the treatment of herpes viral diseases.
Materials and methods. In the work were used the list of drugs from official sources of information, which were processed by methods of generalization, systematization, graphic, comparative and structural analysis.
Results and discussion. The marketing research of drugs for the treatment of herpes viral diseases presented at the pharmaceutical market of Ukraine has been conducted. According to the results it is found that 80 drugs for the treatment of herpes viral diseases of various dosage forms are presented in the domestic market, of which 70 % – tablets; 51 % – medicines produced by the industry of Ukraine; 94 % – are monocomponent preparations.
Soft drugs play important role in the treatment of skin diseases because they provide a direct effect of the drugs on the pathogens, eliminate inflammatory phenomena and eliminate or reduce the symptoms of the disease. Soft dosage drugs have practically no contraindications, they can be used at any age, regardless of the presence of comorbidities.
Conclusions. Domestic drugs (51 % of the total range) dominate in the market structure. The main part (94 %) consist of monocomponent preparations. The results of the research prove the relevance of the development of new combined soft dosage forms for the treatment of herpes viral diseases, which should be produced by the Ukrainian industry and satisfy the medical and economical needs of patients. The results of the marketing analysis show that it is relevant and promising to create new combined the soft dosage forms for the treatment of herpes viral diseases
DIAPHRAGMATIC MYOTRAUMA IN CHILDREN WITH ACUTE RESPIRATORY FAILURE
The aim of the study was to establish the prevalence of diaphragmatic dysfunction (DD), depending on the strategy of mechanical ventilation (MV).
Materials and methods. We completed the prospective single-center cohort study. Data analysis included 82 patients (1 month – 18 years old), divided into I group (lung-protective MV) and II group (diaphragm-protective in addition to lung-protective MV).
Patients were divided into age subgroups. Stages of the study: 1st day (d1), 3rd (d3), 5th (d5), 7th (d7), 9th (d9), 14th (d14), 28th (d28). We studied amplitude of diaphragm movement; thickening fraction, parameters of acid-base balance and MV. Results are described as median [IQR - interquartile range] with level of significance p.
Results. In patients of the 1st age subgroup in I group there were episodes with under-assist during MV, while in II group diaphragm overload was registered only on d5.
In patients of 2nd subgroup in I group we found over-assist of MV with excessive work of the right hemidiaphragm and low contractions of left dome at all stages of study, while in II group – the only episode of diaphragmatic weakness on d3 due to under-assist of MV. In the 3rd subgroup the proper diaphragmatic activity in I group was restored significantly later than in II group. In 4th subgroup of I group there was episode of high work of diaphragm on d5, whereas in II group – all data were within the recommended parameters for diaphragm-protective strategy of MV. In 5th subgroup of I group excessive work of both right and left domes of diaphragm was significantly more often registered than in II group, however, in II group there were found episodes of both type changes – diaphragmatic weakness and excessive work.
Conclusion: The prevalence and variety of manifestations of DD depend on the strategy of MV. Low incidence of DD was associated with lower duration of MV: in 1st age subgroup in 1.5 times; in 2nd age subgroup – in 2.4 times; in 4th age subgroup – in 1.75 times; in 5th age subgroup – in 4.25 times
THE EFFECT OF TRANEXAMIC ACID, CARBETOCIN AND ARGON PLASMA COAGULATION ON UTERINE INVOLUTION AFTER REPEAT CESAREAN SECTION
Repeat cesarean sections (CS) are associated with additional risks of perioperative complications.
The aim of our study was ultrasound evaluation of uterine involution in women after repeat traditional cesarean delivery and after repeat CS using argon plasma coagulation and prophylactic use of tranexamic acid and carbetocin.
Materials and methods. Prospective cohort study has been conducted on 140 patients who underwent second CS. Group I included 70 women who had repeat CS with the use of argon plasma coagulation and administration of 100 mcg carbetocin after cutting the umbilical cord. Ten minutes before the operation, 15 mg/kg of tranexamic acid was injected intravenously. Group II consisted of 70 women who had traditional CS, 10 IU dose of oxytocin, divided between 5 UI intravenous bolus dose and slow intravenous infusion, was administered after delivery of the baby. A serial ultrasonographic examination was carried out on the 2nd and 5th day of the postpartum period.
Results. A study of the dynamics of changes in uterine body and uterine cavity volume revealed a faster rate of uterine cavity involution in group I compared with group II (p<0.05). In group I on the 5th day in comparison to the 2nd day the mean uterine body volume decreased by 27.75%, in group II – by 20.17%. In group I mean uterine cavity volume declined in three days by 21.09%, in group II – 14.22%. Uterine subinvolution was diagnosed in 3 (4,29%) cases in group I and in 19 (27,14%) cases in group II (p<0.05). Faster uterine involution in group I is probably associated with techniques, that were applied during intraoperative period. In addition, 2.86% (2 cases) from the group I versus 31.43% (22 cases) from the group II, needed additional uterotonic therapy after surgery (methylergometrine, misoprostol) (p<0.001).
Conclusion. We have found differences in the course of uterine involution in women in group I in comparison with group II, such as significantly smaller uterine length on the 2nd and 5th day, significantly smaller uterine body and uterine cavity volume, faster rates of involution of the uterine cavity during the first 5 days of the postpartum period, thinner anterior uterine wall in the sutured area.
We therefore conclude that complex use of tranexamic acid, carbetocin and argon plasma coagulation appears to be effective to maintain adequate uterine involution after repeat CS and prevent postoperative complications
CLINICAL AND ULTRASOUND PARALLELS IN DIAGNOSIS OF GASTROESOPHAGEAL REFLUX DISEASE AND AUTOIMMUNE THYROIDITIS
The aim of the work: to study the relationship of ultrasound parameters of the thyroid gland with the clinical course, the level of thyroid hormones and indicators of the motor-secretory function of the gastrointestinal tract in patients with gastroesophageal reflux disease (GERD) and autoimmune thyroiditis (AIT).
Materials and methods. The study involved three groups of patients. The first group consisted of 22 patients with isolated GERD. The second group included 20 patients suffering from GERD in combination with AIT in the hypothyroid stage. And the third group included 20 patients suffering from isolated AIT in the hypothyroid stage. The control group consisted of 15 apparently healthy individuals of the same age and sex.
All patients underwent a comprehensive examination, which included the necessary set of clinical, laboratory and instrumental research methods in accordance with the local treatment protocols such as collection of complaints and anamnesis, patient examination, physical examination, clinical and biochemical analyzes, ECG, EGD, intragastric pH-metry. The patients underwent ultrasound examination of the motor-evacuation function of the esophagus and stomach with water load and an ultrasound examination of thyroid gland. The functional activity of thyroid gland was assessed by determining the level of thyroid hormones (TSH, T4 free, T3) by ELISA and the presence of antibodies to.
Results. The study involved three groups of patients: patients with isolated gastroesophageal reflux disease, patients with a combined course of GERD and AIT in the hypothyroid stage and patients with isolated AIT in the hypothyroid stage. The most pronounced complaints, a higher level of thyroid-stimulating hormone and the most pronounced ultrasound changes are noted in patients with combined pathology. Ultrasound examination of the lower third of esophagus and stomach in patients of all three groups showed the presence of regurgitation, expansion of the lumen of the lower third of esophagus and an increase in the diameter of esophageal opening of diaphragm in varying degrees, in contrast to patients in the control group. Thus according to the results of thyroid sonography and ultrasound examination of the motor function of the lower third of esophagus and stomach, the most pronounced changes were found in the second group of patients who had a combined course of GERD and AIT.
Conclusions. 1. A clear correlation was established between the clinical course, an increase in the level of thyroid hormones and ultrasound picture of thyroid gland and indicators of the motor-secretory function of the gastrointestinal tract in patients with GERD and AIT;
It was shown that disorders in the activity of thyroid hormones affect the formation of motor-secretory disorders in patients with GERD with concomitant AIT which mutually aggravates the course of this comorbid pathology;
Ultrasound diagnostics can be used as an evaluative method in this category of patients
STUDY OF AWARENESS OF PHARMACY EMPLOYEES IN UKRAINE WITH THE PROBLEM OF ANTIBIOTICS RESISTANCE
Antibiotic resistance (ABR) is one of today's pressing challenges for scientists, healthcare, and pharmaceutical professionals. Pharmacies are often the first point of patient’s contact with the healthcare system, so the pharmacist must be a source of objective information about ABR and provide qualified advice on prescription and over-the-counter medications.
The aim: through questionnaires to identify the level of knowledge about antimicrobial drugs of systemic action in pharmacists and pharmacists in Ukraine, as well as to monitor their behavior on the recommendations and release of antimicrobials without a prescription.
Materials and methods. The survey was conducted among pharmacy employees from May 1, 2019, to February 11, 2020. The questionnaire consisted of closed and open questions, multiple-choice, and comparison questions.
Results showed high self-esteem of knowledge about antibiotics among respondents, but their answers to open questions indicate a lack of knowledge. Most pharmacists are ready to recommend antibacterial drugs for colds and SARS, whooping cough, sore throat with fever, diarrhea. 69 % of respondents believe that a doctor should prescribe antibacterial drugs, but at the same time, 74 % of respondents do not always or never ask for doctor's prescription for antibiotics.
Conclusions. We believe that more articles on antibiotic resistance should be published for pharmacists in order to improve their skills. Issues on this issue should be more actively covered in the curriculum and the process of postgraduate education (training). Pharmacists should also be reminded of the possibility of prescribing over-the-counter medicines to help patients with SARS
RESULTS OF COMPARATIVE ANALYSIS OF FORMATION OF PERSONNEL POTENTIAL OF PHARMACEUTICAL SUPPLY SYSTEMS IN UKRAINE, BELARUS, POLAND, COUNTRIES OF THE CIS AND THE EUROPEAN REGION OF WHO
As a result of the radical changes that have taken place in pharmacy at the turn of the century, the role of pharmaceutical workers has acquired a fundamentally new meaning and development in the health care system of the countries. Under these conditions, the issue of forming the human resources of pharmaceutical support systems acquires not only important professional significance, but also acquires socio-economic content for the effective development of the macroeconomic complex of countries as a whole.
The aim. Carrying out a comparative analysis of the process of forming the human resources of pharmaceutical supply systems in Ukraine, Belarus, Poland, the countries of cooperation of the Independent States (CIS) and the European Region countries (ER) of the World Health Organization (WHO).
Materials and methods. The indicators of the total number of graduates-pharmacists, as well as indicators of the number per 100 thousand population in Ukraine, Belarus, Poland, the CIS and ER of WHO during 2000–2013 were studied. These indicators are presented on the European Health Information Portal. (“Information and facts”). The authors used historical, information-analytical, analytical-comparative, systemic, logical, hypothetical-deductive and generalizations, as well as mathematical-statistical research methods.
Results. It is established that during 2000–2013 in Ukraine there was a significant increase in the number of graduates of pharmacists from 2.61 to 14.49 people per 100 thousand people. It is proved that Ukraine had the highest rates (%) of changes in these indicators, compared to other countries. Thus, the average value of the growth rate (%) of this indicator in Ukraine was 15.0 %, in Belarus and the CIS countries – 9.0 %, in Poland – 3.0 %, and in the ER of WHO – 4.0 %. It should be noted that in Ukraine, Belarus and Poland there was an unstable nature of changes in the number of graduates of pharmacists, per 100 thousand of us. According to the comparative analysis, it was found that the average value of the analyzed indicator in Ukraine was 5.8 times higher than in Belarus and 2.3 times higher than in Poland. In addition, the corresponding data of Ukraine were 3.1 and 2.6 times higher than similar indicators calculated in the CIS and ER of WHO countries. It is also noteworthy that the decline in Ukraine was characterized only by one year (2010), and amounted to 3.0 %.
Conclusions. Against the background of a systematic increase in the number of graduates-pharmacists who received diplomas in higher educational institutions of Ukraine, it is promising to analyze the dynamics of changes in the number of specialists who are actively engaged in professional activities in the pharmaceutical system. Confirming the status of a pharmaceutical country, in our opinion, Ukraine should form a socially oriented profile of the human resources potential of the health care system among the ER countries of WHO
DYNAMICS OF FUNCTIONAL CONDITION AND QUALITY OF LIFE IN PATIENTS WITH ASTHMA-COPD OVERLAP AND CONCOMITANT ARTERIAL HYPERTENSION AGAINST THE BACKGROUND OF COMPLEX THERAPY
Patients with ACO have significant poorer health-related quality of life and more severe functional limitations compared to asthma and COPD alone. Most commonly, chronic respiratory disease is associated with cardiovascular disease, such as arterial hypertension. However, the impact of concomitant cardiac diseases on the quality of life and functional status of patients with ACO remains poorly understood.
The aim of the work was to study dynamics of functional condition and quality of life in with ACO and concomitant AH against the background of complex therapy.
Materials and methods. We selected for participating in the study 100 patients with ACO and concomitant AH. Examination of the patients included: clinical methods, spirometry, and questinaries – mMRS, CAT, SGRQ, performing 6MWT.
Results. After 16 weeks of treatment there were no changes in lung functional status in patients on standard treatment, at the same time, in group of patients who had an active rehabilitation program, there was a significant improvement in the bronchial response to the action of bronchodilators, although other indicators of the functional status of the lungs didn't show significant changes. Patients who additionally used an active rehabilitation program had a significant improvement in clinical symptoms, shortness of breath, and quality of life according to CAT, mMRC, and SGRQ scores, respectively. There was also a significant increase in distance during the 6MWT in this group of patients.
Conclusions. Conducting an active rehabilitation program (physical rehabilitation in combination with an educational program and self-management) in group of patients with ACO and concomitant AH, who are on standard medical treatment, significantly improves the bronchial response to the action of bronchodilators, decreases clinical manifestations, shortness of breath and improve quality of life and exercise tolerance, according to CAT, mMRC, SGRQ and 6MWT questionnaires, respectively
ORTHODONTIC TREATMENT WITH Nd-Fe-B MAGNETS
The aim. The development of methodology for experimental and theoretical assessment of interaction forces between magnets in an orthodontic apparatus, the test of corrosion resistance of protective oxide and nitride coatings deposited on Nd-Fe-B magnets surface.
Materials and methods. The Nd-Fe-B permanent magnets with saturation magnetization Ms≈1100 G and bilayer ZrN / ZrO2 coatings were used. To experimental measure of interaction forces between magnets the device was assembled on the base of analytical balance. The distance between the magnets was varied using non-magnetic plates. The ZrO2 and ZrN coatings have been analyzed for their corrosion properties in 0.9 % NaCl quasi-physiological solution.
Results. An original method was proposed for calculating of magnetic interaction forces for materials with high magnetic anisotropy, which has good agreement with experimental measurement of forces. The theoretical model takes into account the size of the magnets and the mutual influence of their opposite faces. An increase of corrosion resistance of magnetic materials can be provided by zirconium oxide or nitride compounds, which contribute to inhibition of electrochemical corrosion of Nd-Fe-B magnets.
Conclusions. A method for calculating of interaction forces between permanent magnets, which are used for correction of malocclusion in orthodontic, has been developed. The passivation of the Nd-Fe-B permanent magnets surface can be achieved by applying of bilayer ZrN / ZrO2 coating
