Causal complexity of sustainable consumption: Unveiling the equifinal causes of purchase intentions of plant-based meat alternatives
International audienceSustainable consumption has received much attention, although its antecedents are not fully elucidated. Existing studies are constrained by the assumption of symmetrical paradigms, emphasizing the net effects of antecedents without considering the complexity of sustainable consumption decision-making. This study draws on the theory of consumption values and the complexity theory to identify the complex combinations of consumption values (health, food safety, ecological, curiosity, novelty, prestige, avoidance of guilt) that explain consumers’ intentions to purchase plant-based meat alternatives (PBMA). An online survey was used to collect data from 252 meat eaters in the United States. Data were analyzed with fsQCA to identify the causal configuration of consumption values that explain meat-eaters’ PBMA purchase intentions. The fsQCA results revealed six heterogeneous combinations of the consumption values that result in high levels of purchase intention toward PBMA. The findings can be used by marketers to develop strategies for promoting sustainable consumption and PBMA

Gaps after linear ablation of persistent atrial fibrillation (Marshall-PLAN): Clinical implication
International audienceBACKGROUND Beyond pulmonary vein (PV) isolation, anatomic isthmus transection is an adjunctive strategy for persistent atrial fibrillation. Data on the durability of multiple lines of block remain scarce.OBJECTIVE The purpose of this study was to evaluate the impact of gaps within such a lesion set.METHODS We followed 291 consecutive patients who underwent (1) vein of Marshall ethanol infusion, (2) PV isolation, and (3) mitral, cavotricuspid, and dome isthmus transection. Dome transec-tion relied on 2 distinct strategies over time: a single roof line with touch-ups applied in case of gap demonstrated by conventional ma-neuvers (first leg), and an alternative floor line if the roof line ex-hibited a gap during high-density mapping with careful electrogram reannotation (second leg).RESULTS Twelve-month sinus rhythm maintenance was 70% after 1 procedure and 94% after 1 or 2 procedures. Event-free survival af-ter the first procedure was lower in case of residual gaps within the lesion set (log-rank, P = .004). Delayed gaps were found in 94% of a second procedure performed in the 69 patients relapsing despite a complete lesion set with PV gaps increasing the risk of recurrence of atrial fibrillation (67% vs 34%; P = .02) and anatomic isthmus gaps supporting a majority of atrial tachycardias (60%). Between the first leg and the second leg, a significant decrease was found in roof lines considered blocked during the first procedure (99% vs 78%; P , .001) and in delayed dome gaps observed during a second procedure (68% vs 43%; P = .05).CONCLUSION Gaps are arrhythmogenic and can be reduced by optimized ablation and assessment of lines of block. Closing these gaps improves sinus rhythm maintenance
Recherche interventionnelle en santé des populations, évaluation des technologies de santé, recherche sur les services de santé et recherche sur la mise en œuvre des interventions : convergences et particularités
International audienceIn public health, intervention is an object of research and evaluation which, over time, has given rise to numerous approaches. The first part of the article proposes to reposition intervention research in population health and intervention evaluation on a continuum. Although the former has a more cognitive objective and the latter a more pragmatic objective, they are not mutually exclusive. The distinction between these two practices is based on the predominance of the following characteristics: the objectives pursued, the scope of the investigations, the regulatory constraints, the financing obtained, the ethical approaches taken, and the deliverables established. The second part of the article offers a glance different fields and approaches within the continuum between these two poles: Health Technology Assessment, Health Services Research and Implementation Research. While all of them have the study of health interventions at their core, but each has developed through specialisation in one or the other type of intervention, in a particular scope or context, in certain evaluation questions, or in specific approaches. all as gateways to the study of public health intervention, these different approaches are by no means mutually exclusive.En santé publique, l'intervention, est un objet de recherche et d’évaluation qui au fil du temps a donné naissance à de nombreuses approches. La première partie de l'article propose de repositionner la recherche interventionnelle en santé des populations et évaluation des interventions sur un continuum. Si l'on reconnait à la première un objectif plus cognitif et à la seconde un objectif davantage pragmatique, elles ne sont pas pour autant opposables l'une à l'autre. La distinction entre ces deux pratiques relève de la prédominance des caractéristiques en présence concernant : les objectifs poursuivis, la portée des investigations, les contraintes règlementaires, le financement obtenu, les démarches éthiques réalisées et les livrables établis. La seconde partie de l'article balaye différents champs et approches s'inscrivant au sein du continuum établi entre ces deux pôles : Health Technology Assessment, Health Services Research et Implementation Research. Tous ont pour noyau et point commun l’étude des interventions pour la santé mais chacun d'entre eux s'est développé par spécialisation sur l'une ou l'autre nature d'intervention, sur un périmètre ou un contexte d'insertion de l'intervention, sur certaines questions évaluatives ou sur des approches spécifiques. Ces différentes approches sont autant de portes d'entrée sur l’étude de l'intervention de santé publique, non exclusives l'une de l'autre

War in Ukraine: From Treaty to Treachery
International audienceWhen Russia invaded Ukraine on 24 February 2022 [1], Russia was in breach of its assurances under the Budapest Memorandum, signed by Boris Yeltsin, on 5 December 1994 in Budapest, Hungary. Consider some consequences and provisional lessons of the Ukraine-Russia War: (i) Do not relinquish nuclear weapons. No nuclear-armed state has ever been invaded; nuclear weapons have served as a deterrent. It is a counterfactual, but it is compellingly arguable, that a nuclear Russia would not have invaded a nuclear Ukraine.(ii) Do not trust treaties. Treaties, whether made with good or ill will, are subject to the double cross. In the case of the Budapest Memorandum, Vladimir Putin is the architect of the double-cross.(iii) The United Nations is a lame duck. The United Nations, is failing at the singular task that could be hoped was its raison d’être - maintain peace, prevent war. The United Nations has failed to prevent the Russia-Ukraine war, it has failed to prevent a nuclear power invading a non-nuclear power, it has failed to rein in a rogue state threatening the world with nuclear catastrophe. These are serious failures that call into question the UN’s fitness for purpose.These three joyless conclusions from the present war are destructive of worthy and cherished values
