    No full text

    No full text

Deep and continuous sedation until death in the French overseas departments

    No full text
Background Continuous deep sedation until death (CDSUD) has been a legally recognised end-of-life right in France since 2016. Data on implementation remain limited, particularly in the French overseas departments, which share the same legal framework as mainland France but present a more rapidly ageing population. This study describes the characteristics of patients who received CDSUD, the profiles of the physicians involved, the modes of implementation, and the emotional impact of CDSUD on physicians. Method We conducted a retrospective, questionnaire-based survey among physicians who certified deaths in four French overseas departments. The study population comprised a random sample of adult deaths between March 2020 and February 2021. CDSUD cases were identified through a multidisciplinary review process. Findings Of 1,082 informed non-sudden deaths, CDSUD was implemented in 128 cases (11.8%), mainly in hospitals, among younger patients without cognitive impairment. midazolam or benzodiazepine were used in 78.8% of cases, often combined with morphine. A colleague was consulted in 80% of cases, though formal collegiality was not systematic. Physicians mainly aimed to relieve unbearable suffering. They acknowledged possible hastening of death, although this was rarely their intention. The emotional impact on physicians was similar, whether or not CDSUD was performed. Interpretation This first population-based study of CDSUD in the French overseas departments highlights the need to improve access to this practice, particularly for older adults and those receiving end-of-life care at home. Further prospective and qualitative research is needed

Quelle reproduction sociale de l'élite socioprofessionnelle ?

    No full text

    No full text

    No full text
Migration and employment policies often cast doubt on the economic productivity of immigrant women who join their husbands in France. Based on the case study of "reunited" women - those who arrived in France under a family reunification procedure - this article examines the impact of this migratory configuration on the gendered division of labour within immigrant families, as well as on family relationships. It demonstrates how female migration is influenced by family power dynamics stemming from gender socialization, which are further exacerbated by migration policies that perpetuate a gendered order. On the one hand, withdrawing from the labour market to undertake domestic work can be associated with a model of upward mobility in migration. However, the desynchronisation of migration schedules can lead to an unequal distribution of paid and unpaid work, even for women who planned to enter the labour market quickly upon arrival. Nevertheless, living conditions in France are likely to disrupt family arrangements around work, both with spouses and descendants, and make entry into the labour market desirable for reunited migrant women. Ultimately, particularly for those with limited resources, their (re)entry into employment tends to reinforce gender relations within families

Modeling smooth and localized mortality patterns across age, time, and space to uncover small-area inequalities

    No full text
Small-area mortality estimation is inherently difficult, as random fluctuations from low death counts can obscure real geographic differences. We introduce a flexible model that borrows strength across age, space, and time to estimate mortality schedules and trends in very small populations. The approach ensures smooth patterns across these dimensions while allowing localized breaks from the spatial structure, capturing broad trajectories as well as sharp local contrasts. We implement our model within a Penalized Spline framework and estimate it using Generalized Linear Array Model techniques, resulting in a computationally fast, interpretable, and parsimonious method. Crucially, it can readily incorporate sudden mortality shocks, such as the Covid-19 pandemic, making it highly versatile for real-world demographic and epidemiological challenges. We demonstrate its application by estimating life expectancy and age-specific mortality inequalities in over 4,800 small areas across the Greater London Authority from 2002 to 2024
